I am submitting this complaint because I believe this issue presents a serious safety concern. My 2014 Hyundai Sonata experienced a failure related to the steering angle sensor, causing the ESC, ABS, and traction control warning lights to illuminate while driving. As a result, these important safety systems lost functionality and were no longer available to assist in maintaining vehicle stability and traction during normal driving conditions. This failure occurred without warning and required replacement of the steering angle sensor, followed by a required calibration. A critical steering-related component should not fail under normal driving conditions, especially when its failure can disable multiple electronic safety systems. Drivers may not immediately realize that these systems are no longer functioning, increasing the potential risk during emergency maneuvers, wet road conditions, or sudden avoidance situations. While researching this issue, I found that many other Hyundai owners have reported similar steering and electronic stability control problems. I respectfully request that NHTSA investigate whether steering angle sensor failures, related steering system components, or the associated calibration requirements represent a widespread defect affecting 2011–2014 Hyundai Sonata vehicles. If this issue is determined to be common, I ask that Hyundai be required to address the defect through an appropriate recall or extended warranty program so owners are not responsible for repairs involving a critical vehicle safety system.

I am submitting this complaint regarding a serious safety issue involving my Hyundai Sonata. While driving under normal conditions, my vehicle suddenly entered a low-power ("limp") mode without warning. The vehicle would not accelerate beyond approximately 30–35 mph, creating a potentially hazardous situation in traffic. At the same time, the check engine light began blinking. The sudden loss of power significantly reduced my ability to maintain traffic speed and safely maneuver the vehicle. This condition could have increased the risk of a rear-end collision or other accident. I brought the vehicle to an authorized Hyundai dealership (Fred Beans Honda Of Abington PH# 215-392-9020) for diagnosis. The dealership confirmed that the vehicle stored Diagnostic Trouble Code (DTC) P1326, related to the Knock Sensor Detection System (KSDS). According to the dealership, the vehicle entered limp mode as a result of this fault. The abrupt reduction in vehicle power and speed occurred without sufficient warning and created a safety concern. I am reporting this incident so that NHTSA can investigate whether this condition represents a broader safety defect affecting Hyundai Sonata vehicles equipped with the KSDS system. My vehicle experienced issues beginning with a faulty knock sensor, which escalated into complete engine failure. This issue appears to be identical to problems that have already been identified in recalls involving similar Hyundai Sonata models. Despite experiencing the same symptoms and failure pattern, my specific VIN is not included in the recall. My vehicle has been well maintained, has under 120,000 miles, and should not have experienced a total engine failure under normal conditions. Based on the known recall issues related to engine defects and knock sensor failures, I believe my vehicle is affected by the same underlying defect. I am concerned that vehicles like mine are being excluded from the recall despite having identical safety-related issues. Engine failure while driving presents a serious safety risk, including potential loss of power, stalling, or inability to accelerate in traffic. I have had the car looked by a professional and advised of the engine failure and this is a common issue with this make and model.

ABS module/brakes system goes into limp mode randomly, which causes brake malfunction, throwing codes c1233, c1234 and c1236. There was a recall repair performed by Hyundai about two years ago, however there was two options on NHTSA recall 23V651000/campaign 251. One was to replace a safety plug fuse issue, TSB 23-01-054H . That is the recall they performed. The other recall repair which they did not perform is to replace the ABS module. The Hyundai dealer says I must pay a $225 diagnostic fee for them established they need to do this recall repair . Does that sound correct to you?

The fire started in the engine bay. There was property damage to the driveway, and the fire was eventually extinguished by the fire department. The contact walked to an AutoZone nearby, where a mechanic diagnosed that the battery and starter needed to be replaced. The vehicle was repaired while with the mechanic, and the contact was informed that the actual mileage on the vehicle was 170,123. The contact reported the issue to the dealer and was informed that it would be investigated; however, the contact had not received any additional information. The dealer had taken possession of the vehicle and informed the contact that payments were still expected despite the odometer discrepancy. While driving with my family on a busy freeway at approximately 70 mph, our vehicle suddenly displayed a loud, urgent warning: “Hybrid System Warning – Safely Stop and Do Not Drive!” At the same moment, the car lost the ability to accelerate without any prior signs of trouble. We were extremely fortunate to have enough momentum to coast to the shoulder and shut the vehicle off, unsure of how serious the issue might be. Being stranded without warning on a high‑speed highway put my entire family in a dangerous situation. After a few minutes, we restarted the car to see if we could at least get it off the freeway. The warning had disappeared, and the vehicle drove normally, allowing us to take back roads home. The lack of any prior symptoms makes this even more concerning. Losing acceleration at highway speeds with no warning is a major safety hazard. We took the vehicle to a dealership immediately, but because the warning light was no longer active, they were unable to diagnose the problem. A few days later, the same warning appeared again—this time only a couple minutes from home. I turned around immediately for safety, parked the car, and once again the warning disappeared as soon as the vehicle was shut off. At this point, we have a car sitting in our driveway that we do not feel safe driving. The dealership cannot identify or repair the issue unless the warning is active, & even then, they cannot guarantee that a diagnostic code will appear. After researching this problem online, it’s clear that many other Hyundai owners are experiencing the same issue, often spending thousands of dollars without a definitive fix. Despite how common & dangerous this problem appears to be, there are no recalls & no consistent guidance from dealerships. This situation is unacceptable from a safety standpoint. A vehicle that can suddenly lose power at highway speeds—with no warning and no reliable way to diagnose the cause—poses a serious risk to EVERYONE and needs to be investigated

The engine failed in my 2014 Hyundai Sonata for the 2nd time while driving in evening traffic on a major interstate with my elderly father in the car with me. The engine completely shut down while in mid drive on the interstate with no power. The engine and oil lamp indicators immediately came on right before the engine lost power on the interstate. This caused my safety and the safety of my father to be put at risk by not being able to have any power while in the lane of evening traffic. My vehicle was towed to the nearby Hyundai dealership, Modern Hyundai of Concord, where their service department conducted inspection of the engine issue and where it was kept for 4 months with no repairs made per the orders of Hyundai Motor America. Hyundai Motor America refused to honor their own warranty to replace the engine (engine in need of replacement for the 2nd time).

I was driving my 2014 Hyundai Sonata in a rural area when smoke suddenly entered the cabin from the engine compartment. Two bystanders waved me down, alerting me that they could see fire underneath the engine compartment. I immediately stopped the vehicle, turned it off, and exited safely. I attempted to contact 911, but reception was poor. The fire spread from the engine compartment and engulfed the vehicle, producing thick smoke that created a serious safety risk. Emergency services arrived approximately 15 minutes after being notified and extinguished the fire. The vehicle burned for approximately 30 minutes in total resulting in total loss. There were no warning lights or messages prior to the fire. The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives beyond the fire report. It was towed and is being stored on private property awaiting investigation.

The Hyundai dealership submitted my case to Hyundai corporate, and Hyundai denied the repair solely because my vehicle has over 150,000 miles. A manufacturer cannot refuse a federal recall repair due to mileage, and Hyundai’s engine recall has no mileage or time limit. The denial places me in an unsafe situation, as the recall defect can cause engine seizure or stalling while driving. I am requesting NHTSA intervention.

The High-Voltage (HV) Hybrid Battery Pack Assembly failed, resulting in a system shutdown and the required replacement. The primary code is DTC P0A80 ("Replace Hybrid Battery Pack"), which indicates an internal manufacturing or design flaw, specifically a weak battery module or compromised internal connection, not normal wear. The defective component (the high-voltage battery) is currently at and is available for inspection. The entire vehicle is also compromised by the Anti-Lock Brake System (ABS) Module defect (NHTSA Recall 23V-651000). My safety was put at risk in two primary ways: 1. Immediate Loss of Power: The hybrid system failure resulted in a loss of drivetrain power, rendering the vehicle nearly immobile. This occurred on at approximately mph, placing me at high risk of a collision. 2. Latent Fire Risk: My vehicle is covered under the NHTSA ID 23V-651000 fire risk recall due to the ABS module. Hyundai specifically advised owners of these vehicles to "park their vehicles outside and away from structures" until the remedy is complete, confirming the severity of the latent fire hazard. The failure of the secondary HV battery system occurred within this already dangerous, electrically unstable environment. Yes, the vehicle was inspected by the Hyundai dealership / independent shop at the time of failure.. The manufacturer's corporate division has been contacted and is aware of the defect and the resulting $10,000 replacement estimate, as they offered a 50% "goodwill" subsidy which I am rejecting, asserting 100% coverage is required for a defect. Yes. The primary preceding warning was the intermittent appearance of the "Check Hybrid System, Turn off engine" warning.

This motor had a recall. Campaign t3g- sonata yfa 2.4L Theta II. 1 month after an oil change at the mechanic the car started making noises. Took it right to the mechanic who said there was no oil and that there was a recall on this motor for this reason. Immediately took it to the Hyundai dealership located in centereach NY. The employee who helped us was not a help. Took days and days to even get the paperwork In for the recall. Did not communicate with us unless we called him. Then eventually just said it was denied. Took weeks to get the proof from them. Completely frustrating. We submitted all proof of oil changes and maintenance and it was denied saying engine neglect! We are struggling to keep up with bills and now cannot get to work because of this. We have 2 toddlers and he is a volunteer firefighter and EMT in Centereach. He works full time at Stony Brook Hospital and is in school full time with rotations in the hospital for paramedic class. This is insane to be treated this way

Hello, I brought my 2014 Hyundai Sonata (approx. 73,000 miles) to an authorized Hyundai dealership (MotorWorld Hyundai, Wilkes-Barre, PA) on August 4, 2025, for Recall 162 (NHTSA 17V-226) due to audible engine knocking. I reported the noise when dropping off the vehicle. The dealer claimed the engine passed the inspection but did not provide any documentation confirming that the required bearing test was performed. They said the issue might be from the top of the engine (possibly the timing chain) and wanted to charge $800 to disassemble the engine, with a repair estimate of $2,900. The reason they wanted to disassemble the engine is because after an initial diagnosis for $160 they discovered the noise is not from the timing chain. Besides, it would be extremely unusual for the issue to be a timing chain at 73k. I later learned from Hyundai Customer Care that the recall still shows as “Incomplete” in their system, meaning the inspection was never properly submitted. I'm conerned that the recall was not properly performed or submitted (why does it still show "incomplete" if they insepcted it and it passed?) and that the symptoms may still indicate the exact defect covered under Recall 162 (rod bearing knock), I believe I was denied proper remedy (in a less polite way - I think they flat out lied to me about the real issue) and I was pressured toward expensive non-recall repairs without conclusive diagnosis. My understanding is that getting a diagnosis for a timing chain shouldn't cost anywhere near $800. I am requesting NHTSA review this situation to ensure Hyundai fulfills its federally mandated recall obligation. Engine Failure. After taking my Hyundai listed above in for repairs to the air conditioning not working properly and while in custody of Jeff's Automotive Service, here in Sterling, IL 61081, the engine suddenly died out while they were test driving the car. They were able to jump start it once, and then it died out again while driving it. This time the car could not be restarted by all means possible. Being in the shop, other possible causes were checked and not one of them worked. The Engine is completely DEAD. After Checking for possible causes online it was found that a recall for the very same Engine Failure had been done back in 2017/2018.\n" + 'For unknown reasons, I was never notified nor did I ever received such notice. Now at this time, we have contacted a local dealer and they state that because the auto had not been taken in for the recall repairs, it no longer "qualifies" for free repairs and any repairs would have to be paid for. Upon checking the NHTSA website, it states that there are no time limits on the repairs. I was going to contact National Hyundai Corp to inquire about it, but decided instead to file a complaint with you. 'The question is: Is my car still eligible for the Recall Repairs under the Hyndai Mfg Recall #162 'of March 31, 2017? Recall Number 17V2260000??? Given the fact that i did not receive their notification if indeed, one was sent, and I was totally unaware of this Recall???. We dropped the vehicle off at a Hyundai dealership (Hyundai Napleton) on July 1st, 2025 for recall repair, following instructions from ACV Auctions. After holding the vehicle for over three weeks, Hyundai Napleton informed us they could not perform the recall due to their internal policy against working on dealer-owned vehicles. This directly contradicts the purpose of a federally mandated safety recall, which is supposed to apply regardless of ownership. On July 23, 2025, we received a call from Hyundai Motor America, stating they would not approve the recall repair because the vehicle had been declared a total loss. This is entirely false—the vehicle has never been declared a total loss by any insurance company or official source. It appears Hyundai Motor America and the dealership are passing responsibility back and forth, providing conflicting and inaccurate information, while the vehicle sits unrepaired and inoperable. This behavior delays a federally mandated repair, puts consumers at risk, and reflects serious breakdowns in how Hyundai handles recall compliance. We need NHTSA’s help to investigate and enforce the proper handling of this safety recall. I have not yet had it diagnosed by a dealer or repair shop, but I plan to schedule a diagnostic check soon. The airbag light remains consistently on during vehicle operation, indicating a persistent fault. After having work done to repair the head gasket and spark plug. I was on mile ~300 after repairs and was nearing to fill up at a gas station, I started losing power and notice the check engine light was on and Gas smell filled the cabin. When I stopped the hood started smoking and there was a fire under the engine cover. My Vin is not listed on the fuel line recalls and I believe this is an oversite as it lists, faulty High Pressure hose can rupture and cause engine fires and that is exactly what happen in my case. I believe this Vin was Overlooked and I need the repairs complete. I have not completed any repairs because I believe this should have been recalled.

My engine seized and I lost power. This is the second engine this has happened to. I was given a extended warranty, but Hyundai is refusing to pay for a new engine. The mechanic advised me they are replacing engines three times a week. Mine was denied even though my engine only has 76,000 miles and my warranty is still good. This engine problem should be a new recall since the same problem is still occurring. The engine light did come on blinking. When I took it to the dealer. They knew the problem before looking at my engine since so many others need to have thiers replaced. The dealership confirmed i need a new engine. They made me send in my oil changes. They were upset I didn't have any oil receipts for my previous engine even thought it was replaced at a Hyundai dealership the first time. Consumers should not have to replace two engines. This is a safety concern with it losing power so quickly.

Incident Summary: While driving on the freeway, the vehicle suddenly displayed the following alarming warnings: “Hybrid System Warning! Safely Stop and Do Not Drive” “Check Charging System” Immediately after these messages appeared, the car lost all power and shut down in the middle of the road. I had my two young children (ages XXX and XXX) in the car, making the situation extremely unsafe and traumatic. The vehicle lost all mobility and provided no opportunity to safely pull over. After contacting Hyundai Corporate multiple times, a case manager was eventually assigned to my case. At Hyundai’s request, I towed the vehicle to AutoNation Hyundai Valencia for diagnostic evaluation. The dealership diagnosed the issue as error code P0A78, indicating a transmission failure. Hyundai Corporate offered to cover 60% of the repair cost, leaving me responsible for the remaining 40%—an amount that exceeds the current value of the vehicle. This proposed resolution is not reasonable considering the severity of the safety risk and the vehicle’s low mileage. My primary concern is that this is an extremely dangerous defect, as the vehicle loses all power without warning, leaving the driver unable to move the car to a safe location. Requested Actions: A formal investigation into the safety and reliability of Hyundai’s hybrid systems, specifically in 2014 Sonata Hybrid vehicles. A determination of whether this defect qualifies under Hyundai’s warranty or federal safety defect regulations. Full compensation from Hyundai Corporate to cover 100% of the repair costs to correct this serious safety issue. Thank you for your time and attention to this critical matter. The dealership stated that the engine needed to be replaced. However, I have maintained the vehicle regularly, with consistent oil changes and maintenance, and I have documentation to prove this. Despite this, Hyundai has refused assistance and claims "exceptional maintenance neglect" without showing inspection photos or detailed evidence. This engine failure fits the exact pattern of a known engine defect already under recall for similar Hyundai models and engines, including issues with oil passages and rod bearings that lead to sudden engine failure. These vehicles have been involved in class-action lawsuits and recall campaigns for this reason. This sudden failure put my safety and that of others at risk, as the engine lost power in traffic without any prior warning. Risk: A cracked fuel hose may leak fuel, which can increase the risk of a fire.

Remedy: Dealers will replace the low pressure fuel hose, free of charge. Owner notification letters were mailed October 10, 2022. Owners may contact Hyundai customer service at 1-855-371-9460. Hyundai's number for this recall is 227. This recall replaces NHTSA recall number 20V-121. Vehicles previously repaired under recall 20V-121 that did not receive a replacement fuel hose will need to have this new recall remedy performed.

Hyundai Motor Company (Hyundai) is recalling certain model year 2011-2014 Sonata vehicles manufactured December 11, 2009, through May 29, 2014. In the affected vehicles, the transmission shift cable may detach from from the shift lever pin.

Risk: If the transmission shift cable detaches, the transmission gear selection may not match the indicated gear and the vehicle may move in an unintended or unexpected direction, increasing the risk of a crash. Furthermore, when the driver parks the vehicle, despite selecting the 'PARK' position, the transmission may not be in 'PARK.' If the vehicle is not in the 'PARK' position and the parking brake is not applied, there is a risk the vehicle will roll away as the driver and other occupants exit the vehicle or anytime thereafter. A vehicle rollaway increases the risk of injury to exiting occupants and bystanders.

Hyundai Motor America (Hyundai) is recalling certain 2013-2014 Sonata vehicles. The low pressure fuel hose that connects the low pressure fuel pump to the direct injection fuel pump may crack over time due to heat generated within the engine compartment.

Risk: If the fuel line cracks, a fuel leak can occur, increasing the risk of a fire.

Remedy: Hyundai will notify owners, and dealers will inspect the low-pressure fuel hose for damage or leaking, replacing it as necessary. If no damage or leaking is found, heat-protective tape will be installed. Repairs will be performed free of charge. The recall began August 10, 2020. Owners may contact Hyundai customer service at 1-855-371-9460. Hyundai's number for this recall is 189. This recall has been replaced by NHTSA recall number 22V-314.

Loss of Motive Power

The National Highway Traffic Safety Administration (NHTSA) received a letter dated July 13, 2022, petitioning the Agency to initiate a safety defect investigation into loss of motive power allegations attributed to oil drain pan assembly failures on certain Hyundai Motor America, Inc. and Kia America, Inc. vehicles spanning Model Years (MY) 2005-2021. The Petitioners cited examples of complaints indicating that the subject vehicles were experiencing rapid loss of engine oil due to oil drain plug back outs, resulting in loss of motive power and/or catastrophic engine damage. Additional complaints were provided which alleged that the oil drain pans lack sufficient structural integrity and are susceptible to cracking. The petition itself can be reviewed at NHTSA.gov under ODI number 11481745. On September 12, 2022, the Office of Defects Investigation (ODI) opened Defect Petition (DP22-003) to evaluate the Petitioners’ request. The Petitioners have provided additional complaints within four supplemental letters dated January 31, 2023, March 21, 2023, May 23, 2023, and August 4, 2023. ODI’s evaluation included the review and analysis of the complaints and supporting information submitted by the Petitioners, as well as additional complaints submitted directly to NHTSA which referenced either the oil drain plug or a crack in the oil pan. After reviewing the available data, ODI has not identified evidence that would support opening a defect investigation into the oil drain pan assemblies of the subject vehicles. Based on a review of the available information, a loss of motive power resulting from an oil drain plug back out has not occurred between the initial sale of a vehicle and the completion of its first oil change. If during an oil change service, a technician fails to remove the existing drain plug gasket before installing a new gasket, it is likely that a proper seal between the drain plug and the oil pan will not be made. An improper seal between the drain plug and oil pan could lead to the backing out on the drain plug, resulting in the sudden loss of oil while driving. Additionally, the provided examples of oil pan cracks were localized to the immediate vicinity of the oil drain plug and/or appeared to be the result of direct-contact with a tool during servicing.  There is no information to support that a vehicle-based defect exists relating to the oil pan assembly, and instead the documented incidents are related to the failure to follow the proper maintenance procedures during oil change service. In October 2023, Hyundai published a Technical Service Bulletin (TSB) that provided detailed instructions for oil change service on its gasoline powered vehicles, specifically highlighting the need to remove and replace the existing drain plug gasket prior to reinstalling the drain plug. After a thorough review of the material submitted by the petitioner, the information already in NHTSA's possession, and the potential risks to motor vehicle safety implicated by the petitioners' allegations, NHTSA does not believe that a formal investigation is warranted. Therefore, the petition is denied. A Federal Register Notice (FRN) further detailing NHTSA’s reasons for denial of the petition will be published. Engine Fires

On March 29, 2019, the Office of Defects Investigation (ODI) opened two Preliminary Evaluations (PE19-003 on Hyundai and PE19-004 on Kia) to broadly investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model year Hyundai and Kia vehicles.  During the PE investigations, Hyundai and Kia issued several recalls to address vehicle fires, including those due to engine failures involving various engine types.  The remedies for the engine failure recalls typically involve engine inspection (to detect existing damage), and if necessary, engine replacement.  Additionally, both manufacturers have been installing an engine control software modification known as Knock Sensor Detection Software (KSDS) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen likelihood of engine failure).KSDS is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties (see recall files). On December 22, 2021, ODI upgraded PE19-003 and PE19-004 to this Engineering Analysis (EA21-003, covering both Hyundai and Kia) to evaluate the scope of the prior recalls related to engine failures/fires and the efficacy of the recall remedies for the recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by Hyundai and Kia.  The former issue involved approximately 550,000 MY 2013 Hyundai Elantra, MY 2011 Kia Sorento, and MY 2012-2015 Kia Rio.  The latter issue involved approximately 2.5 million recalled vehicles, which have been identified as displaying potential remedy concerns: Hyundai recalls 15V-568, 17V-226, 20V-746, and 21V-727; and Kia recalls 17V-224, 19V-120, 20V-750, and 21V-844. Regarding the recall scope issue, among all of the scope vehicles identified at EA opening, ODI found only the MY 2011 Kia Sorento vehicles equipped with both the Theta II 2.4L 4-cylinder Multi-Point Injection (MPI) and the Lambda II 3.5L 6-cylinder MPI engines experienced relatively high rates of reported fires.  In December 2023, Kia issued recall 23V-877 that will cover about 80,000 MY 2011 Kia Sorento vehicles equipped with the Theta II 2.4L MPI engines.  The remedy will involve an inspection, and if necessary, a replacement of the engine, as well as the installation of KSDS.  It should be noted that in 2021 these vehicles were included in a KSDS product improvement campaign with warranty extension.  For the MY 2011 Sorento vehicles equipped with the Lambda II 3.5L engines, ODI believes that future engine failures/fires involving these vehicles will likely be due to a lax in adhering to the proper or scheduled engine maintenance (regular engine oil changes), rather than a design or manufacturing engine issue. Regarding the issue of the efficacy of the recall remedies, ODI found the majority of the recalled Hyundai and Kia vehicles, by model, experienced lower reported fire rates after the remedies were applied.  Furthermore, ODI found that for most vehicles that did not receive the recall remedies, by model, experienced relatively high fire rates.  To address this problem, Hyundai informed ODI in December 2023, that it will renotify all of its owners covered by the Hyundai recalls of engine failures/fires every 8 months for the next 3 years.  In December 2023, Kia stated that it will renotify the majority of its owners every 8 months for the next 3 years.Several previously recalled Kia models are excluded from the renotification due to their lower reported rates of engine fires.  Overall, the renotification actions will help to increase the recall completion rates, thus reduce the frequency of engine fires in the future. In view of the recall action being taken by Kia and renotification actions being taken by both Kia and Hyundai, ODI is closing this Engineering Analysis.  Also, both companies have been released from the 2020-iniated Consent Orders (Kia since November 2022 and Hyundai since November 2023).  The agency reserves the right to take additional action if warranted by future circumstances.  To review the reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.

Throttle control system vulnerabilities

The Office of Defects Investigation (ODI) of the National Highway Traffic Safety Administration (NHTSA) received a petition letter dated October 10, 2021, requesting an investigation of possible defects in the electronic throttle control (ETC) system on certain Kia vehicles resulting in the opening of DP21-003. Accordingly, Model Year (MY) 2005-2016 Kia Optima/K5, MY 2006-2015 Kia Sorento, MY 2007-2016 Hyundai Elantra, MY 2007-2016 Hyundai Santa Fe, MY 2006-2015 Hyundai Sonatas are all part of the subject population. The petition was originally filed as a Vehicle Owner Questionnaire that can be reviewed at NHTSA.gov under reference (ODI) number 11439675.On November 12, 2021, ODI opened Defect Petition DP21-003 to evaluate the petitioner?s request. ODI performed trend analyses of the VOQs related to vehicle speed control issues for all the subject vehicles and found that none of the 52 Model/Model Year Hyundai/Kia vehicles demonstrated any actionable trend that warranted an investigation.Petitioners submitted 1119 VOQs and non-VOQ cases in which sudden uncommanded acceleration (SUA) or loss of motive power (LOMP) incidents were alleged.For the 1119 reports, 220 were incidents involving crashes, from which 69 subject Hyundai-Kia vehicles were inspected by Hyundai/Kia dealer technicians, Hyundai/Kia engineers, and/or third-party representatives.ODI found the inspections identified no vehicle defects related to the ETC system.Of the 220 crash-involved reports, 13 Event Data Recorder (EDR) records were downloaded successfully and 11 of those indicated driver error: (i.e.) pedal misapplication.A condition where the accelerator pedal is mistakenly applied instead of the brake pedal when the driver wants to stop the vehicle.In addition, ODI reviewed a scientific journal paper published in Forensic Science International in 2016 by Mr. Park et al ., as well as related test videos. ODI does not believe there is any evidence of uncontrollable SUA occurrences in any of the tests reviewed. NHTSA's Office of Vehicle Safety Compliance (OVSC) reviewed the allegations regarding the potential noncompliance with Federal Motor Vehicle Safety Standard No. 124.Based on the information provided, the Agency concludes there is insufficient information for us to determine if a possible non-compliance exists.Finally, both Hyundai and Kia informed ODI that all their vehicles manufactured after 2012 were equipped with an ETC management system, in which the engine speed will go to idle when the system sees signals from both the brake pedal and accelerator pedal at the same time; this is commonly known as the brake-throttle override system.After thoroughly assessing the material submitted by the petitioner, information already in NHTSA's possession, information submitted by Hyundai and Kia in response to an Information Request letter, and the potential risks to safety implicated by the petitioner's allegation, NHTSA does not believe that a formal investigation is warranted. After full consideration of appropriate factors, the petition is denied. A Federal Register Notice (FRN) further detailing NHTSA?s reasons for denial of the petition will be published.

Electrical overstress

The Office of Defects Investigation (ODI) opened this investigation to determine if the failure of airbags to deploy during severe crashes, in certain vehicles, was the result of a safety related defect.  During the investigation a complex failure was studied that can result in non-deployment of subject vehicle air bags and other restraint system devices in severe crash events. The subject vehicles may be equipped with an airbag control unit (ACU) for the supplemental restraint system (SRS) Electronic Control Unit (ECU) manufactured by ZF-TRW.  The ECU receives signals from crash sensors mounted in the vehicle and deploys the vehicle air bags and seat belt pretensioners in accordance with manufacturer design specifications.  The ECU in the subject vehicles contains a model DS84 application-specific integrated circuit (ASIC) which controls the communication of the crash sensor signal, firing commands (i.e., when to deploy the airbag(s) and/or pretensioners), and fault information (e.g., diagnostic trouble codes). In September 2016, FCA announced recall 16V-668 for certain model year (MY) 2010 to 2014 Chrysler, Dodge and Jeep products manufactured with the subject ZF-TRW ACU.  In this recall, FCA discussed an EOS condition that resulted in a failure of the subject DS84 ASIC, which caused air bag non-deployment.  FCA noted that the defect condition had only been observed in vehicles equipped with sensor harnessing routed across the front of the vehicle.  Other FCA vehicles that also used the subject ACU, but were not equipped with cross-car harnessing, had not experienced EOS failures, despite similar time in service. During the course of this investigation, ODI sent two separate Information Request (IR) letters to six vehicle manufactures (including FCA, Hyundai, Honda, Kia, Mitsubishi, and Toyota) and one IR letter to ZF-TRW.  These IR letters resulted in ODI receiving comprehensive data from these manufacturers and suppliers. Studies of this data found that the DS84 ASIC does not have sufficient protection against negative electrical transients or electrical overstress (“EOS”) that can be generated in certain severe crashes.  An electrical transient occurs when the electrical power supplied to a circuit changes momentarily over a short duration of time.   In these severe crash cases, the crash sensors and other powered wiring can be damaged and short circuited so as to create a negative electrical transient of sufficient intensity and duration (that are outside the vehicle manufacturer's specification) to damage the ASIC before the restraint device deployment signal is received by the SRS ECU.  This damaged signal can lead to incomplete or nondeployment of the air bags and/or pretensioners.  Airbag non-deployment and/or lack of pretensioner operation can increase the risk or severity of injury in a crash.A total of 8 fatalities and 14 injuries were associated with known EOS events. The common element in all investigated manufacturers vehicles is the SRS ECU containing a DS84 ASIC manufactured by ZF-TRW.  The risk associated with the ASIC is equally shared among all OEMS involved in the investigation.  The actual real-world risk can be mitigated by other factors which were assessed by ODI during this investigation. The first mitigating factor involves protections built into the ACU design which protect the DS84 ASIC from damage.  There are multiple strategies and levels of protection employed by different OEMs that provide effective EOS mitigation.  The two most common strategies at the ACU level are circuit protection diodes on the remote senor signal lines, and current limiting resistors that protect critical components. The second mitigating factor is found at the vehicle level and involves the location and routing of the wires leading from the crash sensors to the SRS ECU.  If the wires are well protected in a crash and are not routed with other power wires carrying large currents, the risk for an EOS event is significantly reduced or eliminated. These design specific factors combine to produce a spectrum of risk for the vehicles equipped with ACUs using the DS84 ASIC.  Given the many of years of field exposure, it is possible to divide the subject population into two groups; vehicles which have experienced EOS events, and vehicles which have not experienced EOS field events. Four of the six OEMs involved in this investigation have experienced EOS field events on at least one of their models equipped with a DS84 ASIC.  All vehicle models (including the Toyota models identified in the Failure Report Summary of the opening resume for this investigation) with field events have been recalled.  In an abundance of caution, ODI kept this investigation open five years to monitor field performance and did not identify any field events on vehicles not included in existing safety recalls. Given the spectrum of risk identified in this investigation and that all vehicles with a demonstrated unreasonable risk have been recalled, ODI is closing this investigation. ODI is closing this investigation with the following manufacturer safety recalls: 16V-668, 18E-043, 18V-137, 18V-363, and 20V-024.  With the recall actions taken by the subject vehicle and equipment manufacturers, this investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exists on other model or model year vehicles outside of the recall scopes. The agency reserves the right to take further action if warranted by the circumstances.

Non-crash Vehicle Fires

During this Preliminary Evaluation (PE), the Office of Defects Investigation (ODI) assessed the scope, frequency and potential safety-related consequences of non-crash vehicle fires caused by both engine failures and failures of other vehicle systems/components in model year (MY) 2011-2014 Hyundai Sonata and Santa Fe vehicles equipped with Theta II and Lambda II engines (approximately 1.3 million vehicles). ODI also reviewed field data of other Hyundai vehicles equipped with Theta II, Gamma, Nu and Lambda engines (approximately 4.8 million). As a result of this PE, Hyundai has issued NHTSA Recalls 20V-746 and 21V-727 to address potential engine failures and fires in certain Hyundai vehicles equipped with various engine types. Additional information on these and other related recall actions is available in the downloadable version of this resume available at NHTSA.gov. In addition, ODI has upgraded this PE to an Engineering Analysis (EA) 21-003 which will focus on potential recall scope concerns and recall remedy concerns related to the engine failure rewlated recalls mentioned above and conducted previously. Non-crash Vehicle Fires

In a letter dated June 11, 2018, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a safety defect investigation into non-collision related fires on model year (MY) 2011-2014 Kia Optima and Sorento, and Hyundai Sonata and Santa Fe vehicles. In a letter dated July 24, 2018, CAS requested that MY 2010-2015 Kia Soul vehicles be added to the petition. CAS stated that there is a higher number of complaints to NHTSA regarding non-crash fires on these vehicles compared to other similar vehicles. The Office of Defects Investigation (ODI) had already opened Recall Queries (RQ17-003 and RQ17-004) on all Kia and Hyundai vehicles equipped with Theta II engines for engine failures that may cause stalling and fires.The queries cover most of the petition subject Optima, Sorento, Sonata, and Santa Fe vehicles but not any of the subject Soul vehicles.On August 21, 2018, ODI opened a Defect Petition (DP18-003) to evaluate whether to grant or deny the CAS petition. ODI sent Information Request letters to Kia and Hyundai (September and October 2018) requesting information on all engine and non-engine related fires in the petition vehicles.ODI performed extensive analysis on the information obtained from Kia and Hyundai. During the DP18-003 analysis, Kia and Hyundai each initiated three additional safety recalls related to engine compartment fires on certain petitioned and non-petitioned vehicles (nearly 740,000 total vehicles: Recalls 18V-907, 18V-934, 19V-101, 19V-063, 19V-120, and 19V-204). Based on ODI?s review of the applicable materials, including responses to Information Requests sent to multiple manufacturers, Vehicle Owner Questionnaires (VOQ), and Early Warning Data, NHTSA decided to grant the petition.The CAS petition was granted on March 29, 2019. Preliminary Evaluations (PE19-003 on Hyundai and PE19-004 on Kia) have been opened to assess the scope, frequency, and potential safety-related consequences of alleged defects relating to non-collision vehicle fires in the 2011-2014 Kia Optima and Sorento, 2011-2014 Hyundai Sonata and Santa Fe, and 2010-2015 Kia Soul.Attached is a list of the reference numbers for the 404 VOQs cited above.

Scope and Timeliness

NHTSA opened Recall Query (RQ) 17-004 to investigate the timeliness and scope of Hyundai?s Theta II GDI engine recalls (Recall Nos. 15V-568 and 17V-226), and Hyundai?s compliance with reporting requirements. On November 23, 2020, the Agency entered into a Consent Order with Hyundai to administratively resolve NHTSA?s assertions of violations of law in connection with RQ17-004. NHTSA?s assertions included that Hyundai may be liable for civil penalties on multiple grounds, including the untimeliness of Recall Nos. 15V-568 and 17V-226, inaccuracies in Hyundai?s recall reports, and that a required report describing potential safety-related issues contained certain inaccuracies or omissions.Under the Consent Order, Hyundai agreed to a total civil penalty of $140,000,000. The total civil penalty included payment of $54,000,000 as a Non-Deferred Amount. Hyundai also agreed to a sum of $46,000,000 as an Abeyance Amount to be deferred and held in abeyance pending its satisfactory completion, as reasonably determined by NHTSA, of the requirements of the Consent Order.Additionally, Hyundai agreed to a $40,000,000 Performance Obligation Amount to be expended by Hyundai to fulfill specified Safety Data Analytics Infrastructure obligations and Test and Inspection Laboratory obligations. The Consent Order also includes other terms designed to enhance Hyundai?s safety practices, and requires Hyundai to retain a Third-Party Auditor who directly reports to NHTSA.Other than RQ17-004, the Consent Order is not intended to affect any other current or future investigation which may involve vehicles equipped with Theta II GDI engines. In particular, Engineering Analysis (EA) 21-003 (which is upgraded from Preliminary Evaluation (PE) 19-003) continues NHTSA?s evaluation of certain performance and design issues relating to the Theta II and other engines.Nothing in the Consent Order discharges Hyundai from any obligation to comply with the Safety Act or regulations thereunder, including the obligation to carry out recalls in accordance with law, and to initiate a recall upon any future determination by Hyundai, or upon any finally adjudicated order from NHTSA, that vehicles equipped with Theta II GDI engines contain a defect related to motor vehicle safety or do not comply with an applicable FMVSS.This RQ is closed. Closure of this RQ has no bearing on the terms of the Consent Order, including Hyundai's ongoing performance obligations, and does not represent a determination by the agency as to Hyundai?s performance under the Consent Order to date.
