I own a 2018 Jeep Grand Cherokee EcoDiesel that has experienced repeated high-pressure fuel pump (HPFP) failures despite completion of the NHTSA HPFP safety recall. After the recall was performed, my vehicle developed the exact failure described in the recall. Metal contamination was found in the fuel system, requiring replacement of the HPFP and major fuel system components. Approximately 13,000 miles later, during routine fuel filter maintenance, I suspected the failure had returned. I asked the dealership to inspect the fuel filters for metal contamination, but they declined. I then removed and cut open the filters myself and documented visible metal shavings consistent with the recall failure. This is the second occurrence of metal contamination after the recall repair. I retained the filters, saved fuel samples, and documented everything with photos and video. I provided this evidence to the dealership and Stellantis. A STAR case was opened, but the Stellantis District Manager instructed the dealership not to perform any repairs because the vehicle has no check engine light and remains operable. The recall warns that HPFP deterioration can contaminate the fuel system and cause a sudden loss of engine power, increasing the risk of a crash. It also states this failure may occur without warning lights. My concern is that Stellantis is requiring complete pump failure before authorizing repairs, despite documented evidence that the recall failure condition has returned. I still possess the contaminated filters, fuel samples, photographs, video, and service records and will provide them upon request. I respectfully request that NHTSA investigate whether the recall remedy adequately corrects this defect and whether Stellantis' refusal to repair a vehicle exhibiting the same safety-related failure condition is appropriate.

while driving the car puts itself in park, then the gear shift D starts blinking and the dashboard says put into Park. I've took it to jeep to figure it out and fix it. i got it back and while on the highway it did it again causing me to almost get hit by several cars. I was on the highway doing the speed limit and no brake lights also when it happens. I took it back to jeep and they still have no clue but my jeep is to dangerous to drive period. There are not warning signs when it happens.

I was traveling south on Lee Jackson hwy in Staunton Va at about 40 to 45 mph and was t-boned on the right passenger side with the impact just in front of the passenger door and front quarter panel. The damage ended up being all the way to the right rear passenger door. None of the air bags or side curtain airbags deployed.

* Upon normal driving the vehicle suddenly began to run rough. At that point the engine light came on and began flashing. Yes, it is available for inspection * I was traveling at highway speeds with my two young son's in the vehicle, when out of nowhere we experienced a major loss in power, Forcing us to get to the far right lane and off on the next exit. * The problem has been Confirmed (Passenger Side intake camshaft has 5 out of 6 lobes significantly worn off. .119" of material missing as well as 2 holes on cylinder 6 piston. * It was inspected by and independent mechanic who discovered the damage to the camshafts and the piston. It was suggested that I return the vehicle to the dealer as it would be needing a new motor seeing as the damages were premature and catastrophic in nature. Upon initial inspection at the dealer, they failed to identify the 2 holes in cylinder 6 piston. Suggesting that the "said holes" were nothing other than flaked carbon deposits. * There were no warning lights or any symptoms that would have indicated anything was wrong.

The Cruise control pacing/follow option does not slow down/break and will rear end/strike any vehicle. The auto breaking does not work. The touch screen constantly cycles randomly changing map directions, radio stations, heat and air conditioning, seat heating and cooling. While driving my 2018 Jeep Grand Cherokee Limited 4x2, the driver side Active Head Restraint deployed without any collision, rear impact, crash, or other triggering event. There was a sudden loud bang directly behind my head, which startled me and created an immediate safety distraction while the vehicle was in motion. I did not know what had happened at the time. The head restraint remains deployed/open and cannot be reset. The vehicle was not involved in a crash, there was no rear impact, and there was no airbag deployment. FCA/Stellantis has issued an X88 Active Head Restraint warranty extension for certain Jeep Grand Cherokee vehicles involving inadvertent deployment of the front Active Head Restraint where it cannot be reset due to internal damage. My vehicle appears to have experienced the same failure mode, but Jeep/Stellantis has advised that my VIN does not qualify for X88 coverage, apparently because the vehicle was built after the selected warranty extension population. My vehicle build date is January 2018, with MDH 011623. I am filing this complaint because this was a safety related head restraint failure that occurred while driving without a collision. The sudden deployment caused a loud impact immediately behind my head and created a dangerous distraction. I am concerned that vehicles outside the current X88 warranty population may still be experiencing the same inadvertent Active Head Restraint failure. The vehicle was factory ordered and delivered in February 2018, which may explain the later January 2018 build date, but it is still a 2018 Jeep Grand Cherokee and appears to have experienced the same inadvertent Active Head Restraint failure mode described in FCA’s X88 warranty extension.

My daughter that is [XXX], her in the passenger seat, me driving were traveling down a street. I slowed to enter a turn lane going maybe 10 mph and heard a loud bang. I said "what was that noise." I glanced at my daughter she looked dazed and said "I don't know but something slammed into the back of my head." I pulled in where we were going and looked to find the head rest was popped open behind her head. There were no bumps, no impacts and not even high speed. This headrest mechanism literally deployed for no reason slamming at high speed into the back of her head and neck. I stopped called Mopar and made a complaint because it was right at 5pm and dealers were closing. She was slurring speech, dazed and repeating herself. Took her to the urgent care that sent me to the ER out of concern. They told me she may need head and neck images. The doctors and staff were so confused as to what I was even talking about. I showed them photos so they could see the situation. I was in the ER with her for 5 hours. She has a concussion. THESE HEADRESTS ARE DANGEROUS AND JUST INJURED A MINOR. WHAT DO YOU MEAN WE ARE NOT CALLING FOR RECALL. THIS ISNT JUST ABOUT EQUIPMENT REPLACEMENT. THIS INJURY COULD HAVE BEEN WORSE AND THERE IS OBVIOUSLY SOMETHING FLAWED HERE. Headlight beams do not function. Beam lights come on upon starting car then turn off moments after. Then I am driving done the street in complete darkness. I have taken the Jeep to the dealership of purchase ( Elmhurst Chrysler/jeep/dodge Elmhurst Illinois 630-834-8000 ) 4 times over several months. They have confirmed the problem. Two days ago they had me replaced the battery. There was NO messages or other warnings that the lights have failed.

While driving my Jeep Grand Cherokee, the front passenger headrest suddenly deployed forward with a loud popping/exploding sound. There was no collision, no impact, and no warning before it happened. Fortunately, no one was sitting in the passenger seat at the time. If a passenger had been seated there, the headrest could have struck them in the back of the head or neck and potentially caused serious injury. This appears to be a dangerous safety defect involving the Active Head Restraint system. Based on information I found from other Jeep Grand Cherokee owners reporting similar failures, the issue may involve a faulty plastic bracket or internal component in the Active Head Restraint that can fail under pressure and cause the headrest to deploy unexpectedly. Other owners have reported injuries such as head, neck, and concussion-type injuries from similar incidents. I am concerned that this failure happened during normal driving conditions and without any crash or trigger event. This creates a serious risk to vehicle occupants and could also startle the driver, potentially causing a crash. I am requesting that NHTSA review this issue and determine whether my vehicle is affected by an existing recall or whether additional investigation or recall action is needed.

The active head restraint on my driver seat deployed and pushed my head forward suddenly. I could not push my head back into position. This happened while driving on the evening of May 3. I have a 2018 Jeep Grand Cherokee Laredo. I was not able to reset the head rest position. I took the vehicle to the dealership. They said that the whole headrest needed to be replaced. It could not be reset because the molded plastic piece was degraded. From a search online this appears to be common in the 2014-2018 Jeep Grand Cherokee with active head restraints. Even though this is a failure of the equipment, and other Jeep Grand Cherokees from 2014 -2018 received a 10-year extension on their head restraints, the dealership and Jeep Customer Care said that my vehicle is not covered. Nor will they let me buy a seat from them that does not use the active head restraint. They said that because My vehicle's VIN specifies a vehicle with active head restraints, they cannot sell me otherwise. I will not be comfortable driving this vehicle again with an active head restraint that could randomly fail and punch my head forward while I am in rush hour traffic. I believe this is a safety risk as it is disorienting to not know why your head is suddenly pushed down and forward while you are driving at 45 mph. Distracting the driver in such a way endangers the driver, passengers, and other vehicles' occupants on the same roadway. It throws off your view of the roadway and your mirrors. There were no warnings ahead of time.

On 5/72026@app 3:14pm after picking up one grandchild, and at a stop light on the way to p/u the other, the Jeep without warning just disengaged when the gas pedal was depressed, but to no avail. While frozen, the dashboard may have displayed something that was not noted in the panic to get the car moving! The internal panel was still illuminated, however the car would not move forwards although still in "Drive" with nothing having been switched, shifted or toggled except engagement of the brake to stop at the traffic light. The Jeep had to be placed in "Park", the starter button pressed, then pressed again to completely restart the vehicle. This was a potentially huge safety issue with many cars around us honking & rushing to pick up their own children, yet fortunately no one ran into us with our precious little rider!

There are parts in the engine that malfunctioned and have caused fluids to leak into the engine. This is causing the engine to fail and requiring replacement. This issue started last year for me personally. The car has been to 2 mechanics and they both have diagnosed the issue , and to fix it requires full engine replacement . It’s a safety issue because the engine can lock up and stop the car.

I recently purchased a 2018 Jeep Grand Cherokee and began experiencing serious issues almost immediately after taking possession of the vehicle. The vehicle has stalled unexpectedly while driving on multiple occasions, including during active driving conditions. It also has difficulty starting and has shut off without warning. The vehicle has already undergone multiple repairs within a short period of time, including work related to the radiator, spark plugs, and ignition coil. Despite these repairs, the issues continue. This creates a significant safety concern, as the vehicle becomes inoperable without warning, especially during daily highway driving. I rely on this vehicle for transportation, and it is currently unreliable and unsafe to operate.

Twice I have been driving on the highway at 60 mph and the transmission had suddenly shifted into low gear and subsequently went into limp mode. The dealer is unable to find an issue with the vehicle. There were no warning lights, before, during, or after. After I pulled onto the shoulder of the road and turned the Jeep off and turned it back on it operated normally.

Electrical goes crazy when driving and parked , radio comes on, switches from AC to heat , on its own , calls people in my contact list , calls 911, screens goes blank at times , flashes on and off , heated seat on driver side gets way to hot , passenger seat ok so far , headlight keeps going out on passenger side fails to get it off heated seats at times or it freezes, it jumps from defrost to vent to floor , heat to AC , have to turn on AC when it rains to keep windows from fogging up. Hard to drive when it’s lighting up like a Christmas tree , radio won’t stay on same station , jumps from station to station , when on speaker phone it mutes the calls, or hangs up the person you are talking to. Navigation won’t work most of the time freezes up also.

Recall on the hydraulic control unit/anti-lock brake system A brake light incorrectly resulting other drivers not receiving proper indication to stop or slow down which can cause a vehicle crash without prior warning. Call my local dealership and MOPAR and informed me that they still do not have the part to fix this recall. It has been more an an year and half without resolution. They have not inspected my vehicle as I did schedule an appointment but the dealership informed me that they is still no solution and would reach out. There has no warning lamps or messages that appears in my vehicle as of right now.

Check Engine light with Codes: PO300, and PO305. Misfire in Cylinder #5. Changed all spark plugs, and coils. Check engine still on, with same codes. Jeep dealer then said it was the cam shaft and needed new cam shaft head of a price tag of $8500. The car has been well maintained and has 75,200 miles on it. There is a known premature failure with the Pentastar Engine that is in this vehicle. Jeep is refusing to stand behind their product.

The contact stated that while unloading groceries out of her vehicle with the vehicle in park(P) and the engine on, grayish-white smoke began to emit from the vehicle without warning. The contact turned off the vehicle and checked under the hood, where she saw flames in the engine. The contact then ran to the fire department, which was nearby, and informed them of the fire. The fire department arrived at the scene with a fire truck and extinguished the fire. The police department also arrived at the scene, and a police report was filed. No injuries were reported due to the fire. The vehicle was destroyed as a result of the fire, but was not towed. The dealer or manufacturer had yet to be notified of the failure. The vehicle was not repaired. The failure mileage was approximately 74,000.

1. The issue involved the engine and emissions system, specifically a cylinder 3 misfire, accompanied by illumination of the check engine light and white smoke emitted from the exhaust. The vehicle was evaluated by an authorized Jeep dealership, which confirmed the cylinder misfire through diagnostic testing. At that time, per the diagnosis, coil 3 and all spark plugs were exchanged. This did not solve the issue. When picking up the car, check engine light was still illuminated and white smoke emitted from the exhaust. The affected components are in possession of the dealership, unsure if available upon request. 2. The engine misfire and associated white exhaust smoke created a risk of engine performance degradation, including potential loss of power, observed hesitation and stalling, which could impact the ability to safely operate the vehicle in traffic. Additionally, white smoke from the exhaust can impair visibility for surrounding drivers and may indicate an underlying engine condition that could worsen without warning, increasing the risk to the driver and others sharing the roadway. The chemical smell could also nauseate the person operating their vehicle, impeding their awareness and concentration. 3. This issue was proposed by an authorized Jeep dealership, then confirmed by two other mechanics. Dealer diagnostic records document the cylinder 3 misfire and investigation related to the check engine light and exhaust condition. 4. No inspection from manufacturer, police, or insurance representative. 5. Yes, the check engine light illuminated and the vehicle exhibited white smoke from the exhaust which prompted the service visit. Auto-start was not functioning as normal, with an error displayed on screen. Engine would sometimes stall upon start up during cold temperature days. NOTE: vehicle is currently with another mechanic awaiting an engine replacement, will upload that invoice once available.

While driving on the highway I received a “SERV 4WD” light and also a traction control unavailable light. Other than that that the car appeared to drive normally other than I could feel the car gears “catching” at starts when I pressed the accelerator. I took the car to the certified dealer and they said it was a transfer case failure and was unsafe to drive as it could have caused the wheels to lock up. This issue is very similar to a recall for other Jeep VINs over the same production timeframe of my Jeep, although my VIN was not included in that recall. I am seeking the immediate inclusion of my VIN into the Jeep recall that exists for manufacturing defects of the transfer case.

To Whom It May Concern, I am writing to formally report a serious safety defect regarding the Active Head Restraint (AHR) system in my vehicle. On 11/28/25 the headrest deployed spontaneously while I was operating the vehicle. Incident Details: • No Collision: There was no impact, collision, or sudden braking event that would trigger a safety deployment. • Safety Risk: The forceful deployment occurred without warning, striking me in the back of the head. • System Failure: The internal plastic bracket responsible for holding the spring-loaded mechanism failed—a documented issue in many similar Stellantis vehicles. I contacted a certified Jeep dealership and Stellantis customer service regarding this failure. They have declined to provide a remedy, repair or reimbursement the component under any "goodwill" or extended warranty program, stating that my specific VIN is not included in existing service bulletins or recalls. Request for Action: The "VIN-specific" limitation is arbitrary when the mechanical failure is identical to those in recalled units. A safety component that deploys violently without an accident is a defect, regardless of whether the manufacturer has acknowledged that specific production batch yet. While driving this vehicle under normal conditions, the engine developed abnormal ticking/knocking noises, loss of power, and misfire symptoms that progressively worsened. The vehicle was frequently used to transport children. Due to the nature of the failure and unpredictability of progression, continued operation posed a serious safety risk. An authorized dealer later confirmed that the engine suffered a catastrophic internal valvetrain failure involving collapsed lifters/rocker arms and severely damaged camshafts. The dealer technician explicitly stated this failure mode is “typical” for this engine design and documented progressive internal damage affecting both cylinder banks, not an isolated component. This failure mode is not sudden; it develops gradually over tens of thousands of miles and can lead to sudden engine power loss, stalling, or seizure without warning. A loss of propulsion at highway speed, during lane changes, intersections, or while transporting children creates a clear risk of crash, injury, or worse. There was no evidence of driver abuse, lack of maintenance, lubrication failure, or misuse. The dealer confirmed oil condition and service history were appropriate. The defect appears to be internal, latent, and manufacturing-related, consistent with numerous consumer complaints, technical service bulletins, and ongoing litigation involving this engine family. Had the vehicle continued to be driven, the failure could have escalated to: •Sudden loss of engine power at speed •Inability to accelerate to avoid hazards •Engine seizure causing loss of steering assist •Increased crash risk with child passengers onboard The vehicle was removed from service due to safety concerns. This defect is repeatable, predictable, and well-documented, yet not subject to recall at this time. I am reporting this to support investigation into a systemic engine defect that presents a serious safety risk to drivers and passengers.

I own a 2018 Jeep Grand Cherokee Overland. On December 5, 2025, the vehicle received a transmission fluid exchange service at a Jeep dealership. Prior to this service, the transmission operated normally with no hesitation, harsh shifts, or warning lights. Within a few days after the service, I began noticing delayed engagement and hesitation during shifts. On December 14, 2025, while driving at highway speed, the transmission suddenly shifted harshly with a rapid increase in engine RPM followed by a hard engagement. Later the same day, when stopped at a traffic light, the vehicle hesitated to move forward, revved abnormally, then engaged abruptly. A check engine light illuminated at that time. The vehicle behavior was unpredictable and created a safety concern while driving in traffic, particularly during acceleration from a stop and at highway speeds. The vehicle was driven directly to the servicing dealership and is currently available for inspection. The issue began only after the transmission service was performed. The transmission had no prior symptoms before the service. Warning indicators: Check engine light illuminated on December 14, 2025 after the transmission exhibited harsh shifting and hesitation. Inspection status: The vehicle is currently at the dealership that performed the transmission service for diagnosis.

I purchased my Jeep in 2023 and have consistently stayed up to date on all required maintenance. I had no issues with the vehicle until recently, when the check engine light and the start/stop warning appeared out of nowhere. After inspection, the mechanic determined that the entire engine needs to be replaced due to severe coolant leakage. My vehicle has now been in the repair shop for over three months because the engine is on national backorder. I contacted the dealership, but they dismissed my concerns and told me that once the car is purchased, it’s no longer their responsibility. I then reached out to the manufacturer requesting a goodwill resolution, given the unreasonable repair delay and the national backorder of such a critical component. Lemon law mileage limits do not apply to goodwill assistance, trade-in support, or corporate escalations. Despite this, the manufacturer has refused to provide any explanation or documentation for why they cannot assist and it has been three months with no progress. There were no warnings or indicators that something was failing internally, and my mechanic confirmed there was absolutely nothing I could have done to prevent this. I’ve also seen numerous reports of this same issue affecting my specific make and model. Stellantis needs to take accountability. It is completely unfair that I am left without reliable transportation for months over an issue that was entirely out of my control.

A malfunction in the antilock-brake system (ABS) module may incorrectly illuminate the brake lights and disable the ABS and electronic stability control (ESC). In addition, the vehicle operator may be able to start and shift out of Park without the brake pedal being applied.

Risk: A vehicle that can be started and shifted out of Park without applying the brake pedal can result in a vehicle rollaway. Disabled ABS and ESC systems can reduce vehicle handling and control in certain driving situations. Brake lights that incorrectly illuminate can fail to properly indicate the driver's intention to following traffic. Any of these scenarios can increase the risk of a crash.

Remedy: Dealers will replace the integrated pressure transducer and update the ABS module software, as necessary free of charge. Owner notification letters were mailed December 16, 2025. Risk: Combustion in the intake manifold can increase the risk of a fire.

High pressure fuel pump failure

The Office of Defects Investigation (ODI) opened PE21-021 on October 14, 2021, to investigate incidents alleging a stall/loss of motive power as a result of high-pressure fuel pump failures in certain model year (MY) 2019-2020 Ram 2500, 3500, 4500, and 5500 heavy duty trucks equipped with 6.7L Cummins turbodiesel engines. During the investigation, ODI sought to determine if the related defect allegation was limited in scope to the recalled population. After review of information request response materials from both FCA and BMW, NHTSA determined that sufficient information to identify a comprehensive recall population could not be produced by FCA and BMW.During the investigation FCA filed recalls 22V406, 22E048, 22V767, and 22E087 which include vehicles not identified in the initial subject population by ODI. ODI also received recall 21V586, involving loss of motive power due to failed CP4 fuel pumps on certain BMW manufactured vehicles. An information request letter response received from BMW indicated that failed pumps on their vehicles were caused by an interaction between pump internal components and US market diesel fuel, leading to increased slip and eventual particle-generating wear surface. Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

Active Head Rest Inadvertent Deployment

The Office of Defects Investigation (ODI) opened PE19014 on September 9, 2019, to investigate 128 consumer complaints alleging that the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge Durango vehicles.When ODI opened the investigation, ODI was aware of thirteen alleged injuries.A small number of Early Warning Reports identified driver and/or passenger front seat AHR inadvertent deployments without a rear impact event.Reported safety consequences resulting from the AHR deployments include concussions, headaches, soft tissue injuries, disorientation, nausea, vomiting and/or loss of vehicle control as a result of being surprised and distracted by being struck in the head by the inadvertent AHR deployment. On September 24, 2019, ODI sent an Information Request (IR) letter to Fiat Chrysler Automobiles (FCA).ODI requested certain detailed information from FCA related to the following subject vehicles: MY 2010-2014 Dodge Avenger 200, MY 2010-2014 Dodge Calber, MY 2010-2014 Chrysler Sebring, MY 2013-2015 Ram Tradesman, MY 2012 Ram Van, MY 2010-2019 Dodge Grand Caravan, MY 2010-2016 Chrysler Town and Country, MY 2010-2017 Jeep Compass, MY 2011-2020 Dodge Durango, MY 2011-2020 Jeep Grand Cherokee, MY 2010-2019 Dodge Journey, MY 2010-2012 Jeep Liberty, MY 2010-2011 Jeep Nitro, and MY 2010-2011 Jeep Patriot vehicles.On December 2, 2019, ODI received a response from FCA that included vehicle production data, consumer complaints, field reports, warranty claims, manufacturer actions, production changes, and other data related to the alleged defect from the start of production (2010) through September 24, 2019.On January 27, 2020, ODI received an update to the December 2, 2019, IR response from FCA.On September 22, 2020, ODI requested additional subject vehicle AHR information from FCA.ODI received FCA's response on October 21, 2020.ODI subsequently received an update to FCA's October 21, 2020, response on February 16, 2021.During the investigation, ODI requested updates to certain IR questions on two occasions.FCA provided updates to question #2 on March 5, 2021, and questions #2 and #5 on May 26, 2023. FCA’s IR response data identified approximately 7.5 million subject vehicles (with approximately 15 million AHR assemblies).Due to the large number of vehicles and failure reports, ODI focused its analysis on the FCA vehicle model with the highest failure rate to represent the worst-case scenario.For this purpose, ODI focused on MY 2012-2015 Jeep Grand Cherokee vehicles for its analysis.This population includes 710,890 vehicles that FCA manufactured between August 19, 2011, and August 8, 2015.ODI identified 2,839 alleged AHR failures within this limited population.ODI identified 263 alleged injuries that were caused by AHR failures in MY 2012-2015 Grand Cherokees vehicles.Of these alleged injuries, ODI categorized them as eighty-two (82) soft tissue (neck/whiplash, bruise) injuries, four (4) concussions, fifty-nine (59) headaches, and one hundred eighteen (118) without a specific injury description.Although complainants have alleged injuries, ODI has been unable to obtain evidence to substantiate or validate any injuries where medical treatment was required.Consumer outreach was conducted by ODI, where sixty-three (63) FCA complainants with alleged injuries were contacted.Twelve of the sixty-three FCA consumers responded and were interviewed by ODI.ODI requested medical records, however, only one response was provided. Attorneys representing the responding complainant provided a synopsis that indicated they had experienced prior neck-related injuries dating to the mid-1990s.ODI also contacted one hundred (100) consumers who submitted Vehicle Owner Questionnaires (VOQ) in an attempt to validate alleged injuries. Despite the efforts of ODI, the office has been unsuccessful at obtaining substantive documentation/evidence supporting complaints that allege serious injuries (where pre-existing medical conditions did not already exist). In addition to the low severity of alleged injuries caused by AHR deployments, there have been eleven alleged crashes and two alleged lane departures within the entire subject vehicle population that has been in the field up to sixteen years (with over 8,500 AHR inadvertent deployments).ODI was not able to validate that the alleged crashes were the result of inadvertent head rest deployments, and the crashes were minor in nature. ODI has conducted a thorough investigation of the alleged safety defect and with up to sixteen years of exposure for some vehicles and over 8,500 inadvertent AHR deployments, no serious crashes or injuries could be validated in the sub-population of focus.The lack of serious injuries validated by ODI with such large period of field exposure is consistent with thework done by NHTSA’s Human Injury Research Division and the Biodynamic Research Corporation, which support that an inadvertent AHR deployment is not likely to cause serious injury.In addition to the low risk of injury, FCA released an extended warranty program that covers inadvertent AHR deployment for ten years / unlimited mileage from the vehicle's in-service date. PE19014 is closed with the FCA extended warranty program.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the Agency reserves the right to take additional action if warranted by new circumstances.
