On 5/18/26 at 6:45 am Eastern Time While driving, both left and right sides of the rear lower control arms completely detached from the vehicle due to severe internal rust perforation of the rear subframe/cradle assembly. The structural mounting bracket on the subframe snapped off entirely, causing a catastrophic suspension failure and a sudden, total loss of vehicle control. This is a severe structural safety hazard that occurred without warning due to the subframe rotting from the inside out.the entire cross member detached from the back of the frame dis lodging the drive shaft as well.the picture I am attaching below is one side both sides of the rear look the same .

Car parked in driveway, passenger Active Head Restraint deployed overnight while parked. Plastic mold holding the latch pin in place is destroyed. Broke into pieces. The latch pin in pic1.jpg is still secured in the lock position. The computer is not throwing codes of deployed headrest, the computer thinks everything is fine. Pic3.jpg has a picture of the driver headrest, NOT deployed. The headrest are designed to deploy only with impact. The plastic molding broke away from the latch pin, and allowed the headrest to deploy prematurely. This should have been a safety recall on the first report. Thousands of complaints have been noted thus far.

I brought my vehicle to the dealership (Axis Chrysler Dodge Jeep Ram in Jersey City, NJ) for open safety recalls. The vehicle has been at the dealership for approximately 3 weeks. I was repeatedly told parts were coming (alternator and brake booster), but the repairs were never completed. I was later told I could pick up the vehicle, but no documentation was provided and I have reason to believe the recall repairs were not performed. This has caused delays, inconvenience, and safety concerns.

The contact owns a 2013 Jeep Grand Cherokee. The contact stated that while driving at an undisclosed speed, the check engine warning light illuminated. The vehicle was taken to the dealer; however, no cause for the failure was found. The dealer informed the contact that the battery charge was extremely low. The vehicle was not repaired. The contact stated that the failure persisted. The vehicle was taken back to the dealer, and was informed that the alternator had failed. The alternator was replaced. The contact stated that on a separate occasion, while stopped at a traffic light, the vehicle sputtered and stalled. The vehicle was pulled over to the side of the road. A tow truck arrived on the scene, and while the tow truck driver was attempting to jump-start the vehicle, sparks were coming from the battery. The tow truck driver became aware that the alternator was red hot, and the wires surrounding the alternator were burned, with a burning odor coming from the vehicle. The tow truck driver did not jumpstart the vehicle due to safety concerns. The vehicle was pushed onto the flatbed truck. The vehicle was towed to the residence and had not been driven since. The contact related the failure to NHTSA Campaign Number: 17V435000 (ELECTRICAL SYSTEM); however, the VIN was not included. The manufacturer was not notified of the failure. The failure mileage was unknown.

Took my vehicle to dealership for fuel pump recall. Was told it wasn’t the actual pump but the wiring and they fixed it. Vehicle continued to have the same issues plus a new issue where the car completely stalled out while driving. I took it back, they said the parts they put on were defective but I’d still have to pay 300$. I paid the money and within 10 hours of getting my car back I stalled on the highway in the fast lane. Luckily it was traffic so no one was hurt and I didn’t cause an accident but this is dangerous and I let the dealership know and all they said was bring it back for a day. I did and they couldn’t tell me anything.

Recall V62/NHTSA 19V-813 that was done on my vehicle in Jan 2023 at 44607 miles failed 0n March 4 2026 the vehicle died and would not start and the repair shop had to replace the fuel pump relay again only 4,811 mile later even though it had been 3 years not many miles and of course Jeep will not reimburse me because it was over 3 years ago

Active head restraint deployed on drivers side while auto was moving in parking lot. This unexpected event made the driver come to a sudden stop. There was no impacts of this vehicle into any object. No injury to the driver in the parking lot but is pssible injury to driver and other in the parking lot could have happened. After inspection it looks like a plastic bracket broke inside the active head rest.

- The core component that failed is the Totally Integrated Power Module (TIPM). The failure was initiated by a fault in the external fuel pump relay (V62 Safety Recall remedy part), which caused improper power delivery. This improper power delivery subsequently damaged the fuel pump motor, requiring replacement of both components. This failure has cascaded, also disabling the circuits for the Engine Cooling Fan (Codes P0480/P0693) and Exterior Lighting (Headlights/B1630), and the Horn. I am in possession of the burnt-out fuel pump relay/fuse component. -The engine stalled while the vehicle was operating, the exact hazard the V62 recall was meant to prevent. This was followed by a catastrophic TIPM breakdown, creating new, simultaneous safety risks: Overheating/Fire Risk: The Cooling Fan circuit failed (P0480/P0693), causing severe engine overheating. No Headlights: The Headlights failed (B1630), causing total loss of visibility at night. No Horn: The Horn failed, eliminating the ability to warn other drivers. - Yes. The initial engine stall and subsequent no-start condition (due to a blown relay fuse) were confirmed by an independent service center (Monro). The cascading failures (overheating, horn, lights) and the diagnostic codes (P0480, P0693, B1630, C2201-00) were confirmed on 11/1. - The vehicle was inspected by an independent service center (Monro), who returned the failed recall component to me (relay). Recommend it go to a certified jeep dealership. It is waiting to be towed there. -The first warning was a history of intermittent starting difficulty. On 10/23, the engine stalled while operating and immediately resulted in a no-start condition. The Check Engine Light illuminated after the subsequent failed restart attempts.

The contact owns a 2013 Jeep Grand Cherokee. The contact stated while driving approximately 15-25 MPH and depressing the accelerator pedal, the vehicle hesitated before accelerating as intended. The contact stated that the brake pedal resistance increased while depressing the brake pedal, which required the brake pedal to be depressed to the floorboard. The contact stated that the Emergency Stop Brake engaged unexpectedly while the vehicle was idling and while driving at various speeds. A "Low Battery" message was displayed, and upon inspection, the battery was hot. The vehicle was taken to a certified mechanic who diagnosed failures with the TIPM (Totally Integrated Power Module) communication and the transmission; however, the vehicle had not been repaired. The contact stated that the vehicle was previously included in unknown recalls and was taken to the dealer where the recall repairs were performed. The manufacturer was made aware of the failure. The failure mileage was unknown.

Hello, I am reporting failure on the 2013 Jeep Grand Cherokee in which the drivers side headrest activated while the vehicle was not in operation and not involved in any accident. The active head restraint had activated while the vehicle was parked and in a non-running condition. No accident occurred during this time frame. The passenger side active headrest is still in the factory location and has not been activated. The drivers side head restraint has activated and can not be returned to original position. My current safety is at risk as the active head restraint is not functioning per original safety design intent. My head and neck are currently constrained in a position that causes discomfort while driving due to the location of the active head restraint. The manufacturer (Authorized dealer) has been contacted and they responded with a response including that a recall was not issued for this failure type. A warranty period was extended to 10 years from manufacture date which my 2013 has eclipsed. No inspections have been completed to date. The parts have been untouched since the date of discovery. There were no signs of this issues prior to discovery. There are no service or warning lights currently illuminated on the dash or driver information screen. This is a safety related failure and needs to be repaired by the OEM with no cost to the customer. A well documented and known issue exists and puts the owner/operator of the vehicle at a increased level of risk of injury by continuing to operate this vehicle in its current condition.

Was inspected @ my local garage. The tipm goes out and you lose turn signals, wipers, transfer case control ( locks in low gear) causes airbag light to go on. And Chrysler does not manufacture them anymore. Jeep had to be towed.

I am reporting repeated safety-critical failures with my 2013 Jeep Grand Cherokee that have rendered the vehicle unreliable, dangerous, and now inoperable. I believe this vehicle suffers from a systemic electrical defect related to the Totally Integrated Power Module (TIPM)—a well-documented issue across Jeep models that causes starting failures, loss of power, and unpredictable behavior of vehicle systems. On [XXX], my vehicle failed to start. The starter clicks but does not engage. These symptoms align with known TIPM-related failures. Despite this, the authorized dealership (Madison Chrysler Dodge Jeep RAM) has refused to test the TIPM, insisting on replacing the starter without confirming the root electrical issue. This places consumers at risk of breakdowns without warning and reflects a pattern of ignoring known defects. My vehicle also has a history of major mechanical failures: Water pump, thermostat, and oil pressure unit replaced (Feb 2024) Rack and pinion, control arm, wheel hub replaced (Jul 2024) Rear brake caliper seized (Aug 2024) Radiator failure (Apr 2025) The vehicle remains with the dealership since 4/30/25. As of 5/14/25, I have received no update or repair. My case with Stellantis (Case [XXX]) has been met with repeated communication failures. I missed one call from the case manager on 5/8 and immediately returned it—leaving three voicemails over two business days and receiving no response. I was promised a callback by 5/13 and still have not heard back. These issues raise serious concerns regarding the safety and reliability of this vehicle. I am requesting the NHTSA investigate potential TIPM-related electrical defects and non-compliance with manufacturer responsibility to address safety defects. I do not believe it is safe for my vehicle to be returned to me in its current state, and I urge your agency to consider further action regarding this recurring and widespread defect. The vehicle’s ECM or TIPM system appears to be malfunctioning, causing intermittent no-start conditions. The engine fails to turn over even though the starter clicks, and all electrical systems are functional. The issue is temporarily resolved by disconnecting and reconnecting the battery, suggesting a logic fault in the ECM or TIPM. The vehicle is available for inspection upon request. How was your safety or the safety of others put at risk? The vehicle fails to start without warning, potentially leaving occupants stranded without transportation. This can be dangerous in remote areas, severe weather, or emergency situations. A failure like this undermines confidence in the vehicle’s reliability and safe operation. Has the problem been reproduced or confirmed by a dealer or independent service center? The issue has been observed and temporarily resolved by resetting the battery. Research shows this is a known issue in similar Jeep models, referenced in TSB 08-007-14. The symptoms align exactly, but Jeep denies coverage because the VIN is not in the affected range. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not yet inspected by the manufacturer or third parties, though Chrysler/Jeep was contacted. They acknowledged similar issues exist but declined to assist based on VIN exclusion. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The engine wouldn’t crank, and diagnostic code P0113 (Intake Air Temp Sensor Circuit High Input) appeared. The vehicle was stationary when the issue occurred and has had prior history of similar electrical faults.

The V62 fuel pump recall was repaired in 2021. Recently this vehicle has had hard starts again, misfires, engine light came on. The vehicle was taken in to certified repair shop. The shop states cannot diagnose without tearing the only thing apart and a high expense. The shop believes it could be lifters or possibly cam but they are unsure even though the systems to this vehicle having a hard start and misfiring relates to this V62 fuel pump relay that is relates to the cam shaft sensor and causes hard starts and misfires. The car is not driveable more than a short distance. The vehicle is unsafe to drive due to the fact you do not know when this vehicle will stall or start again. The V62 recall pet fuel pump relay has malfunctioned. This part was repair but is a faulty part, a lemon, and the manufacturer should be standing behind this part and their product. I have discussed this with the repair shop and Chrysler/Jeep and now pursing with you due to the safety of this vehicle, the defective recall part, and the fact the manufacturer needs to stand behind their product.

While driving my 2013 JEEP Grand Cherokee on [XXX] the passenger side front headrest deployed with no warning. There was a passenger in the seat at the time of the deployment. My local Jeep dealership stated the headrest wasn't covered by warranty even though an extended warranty letter was sent that there was a known issue with the headrest. There are NO warning lamps, messages or other symptoms of the problem prior to the failure. Once the headrest deploys it CAN'T be reset. The local JEEP dealership is unable to inspect or repair UNTIL the headrest deploys. The driver side headrest MAY STILL DEPLOY. This issue can cause an accident, injury, or worse. I had to pay for a repair of a known issue that I could take no preventive action. A call to Chrysler resulted in no resolution or reimbursement of $1,250.23 I paid. I have the deployed headrest. I was driving down a hill, came to a stop, and it died. I put the car in park, and turned it off and on. There were no messages on the dashboard. The car started, so I took a left turn and suddenly the car stopped moving. The RPM went up, but the car didn't drive. It kicked back and it finally started driving but with the car hesitating. I was in the middle of a very busy intersection. The car died 3 times yesterday, and today it died 2 times.

The contact owns a 2013 Jeep Grand Cherokee. The contact stated while driving approximately 45 MPH and slowing for traffic, the brake pedal was depressed; however, the brake pedal was firm. The contact stated that added force was needed to depress the brake pedal to slow the vehicle. The contact almost had to pull onto the shoulder since the vehicle was not slowing as needed. The contact stated that the word "BRAKES” in red was displayed on the instrument panel. The contact had taken the vehicle to an independent mechanic where it was diagnosed and determined that the Ready Alert Braking System had failed and needed to be replaced. The vehicle was not repaired. The manufacturer was not informed of the failure. The contact was advised by the mechanic that the failure was possibly related to NHTSA Campaign Number: 14V104000 (Service Brakes, Hydraulic); for the hard brake pedal; however, the VIN was not included. The failure mileage was approximately 72,000.

Passenger seat headrest exploded and hit my wife in the head/ear/neck and now she is in a lot of pain. This is the most random thing I have ever seen. My wife and daughter were driving through a parking lot going 4mph and the headrest just exploded. The plastic pieces holding the springs back broke and let the headrest fly forward into my wife’s head/ear/neck

The axles are all simultaneously rotting to dust. Manufacturer should’ve included grease and a cap.. no reason all 4 of my tires should be in the verge of falling off. A bit of a safety concern id say. Could’ve been easily prevented. Had a mechanic of 50 years tell me he’s never seen axles in such bad condition. I can’t even replace them myself due to the amount of rust that has replaced the bolts. No where else on the jeep has that much rust. From the hour of research I put in, I learned it’s an extremely common issue with the jeeps and in fact the 2017s were recalled for a similar issue. Seems like the 2013s should’ve been included.

Ok [XXX] 1:30 pm we got into car to go to lunch started driving and smoke started coming from the headliner and smelled like burning plastic on driver side near visor, smoke filled the car we were able to roll down windows and able to stop the car. The area continued to emit smoke and area on headliner felt hot. After Turing the car off the smoke/ fire continued and Had to locate the battery under passenger seat to disconnect power. Extremely stressful and scary situation as we have grandchildren and an elderly mother that we care for. I called Chrysler on 7/16/2024 spoke with Kayla she gave me a case number and informed me I would receive a phone call. I never got a call, so I called today 7/22/24 spoke with Ash she informed me this is not under warranty and my call was sent to a survey . My vehicle while I am driving completely stalls on me and I have to wait at least 45 minutes to turn my vehicle on. In 2001 Jeep was forced to do a recall however the situation is still the same with the jeeps stalling. This is a situation that can be life threatening when on a highway if an accident occurs.

We were traveling at approx 75 mph on [XXX] in Georgia when the Jeep completely stopped and shut off - lost all power and we had to coast to the shoulder. Thankfully no one was close behind us. The power steering went out and we could not start it back up. We had to get it towed twice to a Jeep dealership for diagnosis. Was told the fuel pump relay needed to be replaced bc of active recall, but that also both fuel pumps had to be replaced. Said that those are the reasons why Jeep wouldn't run. Called Jeep Customer Care and was advised to have the dealership repair, pay for the fuel pumps and then turn in cost under Case #[XXX] and they would reimburse for the parts and labor of the fuel pumps. The cost totaled over $3000 for the 2 pumps and labor and it could take 2 weeks. Told the dealer that we live in Virginia and asked for a loaner or rental car. They said they can't provide either. So we had to rent a car and drive back to Virginia. Then had to rent a car to drive back to Georgia to pick up the Jeep. The Service Rep was not very helpful and was actually very rude along with the service tech who did the repairs. He purposely wrote on our paperwork that the fuel pumps didn't go bad bc of the fuel pump relay so that we would not get reimbursed. But research shows that the faulty fuel pump relay does indeed damage the fuel pumps. After spending over $4k between repairs and additional costs caused by this recall issue, I filed the claim as I was told to do through Jeep Customer Care. They denied it saying that since the service tech specifically wrote that it wasn't the fault of the recalled part, they weren't going to pay it. We were told to have the dealer replace the pumps which we could have done ourselves had we known they would deny our claim. We did what Jeep Customer Care told us to and then they denied our claim anyway. We could have been seriously hurt and this wasn't the first time the Jeep had done this. Vehicle has stalled over twenty times, whether driving on the highway over 60 mph or driving locally within 20 MPH. Initially stalled December 25, 2022 while driving home on Christmas Day with my entire family, driving full speed on the highway. I felt the stall and pulled over as quickly as I could. Attempted to restart several times and engine would crank but not start until after 5-7 attempts and several minutes. Since then I continue to experience the same stalling and attempts to restart vehicle. Took to service center multiple times and they are unable to determine the issue. Fuel pump relay was changed twice by the dealership. I had to pay $1500 one time as they said the cost was not covered, which is ridiculous since the problem reoccurred many times after that. Most recently, fast forward to almost 1.5 years later and the issue is still occurring. Now it stalled and appears to be rolling back when in drive until I shift the gear to park and move the gear several times. It has been losing traction and power. I really need someone to determine a resolution to the issue as this is a severe safety issue and I am very concerned about driving it.

Active Headrest Restraint ( AHR) Drivers Side deployed overnight. This safety feature on many Jeeps is intended to protect motorists including passengers, in the event of rear end collisions by deploying. However a cursory review online reveals many many complaints, recalls, & TSBs . But my entire family now is exposed because Jeep wont extend recalls any longer. My recall expired 11 months ago. I was never notified by mail or by dealership that this recall would expire April 2023. I bought this SUV with understanding of its many safety features like AHD among others. Only makes sense that Stellantis /Jeep extend this recall beyong April 2023 as a result of substandard plastic parts used in headrest. If I had been notified by dealership when Jeep was serviced in 2023 it could have been fixed. All my recalls have previously been fixed. Trying to reach Customer Care at Jeep 877-426-5337 only to get complete jibberish and run-a-around. Certainly affect my next buying decision. Here-to -fore pleased with vehicle but this changes everything . I take safety VERY seriously. Inspection clearly indicates plastic parts ( sled bracket) failed.

Shortly after purchasing vehicle we noticed there was an issue when using the remote start, the vehicle would start and then the horn would go off as if the alarm was going off, the vehicle would shut off and not want to start with the even with the power button. I disconnected the batter and let it sit for about 20 minutes and then reconnected and started the vehicle normally with the power button, about 6-7 weeks go by and after not using the remote start we had another issue, (NO KEY DETECTED) with both keys in the vehicle, did the same procedure with battery disconnect and reconnect and holding the key fob on the power button so that it would recognize the fob and it seemed to be ok for the time being until the next issue arrived.

2ND, To continue with the issues of the vehicle lastly provided, while driving the check engine light come on and the traction control turns off, cannot turn back on using dash button, cannot access vehicle system on wheel, change display of dash view, windshield wipers are stuck on constantly, turn signals do not work nor does the hazard lights, vehicle is also stuck in 4x4 drive and cannot be taken out. Vehicle was taken to Preferred JCD of Muskegon MI and diagnostics were run with no clear diagnosis and no fix, suggestion was to replace TIPM box. Now Jan 2024 and vehicle still sits with issues and not driven.

I don't know if the headrest is part of the airbag deployment system or it's own entity. It is available for inspection. I was pulling into my driveway when the front passenger headrest exploded/deployed. I had just dropped my sister, who had been sitting there, and would have been injured by the force. I took my car to the dealership and was told that the 'involuntary deployment' was a known problem to Jeep and they had extended the warranty for the problem but it had expired in June 2023 so it would cost me $900 to fix. I contacted Jeep and was told they could not extend an extended warranty so repair was on me. There were no warning lamps or messages. This happened on 12/22.

Has been having issues with the hydralyft suspension. There has been a leak in both front air suspension fittings that connect to the air lines that supply air to both of the front air bag suspension suspension shocks. Replaced both of those out of pocket. My main reason for contacting you guys today is because I've searched my vin number & after further research I've found out that my 2014 jeep grand Cherokee overland w/the 5.7Litet hemi engine has started to give me some issues the other day with the battery dying while I was driving it home. The very next day i walked outside because I was about to head to work. So I cranked my SUV up noticed the battery light on the dash was back on again but then I seen a little bit of smoke coming out from underneath my hood so I shut my truck off opened the hood & seen it was coming from my alternator. I even got a video of when I opened the hood that morning when I seen the smoke, & when Iooked under the hood the copper wound wrapped diodes inside the alternator were glowing red hot the color of lava. All I smelled was burning copper so it was on the verge of the metal being so hot it melted, like they were just pulled out of a metal furnace. My vin number should be in this request I can provide it if not. I can also provide pics & video of suv & alternator. I pulled the alternator off earlier myself because I've been out of transportation for the past week. So I'm going to get it testing here ina bit.

Was driving down the road with my young son in the back seat and all of a sudden the head rest came flying forward and smacked the back of my head hard enough for me to swerve and almost get into an accident. After further inspection looking at the headrest once I got home I noticed that a pin is mounted onto some obviously brittle plastic that’s secured to a latch. Well i wasn’t in an accident and just out of nowhere this headrest flipped forward enough to rattle my head a bit.

The affected vehicles have electro-hydraulic power steering (EHPS) and are equipped with a 5.7L or a 3.6L engine and a 160, 180 or 220 amp alternator. In the affected vehicles, the alternator may suddenly fail.
Risk: If the alternator fails, the vehicle may stall without warning, increasing the risk of a crash. There is also the possibility that the alternator may short circuit, increasing the risk of a fire.

Risk: The water inside could freeze and limit the braking ability of the vehicle, increasing the risk of a crash.
Remedy: Chrysler will notify owners, and dealers will add a water diverter shield to the booster after the booster has been tested to confirm it can hold an acceptable amount of vacuum pressure. From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
The Office of Defects Investigation (ODI) opened PE19014 on September 9, 2019, to investigate 128 consumer complaints alleging that the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge Durango vehicles.When ODI opened the investigation, ODI was aware of thirteen alleged injuries.A small number of Early Warning Reports identified driver and/or passenger front seat AHR inadvertent deployments without a rear impact event.Reported safety consequences resulting from the AHR deployments include concussions, headaches, soft tissue injuries, disorientation, nausea, vomiting and/or loss of vehicle control as a result of being surprised and distracted by being struck in the head by the inadvertent AHR deployment. On September 24, 2019, ODI sent an Information Request (IR) letter to Fiat Chrysler Automobiles (FCA).ODI requested certain detailed information from FCA related to the following subject vehicles: MY 2010-2014 Dodge Avenger 200, MY 2010-2014 Dodge Calber, MY 2010-2014 Chrysler Sebring, MY 2013-2015 Ram Tradesman, MY 2012 Ram Van, MY 2010-2019 Dodge Grand Caravan, MY 2010-2016 Chrysler Town and Country, MY 2010-2017 Jeep Compass, MY 2011-2020 Dodge Durango, MY 2011-2020 Jeep Grand Cherokee, MY 2010-2019 Dodge Journey, MY 2010-2012 Jeep Liberty, MY 2010-2011 Jeep Nitro, and MY 2010-2011 Jeep Patriot vehicles.On December 2, 2019, ODI received a response from FCA that included vehicle production data, consumer complaints, field reports, warranty claims, manufacturer actions, production changes, and other data related to the alleged defect from the start of production (2010) through September 24, 2019.On January 27, 2020, ODI received an update to the December 2, 2019, IR response from FCA.On September 22, 2020, ODI requested additional subject vehicle AHR information from FCA.ODI received FCA's response on October 21, 2020.ODI subsequently received an update to FCA's October 21, 2020, response on February 16, 2021.During the investigation, ODI requested updates to certain IR questions on two occasions.FCA provided updates to question #2 on March 5, 2021, and questions #2 and #5 on May 26, 2023. FCA’s IR response data identified approximately 7.5 million subject vehicles (with approximately 15 million AHR assemblies).Due to the large number of vehicles and failure reports, ODI focused its analysis on the FCA vehicle model with the highest failure rate to represent the worst-case scenario.For this purpose, ODI focused on MY 2012-2015 Jeep Grand Cherokee vehicles for its analysis.This population includes 710,890 vehicles that FCA manufactured between August 19, 2011, and August 8, 2015.ODI identified 2,839 alleged AHR failures within this limited population.ODI identified 263 alleged injuries that were caused by AHR failures in MY 2012-2015 Grand Cherokees vehicles.Of these alleged injuries, ODI categorized them as eighty-two (82) soft tissue (neck/whiplash, bruise) injuries, four (4) concussions, fifty-nine (59) headaches, and one hundred eighteen (118) without a specific injury description.Although complainants have alleged injuries, ODI has been unable to obtain evidence to substantiate or validate any injuries where medical treatment was required.Consumer outreach was conducted by ODI, where sixty-three (63) FCA complainants with alleged injuries were contacted.Twelve of the sixty-three FCA consumers responded and were interviewed by ODI.ODI requested medical records, however, only one response was provided. Attorneys representing the responding complainant provided a synopsis that indicated they had experienced prior neck-related injuries dating to the mid-1990s.ODI also contacted one hundred (100) consumers who submitted Vehicle Owner Questionnaires (VOQ) in an attempt to validate alleged injuries. Despite the efforts of ODI, the office has been unsuccessful at obtaining substantive documentation/evidence supporting complaints that allege serious injuries (where pre-existing medical conditions did not already exist). In addition to the low severity of alleged injuries caused by AHR deployments, there have been eleven alleged crashes and two alleged lane departures within the entire subject vehicle population that has been in the field up to sixteen years (with over 8,500 AHR inadvertent deployments).ODI was not able to validate that the alleged crashes were the result of inadvertent head rest deployments, and the crashes were minor in nature. ODI has conducted a thorough investigation of the alleged safety defect and with up to sixteen years of exposure for some vehicles and over 8,500 inadvertent AHR deployments, no serious crashes or injuries could be validated in the sub-population of focus.The lack of serious injuries validated by ODI with such large period of field exposure is consistent with thework done by NHTSA’s Human Injury Research Division and the Biodynamic Research Corporation, which support that an inadvertent AHR deployment is not likely to cause serious injury.In addition to the low risk of injury, FCA released an extended warranty program that covers inadvertent AHR deployment for ten years / unlimited mileage from the vehicle's in-service date. PE19014 is closed with the FCA extended warranty program.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the Agency reserves the right to take additional action if warranted by new circumstances. In response to this investigation, Fiat Chrysler Automobiles (FCA) is conducting a recall to address headliner fires that occur post 14V-391.On December 22, 2015 FCA notified the Office of Defects Investigation (ODI) of safety recall 15V-879 to remedy potential headliner fires occurring in 389,252 model year (MY) 2011-2013 Dodge Durango and Jeep Grand Cherokee vehicles manufactured from December 3, 2009 to September 1, 2012.Note this recall, which was scoped based on vehicle design level and field events, does not include all MY 2013 or any MY 2014 Durango and Grand Cherokee vehicles; ODI and FCA will continue to monitor the field experience for this population.ODI opened this investigation based on allegations of fires occurring in the headliner area after the remedy for recall 14V-391 had been conducted.Recall 14V-391 involved MY 2011-2014 Jeep Grand Cherokee and Dodge Durango vehicles manufactured by FCA from January 5, 2010 to December 11, 2013.The recall was influenced by ODI investigation EA14-001.During EA14-001, data provided by FCA indicated fires were caused by an electrical short in the vanity lamp wiring for either visor.Visors are mounted to the (metal) roof of the vehicle through the headliner with three screws. EA14-001 determined it was possible for the wiring to be pierced or abraded by one of the screws, or otherwise become electrically shorted either during initial vehicle assembly or subsequent headliner area repairs presenting a fire risk.Most of the fires occurred while the vehicle was being driven.The 14V-391 remedy consisted of installing a plastic guide way on each visor to route wiring away from the attachment screws and prevent electrical shorting.Additionally abrasion resistant tape was installed in key areas to protect the wiring integrity.In order to install the guide, the headliner had to be lowered and the existing visor and headliner wiring rerouted, a somewhat complex procedure that required sufficient service technician care and expertise.FCA modified the 14V-391 remedy multiple times to improve it.ODI has identified a total of 43 NHTSA complaints across affected vehicles indicating a fire occurring after the vehicle had been remedied under 14V-391.FCA reported 159 additional unique complaints of post-remedy fires. The fire events primarily occurred while driving however a few reported fires starting after the vehicle was parked. Ten customers incurred injuries including minor burns and smoke inhalation, no crashes were reported.According to FCA, the vehicles recalled under 15V-879 were manufactured with longer wiring connecting the visors to the main harness in the headliner.Field experience clearly shows these vehicles are experiencing more post 14V-391 fires, 189 out of 202 known incidents to date.Recall 15V-879 addresses any remaining risk of fire after the 14V-391 recall remedy is performed.The 15V-879 remedy installs new design level visor assemblies containing added wire sheathing, shortened wiring, revised wiring retainer and wiring loop relief.In addition, part of the metal structure (body in white) where the wiring is routed will be modified (removed) using a template to allow more clearance.The recall action initiated by the vehicle manufacturer address the safety risks identified by the investigation.Accordingly, the investigation is closed.The ODI reports cited above can be reviewed at SaferCar.gov under the following identification (ODI) numbers:10640524, 10653417, 10684130, 10691520, 10692710, 10703058, 10705802, 10711836, 10715282, 10717265, 10717266, 10723677, 10726438, 10733003, 10733598, 10735042, 10743410, 10744097, 10745129, 1
In a letter dated August 21, 2014, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a defect investigation of alleged failures associated with the Totally Integrated Power Module (TIPM) installed in Chrysler SUV?s, trucks, and vans beginning in the 2007 model year. The petitioner alleges that TIPM defects may result in engine stall, airbag non-deployment, failure of fuel pump shutoff resulting in unintended acceleration, fire, and other symptoms. On September 25, 2014, the Office of Defects Investigation (ODI) opened DP14-004 to evaluate the petition for a grant or deny decision. ODI analyzed complaint data provided by CAS as well as complaints submitted to ODI from consumers. In total, there were 296 complaints submitted by the petitioner in the original petition and five supplements, including 271 complaints related to the subject vehicles equipped with TIPM-7. Approximately 3 percent of CAS complaints are related to vehicles equipped with TIPM-6 and ODI?s review of these complaints did not identify any safety defect trends. ODI conducted a detailed review of complaints narratives submitted by CAS and consumers including careful analysis of vehicle repair histories, warranty claims obtained from the manufacturer and any available Customer Assistance Inquiry reports (CAIR). Analysis of the field data indicated that MY2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles exhibited significantly higher complaint rates related to fuel pump relay (FPR) failures than other subject vehicles. In a September 3, 2014 letter to NHTSA, Chrysler submitted a Defect Information Report (DIR) identifying a defect in the FPR within the TIPM-7 which can result in a no start or stall condition in approximately 188,723 model year (MY) 2011 Jeep Grand Cherokee (WK) and Dodge Durango (WD) vehicles manufactured from January 5, 2010 through July 20, 2011 (14V-530).In a February 24, 2015 letter, Chrysler submitted a second DIR expanding the scope of the FPR defect condition to include an additional 338,216 MY 2012 through 2013 Jeep Grand Cherokee vehicles manufactured from September 17, 2010 through August 19, 2013 and MY 2012 through 2013 Dodge Durango vehicles manufactured from January 18, 2011 through August 19, 2013 (15V-115).ODI analysis of the CAS allegations of TIPM defects resulting in stall while driving, airbag non-deployment, unintended acceleration, fire and other faults identified a single defect condition related to 1 of over 60 different circuits in the TIPM assembly. The most common effect of this defect condition, related to the fuel pump relay, was a no-start concern, but it could also result in stall while driving (76 complaints). No valid evidence was presented in support of claims related to airbag non-deployment, unintended acceleration or fire resulting from TIPM faults and these claims were found to be wholly without merit based on review of the field data and design of the relevant systems and components.Except insofar as the petitioner's contentions relate to the defect condition addressed by the Chrysler recalls, the factual bases of the petitioner's contentions that any further investigation is necessary are unsupported. In our view, additional investigation is unlikely to result in a finding that a defect related to motor vehicle safety exists or a NHTSA order for the notification and remedy of a safety-related defect as alleged by the petitioner at the conclusion of the requested investigation. On July 1st, 2014 Chrysler Group LLC (Chrysler) notified the Office of Defects Investigation (ODI) that it would conduct a safety recall to remedy a wiring-related fire hazard on the headliner of approximately 661,888 model year (MY) 2011-2014 Jeep Grand Cherokee and Dodge Durango vehicles manufactured between January 5, 2010 and December 11, 2013 (see NHTSA Safety Recall 14V-391), with notifications to begin in August 2014. According to Chrysler, the fire is caused by an electrical short in the vanity lamp wiring for either one of the sun visors mounted on the vehicle. The sun visors are mounted to the roof of the vehicle through the headliner with three metal screws. It is possible for the sun visor wiring to be pierced by one of these screws either during initial vehicle assembly or later headliner area repairs which may cause an electrical short potentially resulting in fire.ODI opened an investigation involving MY 2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles based on consumer reports of headliners catching fire at the front of the passenger's compartment near the sun visors. Complainants reported failure symptoms that ranged from discoloration to open flame burning of the headliner and/or sun visor material. Complainants reported a burning plastic smell and smoke from the headliner, often occurring while the vehicle was in motion. Damage ranged from a small burn spot on the headliner to a near complete burn of the vehicle resulting in a total loss; sunroof damage/glass shattering occurred in some cases. Incidents are more likely to occur when the ignition key is in the on position.However, several vehicles continued to burn after the key was turned off, and key-off fires, while highly unlikely, cannot be fully eliminated as a potential problem. The earliest incidents chronologically were often associated with vehicle assembly or in-plant repair procedures. Chrysler revised assembly and in-plant repair procedures for the headliner which reduced the number of fields incidents.However, anytime a repair is made to the headliner of the vehicle there could still be a risk of shorting the wiring and creating a hazard.Dealership technicians will inspect the vehicles for damaged wiring caused by an improper service procedure and repair any damage. Also, all vehicles will have a plastic guideway installed on each sun visor that routes the wiring away from the attachment screws preventing the wiring from being shorted. All MY 2011-2014 Grand Cherokee and Dodge Durango built before December 11, 2013 are affected by the recall; vehicles manufactured after this date had the guideway installed during vehicle production.ODI received a total of 6 complaints as of June 2014, all of which reported a fire and two of which alleged related injuries. All 6 incidents occurred while the vehicle was being driven. In each case the driver smelled smoke and witnessed the headliner burning on either the driver or passenger side. Two complaints indicate smoke inhalation injuries to the driver. In response to ODI's information request letters of 8/28/2013 and 2/26/2014, Chrysler provided consumer and warranty data which was received by ODI on 10/7/2013 and 3/19/2014. Analysis of the Chrysler data indicated a total of 91 unique incidents related to the investigation of which about two thirds reported an open flame and smoke, the remaining showed only small burn marks with no visible flame. The Office of Defects Investigation (ODI) has identified 5 reports of vehicle headliners catching fire at the front of the vehicle near the sun visors on Jeep Grand Cherokees.Customers reported a range of fire conditions ranging from minor overheating to an open flame at the headliner and/or sun visor material while driving the vehicle. In some reports the fire spread to the front seats and/or door panels of the vehicle.In one report, the sunroof was damaged causing the glass to shatter.The cause of these fires is an electrical short in the vanity lighting wiring circuit that is routed to either one of the sun visors.The sun visors are mounted to the roof of the vehicle through the headliner with three screws.The sun visor wiring may be penetrated, or pierced by one of these screws either during initial vehicle assembly or subsequent headliner area repairs.The piercing causes an electrical short that could result in a fire.There is no dedicated fuse for the affected circuit so the electrical short can continue until the short clears or the vehicle is keyed off.The Dodge Durango uses the same headliner assembly, and Chrysler indicates there are similar headliner fire incidents affecting this model.
