I was caught driving in a hailstorm when hail impacted and broke the panoramic sunroof of my vehicle. I understand that hail can break automotive glass and that the sunroof is constructed from safety glass. My concern is not that the glass broke, but how it shattered. When the sunroof failed, it fragmented into extremely fine particles and powdered glass. The glass was so fine that it became embedded in the vehicle's seats and interior surfaces. It was in my hair and clothing. Fortunately, the interior cloth sunshade was partially closed at the time and contained much of the debris. Had the shade been open, the glass particles would have fallen directly into the cabin while I was driving. My concern is that the shattered glass created a large volume of fine particulate debris that could have been dispersed throughout the cabin and potentially into occupants' eyes, skin, and airways. This is particularly concerning when considering the possibility of children or other passengers seated beneath the sunroof at the time of failure. Given how fine the resulting particles were, I question whether additional protective measures, such as a laminated layer or other containment system, should be required to prevent shattered glass from entering the passenger compartment when a panoramic sunroof fails. I believe this issue warrants review because the manner in which the glass disintegrated may present a safety risk beyond the initial breakage itself.

The engine power cuts of randomly while driving with no warning at all. When it happens the steering locks up veering the vehicle to the left so you have no control and at the same time the power brakes lock up and you are unable to stop the vehicle. Everything else works when this happens lights, radio, heater, air conditioning. No warning lights display. I had the vehicle with a mechanic that kept it for over 4 months to test drive and see if it would occur. It happened while he drove it but no codes show up to determine the cause. I had it towed back to my home and contacted Jeep Chrysler and they gave me a claim number to bring to my local Jeep dealer. They had it for over a week and unfortunately the jeep ran fine so once again no resolution to be able to fix. I bought the Jeep from Carvana 9/7/24 and am still paying it off but yet haven't been able to drive it for the past 15 months because it is unsafe for myself and anyone else on the roadway.

Upon starting my Jeep on the coldest day of the year back in January when I pressed the brake pedal to start it there was a loud and it reminded me, there was a recall out for the break booster pop and the pedal went all the way to the floor. It was fifteen below zero out.So I thought that it blew a brake line. I checked the lines going to all four.Wheels and could not find any fluid. I could not figure out where all the fluid went. I refilled the reservoir with two full cans of brake fluid. Tried pumping the pedal and again it went to the floor. Got out and inspected all lines and the reservoir itself.... Still could not find a leak. I googled the problem and saw a recall for the brake booster. Said it was installed incorrectly and water could enter the booster and master cylinder and freeze if cold outside and cause a serious accident causing serious injury or death. I inspected the system and water had entered the booster and master cylinder and froze. It ruined the master cylinder. I asked the dealership if they would pick up the vehicle and repair it. First they said yes then no they don't pick vehicles up. They ordered only a little shield to fix the booster per the recall but said that's all they would do. Why would I pay 200 out of my pocket to have the vehicle taken to them.Only to have them put a shield on the booster. I still would have no brakes. That is ridiculous. This is supposed to be a major safety issue.And it is! If I had been driving when I pressed on the brake pedal and it went to the floor I could have been killed or other innocent people could have been killed. You the NHTSA 's answer said Chrysler is contractually. Obligated to not only pick up my vehicle but to repair any and all components of the brake system to return the vehicle to previous safety operation at no charge to me. You also said that I should be given a loaner vehicle to use while mine is repaired. Dealership is refusing. I could be dead and so could other innocent people

The vehicle is a 2012 Jeep Grand Cherokee with open safety recalls T59 and V62. The brake warning light is currently illuminated, and based on the symptoms I believe the brake booster may be failing. The braking system is available for inspection upon request. The safety concern is that the vehicle may have reduced braking assist or impaired braking performance while being driven. This creates a risk of increased stopping distance, loss of braking confidence, or inability to stop safely in traffic. I contacted the FCA/Stellantis-recommended Jeep dealership in Lapeer, Michigan multiple times this week to schedule recall-related service. I was repeatedly transferred to service voicemail and did not receive return calls. When I called back and asked to speak with someone else, I reached the service manager. I explained that the vehicle has an illuminated brake warning light and that I suspect the brake booster may be failing. The service manager focused on where I lived, why I was not using a closer dealer, and mentioned that the vehicle is 14 years old, rather than clearly arranging recall service or addressing the brake safety concern. The problem has not yet been inspected or confirmed by a dealer because I have been unable to obtain timely service despite repeated attempts. The vehicle has not been inspected by the manufacturer, police, insurance representatives, or others. The warning lamp currently present is the brake warning light. The exact first date the warning light appeared was within the last 2 weeks.

Vehicle has two open unrepaired recalls: T59 (NHTSA 17V-572, brake booster water shield) and V62 (NHTSA 19V-813, fuel pump relay). The brake booster is currently symptomatic — an audible humming noise is present when the vehicle is stopped and idling, consistent with vacuum loss in the brake booster system. I contacted LaFontaine Chrysler Dodge Jeep Ram of Fenton, Michigan to schedule the recall repairs. An appointment was set for 8:15 AM on April 24, 2026. I requested a loaner or rental vehicle as the dealer indicated they would need to keep the vehicle over the weekend for the work. The dealer never followed up regarding the rental. Instead, the dealer contacted me to appraise my current vehicle and discuss new vehicle inventory. The recall work was never discussed. When I did not appear for the appointment, the dealer did not follow up or offer to reschedule. I then contacted Stellantis customer service directly and explained the situation, including the active brake booster symptom. The Stellantis representative connected me with a second dealership, Bill Snethkamp Chrysler Dodge Jeep Ram on Woodward Avenue in Detroit, Michigan, then disconnected from the call without notice. I was left explaining the situation a second time to a dealership that informed me they do not provide loaner vehicles. The rental option requires me to pay out of pocket upfront and then seek reimbursement from the manufacturer in the form of a check. This is not a viable option. Additionally, this dealership is located approximately one hour from my residence in Flint, making it logistically impractical for recall service. I am currently driving a vehicle with an active safety defect affecting the braking system. The recall remedy language states that limited braking ability can cause a crash without prior warning. Neither the manufacturer nor two dealerships have provided adequate accommodation to complete the recall repair while ensuring I have safe transportation during the service per

The dashboard has warped and lifted significantly from the underlying structure, particularly near the defroster vents and windshield area. The deformation appears to be caused by failure of the internal adhesive or mounting system used to secure the dashboard. The vehicle has only approximately 55,000 miles, and the issue does not appear to be due to normal wear or misuse. The vehicle and dashboard are available for inspection upon request. The warped dashboard creates potential safety concerns related to the passenger-side airbag deployment area, as the deformation occurs near the airbag seam and mounting points. A compromised dashboard structure could potentially interfere with proper airbag deployment in the event of a crash. In addition, the raised and distorted panel can cause glare or visual distraction while driving, which could affect driver visibility and concentration.

The contact owns a 2012 Jeep Grand Cherokee. The contact stated that while driving approximately 5 MPH, the brake pedal was depressed while slowing down; however, the RPM elevated to 5,500 RPM. The contact stated that excessive force was needed to depress the brake pedal. The vehicle was stopped, turned off, restarted, and operated normally. Additionally, the remote start feature was not operating properly. After remote starting the vehicle, the brake pedal was difficult to depress. The contact also stated that a phone call was ringing through the radio even though the phone was not in use. The contact researched online and believed that the vehicle had been hacked. The local dealer was contacted, and the contact was informed that the vehicle could not be hacked. The vehicle was not repaired. The manufacturer was not informed of the failure. I brought my vehicle to the Jeep of concord for a fuel pump relay recall and oil change on 11/12/25. Prior to 11/12 the vehicle did not experience any of the issues described below. On 11/13/25, a dealership representative sent an inspection video stating the vehicle was in immaculate condition following the recall repair. However, within less than 24 hours, the check engine light illuminated, and additional issues began occurring intermittently. After the recall work, the vehicle began experiencing intermittent warning lights, including service four wheel drive, traction control, and check engine. These warnings appear unpredictably and are sometimes accompanied by loss of vehicle functions, creating unsafe driving conditions. The vehicle has also experienced overheating since these issues began. Sometimes turning the vehicle off and then on would temporarily correct the problem if it would recrank. The failures occur intermittently, making them difficult to reproduce or diagnose, and they pose serious safety risks, including the potential for sudden loss of power, steering assistance, or drivability while the vehicle is in motion. On multiple occasions, the issues have left me stranded. I returned the vehicle to the dealership on 1/8/26. On 1/9/26, I received an inspection video from the dealership identifying a transmission issue for the first time. The rep stated they “double-checked” the recall wiring but confirmed that no manufacturer engineer inspected the vehicle, despite my request to escalate this as a STAR case due to safety concerns. The intermittent warning lights, overheating, and transmission concerns remain unresolved and Stellantis refused to escalate the issue. The dealership currently retains possession of my vehicle. I am concerned the recall repair may have been performed wrong or inadequately inspected, resulting in ongoing intermittent safety-related failures affecting multiple vehicle systems. The contact owns a 2020 Ford Escape. The vehicle was repaired under NHTSA Campaign Number: 19V813000 (ELECTRICAL SYSTEM), but the failure worsened. The contact stated that after the vehicle was retrieved from the dealer, the check engine warning light illuminated. The contact stated that on a separate occasion, a week after the vehicle was repaired, the traction control warning light, the check engine warning light, and the yellow light with an arrow across were illuminated. The message "Service 4-WD" was displayed on the instrument panel. In addition, the contact stated that while driving at an undisclosed speed, the vehicle failed to accelerate as intended while depressing the accelerator pedal, and the vehicle stalled. The contact was able to coast to the side of the road and failed to restart. The contact left the vehicle on the side of the road. The following day, the contact sent someone to pick up the vehicle, and the vehicle restarted. The vehicle was taken to the residence. The contact called the dealer and was informed that a diagnostic cost would be charged, and the contact refused. The vehicle was not repaired. The contact stated that on a separate occasion, while driving at an undisclosed speed, the vehicle stalled. The contact was able to coast to the side of the side of the road. The vehicle was restarted. The vehicle was taken to the dealer. The contact was informed that the transmission had failed. The vehicle was not repaired. The manufacturer was made aware of the issue. The contact requested a field inspector to inspect the vehicle. The manufacturer stated that the dealer stated that the failure was not related to the recall, and no assistance was offered. The failure mileage was approximately 190,000.

My Airbags did not deploy and I believe my seatbelt also did not work properly. I was struck head on (front end to front end) inside a tunnel by a pickup truck (ram 3500) Going appx. 40mph and my air bags did not deploy and my head hit my windshield so I’m unsure if seatbelts worked properly as I was using mine. My insurance company is storing it now for me as it is most likely totaled. My insurance is handling my property damage case, as far as I know it has not been to any dealer/mechanic and again is most likely beyond repair unfortunately. Colorado state patrol inspected both vehicles involved, I know my insurance is holding the vehicle now, I’m not sure about the other driver as I am struggling to find help with litigation. I had no warning from my car nor otherr driver, not sure if this is because I was entering the single lane tunnel. he did not use horn flash light anything like that, I laid on my horn and cut my wheel right but being in a tunnel with a wall of rock/mountain, there’s was no avoiding him.

Two similar incidents to report during week of Dec 15th 2025 on surface streets: Was driving 25 mph when 2012 Jeep Grand Cherokee just came to a complete stop -- without any warning signs of a problem. Vehicle stalled with no engine power and loss of power steering. I could not steer the vehicle to the side of the road. Was just stuck. The emergency lights did work. I put the vehicle in park and restarted the engine after about 30 seconds. The vehicle drove normally therafter. THIS COULD HAVE BEEN CATASTROPHIC IS IF I HAD BEEN ON A MAJOR HIGHWAY WITH TRAFFIC. This same stalling of the vehicle with no power or steering ability has happened 2 other times before earlier in 2025. The Dealer insists there is no recall, but this looks like the same problem that this Jeep had in 2019 when the dealer replace the fuel pump relay as part of a previous recall. Obviously that recall was not effective.

Year: 2012 Make/Model: Jeep Grand Cherokee 5.7 L V8, 4WD Mileage: ~140,000 miles Date of Incident: [Insert date] Location: McKinney, Texas (near U.S. 75 exit) Description of the Safety Issue: While driving normally, the Service 4WD System warning illuminated. Seconds later, the lightning-bolt symbol (Electronic Throttle Control) appeared, and the engine stalled abruptly without warning. All power steering and power-brake assist were lost while the transmission was still in Drive. The vehicle rolled backward on a slight grade as if in neutral. Attempts to restart the vehicle failed. Even CarMax technicians, at whose lot the vehicle came to rest, were unable to start it. A strong hot/burning electrical odor was present immediately after shutdown. This indicates a possible overheating or shorting event in the electrical or fuel-relay system (TIPM or throttle-body circuit). Had this failure occurred on U.S. 75 at highway speed (80–85 mph), I would have lost steering assist and braking ability in heavy traffic — a situation that could have led to a severe collision or injury. I respectfully request that NHTSA review this case as a potential recurrence or expansion of the existing recall group. The combination of a sudden stall, complete loss of control systems, no-restart condition, and electrical-burn odor constitutes a serious and ongoing safety defect that endangers vehicle occupants and others on the roadway. Supporting Information: – CarMax technicians confirmed the vehicle would not restart. – Recent maintenance included new fuel pump, injectors, hoses, and cooling-system components (~$10 K). – Warning lights: “Service 4WD System” and “Electronic Throttle Control (lightning-bolt)” appeared immediately before stall.

In August 2025 my 2012 Jeep Grand Cherokee Laredo lost engine power and stalled while driving 15 mph. There was no warning. I was able to restart the car and drive. Then while driving in traffic at 45 mph on 18 Oct 2025, my Jeep abruptly lost engine power and stopped, putting my safety and the safety of my passenger at great risk as I had several cars behind me. Again, the Jeep stalled without warning. I was able to put my emergency warning lights on, then after 30 seconds, I put the car in park and restarted the engine with no problem. The Jeep drove normally. This unexpected loss of power while driving at speed in traffic on Hwy 431 could have been catastrophic. This incident appears to be linked to two previous Fuel Pump Relay recalls, the most recent being V62/NHTSA 19V-813. My Jeep was repaired by the Jeep dealer under both of these recalls which were prompted by no-start conditions and ENGINE STALL CONDITIONS. It appears the FCA repair has been ineffective as my Jeep has experienced two engine stalls while driving and does have intermittant problems with start up. This most recent problem has not yet been confirmed by a dealer as the Jeep Dealer cannot see my vehicle for another 2 weeks, which puts my safety at risk. There were no warning lamps or messages before the abrupt engine stalls while driving. However, for the past 6 months, I had had intermittant instances of the car not starting immediately, particularly in the morning. I would turn the key fob a few times before my Jeep would start. But it always started.

My car apparently overheated which made my oil filter melt and explode, popping my oil filter cap off making oil go all over my engine, ground and everywhere else. I was going about 20 mph when I realized my car was feeling weak on power and it was about to overheat. I then started to smell and see smoke. So I stopped my car in the middle of the street immediately, put on my hazard lights, and turned it off instantly. As soon as I got out, oil started to pour out everywhere. I then popped the hood to see a whole lot of smoke going everywhere. I wasn’t sure what happened, I just got the car 3 and half months ago, they said the car was top of the line, why am I having issues like these now? I did get my oil changed from pit stop (superlube) on lake street in Aurora Illinois, 3 days before that happened. I had to manually push my car to chihuahua tires, where they told me it was my starter, so they fixed my starter, and then told me it’s my gasket. Now apparently my head gasket is gone, and it’s going to take thousands of dollars that I don’t have. I’m already paying off the $800 for this starter repair that I didn’t really need :/ and I’m still playing for a car note on a car I’m not even driving! I can’t get to work, I can’t make money, this is depressing! I got this car because they said at Carfect that my jeep was perfect and in great condition. At this point I had over 10+ mechanical issues wrong with it, now this happened and I cant manage this. This is too much money for me. I need help, direction, a new engine, something! I just got this car! I did nothing wrong, I don’t have thousands of dollars to fix this issue! Please help.

I have 2 issues with this vehicle issue#1 the tire pressure sensor acts up on the front driver side tire pressure goes down to the low 20s then when driving it goes up to 43 and then back down to the 20s , the other is that I was driving from Pennsylvania to New York and the car stalled in New Jersey with no warning I was at the far left side of the highway and came close to getting rear ended twice real bad (my girlfriend who was and is pregnant) was on the passenger seat was hysterically crying because of how close we came to having an accident that would’ve cost us our lives , the car took 5 minutes to start again and we were escorted to safety by tow company that saw us through the cameras they have on the bridges very thankful to them escorting us cause we surely would have died that night

2012 Jeep Grand Cherokee HEMI. As traveling whether on highway or on streets no matter the speeds the Jeep tends to just stall out out of no where especially when braking and coming to a stop it completely shuts off, no engine, no steering and then must have to engage into park or neutral to push to start up the Jeep again. It is very dangerous and scary any time it happens and even more so when happens on highway. It has been an on going issue since I bought the vehicle in NOV. 2018 but only happens every once in a while and out of the blue when it does. I did reached out several years ago to the manufacturer Mopar Jeep Chrysler but they had no solution for me. I had a case number with them but it was so long ago I cannot remember now. I did have the recall on the fuel pump relay in which I thought that would resolve my issue but unfortunately it did not and I still experience this Jeep stalling out at any point while driving, onto braking, and then to the stop. Any help? Thanks

In 2 attempts to resolve the issue, the first time I went in person, (6/10) he said “I will call back Wednesday when Deonte Sutton gets here”. Fine, I waited all day Wednesday, and there was no callback. In attempt to call them, I received no answer. I called today, (6/23), in attempt for updates on the situation. It felt as if he blew me off, saying “Yea me and Deonte have been trying to figure it out. One of the recalls for this vehicle is for "2011-2013 wk wd fuel pump relay". The car stalled on the road but I was able to bring it home and apply a temporary fix by connecting the Pump fuse with the Cigarette lighter fuse (per some online instructions). The car runs now, but it's not a permanent fix. I called a Dealer (Everett, WA) and scheduled an appointment (it was two weeks ago). Today during my appointment I was sent back home saying they don't have the part and I'll need to come back in a couple of weeks, just for the inspection! Complete incompetence. I called another place (Kirkland, WA) and was also told that they are ordering the part now but will need to wait from few to several weeks. If this recall makes my car unsafe to drive (stall happened on the road, my solution is not perfect) why are the dealerships/Jeep not taking the repair seriously?

As I was driving my Jeep Grand Cherokee, I was in the process of braking in order to turn, when the power steering stopped working as the engine stalled. I was able to pull over to the side of the road, put it back in Park and was able to restart it like nothing had happened. According to other owners with a similar issue, all the symptoms describe an open Safety Recall for a failing Fuel Pump that may cause the engine to stall. However, when I search the VIN on the NHTSA website, it states that there are no open recalls. The symptoms are identical to what is outlined in the recall. The only warning that came on was "Check the Gas Cap" warning.

So I just bought this car from a dealer about a year now and I was driving down the fwy. Heading to work And suddenly my car turned off and my steering wheel got really hard to make any turns there was no way I could’ve stopped in the middle of the fwy. To stop and start the engine. luckily I was able to pull over and check the car I was full on gas and I looked up common issues that could turn off a running engine checked all fuses didn’t see any problems with it didn’t see any sort of corrosion in the battery cables and their was no check engine on the dash board. Theirs also been times were it randomly takes a long time cranking and won’t start right away so I literally wait for the engine to sit for a while and start the car. My guess it’s the crankshaft position or the fuel pump going out but that’s why I’m here to find out if theirs any recalls related to this issue. I’ve been told their was a recall for a relay issue

Chrysler (FCA US LLC) is recalling certain model year 2011-2013 Dodge Durango and Jeep Grand Cherokee vehicles manufactured December 3, 2009, to September 1, 2012. In the affected vehicles, the wiring for the vanity lamp in the sun visor may short circuit, after having been remedied for a prior recall for the vanity lamp wiring. This recall is also addressing certain vehicles that have not been remedied under that prior recall.

Risk: The vanity lamp wiring may short, increasing the risk of a vehicle fire.

Remedy: Chrysler will notify owners, and dealer will install clearance for sun visor wiring, protection from sharp edges, and replace the sun visor, free of charge. The recall began on July 28, 2016. The supercharger mounting bracket may contact and damage the ABS line, possibly resulting in a brake fluid leak.

Risk: A brake fluid leak may lengthen the distance needed to stop the vehicle, increasing the risk of a crash.

This recall is to verify that the brake booster shield installation was performed properly.

Risk: If the brake booster shield was not installed properly, the vehicle's braking ability may be reduced, increasing the risk of a crash.

Chrysler (FCA US LLC) is recalling certain 2011-2013 Dodge Durango and Jeep Grand Cherokee vehicles equipped with a 3.6, 5.7, or 6.4 liter engine and previously recalled under NHTSA Recall 14V530 or 15V115. The fuel pump relay inside the Totally Integrated Power Module (TIPM-7) may fail, causing the vehicle to stall without warning.

Risk: A vehicle stall increases the risk of a crash.

Remedy: Chrysler (FCA US LLC) will replace the affected fuel pump relay and related wire harness. Interim notices informing owners of the safety risk were mailed beginning December 19, 2019. Vehicles that experience a fuel pump failure will receive an interim repair. Owners will receive a second notice when the final remedy becomes available. Owner notification letters were mailed on December 10, 2020. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is V62.

Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

Active Head Rest Inadvertent Deployment

The Office of Defects Investigation (ODI) opened PE19014 on September 9, 2019, to investigate 128 consumer complaints alleging that the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge Durango vehicles.When ODI opened the investigation, ODI was aware of thirteen alleged injuries.A small number of Early Warning Reports identified driver and/or passenger front seat AHR inadvertent deployments without a rear impact event.Reported safety consequences resulting from the AHR deployments include concussions, headaches, soft tissue injuries, disorientation, nausea, vomiting and/or loss of vehicle control as a result of being surprised and distracted by being struck in the head by the inadvertent AHR deployment. On September 24, 2019, ODI sent an Information Request (IR) letter to Fiat Chrysler Automobiles (FCA).ODI requested certain detailed information from FCA related to the following subject vehicles: MY 2010-2014 Dodge Avenger 200, MY 2010-2014 Dodge Calber, MY 2010-2014 Chrysler Sebring, MY 2013-2015 Ram Tradesman, MY 2012 Ram Van, MY 2010-2019 Dodge Grand Caravan, MY 2010-2016 Chrysler Town and Country, MY 2010-2017 Jeep Compass, MY 2011-2020 Dodge Durango, MY 2011-2020 Jeep Grand Cherokee, MY 2010-2019 Dodge Journey, MY 2010-2012 Jeep Liberty, MY 2010-2011 Jeep Nitro, and MY 2010-2011 Jeep Patriot vehicles.On December 2, 2019, ODI received a response from FCA that included vehicle production data, consumer complaints, field reports, warranty claims, manufacturer actions, production changes, and other data related to the alleged defect from the start of production (2010) through September 24, 2019.On January 27, 2020, ODI received an update to the December 2, 2019, IR response from FCA.On September 22, 2020, ODI requested additional subject vehicle AHR information from FCA.ODI received FCA's response on October 21, 2020.ODI subsequently received an update to FCA's October 21, 2020, response on February 16, 2021.During the investigation, ODI requested updates to certain IR questions on two occasions.FCA provided updates to question #2 on March 5, 2021, and questions #2 and #5 on May 26, 2023. FCA’s IR response data identified approximately 7.5 million subject vehicles (with approximately 15 million AHR assemblies).Due to the large number of vehicles and failure reports, ODI focused its analysis on the FCA vehicle model with the highest failure rate to represent the worst-case scenario.For this purpose, ODI focused on MY 2012-2015 Jeep Grand Cherokee vehicles for its analysis.This population includes 710,890 vehicles that FCA manufactured between August 19, 2011, and August 8, 2015.ODI identified 2,839 alleged AHR failures within this limited population.ODI identified 263 alleged injuries that were caused by AHR failures in MY 2012-2015 Grand Cherokees vehicles.Of these alleged injuries, ODI categorized them as eighty-two (82) soft tissue (neck/whiplash, bruise) injuries, four (4) concussions, fifty-nine (59) headaches, and one hundred eighteen (118) without a specific injury description.Although complainants have alleged injuries, ODI has been unable to obtain evidence to substantiate or validate any injuries where medical treatment was required.Consumer outreach was conducted by ODI, where sixty-three (63) FCA complainants with alleged injuries were contacted.Twelve of the sixty-three FCA consumers responded and were interviewed by ODI.ODI requested medical records, however, only one response was provided. Attorneys representing the responding complainant provided a synopsis that indicated they had experienced prior neck-related injuries dating to the mid-1990s.ODI also contacted one hundred (100) consumers who submitted Vehicle Owner Questionnaires (VOQ) in an attempt to validate alleged injuries. Despite the efforts of ODI, the office has been unsuccessful at obtaining substantive documentation/evidence supporting complaints that allege serious injuries (where pre-existing medical conditions did not already exist). In addition to the low severity of alleged injuries caused by AHR deployments, there have been eleven alleged crashes and two alleged lane departures within the entire subject vehicle population that has been in the field up to sixteen years (with over 8,500 AHR inadvertent deployments).ODI was not able to validate that the alleged crashes were the result of inadvertent head rest deployments, and the crashes were minor in nature. ODI has conducted a thorough investigation of the alleged safety defect and with up to sixteen years of exposure for some vehicles and over 8,500 inadvertent AHR deployments, no serious crashes or injuries could be validated in the sub-population of focus.The lack of serious injuries validated by ODI with such large period of field exposure is consistent with thework done by NHTSA’s Human Injury Research Division and the Biodynamic Research Corporation, which support that an inadvertent AHR deployment is not likely to cause serious injury.In addition to the low risk of injury, FCA released an extended warranty program that covers inadvertent AHR deployment for ten years / unlimited mileage from the vehicle's in-service date. PE19014 is closed with the FCA extended warranty program.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the Agency reserves the right to take additional action if warranted by new circumstances. Post 14V-391 Headliner Fires

In response to this investigation, Fiat Chrysler Automobiles (FCA) is conducting a recall to address headliner fires that occur post 14V-391.On December 22, 2015 FCA notified the Office of Defects Investigation (ODI) of safety recall 15V-879 to remedy potential headliner fires occurring in 389,252 model year (MY) 2011-2013 Dodge Durango and Jeep Grand Cherokee vehicles manufactured from December 3, 2009 to September 1, 2012.Note this recall, which was scoped based on vehicle design level and field events, does not include all MY 2013 or any MY 2014 Durango and Grand Cherokee vehicles; ODI and FCA will continue to monitor the field experience for this population.ODI opened this investigation based on allegations of fires occurring in the headliner area after the remedy for recall 14V-391 had been conducted.Recall 14V-391 involved MY 2011-2014 Jeep Grand Cherokee and Dodge Durango vehicles manufactured by FCA from January 5, 2010 to December 11, 2013.The recall was influenced by ODI investigation EA14-001.During EA14-001, data provided by FCA indicated fires were caused by an electrical short in the vanity lamp wiring for either visor.Visors are mounted to the (metal) roof of the vehicle through the headliner with three screws. EA14-001 determined it was possible for the wiring to be pierced or abraded by one of the screws, or otherwise become electrically shorted either during initial vehicle assembly or subsequent headliner area repairs presenting a fire risk.Most of the fires occurred while the vehicle was being driven.The 14V-391 remedy consisted of installing a plastic guide way on each visor to route wiring away from the attachment screws and prevent electrical shorting.Additionally abrasion resistant tape was installed in key areas to protect the wiring integrity.In order to install the guide, the headliner had to be lowered and the existing visor and headliner wiring rerouted, a somewhat complex procedure that required sufficient service technician care and expertise.FCA modified the 14V-391 remedy multiple times to improve it.ODI has identified a total of 43 NHTSA complaints across affected vehicles indicating a fire occurring after the vehicle had been remedied under 14V-391.FCA reported 159 additional unique complaints of post-remedy fires. The fire events primarily occurred while driving however a few reported fires starting after the vehicle was parked. Ten customers incurred injuries including minor burns and smoke inhalation, no crashes were reported.According to FCA, the vehicles recalled under 15V-879 were manufactured with longer wiring connecting the visors to the main harness in the headliner.Field experience clearly shows these vehicles are experiencing more post 14V-391 fires, 189 out of 202 known incidents to date.Recall 15V-879 addresses any remaining risk of fire after the 14V-391 recall remedy is performed.The 15V-879 remedy installs new design level visor assemblies containing added wire sheathing, shortened wiring, revised wiring retainer and wiring loop relief.In addition, part of the metal structure (body in white) where the wiring is routed will be modified (removed) using a template to allow more clearance.The recall action initiated by the vehicle manufacturer address the safety risks identified by the investigation.Accordingly, the investigation is closed.The ODI reports cited above can be reviewed at SaferCar.gov under the following identification (ODI) numbers:10640524, 10653417, 10684130, 10691520, 10692710, 10703058, 10705802, 10711836, 10715282, 10717265, 10717266, 10723677, 10726438, 10733003, 10733598, 10735042, 10743410, 10744097, 10745129, 1

Totally Integrated Power Module Failure

In a letter dated August 21, 2014, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a defect investigation of alleged failures associated with the Totally Integrated Power Module (TIPM) installed in Chrysler SUV?s, trucks, and vans beginning in the 2007 model year. The petitioner alleges that TIPM defects may result in engine stall, airbag non-deployment, failure of fuel pump shutoff resulting in unintended acceleration, fire, and other symptoms. On September 25, 2014, the Office of Defects Investigation (ODI) opened DP14-004 to evaluate the petition for a grant or deny decision. ODI analyzed complaint data provided by CAS as well as complaints submitted to ODI from consumers. In total, there were 296 complaints submitted by the petitioner in the original petition and five supplements, including 271 complaints related to the subject vehicles equipped with TIPM-7. Approximately 3 percent of CAS complaints are related to vehicles equipped with TIPM-6 and ODI?s review of these complaints did not identify any safety defect trends. ODI conducted a detailed review of complaints narratives submitted by CAS and consumers including careful analysis of vehicle repair histories, warranty claims obtained from the manufacturer and any available Customer Assistance Inquiry reports (CAIR). Analysis of the field data indicated that MY2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles exhibited significantly higher complaint rates related to fuel pump relay (FPR) failures than other subject vehicles. In a September 3, 2014 letter to NHTSA, Chrysler submitted a Defect Information Report (DIR) identifying a defect in the FPR within the TIPM-7 which can result in a no start or stall condition in approximately 188,723 model year (MY) 2011 Jeep Grand Cherokee (WK) and Dodge Durango (WD) vehicles manufactured from January 5, 2010 through July 20, 2011 (14V-530).In a February 24, 2015 letter, Chrysler submitted a second DIR expanding the scope of the FPR defect condition to include an additional 338,216 MY 2012 through 2013 Jeep Grand Cherokee vehicles manufactured from September 17, 2010 through August 19, 2013 and MY 2012 through 2013 Dodge Durango vehicles manufactured from January 18, 2011 through August 19, 2013 (15V-115).ODI analysis of the CAS allegations of TIPM defects resulting in stall while driving, airbag non-deployment, unintended acceleration, fire and other faults identified a single defect condition related to 1 of over 60 different circuits in the TIPM assembly. The most common effect of this defect condition, related to the fuel pump relay, was a no-start concern, but it could also result in stall while driving (76 complaints). No valid evidence was presented in support of claims related to airbag non-deployment, unintended acceleration or fire resulting from TIPM faults and these claims were found to be wholly without merit based on review of the field data and design of the relevant systems and components.Except insofar as the petitioner's contentions relate to the defect condition addressed by the Chrysler recalls, the factual bases of the petitioner's contentions that any further investigation is necessary are unsupported. In our view, additional investigation is unlikely to result in a finding that a defect related to motor vehicle safety exists or a NHTSA order for the notification and remedy of a safety-related defect as alleged by the petitioner at the conclusion of the requested investigation. Headliner Fires

On July 1st, 2014 Chrysler Group LLC (Chrysler) notified the Office of Defects Investigation (ODI) that it would conduct a safety recall to remedy a wiring-related fire hazard on the headliner of approximately 661,888 model year (MY) 2011-2014 Jeep Grand Cherokee and Dodge Durango vehicles manufactured between January 5, 2010 and December 11, 2013 (see NHTSA Safety Recall 14V-391), with notifications to begin in August 2014. According to Chrysler, the fire is caused by an electrical short in the vanity lamp wiring for either one of the sun visors mounted on the vehicle. The sun visors are mounted to the roof of the vehicle through the headliner with three metal screws. It is possible for the sun visor wiring to be pierced by one of these screws either during initial vehicle assembly or later headliner area repairs which may cause an electrical short potentially resulting in fire.ODI opened an investigation involving MY 2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles based on consumer reports of headliners catching fire at the front of the passenger's compartment near the sun visors. Complainants reported failure symptoms that ranged from discoloration to open flame burning of the headliner and/or sun visor material. Complainants reported a burning plastic smell and smoke from the headliner, often occurring while the vehicle was in motion. Damage ranged from a small burn spot on the headliner to a near complete burn of the vehicle resulting in a total loss; sunroof damage/glass shattering occurred in some cases. Incidents are more likely to occur when the ignition key is in the on position.However, several vehicles continued to burn after the key was turned off, and key-off fires, while highly unlikely, cannot be fully eliminated as a potential problem. The earliest incidents chronologically were often associated with vehicle assembly or in-plant repair procedures. Chrysler revised assembly and in-plant repair procedures for the headliner which reduced the number of fields incidents.However, anytime a repair is made to the headliner of the vehicle there could still be a risk of shorting the wiring and creating a hazard.Dealership technicians will inspect the vehicles for damaged wiring caused by an improper service procedure and repair any damage. Also, all vehicles will have a plastic guideway installed on each sun visor that routes the wiring away from the attachment screws preventing the wiring from being shorted. All MY 2011-2014 Grand Cherokee and Dodge Durango built before December 11, 2013 are affected by the recall; vehicles manufactured after this date had the guideway installed during vehicle production.ODI received a total of 6 complaints as of June 2014, all of which reported a fire and two of which alleged related injuries. All 6 incidents occurred while the vehicle was being driven. In each case the driver smelled smoke and witnessed the headliner burning on either the driver or passenger side. Two complaints indicate smoke inhalation injuries to the driver. In response to ODI's information request letters of 8/28/2013 and 2/26/2014, Chrysler provided consumer and warranty data which was received by ODI on 10/7/2013 and 3/19/2014. Analysis of the Chrysler data indicated a total of 91 unique incidents related to the investigation of which about two thirds reported an open flame and smoke, the remaining showed only small burn marks with no visible flame. Headliner Fires

The Office of Defects Investigation (ODI) has identified 5 reports of vehicle headliners catching fire at the front of the vehicle near the sun visors on Jeep Grand Cherokees.Customers reported a range of fire conditions ranging from minor overheating to an open flame at the headliner and/or sun visor material while driving the vehicle. In some reports the fire spread to the front seats and/or door panels of the vehicle.In one report, the sunroof was damaged causing the glass to shatter.The cause of these fires is an electrical short in the vanity lighting wiring circuit that is routed to either one of the sun visors.The sun visors are mounted to the roof of the vehicle through the headliner with three screws.The sun visor wiring may be penetrated, or pierced by one of these screws either during initial vehicle assembly or subsequent headliner area repairs.The piercing causes an electrical short that could result in a fire.There is no dedicated fuse for the affected circuit so the electrical short can continue until the short clears or the vehicle is keyed off.The Dodge Durango uses the same headliner assembly, and Chrysler indicates there are similar headliner fire incidents affecting this model. Power steering hose failure

The Office of Defects Investigation (ODI) analyzed complaint data provided by Chrysler as well as complaints submitted to ODI from consumers. ODI's analysis identified a total of 24 unique reports indicating power steering hose blow-offs resulting from a dislodged power steering cooler turbulator fin in some early production vehicles.According to Chrysler, a dislodged fin can occur when the turbulator is inserted into the power steering cooler or loaded into the assembly machine.A dislodged fin may block the flow of power steering fluid creating excess back-pressure in the power steering system leading to the possibility of a return hose blow-off.Chrysler and its power steering cooler supplier, Dana, modified the cooler assembly process to address factors that may contribute to hose blow-offs. The redesigned turbulator assembly stations were completely implemented in production by April 2012.During PE12-020, Chrysler identified occurrences where the power steering return hose may blow-off at the power steering cooler.This is a condition that typically occurs early in the life of the vehicle with 65% occurring at less than 2,000 miles and 96% occurring with less than 8,000 miles. The data also shows that 88% of the conditions occurred with less than 4 months in service. Over 50% of the blow-off complaints occurred on vehicles with build dates between November 22, 2011 and December 23, 2011. None of the conditions occurred with more than 12,000 miles or 7 months in service. Chrysler?s complaint analysis indicates up-to 14 additional vehicles may experience the hose blow off condition.None of the 24 unique reports with the hose blow-off condition alleged smoke or fire in the engine compartment. In the event of a blow-off, it is highly unlikely for power steering fluid to reach any of the ignition sources in the engine compartment and result in an engine fire. Complaint analysis identified five reports alleging engine compartment fires that resulted from other or unknown causes, including one attributed to an overheated transmission fluid. Two fires of unknown cause cited in the opening resume for PE12-020 involved vehicles equipped with V8 SRT 6.4L engines which are not equipped with the steering system affected by the turbulator fin concern. ODI will continue to monitor field experience in Jeep Grand Cherokee vehicles equipped with that engine.There have been no reports of loss of steering control, crashes or injuries as result of this condition.Additionally, all affected vehicles that may have experienced a hose blow-off condition were repaired under Chrysler?s 3 year/36,000 mile manufacturer?s warranty at no cost to the owner and supplier?s process improvements put in place have eliminated the likelihood of power steering hose blow offs. There is no indication of loss of motive power or unreasonable safety risk associated with the alleged defect in the subject vehicles.
