As I was merging onto the interstate I put my foot on the gas pedal to give it a little more gas then usual to get pulled out and all of a sudden all the lights on the dash board illuminated and it slowly lost all power I was able to move it out of the road ... There was no loud ,knocking or ticking noises prior to this happening there was no type of any kind of warning .. my car is the push start button so after the car died completely I hit the push button and all the lights on the dash came on the radio worked and everything else seemed to be working it was when I went to put my foot on the brake to start it it would do nothing it would not start at all .. I was still able to put it in neutral even after the incident happened . I had it towed t Hyundai of Bristol to have it inspected well they did verifi that the engine was locked up but failed to give me any information on the test they ran or how they keep it was locked up and what locked it up so then they have my car over a week and they tell me I need a new engine but then contact me to tell ke that HMA refused to replace the engine on warranty and I had the campaign 953 completed the knox sensor thing before my car broke down so then I had to pay for towing once again out of my pocket to go pick the car from their dealership so when I got there to get my car the service was not that great and my car was in a mess very uncalled for the battery to the car was completely dead with that being said we had to use another battery to just get it turned on to even put it in neutral well then fuel started spraying from the fuel line , the engine cover was left off right along with engine mount was taken off and they even took the belt off there were bolts laying all over my hood of my car and I'm pretty sure we lost a few transporting the car back ..their service they provided was not business/professional behavior and very disrespectful at that .. returning my car in that condition was very unnecessary

Around the end of May 2026, my vehicle stalled without warning while traveling at highway speed. The engine light and EPS warning light illuminated simultaneously. The engine cranks but will not start. The vehicle was towed to a Hyundai dealership on June 15, 2026 where the engine failure was confirmed. My safety was directly at risk. The vehicle lost power on an active highway with no warning, creating a collision hazard for myself and other drivers. The failure has been confirmed by the dealership. Hyundai Motor America acknowledged the engine failure falls under the TXXI Settlement Agreement and offered a buyback or cash settlement. However, Hyundai has had physical possession of my vehicle since June 15, 2026 and has repeatedly failed to provide a written buyback offer despite multiple commitments to do so. Prior warning signs included the engine and EPS lights activating at the moment of failure. No prior warning lamps appeared before that day. The vehicle has 2 open safety recalls as of the date of purchase in November 2025.

Knock sensor needs to be replaced upon evaluation of an inspection on March 27, 2026. It failed a mission codes and upon my understanding the knock sensor one circuit range performance pink wonder single sensor is an engine failure as well as I did an ODB test myself on May 19 of 2026 for the Knox sensor as well and the check engine dominator reading at 2111 the the code indicated that bank one Knox and electrical circuit was out of it upper range or had it perform problem for a predetermined period of time and the code is P0326 and the secondary DTC is a combustion vibration sensor a circuit range performance which affects the powertrain. Also, I have switches with the electrical features in the vehicle as far as the ABS and the steering wheel controls and glitching for the cigarette adapter and plug-in area as well on top of the rear passenger side window, having issues with going up and down.

2012 Hyundai Sonata Hybrid well maintained by original owner, under 130,000 miles. May of 2025 AC stopped working intermittently (Hyundai dealer replaced AC hose). Aug 2025 AC stopped working AGAIN (same dealer could NOT identify problem). Car has consistent Hybrid System Warning! Safely Stop and Do not Drive messages lights and sirens when braking. Code PA186 comes up (low state of charge code for hybrid battery). December 2025 brought car back to SAME Hyundai dealer who NOW says car suddenly needs a 10k transmission (BUT still no idea what PA186 code is related to). Got transmission drained at different shop and NO NEW TRANSMISSION NEEDED! Every repair shop OUTSIDE OF HYUNDAI who has scanned for codes gets the PA186 error and say it is related to HYBRID BATTERY LOW STATE OF CHARGE (I am the original owner THUS lifetime hybrid battery warranty). June 2026 different Hyundai dealer for repair of AC and PA186 code. NEW Hyundai dealer says AC issue is NOT related to the PA186 code. They still dont know what PA186 code is but want to charge me $5100 for engine wiring harness electrical short. SIDE NOTE!! Engine wiring harness electrical short is known issue for ABS module recall 251 for this car. Hyundai has denied the engine wiring harness electrical short repair as part of ABS recall 251 warranty and STILL cant tell me why Hybrid System Warning Lights and PA186 code is come on. Is electrical issue really harness or HYBRID battery?? Car is currently at Hyundai dealer.

After incident vehicle will not crank or restart , push -button start activates accessories but engine does not turn over when brake is pressed. Vehicle has Campaign 953\Knock Sensor Detection system update completed on October 14 2020. This failure matches NHTSA PE-18-003 and Hyundai TSB 22 -EM-003H for Theta II Engine connecting rod bearing failure.This is a known safety defect causing sudden engine stall at highway speeds .. I am reporting a pattern of systemic failures on a 2012 Hyundai Sonata Hybrid with approximately 131,000 miles. This is my second NHTSA complaint regarding this vehicle and manufacturer. Primary Safety Concern — Engine Wiring Harness: During a recent engine replacement at an authorized Hyundai dealership, technicians identified damage to the engine wiring harness and documented in writing that the owner “might have issues with drive ability.” The dealership declined to repair it under the same goodwill program used to replace the engine and returned the vehicle with this known defect unresolved. The harness directly controls engine operation, hybrid system communication, and overall vehicle control. On a hybrid vehicle, a compromised harness presents a serious and ongoing safety risk. This component is available for inspection upon request. Engine Failure: The hybrid system warning light appeared prior to failure. The vehicle was towed to an authorized dealership where diagnostic code P1326 confirmed engine failure. The manufacturer acknowledged this and replaced the engine under goodwill. Additional System Failures: A pre-scan diagnostic revealed simultaneous fault codes across six systems — engine, transmission, ABS, active hydraulic booster, battery management, and motor control unit — including codes U0293 and U1004 indicating lost communication with the hybrid powertrain control module across multiple systems simultaneously. Independent Safety Advisory: A separate independent repair facility documented in writing that the vehicle presented a severe wheel hub bearing noise and formally advised of an extremely dangerous driving condition. Prior NHTSA Complaint: My first complaint to NHTSA involved this same vehicle when the manufacturer refused to provide a loaner during an extended recall-related repair hold, requiring formal escalation before they complied. The wiring harness defect remains unrepaired. The vehicle is driven daily out of necessity.

Vehicle: 2012 Hyundai Sonata Incident Date: August 14, 2024 Location: [XXX] Description of the Problem: The vehicle experienced a sudden and catastrophic fire without warning while in normal use. There were no prior warning lights, messages, or symptoms before the incident. The failure appears to have originated from the electrical system and/or engine (exact cause UNKNOWN), which resulted in the vehicle igniting unexpectedly. Safety Risk: This failure created an immediate and life-threatening safety hazard to all occupants. The vehicle caught fire suddenly, leaving no time to react or prevent injury. There was no crash or external cause associated with this fire. The incident required emergency response from Butts County Fire and Emergency Services, who generated an official report. Injuries and Trauma: My son suffered burn injuries to his fingers, requiring emergency medical treatment. There is neurological damage to the fingertips, indicating lasting physical injury. In addition, my son experienced significant psychological trauma due to sudden exposure to a life-threatening vehicle fire, including acute emotional distress during the incident and ongoing fear associated with vehicle safety. This demonstrates the extreme danger posed by a sudden, unpreventable vehicle fire. Inspection / Confirmation: The incident was documented by Butts County Fire and Emergency Services. At this time, manufacturer inspection is UNKNOWN, dealer inspection is UNKNOWN, and insurance inspection is UNKNOWN. Warning Signs Prior to Failure: There were no warning lights, messages, or symptoms prior to the fire. The failure occurred suddenly and without notice. Defect Pattern Concern: This incident is consistent with known issues involving Hyundai Sonata vehicles, including engine and electrical system failures and reports of vehicle fires. I purchased a used 2012 Hyundai Sonata. Within about 2 weeks it broke down while I was in the far left lane of the freeway. The car suddenly stopped accelerating and made a very loud knocking noise. Thankfully I was able to put on my hazards and make it to the right shoulder without getting into an accident but as soon as I pulled over it shut off and the panel lights all came on. Evidently there was a recall on an engine rod and the previous owners advised they were not aware of it. Hyundai has declined the repair on the recalled engine several times and their reasons keep changing. First they said I was missing records (admittedly I was missing one record but then provided it to them), then they stated excessive neglect due to the condition of the valve train (I saw pics of the valve train and is in good condition), and now they state because the KSDS wasn't installed. From what I understand, being the second owner, if I can prove the upkeep of the vehicle the extended waranty should cover the repair. I even have the letter from the car mechanic stating the vehicle was maintained and in good condition. It seems a lot of good people are getting declined for repairs on these faulty engines. I have many more documents I could provide showing all of the services done and communications with the company. Due to the size limit I included these ones for your review. The airbag warning light came on, so we took it in to see about a recall on the airbag control unit issue that required a wire filter kit as out lined in a recall in 2011-2013 Sonatas and Sonata Hybrids. In fact, upon reading the invoice filling out this form, I think that is exactly the issue, and Potamkin Hyundai is ignoring the recall. The invoice states that "AIRBAG MODULE NEED FIXING caused by the DTC for the driver airbag resistance is too high, and has liquid leaking out of it through the connector into the clock spring, and need to replace the air bag incongruent with clock spring to perform repair correctly" The technician explained to my son that driving the car in it's current state, the air bag could go off at any moment, possibly killing him. Their airbag could kill my child. The repair is quoted at $1588 and the solution was to turn off the airbag until a decision could be made if my son wanted to drive the car home, which he did, so not it is disabled. It is a Hyundai manufactured airbag, a safety feature installed into this vehicle that should not degrade over time. It is actually the reason my aunt bought the car for her child and why my son bought the car from her. The car has never been wrecked, the airbag had never been deployed or otherwise molested, has been dealer maintained, and had an extended warranty. I spoke to a Hyundai representative and am awaiting a call back, I do not think that he should be responsible for repair of a defective, degrading safety feature built and manufactured by Hyundai, installed a vehicle built and manufactured by Hyundai. I believe other people should be warned that their Sonata airbags might also have this dangerous issue and it should be immediately investigated. Attached is the invoice given to my son.

Despite this, the manufacturer has denied coverage and attributed the issue to maintenance, even though regular maintenance was performed. This is a serious safety concern due to sudden loss of power while driving.

My 2012 Hyundai Sonata recently had an engine replacement completed under a Hyundai recall. Immediately after the recall repair, the vehicle began shaking while driving, produced a burning smell, and the air conditioning stopped working, none of which occurred prior to the recall repair. These symptoms started immediately after the engine replacement, indicating a possible improper or unsafe installation. I believe the vehicle is unsafe to drive due to the burning smell and shaking. I contacted Hyundai Customer Service and was told the issue must be resolved with the dealership. However, the dealership has refused timely inspection, has dismissed my safety concerns, and has declined to provide a rental or loaner vehicle, leaving me without safe transportation. This is a post-recall safety issue, and I am concerned about the risk of fire or mechanical failure due to the burning smell and vehicle instability. I am requesting this issue be formally investigated.

The engine in my Hyundai Sonata (replacement engine VIN [XXX] ) experienced a complete internal failure. The component that failed is the 2.4L Theta II GDI engine, which is known for issues related to metal debris, oil restriction, rod bearing wear, and resulting piston and compression failure. The failed engine is currently at the dealership (Carolina Hyundai) and is available for inspection upon request. My safety was put at risk because the engine lost power unexpectedly, ran extremely rough, and became inoperable. A sudden loss of power while driving could have caused a crash, especially in traffic situations. The problem has been reproduced and confirmed by the dealership. They performed a compression test and found all cylinders at extremely low PSI (35–45 psi). The dealer stated the engine cannot perform the required bearing clearance test due to the level of internal damage. Hyundai Corporate initially declined assistance, calling it a piston failure, but the symptoms and damage are consistent with the known Theta II recall issues. The vehicle has been inspected by the dealership, and all diagnostic information has been submitted to Hyundai under PA Reference #XXX. Hyundai Corporate has not yet responded to me with a case manager. Before the engine failure, the vehicle had rough running and loss of power, but no warning lights or messages illuminated on the dashboard. Symptoms appeared shortly before the engine became undriveable. Based on the dealer’s findings and the engine’s known defect history, it is my assessment that the failure is related to the same issues described in Hyundai’s Theta II engine recall (Campaign 132). My 2012 Hyundai Sonata cut off and stopped working! The vehicle locked up in the middle of a main street and would not start back up causing a very unsafe situation. There were no check engine lights on or anything at the time of the incident I immediately had the vehicle towed and inspected it has been determined that the engine is failing! Upon research I found out that these engines have had recalls in the past. Carfax shows that the vehicle has been taken in for routine maintenance and oil changes when needed. I am looking to see if the engine replacement could be covered for this vehicle.

My engine seized on the highway on my way home from work. The service department has been horrid. They lied about who the manager is. They lied about the reason behind not wanting to honor the recall. They are rude - threatened to tow my car today after I waited 6 days to hear back from them about the issue with my car and determination. Was extremely rude. Stated that they were not honoring the recall because there was obvious signs of my engine being tampered with or certain parts on my engine, but could not provide a name of the part or a serial or model number so that this could be verified. I need this addressed immediately.

I should have reported it at the time. I didn't realize that I could and now it's too late. The claim has already been submitted. Now, we have the same exact car and a crack has appeared on the passenger dash. My concern is the airbag. The crack just appeared from nowhere. My 2012 Hyundai Sonata originally had its engine replaced under Hyundai’s recall (NHTSA Recall #17V226000) due to the Theta II GDI engine defect. The replacement engine was installed by the dealership as part of the recall. However, the replacement engine has now failed after only ~50,000 miles, exhibiting the same issues (engine knocking, stalling, and eventual failure). This occurred despite proper maintenance and care. The dealership contacted Hyundai, and I also contacted Hyundai Consumer Affairs directly. Both refused to replace or assist with the failed replacement engine. It is unacceptable that a recall replacement engine would fail so soon and that Hyundai will not stand behind the part they used to correct a safety defect. The issue remains a serious safety concern, especially since engine failure can lead to fire. I request that NHTSA investigate the durability and safety of replacement engines and Hyundai’s refusal to support affected owners when these replacement engines fail prematurely. This is the first time I have an engine failure. I drove my car 3 min before merging into the route 1 highway. In approximately 800 feet, my car went haywire, all lights in the dashboard went ON, the steering wheel got stiff, and the car speed dropped off, I almost got rear-ended. I made an effort to pull over to the break lane. The car shut off and I could not get it to start. I opened the hood, a light smoke was coming out of the engine. I left it open to make sure it doesn't get worse. After 10 min, a State trooper stopped by to check on my safety, I informed him that I called the insurance to report the incident and request a tow truck. After the tow truck dropped off my car in front of my house, I called a mechanic to check it, he informed me that the engine is seized.

That garage acknowledged problems with the cars engine stating rod bearing clearance and the engine had to be replaced then charged me denied the extended warranty they initially said would classify for. I couldn't keep up with traffic and was very busy almost cause a crash.

It first started when the engine needed to be replaced, I spent thousands trying to get it repaired, soon after they fixed the engine everything started going wrong ( I truly think they put in a faulty or wrong engine @ Hyundai) the car was overheating, I would take it in they would say it’s one thing pay thousands and it wasn’t it, changed the coolant that goes into the cabin, there’s always been a chemical smell since…still overheats, they said maybe it’s the thermostat, spent more money still didn’t fix it, the battery now won’t stay charged(trunk battery) not sure if they hooked it up wrong…the seatbelt hasn’t worked in years, I take it in they say it’s fine, it never clicks, I have to click it in to the passenger seat for safety. They always say it’s fine! I will never ever buy another Hyundai in life due to all of the issues I’ve experienced with mainly Hyundai dealership service mechanics and dealerships, I’m now dealing with a faulty car that could still be a great car if they didn’t mess it up, I thought I was doing right by always only taking my car to Hyundai and goodness I was wrong! There are no “recalls” currently on my car but honestly, did they even do the recalls correctly?! I disagree! They have literally lied, drained my wallet and disappointed me all while making my car worse, it’s like dr’s take these meds that might fix the issue but it’s going to mess up something else, it’s a guessing game all while killing you or in this case my car!!! I don’t know dates, all I know is Hyundai has all the records! Oh and the last straw was them driving my car without my permission mint you, they took it to a totally different shop without my knowledge & had someone else repair the frame which I was unaware of & drive it with ZERO oil in my car which now my car has been sitting for the most part since they repaired my car(or lack there of) because they broke my car! yes I turned it in when it happened, I just never heard anything from anyone now I’m carless

On [XXX] while driving approximately 1.5 hours away from home, a warning message appeared on my dashboard stating: “Hybrid System Warning. Stop and Do Not Drive.” I immediately pulled over and contacted my insurance company, who arranged for the vehicle to be towed to the nearest authorized Hyundai dealership (Thomas Hyundai in Cumberland). The warning clearly indicated that it was not safe to continue driving. Later, I found out that the issue reported (recall 198) have presented cases ending with fire of the engine. It is critical to mention that in January 2025, I attended the official manufacturer recall for this vehicle, which was supposed to ensure safety and reliability. Despite this, the vehicle failed and left me stranded in an unsafe area. Since then, I have been without my vehicle for over three weeks. I contacted Hyundai customer care and opened a case. I was repeatedly told a case manager would call me within three business days, but I never received any follow-up until I insisted. I was eventually assigned a case manager named Snowy, and also remained in contact with the dealership. On several occasions, both Snowy and the dealership assured me that the vehicle would be ready by Thursday, July 10 and that It was not until then that I could start to request a reimbursement for my rental expenses. I repeatedly asked for confirmation because I am renting a car at my own expense in order to go to work and manage my life. However, on Thursday, July 10, I called again and was told the vehicle would instead be ready by Tuesday, July 15. Then, on Friday, July 11, I received an email stating that the engine had just arrived, and that the dealership was only now able to begin repairs. After that, and only once the car is finished, Hyundai will “review” the case and determine whether I qualify for reimbursement. This is unacceptable. Hyundai Motor America (Hyundai) is recalling certain model year 2011-2012 Sonata vehicles manufactured December 11, 2009, to April 12, 2012 at Hyundai Motor Manufacturing Alabama and equipped with either a 2.0 liter or 2.4 liter Gasoline Direct injection engine. In the affected vehicles, metallic debris may not have been fully removed during manufacturing of the engine crankshaft. If the debris was not completely removed, oil flow may be restricted through the connecting rod bearings, causing connecting rod damage. A worn connecting rod bearing will produce a metallic, cyclic knocking noise from the engine and possible engine failure.
Risk: Engine failure would result in a vehicle stall, increasing the risk of a crash.
Remedy: Hyundai will notify owners, and dealers will inspect the vehicles and replace the engine assembly, as necessary, free of charge. Additionally, Hyundai Motor America will increase the warranty for the engine sub-assembly (short block) to 10 years/120,000 miles for both original and subsequent owners of 2011 and 2012 Sonatas manufactured at Hyundai Motor Manufacturing Alabama equipped with 2.0 liter and 2.4 liter Gasoline Direct injection engines. Hyundai Motor Company (Hyundai) is recalling certain model year 2011-2014 Sonata vehicles manufactured December 11, 2009, through May 29, 2014. In the affected vehicles, the transmission shift cable may detach from from the shift lever pin.
Risk: If the transmission shift cable detaches, the transmission gear selection may not match the indicated gear and the vehicle may move in an unintended or unexpected direction, increasing the risk of a crash. Furthermore, when the driver parks the vehicle, despite selecting the 'PARK' position, the transmission may not be in 'PARK.' If the vehicle is not in the 'PARK' position and the parking brake is not applied, there is a risk the vehicle will roll away as the driver and other occupants exit the vehicle or anytime thereafter. A vehicle rollaway increases the risk of injury to exiting occupants and bystanders.

Hyundai Motor America (Hyundai) is recalling certain model 2011-2012 Sonata vehicles. The brake pedal stopper pad can deteriorate allowing the brake light switch plunger to remain extended when the brake pedal is released.
Risk: If the brake light switch plunger does not retract as it should when the brake pedal is not being pressed, the brake lights may stay illuminated preventing accurate communication to rearward following vehicles that the vehicle is slowing or stopping. Additionally, if the brake switch plunger is not retracted, then the transmission can be shifted out of PARK without depressing the brake pedal. Either condition increases the risk of a crash.

In the event of a crash, the air bag control unit (ACU) may short circuit, preventing the frontal air bags, seat belt pretensioners, and side air bags from deploying.
Risk: If the frontal air bags, seat belt pretensioners, and side air bags are disabled, there is an increased risk of injury to the vehicle occupants in the event of a vehicle crash that necessitates deployment of these safety systems.

The National Highway Traffic Safety Administration (NHTSA) received a letter dated July 13, 2022, petitioning the Agency to initiate a safety defect investigation into loss of motive power allegations attributed to oil drain pan assembly failures on certain Hyundai Motor America, Inc. and Kia America, Inc. vehicles spanning Model Years (MY) 2005-2021. The Petitioners cited examples of complaints indicating that the subject vehicles were experiencing rapid loss of engine oil due to oil drain plug back outs, resulting in loss of motive power and/or catastrophic engine damage. Additional complaints were provided which alleged that the oil drain pans lack sufficient structural integrity and are susceptible to cracking. The petition itself can be reviewed at NHTSA.gov under ODI number 11481745. On September 12, 2022, the Office of Defects Investigation (ODI) opened Defect Petition (DP22-003) to evaluate the Petitioners’ request. The Petitioners have provided additional complaints within four supplemental letters dated January 31, 2023, March 21, 2023, May 23, 2023, and August 4, 2023. ODI’s evaluation included the review and analysis of the complaints and supporting information submitted by the Petitioners, as well as additional complaints submitted directly to NHTSA which referenced either the oil drain plug or a crack in the oil pan. After reviewing the available data, ODI has not identified evidence that would support opening a defect investigation into the oil drain pan assemblies of the subject vehicles. Based on a review of the available information, a loss of motive power resulting from an oil drain plug back out has not occurred between the initial sale of a vehicle and the completion of its first oil change. If during an oil change service, a technician fails to remove the existing drain plug gasket before installing a new gasket, it is likely that a proper seal between the drain plug and the oil pan will not be made. An improper seal between the drain plug and oil pan could lead to the backing out on the drain plug, resulting in the sudden loss of oil while driving. Additionally, the provided examples of oil pan cracks were localized to the immediate vicinity of the oil drain plug and/or appeared to be the result of direct-contact with a tool during servicing. There is no information to support that a vehicle-based defect exists relating to the oil pan assembly, and instead the documented incidents are related to the failure to follow the proper maintenance procedures during oil change service. In October 2023, Hyundai published a Technical Service Bulletin (TSB) that provided detailed instructions for oil change service on its gasoline powered vehicles, specifically highlighting the need to remove and replace the existing drain plug gasket prior to reinstalling the drain plug. After a thorough review of the material submitted by the petitioner, the information already in NHTSA's possession, and the potential risks to motor vehicle safety implicated by the petitioners' allegations, NHTSA does not believe that a formal investigation is warranted. Therefore, the petition is denied. A Federal Register Notice (FRN) further detailing NHTSA’s reasons for denial of the petition will be published. On March 29, 2019, the Office of Defects Investigation (ODI) opened two Preliminary Evaluations (PE19-003 on Hyundai and PE19-004 on Kia) to broadly investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model year Hyundai and Kia vehicles. During the PE investigations, Hyundai and Kia issued several recalls to address vehicle fires, including those due to engine failures involving various engine types. The remedies for the engine failure recalls typically involve engine inspection (to detect existing damage), and if necessary, engine replacement. Additionally, both manufacturers have been installing an engine control software modification known as Knock Sensor Detection Software (KSDS) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen likelihood of engine failure).KSDS is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties (see recall files). On December 22, 2021, ODI upgraded PE19-003 and PE19-004 to this Engineering Analysis (EA21-003, covering both Hyundai and Kia) to evaluate the scope of the prior recalls related to engine failures/fires and the efficacy of the recall remedies for the recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by Hyundai and Kia. The former issue involved approximately 550,000 MY 2013 Hyundai Elantra, MY 2011 Kia Sorento, and MY 2012-2015 Kia Rio. The latter issue involved approximately 2.5 million recalled vehicles, which have been identified as displaying potential remedy concerns: Hyundai recalls 15V-568, 17V-226, 20V-746, and 21V-727; and Kia recalls 17V-224, 19V-120, 20V-750, and 21V-844. Regarding the recall scope issue, among all of the scope vehicles identified at EA opening, ODI found only the MY 2011 Kia Sorento vehicles equipped with both the Theta II 2.4L 4-cylinder Multi-Point Injection (MPI) and the Lambda II 3.5L 6-cylinder MPI engines experienced relatively high rates of reported fires. In December 2023, Kia issued recall 23V-877 that will cover about 80,000 MY 2011 Kia Sorento vehicles equipped with the Theta II 2.4L MPI engines. The remedy will involve an inspection, and if necessary, a replacement of the engine, as well as the installation of KSDS. It should be noted that in 2021 these vehicles were included in a KSDS product improvement campaign with warranty extension. For the MY 2011 Sorento vehicles equipped with the Lambda II 3.5L engines, ODI believes that future engine failures/fires involving these vehicles will likely be due to a lax in adhering to the proper or scheduled engine maintenance (regular engine oil changes), rather than a design or manufacturing engine issue. Regarding the issue of the efficacy of the recall remedies, ODI found the majority of the recalled Hyundai and Kia vehicles, by model, experienced lower reported fire rates after the remedies were applied. Furthermore, ODI found that for most vehicles that did not receive the recall remedies, by model, experienced relatively high fire rates. To address this problem, Hyundai informed ODI in December 2023, that it will renotify all of its owners covered by the Hyundai recalls of engine failures/fires every 8 months for the next 3 years. In December 2023, Kia stated that it will renotify the majority of its owners every 8 months for the next 3 years.Several previously recalled Kia models are excluded from the renotification due to their lower reported rates of engine fires. Overall, the renotification actions will help to increase the recall completion rates, thus reduce the frequency of engine fires in the future. In view of the recall action being taken by Kia and renotification actions being taken by both Kia and Hyundai, ODI is closing this Engineering Analysis. Also, both companies have been released from the 2020-iniated Consent Orders (Kia since November 2022 and Hyundai since November 2023). The agency reserves the right to take additional action if warranted by future circumstances. The Office of Defects Investigation (ODI) of the National Highway Traffic Safety Administration (NHTSA) received a petition letter dated October 10, 2021, requesting an investigation of possible defects in the electronic throttle control (ETC) system on certain Kia vehicles resulting in the opening of DP21-003. Accordingly, Model Year (MY) 2005-2016 Kia Optima/K5, MY 2006-2015 Kia Sorento, MY 2007-2016 Hyundai Elantra, MY 2007-2016 Hyundai Santa Fe, MY 2006-2015 Hyundai Sonatas are all part of the subject population. The petition was originally filed as a Vehicle Owner Questionnaire that can be reviewed at NHTSA.gov under reference (ODI) number 11439675.On November 12, 2021, ODI opened Defect Petition DP21-003 to evaluate the petitioner?s request. ODI performed trend analyses of the VOQs related to vehicle speed control issues for all the subject vehicles and found that none of the 52 Model/Model Year Hyundai/Kia vehicles demonstrated any actionable trend that warranted an investigation.Petitioners submitted 1119 VOQs and non-VOQ cases in which sudden uncommanded acceleration (SUA) or loss of motive power (LOMP) incidents were alleged.For the 1119 reports, 220 were incidents involving crashes, from which 69 subject Hyundai-Kia vehicles were inspected by Hyundai/Kia dealer technicians, Hyundai/Kia engineers, and/or third-party representatives.ODI found the inspections identified no vehicle defects related to the ETC system.Of the 220 crash-involved reports, 13 Event Data Recorder (EDR) records were downloaded successfully and 11 of those indicated driver error: (i.e.) pedal misapplication.A condition where the accelerator pedal is mistakenly applied instead of the brake pedal when the driver wants to stop the vehicle.In addition, ODI reviewed a scientific journal paper published in Forensic Science International in 2016 by Mr. Park et al ., as well as related test videos. ODI does not believe there is any evidence of uncontrollable SUA occurrences in any of the tests reviewed. NHTSA's Office of Vehicle Safety Compliance (OVSC) reviewed the allegations regarding the potential noncompliance with Federal Motor Vehicle Safety Standard No. 124.Based on the information provided, the Agency concludes there is insufficient information for us to determine if a possible non-compliance exists.Finally, both Hyundai and Kia informed ODI that all their vehicles manufactured after 2012 were equipped with an ETC management system, in which the engine speed will go to idle when the system sees signals from both the brake pedal and accelerator pedal at the same time; this is commonly known as the brake-throttle override system.After thoroughly assessing the material submitted by the petitioner, information already in NHTSA's possession, information submitted by Hyundai and Kia in response to an Information Request letter, and the potential risks to safety implicated by the petitioner's allegation, NHTSA does not believe that a formal investigation is warranted. After full consideration of appropriate factors, the petition is denied. A Federal Register Notice (FRN) further detailing NHTSA?s reasons for denial of the petition will be published.
During this Preliminary Evaluation (PE), the Office of Defects Investigation (ODI) assessed the scope, frequency and potential safety-related consequences of non-crash vehicle fires caused by both engine failures and failures of other vehicle systems/components in model year (MY) 2011-2014 Hyundai Sonata and Santa Fe vehicles equipped with Theta II and Lambda II engines (approximately 1.3 million vehicles). ODI also reviewed field data of other Hyundai vehicles equipped with Theta II, Gamma, Nu and Lambda engines (approximately 4.8 million). As a result of this PE, Hyundai has issued NHTSA Recalls 20V-746 and 21V-727 to address potential engine failures and fires in certain Hyundai vehicles equipped with various engine types. In a letter dated June 11, 2018, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a safety defect investigation into non-collision related fires on model year (MY) 2011-2014 Kia Optima and Sorento, and Hyundai Sonata and Santa Fe vehicles. In a letter dated July 24, 2018, CAS requested that MY 2010-2015 Kia Soul vehicles be added to the petition. CAS stated that there is a higher number of complaints to NHTSA regarding non-crash fires on these vehicles compared to other similar vehicles. The Office of Defects Investigation (ODI) had already opened Recall Queries (RQ17-003 and RQ17-004) on all Kia and Hyundai vehicles equipped with Theta II engines for engine failures that may cause stalling and fires.The queries cover most of the petition subject Optima, Sorento, Sonata, and Santa Fe vehicles but not any of the subject Soul vehicles.On August 21, 2018, ODI opened a Defect Petition (DP18-003) to evaluate whether to grant or deny the CAS petition. ODI sent Information Request letters to Kia and Hyundai (September and October 2018) requesting information on all engine and non-engine related fires in the petition vehicles.ODI performed extensive analysis on the information obtained from Kia and Hyundai. During the DP18-003 analysis, Kia and Hyundai each initiated three additional safety recalls related to engine compartment fires on certain petitioned and non-petitioned vehicles (nearly 740,000 total vehicles: Recalls 18V-907, 18V-934, 19V-101, 19V-063, 19V-120, and 19V-204). Based on ODI?s review of the applicable materials, including responses to Information Requests sent to multiple manufacturers, Vehicle Owner Questionnaires (VOQ), and Early Warning Data, NHTSA decided to grant the petition.The CAS petition was granted on March 29, 2019. Preliminary Evaluations (PE19-003 on Hyundai and PE19-004 on Kia) have been opened to assess the scope, frequency, and potential safety-related consequences of alleged defects relating to non-collision vehicle fires in the 2011-2014 Kia Optima and Sorento, 2011-2014 Hyundai Sonata and Santa Fe, and 2010-2015 Kia Soul.Attached is a list of the reference numbers for the 404 VOQs cited above.
The Office of Defects Investigation (ODI) opened PE18-003 on 3/16/18 to investigate incidents involving crashes, resulting in significant collision related damage, where the frontal airbags failed to deploy. ODI reviewed complaint data, available design, test, and field data on the model year (MY) 2011 Hyundai Sonata, MY 2012-2013 Kia Forte and Forte Koup vehicles. ODI also evaluated data from the automotive air bag system supplier, ZF-TRW (ZF), to review vehicles that may contain the same air bag control unit (ACU) used in the subject vehicles. Hyundai Motor America (HMA) issued recall No. 18V-137 for MY 2011-2013 Sonata and MY 2012 Sonata Hybrid vehicles. Kia Motors America (KMA) issued recall No. 18V-363 for MY 2010-2013 Kia Forte and Forte Koup, MY 2011-2013 Optima, MY 2011-2012 Optima Hybrid, and MY2011-2012 Sedona vehicles. The two recalls address vehicles containing ACUs manufactured without adequate circuit protection by installing a wire harness filter kit to prevent internal damage to the ACU. In the event of a crash, ACU damage may result in the disablement of the vehicle's airbags. Each respective manufacturer has mailed letters to affected vehicle owners informing them of the remedy and its availability. ODI plans to evaluate the susceptibility of the subject ACU to electrical signals, as well as other vehicle factors that can either lead to, or reduce the likelihood of, an electrical overstress event in other vehicles manufactured with the subject ZF ACU.Accordingly, this Preliminary Evaluation has been upgraded to an Engineering Analysis (EA19-001), to expand the scope of the investigation to include the Tier-one supplier and any manufacturers who installed the subject ACU in production vehicles. NHTSA opened Recall Query (RQ) 17-004 to investigate the timeliness and scope of Hyundai?s Theta II GDI engine recalls (Recall Nos. 15V-568 and 17V-226), and Hyundai?s compliance with reporting requirements. On November 23, 2020, the Agency entered into a Consent Order with Hyundai to administratively resolve NHTSA?s assertions of violations of law in connection with RQ17-004. NHTSA?s assertions included that Hyundai may be liable for civil penalties on multiple grounds, including the untimeliness of Recall Nos. 15V-568 and 17V-226, inaccuracies in Hyundai?s recall reports, and that a required report describing potential safety-related issues contained certain inaccuracies or omissions.Under the Consent Order, Hyundai agreed to a total civil penalty of $140,000,000. The total civil penalty included payment of $54,000,000 as a Non-Deferred Amount. Hyundai also agreed to a sum of $46,000,000 as an Abeyance Amount to be deferred and held in abeyance pending its satisfactory completion, as reasonably determined by NHTSA, of the requirements of the Consent Order.Additionally, Hyundai agreed to a $40,000,000 Performance Obligation Amount to be expended by Hyundai to fulfill specified Safety Data Analytics Infrastructure obligations and Test and Inspection Laboratory obligations. The Consent Order also includes other terms designed to enhance Hyundai?s safety practices, and requires Hyundai to retain a Third-Party Auditor who directly reports to NHTSA.Other than RQ17-004, the Consent Order is not intended to affect any other current or future investigation which may involve vehicles equipped with Theta II GDI engines. In particular, Engineering Analysis (EA) 21-003 (which is upgraded from Preliminary Evaluation (PE) 19-003) continues NHTSA?s evaluation of certain performance and design issues relating to the Theta II and other engines.Nothing in the Consent Order discharges Hyundai from any obligation to comply with the Safety Act or regulations thereunder, including the obligation to carry out recalls in accordance with law, and to initiate a recall upon any future determination by Hyundai, or upon any finally adjudicated order from NHTSA, that vehicles equipped with Theta II GDI engines contain a defect related to motor vehicle safety or do not comply with an applicable FMVSS.This RQ is closed. Closure of this RQ has no bearing on the terms of the Consent Order, including Hyundai's ongoing performance obligations, and does not represent a determination by the agency as to Hyundai?s performance under the Consent Order to date.
