My 2014 Jeep Grand Cherokee suffered a total intake manifold failure directly resulting from a secondary breakdown of Safety Recall W79 (NHTSA Campaign 20V-699). Physical teardown at an independent facility exposed a 70% engine runner restriction and seized glow plugs due to volatile exhaust sulfur, creating an active fire risk. On July 9, 2026, LaFontaine Fenton was presented with clear mechanical photos and a continuous 2023 dealer repair loop under Customer ID [XXX], but formally refused to open a corporate STAR Case or dispatch flatbed transport to remedy this active safety defect. The vehicle has a severe, life-threatening safety defect where it randomly drops into neutral while driving at speed, physically locks the shifter, and actively engages the electronic braking system on its own, stranding me in active traffic. I brought the vehicle to a certified Jeep dealership to resolve this exact symptom. They misdiagnosed the vehicle and charged me nearly $3,000 to replace the valve body and connectors. The dangerous symptom remains completely unchanged, meaning the vehicle is unsafe to operate and the manufacturer-certified repair facility has failed to remedy a critical safety defect. The issue hasn’t been fixed and it has been 5-6 months.

My passenger seat headrest popped open without any incident or warning and the clip is broken. Air bag deployed from head rest. There was a warranty in place that expired in 2024 but mine just happened while I was driving. It scared me and caused me to swerve. It’s a known issue. Why no recall for safety? My 2014 Jeep Grand Cherokee suddenly developed a no-start condition after previously operating normally. The vehicle would crank but would not start. The battery and fuel injectors were checked and replaced as needed, but the problem persisted. Based on the symptoms and known issues affecting 2011-2014 Jeep Grand Cherokees, the internal fuel pump relay/TIPM is suspected. This defect has been the subject of previous safety recalls because it can cause engine stalling or a no-start condition. Although my vehicle is not currently included in an open recall, it is experiencing the same type of failure. A failure of the fuel pump relay can leave a vehicle stranded or cause the engine to stall unexpectedly, creating a serious safety hazard if it occurs while driving. I believe this issue should be investigated to determine whether additional vehicles should be covered by the recall.

My Son just bought the car from a private party. On our way home the engine light and a lightning bolt icon illuminated on the dash, followed by the vehicle going into limp mode. We had the vehicle scanned for codes and it came back with P0336 (crankshaft position sensor performance). After much investigation online, we found out that this is a known issue with the crankshaft position sensor and, most importantly, the tone wheel on the back of the engine. We looked up recalls on the vehicle and found a safety recall had been issued. While some vehicles affected received replacement of the tone wheel and sensor, others just received a software update. This particular vehicle received the software update. The issue is that the magnetic material on the tone wheel delaminates from the tone wheel. This is a costly repair that should have been fixed correctly from the beginning, not just a software patch. It is a safety concern because it is dangerous when you are traveling at highway speeds and it goes into limp mode.

The contact owns a 2014 Jeep Grand Cherokee. The contact stated that while driving at various speeds, the contact felt that the vehicle was misaligned. The vehicle was taken to a dealer, where the contact was informed that the rear brake shocks needed to be replaced. The vehicle was then taken to an independent mechanic, who informed the contact that no braking system failure was found; however, the rear-end air bag suspension had blown, causing the vehicle to lean on its right side. The contact was then referred back to the dealer, who scheduled a diagnosis appointment. The manufacturer was not notified of the failure. The vehicle was not repaired. The failure mileage was approximately 106,000.

While driving on the highway my Jeep GC shifts into neutral. Once I'm able to pull the car over, I'm not able to put the car into park or shut the car off. After a bit of time the car will start back up and will drive. It has been to the shop and they can not find what is the problem. there are no codes to be read. In reading various forums that is a common problem with the 3.2l engine in the Jeeps. It is dangerous as you lose ability to drive the car on teh highway, when it happens and hopefully you are close to the left lane so you can pull over without being hit.

My 2014 Jeep Grand Cherokee Summit EcoDiesel (VIN: [XXX] ) experienced a catastrophic engine failure at approximately 116,000 miles while driving. The vehicle suddenly displayed an oil pressure warning, and I immediately shut the engine down to avoid further damage and potential safety risks. At the time I shut the engine off, the engine was not making catastrophic knocking noises and was not leaking oil heavily. The vehicle was towed to Astorg Dodge Chrysler Jeep in Parkersburg, WV. Before inspection, I specifically instructed dealership personnel not to start the engine because I suspected a serious oil-related issue and wanted to avoid causing additional damage. Despite these instructions, the dealership started and ran the engine anyway. After the dealership started the vehicle, the engine condition reportedly worsened significantly, including severe engine noises and much heavier oil leakage. I was later informed the vehicle would require complete engine replacement, with estimated repair costs ranging from approximately $10,000 to $24,000. At the time of failure, the vehicle was also subject to the unresolved Z46 High Pressure Fuel Pump (HPFP) recall, which had not yet been completed. I had previously been informed that recall parts were available and the repair would be scheduled. I am concerned that the unresolved recall and/or related fuel system issues may have contributed to this catastrophic engine failure. I am also concerned about the potential safety risk associated with sudden oil pressure loss and catastrophic engine damage while operating the vehicle. I am submitting this complaint so the failure and unresolved recall involvement are documented and reviewed. While driving home from work on March 13, 2026, the alternator in my 2014 Jeep Grand Cherokee (3.0L EcoDiesel) failed, resulting in a progressive loss of electrical power. As battery voltage dropped, the vehicle entered “limp mode.” When I was aproximatly 1 mile from my home when I was forced to stop at a railroad crossing while a freight train was passing. At that time, due to low voltage, the electronic shifter and ignition system became completely unresponsive. I was unable to shift the vehicle into Park or Neutral and was also unable to shut off the engine. The vehicle remained in Drive, requiring constant brake pressure to prevent forward movement. This created an immediate and dangerous situation, as I was stopped very close to a moving train with no functional means of securing or disabling the vehicle. Any loss of braking force or control could have caused the vehicle to enter the path of the train, creating a high likelihood of serious injury or death. After reaching my residence, the condition persisted. I remained unable to shift or shut down the vehicle and was forced to manually disconnect the negative battery cable, which is located under the passenger seat, in order to stop the engine. I am very lucky that I had a small metric socket set in the vehice, that I am somewhat mechanically enclined, and that I have long arms & legs, as this process was extremely difficult. I had to disconect te battery cable while maintaining my foot on the brake to avoid crashing into my garage. The vehicle appears to be designed such that engine shutdown is not permitted unless the vehicle is in Park, while the electronic shifter becomes inoperative during low-voltage conditions. This combination creates a hazardous failure mode in which the driver cannot place the vehicle in Park or turn off the engine during an electrical failure. While driving down the road my active headrest randomly deployed and hit me hard in the back of the head. When I went to look at it to reset (because there had been no accident or collision) I saw that the plastic locking pins had broken. I researched the issue and saw that there was already a MOPAR recall for the same issue on the same year and model vehicle I have, but my VIN is not listed as an affected VIN so Jeep will not honor the recall for my product.

RE: The Power Liftgate Module Malfunction Approximately 45 days ago I began to see water build up inside the well where the liftgate locks. After seeing this twice - after it rained - I was at the dealership for an oil change and asked them to take a look. After review they suspected water intrusion, as there was a recall but not specifically on my VIN. Dealership indicated water tests were needed and I needed to drop my car off for the day or 2. They also indicated to seek your help because of the recall. I couldn't leave my car there that day. On 3/28/26 I did several water tests on the hatchback, using a hose and controlled the flow over the different sections of the hatchback in grid fashion of 6 sections. Each section I sprinkled water like a light rain and then waited 60 mins to see if the water would make it into the lock well. After checking I dried the lock well and then waited another hour to be sure no additional water came into the lock well, before continuing the process over the course of that day and the next. Each time water intruded, entered the inside of the vehicle into the lock well. After conducting research on this issue, the recall stated that this could cause a fire, and not to park the car inside of the garage or near other vehicles. As a result, the hatchback is malfunctioning, it opens and then automatically reverses, and it closes. It will not stay open and is not usable. I'd like to bring it back to the dealership for further testing however, I'd also like to get this resolved in the same visit as I'm sure they will find the same outcome of water intruding causing the electrical issues with the liftgate. Due to it being a fire hazard, I'm afraid to use my vehicle. Can you assist so I don't need to leave my one and only vehicle with the dealership for more than a day or two?

The contact owns a 2014 Jeep Grand Cherokee. The vehicle was repaired under NHTSA Campaign Numbers: 23V411000 (ENGINE AND ENGINE COOLING) and 22V406000 (FUEL SYSTEM, DIESEL) but the failure occurred. The contact stated that two days after the vehicle was repaired, while driving at an undisclosed speed, the check engine warning light illuminated. The contact arrived at the grocery store and after the contact walked back to the vehicle, the contact became aware of an unknown part hanging down from underneath the vehicle. Upon inspection, the contact became aware that the reluctor ring had detached from the vehicle. The vehicle was taken to the dealer where it was confirmed that the reluctor ring had detached. The contact was informed that the part was not available and needed to be ordered. The contact was advised to not drive the vehicle for long periods to avoid the engine overheating. The vehicle was not repaired; however, the dealer wrapped a rag around the connector in case of any leaks. The contact stated that failure persisted. The vehicle remained at the residence and had not been driven since. The contact inspected the vehicle and noticed that the rag the dealer placed in the vehicle was soaked. The manufacturer was not made aware of the failure. The failure mileage was approximately 126,900.

Component/System Failure: The internal fuel pump relay within the Totally Integrated Power Module (TIPM-7) failed. This component is not available for inspection as it is an internal board-mounted relay, but the dealership-installed External Bypass Relay (Part #CBWPR091AA) and the failed fuel pumps are documented on the attached service invoice. Safety Risk: The failure caused an unpredictable engine stall at highway speeds in heavy traffic. This resulted in a complete loss of motive power while moving, creating an immediate risk of a rear-end collision and leaving the vehicle stranded in a live lane of traffic. Confirmation: The technician documented a terminal 'static pressure drop' to 35 PSI (Spec is 58 PSI). Based on this confirmed low-pressure state, the dealer required the replacement of both the Main Fuel Pump and the Siphon/Auxiliary Transfer Pump, alongside the installation of the External Bypass Relay Kit (Part #CBWPR091AA) to restore safe operation." Inspection: The vehicle was inspected and repaired by the authorized manufacturer representative (Larson Chrysler Jeep). I have also opened a formal case with Stellantis Corporate (Case # 96059987). Warning Symptoms: There were no warning lamps, MIL messages, or starting issues prior to the initial stall. The 'Long Crank' and failure-to-start symptoms occurred immediately after the highway-speed stalls. The vehicle would not restart until it had sat for a period of time, indicating a thermal failure (heat soak) of the internal fuel pump relay. This resulted in the vehicle being stranded in live traffic multiple times while waiting for the component to cool and allow a restart "Note: The attached repair invoice from Larson Jeep contains a clerical typo in the VIN (listing a 'D' instead of a 'B'). The side impact airbags deployed on impact. Head injuries were sustained when drivers head hit the sun visor then the windshield. There are pictures of the vehicle after the accident. The vehicle is currently in possession of the insurance company in another state. There was no indication that the airbag was defective prior to the accident.

I am reporting a safety-related issue involving a 2014 Jeep Grand Cherokee Summit 5.7 HEMI. Within days of purchase, the vehicle experienced transmission slipping and delayed shifting, as well as braking issues. These conditions affected drivability and raised safety concerns during normal operation. An independent mechanic found burnt transmission fluid, indicating that the transmission condition likely existed prior to purchase. Although a fluid change provided limited, temporary improvement, delayed shifting continues to occur. As I was on my home on 2/20/26, I encountered some issues. I made a right turn onto the highway and my vehicle did a little drift but made this weird sound and the front end shook. There were no warning lights or dings that came across the dash. I pulled over into the closest parking lot to see if I could notice anything. I couldn't because I only had the light from a parking lot lamp. So decided to try and make it home which was another 15 minutes away. As I got back on the road and pressed the accelerator, I could here ticking sounds coming from the rear of the vehicle. When I approached the next traffic light and pressed the brakes, the front end shook, making the steering wheel shake, and the rear end drifted left. I made another left and pressed the accelerator. I got up to around 40 phone. The vehicle started pulling to the left and then stabilizing. I turned to my suspension articulation screen. As I was driving and the vehicle still scaring me to the point of a heart-a-stroke, I could see the axles trying to maintain contact with the road. At the same time giving me the feel of a loose steering wheel and a bad suspension. The vehicle continued to shift and shake and feel loose. When you turn into my neighborhood and before my house, there are two deep curves and a lake. I went into the first curve fine until the end of it. (At this time the lake is on my passenger side). As I was coming out of it, the steering wheel feels like its hard to turn or locked. I have to turn left into the second curve. The vehicle would not turn and the rear end felt like it was drifting right. At the edge of the road there is a slight downhill and the lake about 50 ft or so. Luckily, I was able to make the turn before going of the side of the road. Once I made it through the curve, the vehicle did do some shaking, loose steering, shaking, and back end motion until I parked in the yard. Its been parked since then. I took my car to the dealer for the recalls. They are basically only putting a band aid on the issue. They are telling me they did both recalls but it didn't fix the issue. Klaben in Kent Ohio. They basically did just a flash on it but that didn't fix the issue (LOSS OF CRANKSHAFT POSITION). Everything I read said the reluctor wheel needs replaced and they are telling me the vehicle is too old and won't fix. This is all part of the recall. If you have any questions please call me. The contact owns a 2014 Jeep Grand Cherokee. The contact stated that while the vehicle was parked, oil was leaking from the vehicle. There was no warning light illuminated. The contact stated that a mobile mechanic diagnosed the vehicle at the residence and determined that the oil housing unit had failed and needed to be replaced. The vehicle was repaired; however, the failure reoccurred. The vehicle was then taken to an independent mechanic, where it was diagnosed that the oil housing unit had failed again and needed to be replaced. The vehicle was repaired. While driving at an undisclosed speed, there was a loud ticking sound coming from the engine. There were no warning lights illuminated. The vehicle was taken to several independent mechanics to be diagnosed, and it was determined that the rocker arm, piston, and camshaft had failed, and the engine needed to be replaced. The vehicle was not repaired. The mechanic referred the contact to the NHTSA Hotline for assistance. The dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 170,000.

Had a catastrophic fuel system failure and was repaired on the Z46 recall the dealer had my vehicle for 2 months and finally got me a loaner after 30 days. Vehicle was finally repaired after 2 months, drove for approximately 2 months and less than 400 miles and it failed again showing a code of low fuel pressure the same as last time had vehicle towed back to same dealership after making appointment and they have had it for 2 weeks and still have not even run a diagnostic check and are refusing to offer me a loaner.

The seat belt does not retract. There is a crack where the seat belts enter the post and compresses the seat belt. I took it to the dealer and they would love to fix it but there are no parts available so they can not do anything. In summary we use the seatbelt only as a decoration piece so we do not get ticket it by non-using a seat belt. The headrest airbag deploying could have caused an accident but lucky for me I was able to pull my vehicle over safely. There is a possibility of going to the doctors if neck pain and headaches continue. I will be taking it to the dealership on Tuesday January 20th, 2026 to find out what caused the airbag to deploy. It has not been inspected yet but will be tomorrow January 20th, 2026. No warning lights came on before or during the airbag deploying.

Mechanic now say only way to make vechicle safe at this point is to put a new engine in.this will cost over 13000 dollars over the 9000.00. I have already spent. Scariest part of this vechicle is when on a 70mph interstate and vechicle goes to a top speed of 40mph.

On November 6, 2025 at 11:59 AM, the active head restraint on my 2014 Jeep Grand Cherokee Limited deployed without impact as I adjusted the driver’s seat into an upright position. This was a spontaneous mechanical deployment inside the cabin of a stationary vehicle. The dealership confirmed this as a known failure involving the internal plastic retaining clip that holds the deployment pin. This defect was previously covered under a 10-year extended warranty that expired in November 2024. At the time of the incident, I was alone in the vehicle. I later sought medical evaluation for head, neck, shoulder, and lower back pain and was diagnosed with a cervical strain. Because this failure occurs without collision, it presents a serious risk of injury to drivers and passengers. I regularly spend time with elderly individuals in personal, non-commercial contexts and occasionally transport others in my personal vehicle. In such a population, an in-cabin mechanical deployment of this force could cause catastrophic injury. The manufacturer and warranty programs have declined to assist with repair due to expiration despite acknowledging this as a known defect. Estimated repair cost is approximately $1,000. The monostable shifter malfunctions constantly. Safety-related concerns include it not staying in park when parked, rolling has occurred; not being able to start due to it not actually being in park; it will switch gear while driving and vehicle will go into limp mode and shut off within seconds, then will not be able to restart. I cannot count on it starting or getting me anywhere safely and have towed multiple times now. The problem has unfortunately not been reproduced or realized by local dealership even with ‘service shifter’, ‘until you reach a desired location you may not be able to shift again NO BUS’, and engine light warning lights appearing when driven. It has been taken twice to John Hiester Chrysler Dodge Jeep Ram of Sanford. (NC). I have spoken with Jeep Customer Care to no avail and the answer is take to a certified dealership but that option is proving to not be successful. I purchased in February 2025 but became disabled right after and haven’t realized it’s very major issues. Not being able to use my vehicle is quite isolating [XXX]. The contact stated that the dealer was unable to repair the headlights and stated that there was a fault in the headlamp assemblies that could not be repaired. The contact stated that the same dealer had replaced both headlamps in 2019 due to a failure to operate as needed. The manufacturer was informed of the failure. The failure mileage was 75,875.

6 months ago, I was driving on the interstate and the ESC system light came on, the vehicle lost power/ability to accelerate, and would only allow acceleration after pulling over and restarting the car. This happened several more times on the interstate, and when the ESC system kicked in it would suddenly lurch the car down about 15 mph and leave no ability to accelerate, causing near collisions at high rates of speed. After eventually taking it to the dealership, it turned out to be failed lifters on the driver's side cam shaft, a known design issue that has apparently plagued the Pentastar engines. This all happened in spite of meticulous maintenance and no warning signs from the engine whatsoever. How is such a significant, known design defect that causes such dangerous driving conditions never been flagged for a recall?

Thankfully no one was in the front seat when it happened. I could only imagine there would have been some pain/injury sustained if they were. There were no warnings. No other malfunctions. The crankshaft position sensor tone wheel may delaminate, causing the engine to lose its ability to synchronize the fuel injector pulses and cam shaft timing, possibly resulting in an engine stall.

Risk: An engine stall can increase the risk of a crash.

Remedy: Dealers will update the powertrain control module software to maintain vehicle propulsion, free of charge. Risk: Combustion in the intake manifold can increase the risk of a fire.

Remedy: FCA US LLC will notify owners, and dealers will replace the EGR cooler, free of charge. Dealers will also inspect and, as necessary, replace the intake manifold. Parts are not currently available. Owners were mailed an interim notification on December 21, 2021. Due to a disruption of computer communications and loose alternator ground wires, the affected vehicles may experience random illumination of multiple instrument cluster warning lights, loss of cluster illumination and loss of anti-lock brake system (ABS) and electronic stability control (ESC) function. Because of these conditions, the vehicles fail to conform to the requirements of Federal Motor Vehicle Safety Standard (FMVSS) No. 101, "Controls and Displays," and No. 126, "Electronic Stability Control Systems."

Risk: A loss of ABS and ESC function reduces the driver's ability to control the vehicle. Drivers would not be warned of brake system failures or any other failures which would be illuminated on the dashboard display. Either condition increases the risk of a crash.

High pressure fuel pump failure

The Office of Defects Investigation (ODI) opened PE21-021 on October 14, 2021, to investigate incidents alleging a stall/loss of motive power as a result of high-pressure fuel pump failures in certain model year (MY) 2019-2020 Ram 2500, 3500, 4500, and 5500 heavy duty trucks equipped with 6.7L Cummins turbodiesel engines. During the investigation, ODI sought to determine if the related defect allegation was limited in scope to the recalled population. After review of information request response materials from both FCA and BMW, NHTSA determined that sufficient information to identify a comprehensive recall population could not be produced by FCA and BMW.During the investigation FCA filed recalls 22V406, 22E048, 22V767, and 22E087 which include vehicles not identified in the initial subject population by ODI. ODI also received recall 21V586, involving loss of motive power due to failed CP4 fuel pumps on certain BMW manufactured vehicles. An information request letter response received from BMW indicated that failed pumps on their vehicles were caused by an interaction between pump internal components and US market diesel fuel, leading to increased slip and eventual particle-generating wear surface. Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

Seat Belt Retraction

The Office of Defects Investigation (ODI) opened this Preliminary Evaluation to investigate complaints alleging improper retraction of the front seat belts (driver or passenger) due to the plastic guide of the D-ring cracking in model year (MY) 2014 Jeep Grand Cherokee vehicles. The purpose of the investigation was to understand the scope, frequency and safety related consequence of the alleged defect. The subject condition occurs when the plastic material encapsulating the metal substructure of the front seat belt, B-pillar mounted D-ring cracks. The structural integrity of the D-ring remains intact; however, cracking of the plastic material pinches the webbing at the D-ring, thereby preventing it from properly retracting, and leaving the belt loose on the occupant. In addition to the detectability of the loose belt, the cracks in the plastic guide are also apparent by visual inspection, and many reports indicate the webbing can be manually fed back into the B-pillar/retractor to improve seat belt fit. ODI review of the available data has not identified any injuries attributable to this failure.In its November 6, 2019 response to ODI's September 8, 2019 information request letter, which was scoped to include MY 2013 to 2015 Grand Cherokee and Dodge Durango vehicles, FCA providedinformation (portions of which were submitted with a request for confidentiality) detailing several material and process changes involved in the production of the subject D-ring. Based on these changes, FCA sub-divided certain production periods into population groups representing different design levels of the D-ring. FCA's response also confirmed the same D-rings were used in Dodge Durango front seat belts. ODI notes that installation of seat belts during vehicle production involves highly controlled processes, in comparison to field service, which ensures all mechanical and electrical connections are properly secured.ODI analyzed complaints submitted directly to the agency and complaint data received from FCA. During this review, ODI identified elevated failure rates for Grand Cherokee and Durango vehicles produced from mid-June 2013 through July 2015 when compared to vehicles produced both prior to and after this period. Additionally, review of FCA warranty data indicated a similar pattern of elevated warranty claims in the suspect July 2013-July 2015 period. The primary factors for the failure rate differences appear to be a D-ring material change (from nylon to a Celcon plastic) starting in mid-June 2013 and a subsequent change (to a Delrin material) in August 2015, however other production changes occurred during the suspect period, as discussed in greater detail in FCA's November 2019 response. ODI's analysis of the various data sources showed vehicles produced in the suspect period experienced failure rates approximately 2 to 4 times higher, depending on the data source and production period evaluated. However the analysis also indicated a declining failure trend.Given the detectability of the condition, the failure frequency combined with a currently declining failure trend, and lack of injuries attributable to this condition despite significant time in service, ODI is closing this Preliminary Evaluation without further action. The closing of the investigation does not constitute a finding by NHTSA that a defect does not exist, and NHTSA will take further action if warranted by future circumstances.In the course of the investigation, FCA has informed the agency of its intention to offer an extended warranty for Grand Cherokee and Durango vehicles built during the suspect period. Active Head Rest Inadvertent Deployment

The Office of Defects Investigation (ODI) opened PE19014 on September 9, 2019, to investigate 128 consumer complaints alleging that the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge Durango vehicles.When ODI opened the investigation, ODI was aware of thirteen alleged injuries.A small number of Early Warning Reports identified driver and/or passenger front seat AHR inadvertent deployments without a rear impact event.Reported safety consequences resulting from the AHR deployments include concussions, headaches, soft tissue injuries, disorientation, nausea, vomiting and/or loss of vehicle control as a result of being surprised and distracted by being struck in the head by the inadvertent AHR deployment. On September 24, 2019, ODI sent an Information Request (IR) letter to Fiat Chrysler Automobiles (FCA).ODI requested certain detailed information from FCA related to the following subject vehicles: MY 2010-2014 Dodge Avenger 200, MY 2010-2014 Dodge Calber, MY 2010-2014 Chrysler Sebring, MY 2013-2015 Ram Tradesman, MY 2012 Ram Van, MY 2010-2019 Dodge Grand Caravan, MY 2010-2016 Chrysler Town and Country, MY 2010-2017 Jeep Compass, MY 2011-2020 Dodge Durango, MY 2011-2020 Jeep Grand Cherokee, MY 2010-2019 Dodge Journey, MY 2010-2012 Jeep Liberty, MY 2010-2011 Jeep Nitro, and MY 2010-2011 Jeep Patriot vehicles.On December 2, 2019, ODI received a response from FCA that included vehicle production data, consumer complaints, field reports, warranty claims, manufacturer actions, production changes, and other data related to the alleged defect from the start of production (2010) through September 24, 2019.On January 27, 2020, ODI received an update to the December 2, 2019, IR response from FCA.On September 22, 2020, ODI requested additional subject vehicle AHR information from FCA.ODI received FCA's response on October 21, 2020.ODI subsequently received an update to FCA's October 21, 2020, response on February 16, 2021.During the investigation, ODI requested updates to certain IR questions on two occasions.FCA provided updates to question #2 on March 5, 2021, and questions #2 and #5 on May 26, 2023. FCA’s IR response data identified approximately 7.5 million subject vehicles (with approximately 15 million AHR assemblies).Due to the large number of vehicles and failure reports, ODI focused its analysis on the FCA vehicle model with the highest failure rate to represent the worst-case scenario.For this purpose, ODI focused on MY 2012-2015 Jeep Grand Cherokee vehicles for its analysis.This population includes 710,890 vehicles that FCA manufactured between August 19, 2011, and August 8, 2015.ODI identified 2,839 alleged AHR failures within this limited population.ODI identified 263 alleged injuries that were caused by AHR failures in MY 2012-2015 Grand Cherokees vehicles.Of these alleged injuries, ODI categorized them as eighty-two (82) soft tissue (neck/whiplash, bruise) injuries, four (4) concussions, fifty-nine (59) headaches, and one hundred eighteen (118) without a specific injury description.Although complainants have alleged injuries, ODI has been unable to obtain evidence to substantiate or validate any injuries where medical treatment was required.Consumer outreach was conducted by ODI, where sixty-three (63) FCA complainants with alleged injuries were contacted.Twelve of the sixty-three FCA consumers responded and were interviewed by ODI.ODI requested medical records, however, only one response was provided. Attorneys representing the responding complainant provided a synopsis that indicated they had experienced prior neck-related injuries dating to the mid-1990s.ODI also contacted one hundred (100) consumers who submitted Vehicle Owner Questionnaires (VOQ) in an attempt to validate alleged injuries. Despite the efforts of ODI, the office has been unsuccessful at obtaining substantive documentation/evidence supporting complaints that allege serious injuries (where pre-existing medical conditions did not already exist). In addition to the low severity of alleged injuries caused by AHR deployments, there have been eleven alleged crashes and two alleged lane departures within the entire subject vehicle population that has been in the field up to sixteen years (with over 8,500 AHR inadvertent deployments).ODI was not able to validate that the alleged crashes were the result of inadvertent head rest deployments, and the crashes were minor in nature. ODI has conducted a thorough investigation of the alleged safety defect and with up to sixteen years of exposure for some vehicles and over 8,500 inadvertent AHR deployments, no serious crashes or injuries could be validated in the sub-population of focus.The lack of serious injuries validated by ODI with such large period of field exposure is consistent with thework done by NHTSA’s Human Injury Research Division and the Biodynamic Research Corporation, which support that an inadvertent AHR deployment is not likely to cause serious injury.In addition to the low risk of injury, FCA released an extended warranty program that covers inadvertent AHR deployment for ten years / unlimited mileage from the vehicle's in-service date. PE19014 is closed with the FCA extended warranty program.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the Agency reserves the right to take additional action if warranted by new circumstances. Vehicle rollaway, engine on

On April 22, 2016, Fiat Chrysler Automotive US LLC, (FCA) submitted a Defect Information Report (DIR) to NHTSA regarding a defect that could result in unattended vehicle rollaway with the engine running in approximately 811,586 model year (MY) 2012 to 2014 Dodge Charger and Chrysler 300 vehicles and MY 2014 to 2015 Jeep Grand Cherokee vehicles equipped with 8-speed transmissions and a monostable gear selector (NHTSA Recall #16V-240, FCA Recall #S27). According to FCA's DIR, The existing strategies built into these vehicles to deter drivers from exiting the vehicle after failing to put the transmission into PARK have not stopped some from doing so. Drivers erroneously concluding that their vehicle's transmission is in the PARK position may be struck by the vehicle and injured if they attempt to get out of the vehicle while the engine is running and the parking brake is not engaged.The vehicles recalled by FCA are equipped with Monostable electronic (shift-by-wire) gearshift assemblies supplied by ZF Group (see Figure 1).The subject Monostable shifter has a single neutral position that it snaps back to when the driver releases the shift knob.Although the Monostable gearshift has the familiar appearance of a conventional console mechanical gearshift assembly, it has an unfamiliar movement that does not provide the tactile or visual feedback that drivers are accustomed to receiving from conventional shifters.Consequently, the driver must take additional time to verify that the desired gear position was achieved by checking the PRNDL display on the shift knob or the Electronic Vehicle Information Center (EVIC) display.

Powered vehicle rollaway

The Office of Defects Investigation (ODI) opened Preliminary Evaluation PE15-030 on August 20, 2015,to investigate 14 complaints alleging incidents of rollaway after intended shifts to Park in Model Year (MY) 2014 and 2015 Jeep Grand Cherokees vehicles. The MY 2014-2015 Grand Cherokee vehicles are equipped with Monostable electronic (E-shift) gearshift assemblies supplied by ZF Group (ZF). The E-shift system operates electronically and the gear requested by the driver is transmitted from the shifter via the CAN Bus to the Transmission Control Module which makes the requested shift. The Monostable gearshift does not move into a detent but springs back to a centered/neutral position after the driver selects a gear and releases the shifter. A button on the shift knob must be depressed to shift out of Park, shift out of Neutral, and to shift from Drive to Reverse or Park.The gear selected is shown on a display in the dash and illuminated letters on the shifter. If the driver's door is opened when the gearshift is not in Park, a chime sounds and a message is displayed on the instrument panel to warn the driver.In addition, the engine Start/Stop push-button control logic does not permit normal engine shut-off when the transmission is not in Park. This logic may provide feedback to drivers who attempt to turn the engine off when the transmission is not in Park. However, this function does not protect drivers who intentionally leave the engine running or drivers who do not recognize that the engine is still running.NHTSA testing during PE15-030 indicates that operation of the Monostable shifter is not intuitive and provides poor tactile and visual feedback to the driver, increasing the potential for unintended gear selection.ODI?s analysis of the PE15-030 complaint and field report data identified 306 incidents of vehicle rollaway followingintended shifts to Park in the 2014-2015 Grand Cherokee. These resulted in 117 alleged crashes. Twenty-eight of the crashes reportedly caused injuries, including 3 with a fractured pelvis and 4 others requiring some degree of hospitalization (a ruptured bladder, fractured kneecap, broken ribs, damaged to right leg). Other injuries include reports of a broken nose, facial lacerations requiring stitches, sprained knees, severe bruising, and trauma to legs.An Engineering Analysis (EA16-002) has been opened to assess the scope, frequency, and safety-related consequences of the alleged defect. Software security vulnerability

On July 23, 2015, Fiat Chrysler Automobiles (FCA) launched Safety Recall 15V-461 to remedy security vulnerabilities in approximately 1.4 million model year (MY) 2013 through 2015 vehicles equipped with Uconnect head units (HU) 8.4A (RA3 radio) and 8.4AN (RA4 radio) manufactured by Harman International.On July 24, 2015, the Office of Defects Investigation (ODI) opened Recall Query, RQ 15-004, to investigate HU security vulnerabilities and remedy effectiveness in the recalled population and to determine whether similar units have been supplied for use in other FCA vehicles.In an August 11, 2015 letter, FCA submitted a second Part 573 safety recall report expanding the scope of the Uconnect RA4 model radio to include additional 7,810 MY 2015 Jeep Renegade vehicles manufactured from September 18, 2014 through June 25, 2015 (Recall 15V-508).Scope analysis indicated that Uconnect radios installed in FCA vehicles not included in recalls 15V-461 or 15V-508 (subject recalls) are not equipped with built-in cellular access or short range wireless communication features and, thus, do not contain the security vulnerabilities addressed by the subject recalls. SUBJECT VEHICLES: MY2014 through 2015 Dodge Durango, Jeep Grand Cherokee and Jeep Cherokee sport utility vehicles; MY2013 through 2015 Ram 1500, 2500, 3500 and 4500/5500 pickup trucks; MY2013 through 2015 Dodge Viper vehicles; and MY2015 Chrysler 200, 300, Jeep Renegade, Dodge Charger and Challenger vehicles.According to FCA, long and short range wireless vulnerabilities identified in the recalled vehicles could allow unauthorized third-party access to, and manipulation of, networked vehicle control systems.Successful exploitation of the vulnerabilities, coupled with reverse engineering of networked microprocessor control modules, could result in unauthorized manipulation of vehicle control systems. This unauthorized manipulation of vehicle controls and systems could expose the driver, vehicle occupants or other highway users to an increased risk of injury.FCA and its network provider, Sprint, conducted a nationwide campaign to block access to a radio communications port that was unintentionally left open.On July 27, 2015, short range wireless vulnerabilities were also blocked.Finally, third party security evaluation and regression testing identified vulnerabilities that were either remedied by Sprint or through updates to the FCA Uconnect software.ODI identified a total of 30 complaints or field reports on unique vehicles submitted by FCA (29) or received by NHTSA (1) alleging incidents of theft from a vehicle or anomalous performance that the owner alleged were caused by, or may have been caused by, remote hacking.Twenty-six (87%) of these reports were submitted after a magazine article was published on July 21, 2015, describing the remote hacking of an FCA vehicle by researchers who were able to affect the operation of various vehicle control systems, including the service brakes, steering, throttle and ignition.Most of the complaints involved vehicle systems that were not safety critical (e.g., complaints related to radio, navigation system, or air-conditioning control) and did not affect vehicle control.Three complaints reported engine stalls.One owner reported sudden unintended acceleration allegedly related to hacking.None of the complaints or field reports reviewed involved the steering and braking vehicle control effects demonstrated by the research hackers prior to the recall.There were no confirmed incidents of hacking in any of the records reviewed by ODI.The remedies completed by Sprint and FCA appear to have eliminated vulnerabilities that mi

False Positive Automated Braking

On June 1, 2015, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE15-021 to investigate 9 complaints alleging incidents of unintended braking in model year (MY) 2014 Jeep Grand Cherokee vehicles, including 2 alleging incidents of full ABS braking.ODI expanded the scope to include MY 2015 Grand Cherokee vehicles and identified a total of 176 incidents in complaints provided by FCA or submitted to ODI by consumers.ODI conducted the following work to assess the alleged defect in the subject vehicles:1) evaluated the design of the subject Forward Collision Warning (FCW) system, including the maximum braking authority during automated braking; 2) analyzed the frequency and severity of field incidents of unintended braking; 3) analyzed warranty data for evidence of system faults that may be related to the alleged defect; and 4) reviewed the performance of the subject system in testing conducted as part of NHTSA?s 2014 Automatic Emergency Braking (AEB) test program.ODI?s analysis found that the maximum brake activations that can be commanded by the subject system are limited in both braking force and duration.The system is not capable of achieving full ABS braking, as alleged in some complaints, unless driver brake application triggers the Advanced Brake Assist (ABA) mode.Analysis of field data found that unintended braking events occur randomly for a variety of causes and are not driven by any single environmental factor, traffic condition, or vehicle fault.The incidents involve short duration moderate braking events with minimal changes in vehicle speed.None of the incidents resulted in crash or injuries.The subject vehicles performed as well or better than peer vehicles in testing conducted as part of NHTSA?s AEB test program.This preliminary evaluation is closed. Post 14V-391 Headliner Fires

In response to this investigation, Fiat Chrysler Automobiles (FCA) is conducting a recall to address headliner fires that occur post 14V-391.On December 22, 2015 FCA notified the Office of Defects Investigation (ODI) of safety recall 15V-879 to remedy potential headliner fires occurring in 389,252 model year (MY) 2011-2013 Dodge Durango and Jeep Grand Cherokee vehicles manufactured from December 3, 2009 to September 1, 2012.Note this recall, which was scoped based on vehicle design level and field events, does not include all MY 2013 or any MY 2014 Durango and Grand Cherokee vehicles; ODI and FCA will continue to monitor the field experience for this population.ODI opened this investigation based on allegations of fires occurring in the headliner area after the remedy for recall 14V-391 had been conducted.Recall 14V-391 involved MY 2011-2014 Jeep Grand Cherokee and Dodge Durango vehicles manufactured by FCA from January 5, 2010 to December 11, 2013.The recall was influenced by ODI investigation EA14-001.During EA14-001, data provided by FCA indicated fires were caused by an electrical short in the vanity lamp wiring for either visor.Visors are mounted to the (metal) roof of the vehicle through the headliner with three screws. EA14-001 determined it was possible for the wiring to be pierced or abraded by one of the screws, or otherwise become electrically shorted either during initial vehicle assembly or subsequent headliner area repairs presenting a fire risk.Most of the fires occurred while the vehicle was being driven.The 14V-391 remedy consisted of installing a plastic guide way on each visor to route wiring away from the attachment screws and prevent electrical shorting.Additionally abrasion resistant tape was installed in key areas to protect the wiring integrity.In order to install the guide, the headliner had to be lowered and the existing visor and headliner wiring rerouted, a somewhat complex procedure that required sufficient service technician care and expertise.FCA modified the 14V-391 remedy multiple times to improve it.ODI has identified a total of 43 NHTSA complaints across affected vehicles indicating a fire occurring after the vehicle had been remedied under 14V-391.FCA reported 159 additional unique complaints of post-remedy fires. The fire events primarily occurred while driving however a few reported fires starting after the vehicle was parked. Ten customers incurred injuries including minor burns and smoke inhalation, no crashes were reported.According to FCA, the vehicles recalled under 15V-879 were manufactured with longer wiring connecting the visors to the main harness in the headliner.Field experience clearly shows these vehicles are experiencing more post 14V-391 fires, 189 out of 202 known incidents to date.Recall 15V-879 addresses any remaining risk of fire after the 14V-391 recall remedy is performed.The 15V-879 remedy installs new design level visor assemblies containing added wire sheathing, shortened wiring, revised wiring retainer and wiring loop relief.In addition, part of the metal structure (body in white) where the wiring is routed will be modified (removed) using a template to allow more clearance.The recall action initiated by the vehicle manufacturer address the safety risks identified by the investigation.Accordingly, the investigation is closed.The ODI reports cited above can be reviewed at SaferCar.gov under the following identification (ODI) numbers:10640524, 10653417, 10684130, 10691520, 10692710, 10703058, 10705802, 10711836, 10715282, 10717265, 10717266, 10723677, 10726438, 10733003, 10733598, 10735042, 10743410, 10744097, 10745129, 1

Headliner Fires

On July 1st, 2014 Chrysler Group LLC (Chrysler) notified the Office of Defects Investigation (ODI) that it would conduct a safety recall to remedy a wiring-related fire hazard on the headliner of approximately 661,888 model year (MY) 2011-2014 Jeep Grand Cherokee and Dodge Durango vehicles manufactured between January 5, 2010 and December 11, 2013 (see NHTSA Safety Recall 14V-391), with notifications to begin in August 2014. According to Chrysler, the fire is caused by an electrical short in the vanity lamp wiring for either one of the sun visors mounted on the vehicle. The sun visors are mounted to the roof of the vehicle through the headliner with three metal screws. It is possible for the sun visor wiring to be pierced by one of these screws either during initial vehicle assembly or later headliner area repairs which may cause an electrical short potentially resulting in fire.ODI opened an investigation involving MY 2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles based on consumer reports of headliners catching fire at the front of the passenger's compartment near the sun visors. Complainants reported failure symptoms that ranged from discoloration to open flame burning of the headliner and/or sun visor material. Complainants reported a burning plastic smell and smoke from the headliner, often occurring while the vehicle was in motion. Damage ranged from a small burn spot on the headliner to a near complete burn of the vehicle resulting in a total loss; sunroof damage/glass shattering occurred in some cases. Incidents are more likely to occur when the ignition key is in the on position.However, several vehicles continued to burn after the key was turned off, and key-off fires, while highly unlikely, cannot be fully eliminated as a potential problem. The earliest incidents chronologically were often associated with vehicle assembly or in-plant repair procedures. Chrysler revised assembly and in-plant repair procedures for the headliner which reduced the number of fields incidents.However, anytime a repair is made to the headliner of the vehicle there could still be a risk of shorting the wiring and creating a hazard.Dealership technicians will inspect the vehicles for damaged wiring caused by an improper service procedure and repair any damage. Also, all vehicles will have a plastic guideway installed on each sun visor that routes the wiring away from the attachment screws preventing the wiring from being shorted. All MY 2011-2014 Grand Cherokee and Dodge Durango built before December 11, 2013 are affected by the recall; vehicles manufactured after this date had the guideway installed during vehicle production.ODI received a total of 6 complaints as of June 2014, all of which reported a fire and two of which alleged related injuries. All 6 incidents occurred while the vehicle was being driven. In each case the driver smelled smoke and witnessed the headliner burning on either the driver or passenger side. Two complaints indicate smoke inhalation injuries to the driver. In response to ODI's information request letters of 8/28/2013 and 2/26/2014, Chrysler provided consumer and warranty data which was received by ODI on 10/7/2013 and 3/19/2014. Analysis of the Chrysler data indicated a total of 91 unique incidents related to the investigation of which about two thirds reported an open flame and smoke, the remaining showed only small burn marks with no visible flame.
