This vehicle had a recall, 201, for the smart junction box. This was due to the blinkers not working properly. The software update was performed, however, the problem still exists. This was due to the smart junction box. Now, aside from the blinker issue, the ac stays on when the engine is turned off, lights come on by themselves, the media screen goes randomly into night mode, daylight running lights are constantly in night mode, smart junction box failure. I have not taken this to a mechanic as of yet, the dealership is aware, and I opened a case with Hyundai case #42940056, however, I have not received a call from my service advisor or Hyundai. They offered to do diagnostics, but, if these problems are not about the safety recall, I would have to pay $350 for diagnostics, unless I get the problem fixed by them. This car has 66k miles with a new engine installed at 59400 miles! At this point, I have the car parked, battery disconnected, and looking for the most efficient way to fix the problem, which must be performed by specialized equipment.

On June 22, my 2015 Hyundai Sonata suddenly lost all electrical power without warning. The vehicle stalled and would not restart. There were no warning signs beforehand. After the incident, the car had no electrical power and was inoperable. Further inspection determined that the engine had failed and required replacement. I maintained the vehicle with regular oil changes and was unaware of Hyundai’s Service Campaign 953 (KSDS update). I was never notified that this campaign needed to be completed, and I believe that failure to receive notice contributed to Hyundai’s denial of my engine replacement claim. I purchased the vehicle December 19, 2019. I was never notified by the dealer or manufacturer

I had the Service Campaign 9C2 and TSB 25-01-087H recall emissions update performed. I immediately noticed degraded performance with my vehicle. It's very jumpy/stuttery after I first start driving and feels less responsive. Even after driving for a bit, it feels like it drives and shifts oddly. It is not as smooth as it used to be. My car drove fine prior to the update and immediately felt different after the update. It's now been over a week and it's still like this, so it's not the engine computer adapting or something like that.

The incident began following a routine maintenance visit on March 16, 2026, and has been a continuous, ongoing safety hazard every single day since. While the vehicle was in for routine brake service and an oil change, the dealership automatically applied an open, manufacturer-mandated ECU S/W Update THETA II (26-01-013H-1) under Service Campaign 9C2 Emissions (Code NP001-SC9C2) without my prior knowledge of its drivability side effects. This software flash directly compromised the vehicle's powertrain logic, creating two notable, daily safety failures. First, within the first 5 minutes of engine startup, and most pronounced on every cold engine startup, the throttle delivery is highly erratic, causing uncommanded low-speed surging. In a specific incident in a parking lot following a cold start, I accelerated to approximately 15 MPH and completely removed my foot from the accelerator pedal to coast. Instead of slowing down, the vehicle unexpectedly lunged and pulled ahead forcefully against my intent. Second, even after reaching full operating temperature, the vehicle completely loses safe and predictable engine braking at greater speeds; when lifting off the gas pedal, engine RPMs aggressively drop, causing the vehicle to coast freely as if in neutral. Re-applying the accelerator causes a violent "phantom shifting" downshift and a hard transmission lockup. My safety and the safety of others are put at risk every time the vehicle is driven. The low-speed throttle surging creates an ongoing risk of hitting pedestrians or cross-traffic, while the permanent loss of predictable engine braking drastically extends stopping distances. No warning lamps or diagnostic trouble codes (DTCs) appeared prior to or during these continuous failures. The vehicle remains available for inspection.

On October 2024,I brought the car in to RickCase Hyundai for maintenance and to address an oil leaking issue. I texted the service advisor, “Please do not do the oil change till you find out where the oil is leaking”, I’m replacing it frequentlyWhen I picked up the car and inquired about the leaking, the he told me that there was not an issue. January 2025, the car was driving rough again, noticed the oil low and filled it up. February 2025 driving on 595, The car stopped. I had it towed to Rick Case, they told me I needed to replace the piston. I said no this has to do with the oil consumption issue that I brought the car in October that you never addressed. The four new spark plugs that were put in were burnt on the tips. They went through the warranty issue to get the engine repaired and it was denied. I said this is not a warranty issue. This was a mistake on your part because you never address the oil consumption issue in October 2024. As of June 12,2026, the dealership still has my car. When I call and ask, What’s going on with my car. The only response I get is we are trying something else it’s going to take some time. Last summer, the service advisor told me he was going to fix my car, wanted me to come pick it up and drive it for 1000 miles and bring it back. They couldn’t fix the car. They never offered a loaner. The vehicle suffered a catastrophic engine failure and completely stalled in the middle of a live road, creating an immediate highway safety hazard. the vehicle is still being held at Rick Case Hyundai Weston for over a year regarding excessive oil consumption issues, which the dealer failed to resolve. The vehicle has 85,000 miles and a complete maintenance history. Hyundai Corporate is currently denying a replacement engine under the active engine settlement framework, leaving a known stalling safety hazard unresolved."

In August 2025, my car went into "limp mode" (go no faster than 40 MPH and it was lurching and speed dropping) on the freeway, which was scary and dangerous. Once home, I called a certified mechanic. When this mechanic tried to start the car, it made a terrible clacking noise. He told me it sounded like the main bearings were going out. He discovered that there was a recall on this particular vehicle because the main bearings would fail. The local dealer confirmed that the car was subject to the recall and requested that the car be brought in. Because I am on limited income, I could not afford to have the car towed until November 2025. Once there, the dealership verified that the main bearings went out. A claim was made to the manufacturer. The claim was denied claiming "outside influence." I provided service records to the manufacturer, but before they had all the maintenance records, they closed my case and will no longer take documents or even speak to me or the dealer. I have been without a car for almost 10 months. I have seen documented cases that trying to start the car could lead to dangerous engine fires.

On 5/27/2026 I was driving home from work. Had just gotten off the frwy , was driving around 35-40mph, all of a sudden , engine turned off, after coming to a stop, holding up traffic, i put in park, and nothing, not crancking, a new battery was imstalled, 3 weeks prior, have recoreds of all maintenance! Checked oil level was good, radiator, had fluid, and fas tank was 1/2 full! Pushed to a side street ,Uberd, back home, later that evening , had car towed approx 5 miles to my house. Checked all fuses, ignition, and starter! Spoke with Local Hundai Dealership, scheduled a maintenance diagnostic to be performed! Could have been an accident, first time, ever experienced, such an issue!

My infotainment unit stoped working after an update that was pushed to the car by Hyundai. My battery was beginning to show signs that it needed replacing (not cranking in the cold temps or needing a few tries to start), so I went to Advance Auto to have the battery replaced. Once back in the car, everything was working fine (radio with sirius XM, back up camera, Navigation, Android and Apple car plan, the clock, everything). When I went to reset the clock, the car showed a software update was availabe and needed to be downloaded. Never before, had I seen this. So, I selected update. After 30 minutes of seeing it say downloading, it then started to say install. It gets to where it says, the radio needs to be restarted to complete the update and then gives error code 604. Now the telematics or infotainment works. I reached out to mapit, Hyundia and Bluelink who all say, "to bad for you, buy a new one for $1600". I did not ask for the update, I did not go looking for the update and I did not push the update to my vehicle, Hyundia did and me and the other 2000 owners who are going through this should not have to pay that kind of money to have it repaired. If they new they were shutting down 3G service, then don't push the update to my car. Hyundia has admitted the issue for Hyundia Tusons and a few other vehicles but not for 2015 model Sonata's. So for Tusons they send out 3 SD cards to fix the issue, but refuse to do so for all the cars they pushed an update to that as they called it Bricked the head of the unit. I am looking for help to get this fix without me paying $1600. If Hyundia recognizes the problem in some cars and not others thats an issue. One that we the public would love help addressing.

Engine – connecting rod bearing failure. Theta II engine. Hyundai confirmed the defect via Bearing Clearance Test (BCT) on October 3, 2025. The engine can seize without warning, causing sudden loss of power while driving – a clear safety risk. The problem has been confirmed by an authorized Hyundai dealership (Hertrich Hyundai of New Castle). The manufacturer issued Technical Service Bulletin 22‑EM‑001H (TXXI lifetime warranty) covering this exact defect. Despite confirming the failure, the manufacturer has refused to repair the engine. The vehicle is available for inspection upon request. No crash, fire, or injury occurred. Warning signs included engine noise and illumination of check engine light before failure.

I have the 2015 Hyundai sonata with the 2.5 GDI engine that eventually will seize due to internal damaged caused by manufacture design flaws. My Hyundai threw a P0010 code which causes my car to idle high and causing low vacuum affecting my brakes. I had a diagnostic test done at a Hyundai dealership that told me it was a cvvt cam sprocket issue and that it was the beginning of the end of the engine, They quoted me 3700.00 but the engine was to far gone and this repair would only accelerate the engine to seize. They said since the engine isn't seize Hyundai will not cover the cost to repair or exchange the engine, I contacted the Hyundai customer service who stated since the repair that is needed wasn't the root cause of the problems associated with the engine swap warranty they were not going to help me even though I explained to them that the cars high accelerating and the braking system been jeopardized and been told the engine is beyond repairs I feel Hyundai should reconsider their decision and help a loyal customer. I have purchased 12 Brand New Hyundaj since 2001. The customer Representative was Extremely un Professional even suggesting I scrap my car, Also I contacted My Hyundai service Technician who said Hyundai customer service never contacted them about my car, When they said they spoke with the technician. Also I received an Extended warranty on the 2.4 GDI engine from Hyundai who many Hyundai customers are or have dealt with.I also purchased an Extended warranty Which I believe should have been extended as well Thank You

This vehicle has experienced three separate incidents where warning messages related to the EV/hybrid battery system appeared, followed by a sudden loss of forward propulsion. During these events, the vehicle would not move properly or lost the ability to accelerate safely. This defect creates a major safety hazard because it can occur while driving, increasing the risk of being rear-ended, stranded in traffic, collision, injury, or property damage. I transport my small children in this vehicle and no longer feel it is safe. Detailed Timeline: Initial Incident: Vehicle displayed warning messages involving the EV/hybrid battery system and lost the ability to move forward properly. Vehicle had to be shut off and restarted. Independent Mechanic Inspection: Vehicle was inspected by an independent mechanic who advised the hybrid battery system likely needed repair or replacement and recommended service through Hyundai dealership. Dealer Visit #1 – Stew Hansen Hyundai (3/18/2026): Vehicle was taken to authorized Hyundai dealer for diagnosis. Dealer stated they could not duplicate the issue and no repair was made. Second Failure: After release from dealer, same dangerous loss-of-power condition occurred again, resulting in towing and additional inconvenience. Dealer Visit #2 – Stew Hansen Hyundai (4/8/2026): Vehicle returned to dealer for same issue. Dealer performed software update and stated this should correct problem. Dealer advised vehicle had been driven approximately 40 miles and was safe to release. Third Failure (4/12/2026 approx.): Only two days after picking vehicle up on 4/10/2026, same warning messages and propulsion failure occurred again. Safety Concern: This repeated loss of propulsion without warning is extremely dangerous.

Engine seized driving down the interstate doing 65 MPH in rush hour traffic could have been rear ended or hit vehicles around me trying to get off the road. Complete loss of power. Power steering was still active brakes were barely working. No warning lights (I had the knock sensor recall performed). Engine simply locked up solid. I was able to safely make it to the disabled vehicle lane and put the car in park. Engine had no sludge buildup and looked excellent once it was disassembled by the dealer. All engine bearings had spun. Engine was completely replaced under the extended warrenty.

The smoke quickly filled the cabin, making it difficult to breathe and creating an urgent safety hazard. My passenger and I had to exit the vehicle right away due to the amount of smoke. We were exposed to smoke inhalation and were stranded on the side of the road in unsafe weather conditions for over an hour while waiting for a tow truck. I later learned that the vehicle had an open recall related to engine issues; however, I was never notified of this recall. Regardless of recall status, the sudden loss of power combined with heavy smoke and potential fire risk created a dangerous situation that could have resulted in serious injury or death. I believe this issue represents a serious safety defect and fire hazard that could endanger other drivers.

However, Hyundai has refused to perform further inspection or goodwill repair solely due to the vehicle’s salvage branding. Inspection History: The vehicle has been inspected by a Hyundai dealership. No engine replacement was completed. Hyundai Customer Care has updated case notes but is declining remedy based on title status rather than mechanical condition. This complaint concerns a repeat failure of both front CV axles on my 2015 Hyundai Sonata. The CV axles and boots were replaced by Gettel Hyundai of Charlotte County using OEM parts in November 2024. The same components failed again just 7,876 miles later in January 2026; requiring a second full replacement. A CV axle replaced with factory OEM parts by a certified Hyundai tech should not fail within 7,876 miles under normal driving conditions. This pattern of repeat failure indicates either * A defect in the OEM replacement parts supplied by Hyundai * An installation defect at the authorized dealership * An unresolved underlying mechanical condition that causes accelerated CV axle wear The rapid repeat failure represents a potential safety hazard. CV axle failure can result in sudden loss of drive power, difficulty controlling the vehicle, and in severe cases, wheel separation --all of which create dangerous conditions for the driver and others on the road. Chronological Timeline of Events: Nov 5, 2024 55,579 miles; both front CV axles replaced by authorized Hyundai dealership using OEM parts. Labor and parts performed per Hyundai standards. Jan 29, 2026 63,451 miles Same CV axles failed again. Second replacement performed at same authorized Hyundai dealership. Total repair cost $3227.16 paid out of pocket by me. Feb 16, 2026 Hyundai Motor America denied goodwill reimbursement request (Case #XXX), citing the malfunction being out of time in service of the vehicle. Reported to Hyundai Motor, their response did not address the repeated nature of failure, the short mileage interval, or the possibility of a defective OEM part or improper installation. I believe the NHTSA should be aware of this pattern and investigate other 2015 Hyundai Sonata owners have reported similar rapid repeat CV axle failures, which could indicate a systemic defect in either OEM parts or dealership repair procedures. Seemed like it was changing to gas or the electric mode,or the other way around. The airbag on the passenger side came out deflated,and filled with smoke going into the car. He was the belted front passenger and his elbow went in to the front window. You can tell by looking at it,the hole and spidered around it. He broke his right elbow. It is available anytime at the moment. It's at Hyundaii in Glen burnie. I need to get it to state farms lot,to get the Kelly blue book value. We look forward to hearing from you

Hyundai Motor America (Hyundai) is recalling certain model year 2015-2016 Sonata vehicles manufactured May 28, 2014, to August 21, 2015. The affected vehicles are equipped with an Antilock Braking System (ABS) warning light intended to warn the driver if there is an ABS malfunction. In the event of an ABS malfunction, the Brake System warning light will illuminate, but the ABS warning light will not illuminate. Since the vehicles have ABS malfunction lights that do not illuminate, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 135, "Light Vehicle Brake Systems."

Risk: If the driver is alerted there is a brake system malfunction, but not specifically with the ABS system, the driver may operate the vehicle unaware that the vehicle's brakes may lock up during hard braking, increasing the risk of a crash.

Loss of Motive Power

The National Highway Traffic Safety Administration (NHTSA) received a letter dated July 13, 2022, petitioning the Agency to initiate a safety defect investigation into loss of motive power allegations attributed to oil drain pan assembly failures on certain Hyundai Motor America, Inc. and Kia America, Inc. vehicles spanning Model Years (MY) 2005-2021. The Petitioners cited examples of complaints indicating that the subject vehicles were experiencing rapid loss of engine oil due to oil drain plug back outs, resulting in loss of motive power and/or catastrophic engine damage. Additional complaints were provided which alleged that the oil drain pans lack sufficient structural integrity and are susceptible to cracking. The petition itself can be reviewed at NHTSA.gov under ODI number 11481745. On September 12, 2022, the Office of Defects Investigation (ODI) opened Defect Petition (DP22-003) to evaluate the Petitioners’ request. The Petitioners have provided additional complaints within four supplemental letters dated January 31, 2023, March 21, 2023, May 23, 2023, and August 4, 2023. ODI’s evaluation included the review and analysis of the complaints and supporting information submitted by the Petitioners, as well as additional complaints submitted directly to NHTSA which referenced either the oil drain plug or a crack in the oil pan. After reviewing the available data, ODI has not identified evidence that would support opening a defect investigation into the oil drain pan assemblies of the subject vehicles. Based on a review of the available information, a loss of motive power resulting from an oil drain plug back out has not occurred between the initial sale of a vehicle and the completion of its first oil change. If during an oil change service, a technician fails to remove the existing drain plug gasket before installing a new gasket, it is likely that a proper seal between the drain plug and the oil pan will not be made. An improper seal between the drain plug and oil pan could lead to the backing out on the drain plug, resulting in the sudden loss of oil while driving. Additionally, the provided examples of oil pan cracks were localized to the immediate vicinity of the oil drain plug and/or appeared to be the result of direct-contact with a tool during servicing.  There is no information to support that a vehicle-based defect exists relating to the oil pan assembly, and instead the documented incidents are related to the failure to follow the proper maintenance procedures during oil change service. In October 2023, Hyundai published a Technical Service Bulletin (TSB) that provided detailed instructions for oil change service on its gasoline powered vehicles, specifically highlighting the need to remove and replace the existing drain plug gasket prior to reinstalling the drain plug. After a thorough review of the material submitted by the petitioner, the information already in NHTSA's possession, and the potential risks to motor vehicle safety implicated by the petitioners' allegations, NHTSA does not believe that a formal investigation is warranted. Therefore, the petition is denied. A Federal Register Notice (FRN) further detailing NHTSA’s reasons for denial of the petition will be published. Engine Fires

On March 29, 2019, the Office of Defects Investigation (ODI) opened two Preliminary Evaluations (PE19-003 on Hyundai and PE19-004 on Kia) to broadly investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model year Hyundai and Kia vehicles.  During the PE investigations, Hyundai and Kia issued several recalls to address vehicle fires, including those due to engine failures involving various engine types.  The remedies for the engine failure recalls typically involve engine inspection (to detect existing damage), and if necessary, engine replacement.  Additionally, both manufacturers have been installing an engine control software modification known as Knock Sensor Detection Software (KSDS) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen likelihood of engine failure).KSDS is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties (see recall files). On December 22, 2021, ODI upgraded PE19-003 and PE19-004 to this Engineering Analysis (EA21-003, covering both Hyundai and Kia) to evaluate the scope of the prior recalls related to engine failures/fires and the efficacy of the recall remedies for the recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by Hyundai and Kia.  The former issue involved approximately 550,000 MY 2013 Hyundai Elantra, MY 2011 Kia Sorento, and MY 2012-2015 Kia Rio.  The latter issue involved approximately 2.5 million recalled vehicles, which have been identified as displaying potential remedy concerns: Hyundai recalls 15V-568, 17V-226, 20V-746, and 21V-727; and Kia recalls 17V-224, 19V-120, 20V-750, and 21V-844. Regarding the recall scope issue, among all of the scope vehicles identified at EA opening, ODI found only the MY 2011 Kia Sorento vehicles equipped with both the Theta II 2.4L 4-cylinder Multi-Point Injection (MPI) and the Lambda II 3.5L 6-cylinder MPI engines experienced relatively high rates of reported fires.  In December 2023, Kia issued recall 23V-877 that will cover about 80,000 MY 2011 Kia Sorento vehicles equipped with the Theta II 2.4L MPI engines.  The remedy will involve an inspection, and if necessary, a replacement of the engine, as well as the installation of KSDS.  It should be noted that in 2021 these vehicles were included in a KSDS product improvement campaign with warranty extension.  For the MY 2011 Sorento vehicles equipped with the Lambda II 3.5L engines, ODI believes that future engine failures/fires involving these vehicles will likely be due to a lax in adhering to the proper or scheduled engine maintenance (regular engine oil changes), rather than a design or manufacturing engine issue. Regarding the issue of the efficacy of the recall remedies, ODI found the majority of the recalled Hyundai and Kia vehicles, by model, experienced lower reported fire rates after the remedies were applied.  Furthermore, ODI found that for most vehicles that did not receive the recall remedies, by model, experienced relatively high fire rates.  To address this problem, Hyundai informed ODI in December 2023, that it will renotify all of its owners covered by the Hyundai recalls of engine failures/fires every 8 months for the next 3 years.  In December 2023, Kia stated that it will renotify the majority of its owners every 8 months for the next 3 years.Several previously recalled Kia models are excluded from the renotification due to their lower reported rates of engine fires.  Overall, the renotification actions will help to increase the recall completion rates, thus reduce the frequency of engine fires in the future. In view of the recall action being taken by Kia and renotification actions being taken by both Kia and Hyundai, ODI is closing this Engineering Analysis.  Also, both companies have been released from the 2020-iniated Consent Orders (Kia since November 2022 and Hyundai since November 2023).  The agency reserves the right to take additional action if warranted by future circumstances.  To review the reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.

Throttle control system vulnerabilities

The Office of Defects Investigation (ODI) of the National Highway Traffic Safety Administration (NHTSA) received a petition letter dated October 10, 2021, requesting an investigation of possible defects in the electronic throttle control (ETC) system on certain Kia vehicles resulting in the opening of DP21-003. Accordingly, Model Year (MY) 2005-2016 Kia Optima/K5, MY 2006-2015 Kia Sorento, MY 2007-2016 Hyundai Elantra, MY 2007-2016 Hyundai Santa Fe, MY 2006-2015 Hyundai Sonatas are all part of the subject population. The petition was originally filed as a Vehicle Owner Questionnaire that can be reviewed at NHTSA.gov under reference (ODI) number 11439675.On November 12, 2021, ODI opened Defect Petition DP21-003 to evaluate the petitioner?s request. ODI performed trend analyses of the VOQs related to vehicle speed control issues for all the subject vehicles and found that none of the 52 Model/Model Year Hyundai/Kia vehicles demonstrated any actionable trend that warranted an investigation.Petitioners submitted 1119 VOQs and non-VOQ cases in which sudden uncommanded acceleration (SUA) or loss of motive power (LOMP) incidents were alleged.For the 1119 reports, 220 were incidents involving crashes, from which 69 subject Hyundai-Kia vehicles were inspected by Hyundai/Kia dealer technicians, Hyundai/Kia engineers, and/or third-party representatives.ODI found the inspections identified no vehicle defects related to the ETC system.Of the 220 crash-involved reports, 13 Event Data Recorder (EDR) records were downloaded successfully and 11 of those indicated driver error: (i.e.) pedal misapplication.A condition where the accelerator pedal is mistakenly applied instead of the brake pedal when the driver wants to stop the vehicle.In addition, ODI reviewed a scientific journal paper published in Forensic Science International in 2016 by Mr. Park et al ., as well as related test videos. ODI does not believe there is any evidence of uncontrollable SUA occurrences in any of the tests reviewed. NHTSA's Office of Vehicle Safety Compliance (OVSC) reviewed the allegations regarding the potential noncompliance with Federal Motor Vehicle Safety Standard No. 124.Based on the information provided, the Agency concludes there is insufficient information for us to determine if a possible non-compliance exists.Finally, both Hyundai and Kia informed ODI that all their vehicles manufactured after 2012 were equipped with an ETC management system, in which the engine speed will go to idle when the system sees signals from both the brake pedal and accelerator pedal at the same time; this is commonly known as the brake-throttle override system.After thoroughly assessing the material submitted by the petitioner, information already in NHTSA's possession, information submitted by Hyundai and Kia in response to an Information Request letter, and the potential risks to safety implicated by the petitioner's allegation, NHTSA does not believe that a formal investigation is warranted. Electrical overstress

The Office of Defects Investigation (ODI) opened this investigation to determine if the failure of airbags to deploy during severe crashes, in certain vehicles, was the result of a safety related defect.  During the investigation a complex failure was studied that can result in non-deployment of subject vehicle air bags and other restraint system devices in severe crash events. The subject vehicles may be equipped with an airbag control unit (ACU) for the supplemental restraint system (SRS) Electronic Control Unit (ECU) manufactured by ZF-TRW.  The ECU receives signals from crash sensors mounted in the vehicle and deploys the vehicle air bags and seat belt pretensioners in accordance with manufacturer design specifications.  The ECU in the subject vehicles contains a model DS84 application-specific integrated circuit (ASIC) which controls the communication of the crash sensor signal, firing commands (i.e., when to deploy the airbag(s) and/or pretensioners), and fault information (e.g., diagnostic trouble codes). In September 2016, FCA announced recall 16V-668 for certain model year (MY) 2010 to 2014 Chrysler, Dodge and Jeep products manufactured with the subject ZF-TRW ACU.  In this recall, FCA discussed an EOS condition that resulted in a failure of the subject DS84 ASIC, which caused air bag non-deployment.  FCA noted that the defect condition had only been observed in vehicles equipped with sensor harnessing routed across the front of the vehicle.  Other FCA vehicles that also used the subject ACU, but were not equipped with cross-car harnessing, had not experienced EOS failures, despite similar time in service. During the course of this investigation, ODI sent two separate Information Request (IR) letters to six vehicle manufactures (including FCA, Hyundai, Honda, Kia, Mitsubishi, and Toyota) and one IR letter to ZF-TRW.  These IR letters resulted in ODI receiving comprehensive data from these manufacturers and suppliers. Studies of this data found that the DS84 ASIC does not have sufficient protection against negative electrical transients or electrical overstress (“EOS”) that can be generated in certain severe crashes.  An electrical transient occurs when the electrical power supplied to a circuit changes momentarily over a short duration of time.   In these severe crash cases, the crash sensors and other powered wiring can be damaged and short circuited so as to create a negative electrical transient of sufficient intensity and duration (that are outside the vehicle manufacturer's specification) to damage the ASIC before the restraint device deployment signal is received by the SRS ECU.  This damaged signal can lead to incomplete or nondeployment of the air bags and/or pretensioners.  Airbag non-deployment and/or lack of pretensioner operation can increase the risk or severity of injury in a crash.A total of 8 fatalities and 14 injuries were associated with known EOS events. The common element in all investigated manufacturers vehicles is the SRS ECU containing a DS84 ASIC manufactured by ZF-TRW.  The risk associated with the ASIC is equally shared among all OEMS involved in the investigation.  The actual real-world risk can be mitigated by other factors which were assessed by ODI during this investigation. The first mitigating factor involves protections built into the ACU design which protect the DS84 ASIC from damage.  There are multiple strategies and levels of protection employed by different OEMs that provide effective EOS mitigation.  The two most common strategies at the ACU level are circuit protection diodes on the remote senor signal lines, and current limiting resistors that protect critical components. The second mitigating factor is found at the vehicle level and involves the location and routing of the wires leading from the crash sensors to the SRS ECU.  If the wires are well protected in a crash and are not routed with other power wires carrying large currents, the risk for an EOS event is significantly reduced or eliminated. These design specific factors combine to produce a spectrum of risk for the vehicles equipped with ACUs using the DS84 ASIC.  Given the many of years of field exposure, it is possible to divide the subject population into two groups; vehicles which have experienced EOS events, and vehicles which have not experienced EOS field events. Four of the six OEMs involved in this investigation have experienced EOS field events on at least one of their models equipped with a DS84 ASIC.  All vehicle models (including the Toyota models identified in the Failure Report Summary of the opening resume for this investigation) with field events have been recalled.  In an abundance of caution, ODI kept this investigation open five years to monitor field performance and did not identify any field events on vehicles not included in existing safety recalls. Given the spectrum of risk identified in this investigation and that all vehicles with a demonstrated unreasonable risk have been recalled, ODI is closing this investigation. ODI is closing this investigation with the following manufacturer safety recalls: 16V-668, 18E-043, 18V-137, 18V-363, and 20V-024.  With the recall actions taken by the subject vehicle and equipment manufacturers, this investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exists on other model or model year vehicles outside of the recall scopes. The agency reserves the right to take further action if warranted by the circumstances.
