The contact owns a 2016 BMW X5 PHEV. The vehicle was serviced under NHTSA Campaign Number: 24V608000 (ENGINE AND ENGINE COOLING, ELECTRICAL SYSTEM), but the failure persisted. The contact stated that a month after the repairs were completed, the coolant connector would stay on and drain the battery. The contact stated that after the vehicle was turned off at night, the coolant connector overheated and the battery drained. The contact needs to jump-start the battery every day. The vehicle was taken to the dealer, and the dealer refused to repair the vehicle, stating that it was already repaired. The contact stated that the coolant connector damaged the EME (Electrical Machine Electronics). The dealer confirmed that the problem may be EME. The vehicle remained at the dealer unrepaired. The component that failed appears to be the hybrid propulsion and charging system, including the high-voltage battery management system and/or integrated charging electronics. The vehicle will not accept a charge from any home or public charging source. The high-voltage battery was replaced approximately one year ago; however, the charging failure persists. The vehicle is available for inspection upon request. In addition to the charging failure, the vehicle displayed drivetrain malfunction and start/stop malfunction warnings. While driving on the highway at normal speed, the vehicle lost power and stopped, creating a hazardous situation in active traffic. This sudden loss of propulsion placed my safety and the safety of other drivers at risk, as surrounding vehicles were traveling at highway speeds. Prior to the failure, warning messages appeared intermittently, including charging system faults and start/stop malfunction alerts. These warnings began approximately one year ago around the time the charging issue started. The vehicle was previously serviced and the high-voltage battery was replaced; however, the underlying issue was not corrected. The root cause is UNKNOWN, but the symptoms suggest a failure within the hybrid control system, charging module, DC-DC converter, or related electrical components. The failure has been persistent and unresolved. I am concerned that defects within the hybrid propulsion system may lead to additional sudden loss of power events.

During a recent auto collision my airbags did not deploy. I swerved to avoid another vehicle entering the intersection from my left, he hit my rear left driver's side quarter panel and I subsequently hit a tree. This was noted by the paramedics first on scene as well as by the firemen and police when they arrived. I have notified BMW of North America, USAA insurance (my insurance company), and now you. The vehicle is a total loss and was towed to Leesburg Virginia by roadrunner wrecker service.com

Subject: Consequential Damage and Failure of Remedy regarding Recall 24V-608 Incident Description: "I am reporting a failure of the manufacturer to provide an effective remedy for Safety Recall 24V-608 (Electric Water Pump) and subsequent consequential damage to the High-Voltage battery system. In September 2023, at approximately 78,000 miles, this vehicle exhibited 'Drivetrain Malfunctions' including Fault Code 0x222626 (EME: internal fault - active short circuit requested) and 0x22271C (Overvoltage in intermediate circuit). The BMW dealer (Fields BMW) failed to diagnose the root cause and instructed me to 'keep driving'. Since the release of Recall 24V-608, it is clear these 2023 faults were early indicators of the coolant pump short-circuiting. This persistent electrical instability has now resulted in a permanent failure of Cell Supervision Circuit #3, causing the high-voltage contactors to open and leaving the vehicle disabled. The dealer is currently attempting to charge for diagnostics and battery removal. I am asserting that the lack of a timely remedy for the coolant pump (which was only recently shielded per the recall) caused a power surge that 'fried' the internal battery circuitry. This is a safety issue as the vehicle experienced a sudden loss of propulsion while in motion. I request that NHTSA investigate BMW’s refusal to cover battery failures caused by the electrical interference of the recalled coolant pump."

Because of this water intrusion, the following safety systems have malfunctioned or failed: • ABS and DSC (stability control) warnings • Chassis stabilization warning • Frontal collision and pedestrian warning malfunction • Airbag system warning • Loss of audible warning tones • MOST bus communication failure • Electrical modules intermittently shutting down or throwing codes These failures affect braking, stability control, crash avoidance, and driver alerts, creating a significant safety risk while driving. This is not normal wear or maintenance. Many BMW owners have reported the same issue: clogged or poorly designed sunroof drains that route water directly into areas containing critical electronics. Once water reaches these modules, it causes electrical shorts and safety system failures that require expensive repairs. BMW dealerships typically diagnose this as “water damage,” but the location of the drains and the placement of the electronics indicate a design flaw that allows a simple drain issue to disable major safety systems. I am filing this complaint because the water intrusion should not be able to cause total failure of ABS, DSC, collision warning, and airbag-related systems. This is a safety-related design defect, not wear and tear. I request NHTSA review and investigate BMW sunroof drain failures and their impact on essential vehicle safety functions.

I am requesting that NHTSA investigate whether BMW is properly honoring recall 18V-755 and ensuring safe, effective remedies are provided to affected customers like myself. I was driving the car when this incident took place Please investigate this matter as it’s very concerning from a risk & safety matter

Hello, I took this vehicle in for a service at 100,000 miles. Part of that service was to change the transmission fluid. While the BMW dealership replaced the fluid, they found that the level was higher than it should have been. They voluntarily removed the transmission, and found that the electric drive buffer spring had been making extremely excessive contact with the transmission, so much so that the buffer spring had worn a hole in the transmission casing. Not a single code, or check engine light had illuminated saying there was anything wrong with any part of the transmission. The only reason this was found was because of regularly scheduled preventative maintenance. After discussing options with the BMW dealership, their recommendation was to put it back together as is, drive it to somewhere like CARMAX, and trade it in with the issue, since it was not showing any faults, codes, or errors with the transmission. The BMW dealership also stated this is not the first X5 edrive that they have seen with the exact same problem, so there is the start to a trend with this vehicle. The transmission could have failed even more catastrophically while driving at highway speed, with no indications that a long term problem was occurring within the transmission, as there is no sensor or error that is being monitored for this type of failure. Although the problem was found at 100,000, the BMW service has stated this issue must have been going on for a significant amount of time for the significant amount of damage. Thank you for your time and consideration in this matter. I am really shook up that my family was driving what I thought was a very safe vehicle with a ticking time bomb underneath it. Please please look into this. I am a helicopter pilot in the USAF, as well as Mechanical Engineer, and everything I know about vehicles and systems tells me this is a serious problem. The engine light came on while I was 250 miles from home and on a Saturday. Was able to contact my dealer's service department who said to drive home and just monitor for any issues and hopefully it will be ok. I brought the vehicle in for diagnostic check following week. Faults were for the diesel particulate filter sensor and recommended $1300. replacement. They couldn't tell me if this part would impair the driving ability of the vehicle, but the cost seemed excessive for a sensor so I haven't made the repair yet (nor driven far) pending checking more into this. They said the vehicle may go into limp mode at any time which would be a safety issue. This sensor has been on a limited warranty extension-10 years/120,000 miles for the same model/engine as mine for 2014-2015 models (mine is 2016). [XXX] with 2016 X3 added in 2019. I have to ask why the 2016 yr with a faulty sensor like mine is not part of this extended warranty?? My vehicle has 80K miles and 8 yrs old.and the dealer says the part is faulty. The electronic A/C compressor destroys itself internally on BMW hybrid and electric vehicles and should be considered defective. The A/C compressor is used to cool the cabin as well as the high voltage battery packs, and is considered an emissions component warranties for 7 years or 70,000 miles. Without a functioning A/C system the vehicle cannot charge or use the high voltage batteries. If the electric A/C compressor fails it could cause the high voltage battery system to overheat and risk of fire. I just had it replaced at 75,508 miles before it completely failed and there are plenty of other owners of BMW hybrid or electric vehicles experiencing the same.

Service Repair light is on indicating there is an issue but when you go to check there is no issue showing. I checked into the BMW service In Southampton with a complaint that the time on the dashboard wasn’t synching for daylight time savings. They said they’ll troubleshoot it. They did a hard reset with the battery and gave me the car back with service light on which wasn’t on when I drove into the service. I had the car serviced for 60k miles and they said this is a typical issue with 2016 BMW with electrical malfunction and it would cost anywhere from $200-$2500 to fix it. Now I am unable to get alerts on my vehicle. This is a big safety issue for me and my family as well as other vehicles on the road if anything goes wrong with the car, I won’t know what is the issue.

I have received two emails from two different shop foreman’s. One from Niello BMW and the other BMW Fairfield. They both said the malfunctions I am experiencing- slipping, delay in accelerations- is the way BMW manufacturer made the car. One foreman said all the cars like mine he has driven have the same issue. It is a hybrid that needs the hybrid battery to have charge to not experience these malfunctions, but the car was not made to allow me to have constant battery charge as it only lasts up to 14 miles for 2016 X5 40e hybrid. I’ve been in situations where charging is not possible- like on vacation at a hotel with no way to charge car. Due to this, I have to deal with these malfunctions that BMW says is not fixable. This has put me in situations where I’m at risk of an accident because my car won’t properly accelerate when I need it to. The approximate date I’ve given is only one situation, but has happened several times before and after.

My bmw x5 40e is having issues accelerating from a dead stop. My safety was put at risk because I was stuck in the intersection going very slowly even after put the accelerating pedal to the floor. It did not properly engage until I got to the other side of the intersection and the stoplight went back to red. It has happened several times after. BMW North America stated that they could not duplicate but the foreman actually did feel it when pulling the car up the day I was to pick it back up and recorded the issue. There were no warning lamps or faults and still do not show any faults, though it still has the same issue. BMW North America Engineer was shown a video I recorded of it in a parking garage going to 4000 rpm but speed remaining under 10 mph for proof after they returned the car to me and he still states “vehicle is operating as designed”. I noticed 2016 bmw x5 40e has a recall on it but only for those produced 7/2015-9/2015, mine was produced in 2/2016. Yet, when reading the recall my car acts very similar to the discription. This is a safety hazard because it puts me at an increased risk of a crash. I cannot get my car worked on because BMW of North America says nothing is wrong with it and won’t allow it to be fixed. We have tried talking to several people with bmw North America and they either say nothing can be done or stop communicating with us. I’ve tried communication through both email and phone.

We also have a home in Cincinnati OH. where our second car is also a 2016 BMW X5, as was our 2007 and our 2002 X5's. Our local mechanic diagnoses is a broken front axel !! We hope BMW will make a one time exception to tow our 2016 X5 to Boise ID. BMW dealership and have it repaired at no cost to us. We also own presently two Mini Coopers and had one previous Mini also. My BMW X5 suddenly will have the power steering fail, then it will start working again. This issue will happen occasionally and is dangerous. There was a recall campaign 16v-704 on January of 2017 that the BMW x5's stating the following "BMW is conducting a Voluntary Safety Recall involving certain Model Year 2015 - 2017 vehicles’ Electronic Power Steering (EPS) control unit." SOS Automotive in Covina, CA diagnosed this vehicle and said the EPS has to be replaced. Either my car was not addressed by BMW or should have been addressed and fixed. The price to fix this is over $4,000 and the mechanic said he rarely sees an EPS fail. The electric drivetrain, electric engine and the gasoline malfunctioned while the car was driving at speed from 40-50 miles. The car was build with two engine, the electric motor and a twin turbo gasoline engine. The Electronic drivetrain system was designed for first off start the electric engine will drive car until the battery is depleted then the gasoline engine will take over or some time inter exchange the drive. The problem was in cold weather regarding event with the electric battery is fully charged while driving at speed from 30 miles up to 50 miles per hours the electric engine stall for 5-10 second and the gasoline engine does not run either. As result the car was moving at speed 30-50 mile per hours with no power pushing the car at all, when the driver push on the accelerator pedal the car electric engine or the gasoline engine is not respond in 5-10 second. When driving under certain condition that needed the driver to change lane or accelerate the car out of traffic the car simply stall and not respond in 5-10 second on the road or freeway during traffic hours is serious dangerous. BMW does acknowledge of the problem and called " Electrical Machine Electronics' EME wiring harness connector warranty but not recall or admit the dangerous nor the serious of the issue. BMW simply sending out letter to car owner saying due to customer satisfaction regard EME common issue they extend the EME (Electrical Machine Electronics) to 10 years/120,0000 miles warranty. And this is not recall just an extend warranty. BMW even explained by their own word in the letter "if the issue occur your vehicle will store the corresponding fault codes that will be generated, and additionally, the malfunction Powertrain Malfunctions warning light will be on with an illuminated." I took the car to dealer, they keep the car 3 days concluded it was faulty batter cable, charging me $680, one month later same issue, they claim the car batter was the issue, replaced, same issue again

BMW’s failure to assure the functionality of its safety options beyond 5 years of use is inexcusable and unbelievable as is the claim that there is no technical fix. I am almost 80 years old and the features that I will lose are even more important to me now than they were when I purchased the car .BMW chose to put obsolete 3 g technology in the 2016 X5, on the cheap, instead of the 4g technology that had been available since 2010. BMW must have known that 3g had a limited life span since BMW should had a similar problem with older models BMW cars with 2 g that they upgraded for customers

WE WERE TRAVELING UPHILL ON A BUSY HIGHWAY IN A CONSTRUCTION ZONE AND WERE FORTUNATE TO NOT BE IN AN ACCIDENT. AFTER THE STALL A WARNING INDICATOR MESSAGE INDICATED THAT THE AUTOMATIC START/STOP MALFUNCTIONED. THE CAR WILL EVENTUALLY RESTART AFTER ABOUT 5 MINUTES WITHOUT ANY WARNING INDICATOR AT ALL, AND THE PROBLEM WILL RANDOMLY REPEAT ITSELF WITHOUT WARNING. PROPULSION DOESN'T SLOWLY STOP, IT CUTS OUT COMPLETELY WITH NO WARNING. THE BMW SERVICE DEPARTMENT HAS INDICATED THAT THERE WAS A FAILURE INTERNAL TO THE TRANSMISSION. THIS CAR IS 5 YEARS OLD AND HAS 89,000 MILES ON IT.

The affected vehicles have a Digital Motor Electronic (DME) control unit that may have a loose electrical ground cable, possibly causing an under-voltage condition which would result in the transmission shifting to neutral.

Risk: If the vehicle unexpectedly shifts to neutral, there would be an increased risk of a crash.

Remedy: BMW will notify owners, and dealers will tighten the ground cable, free of charge. The recall began February 6, 2017. Owners may contact BMW customer service at 1-800-525-7417.

If the EGR cooler leaks internally, the coolant can mix with diesel engine soot. The high EGR temperatures may result in these particles possibly smoldering and melting the intake manifold.

Risk: The melting intake manifold can increase the risk of a fire.

Remedy: BMW will notify owners, and dealers will inspect and replace the EGR cooler as necessary. If a leak has already occurred, the engine intake manifold will also be replaced. These repairs will be made free of charge. The recall began May 13, 2019. Owners may contact BMW customer service at 1-800-525-7417.

High pressure fuel pump failure

The Office of Defects Investigation (ODI) opened PE21-021 on October 14, 2021, to investigate incidents alleging a stall/loss of motive power as a result of high-pressure fuel pump failures in certain model year (MY) 2019-2020 Ram 2500, 3500, 4500, and 5500 heavy duty trucks equipped with 6.7L Cummins turbodiesel engines. During the investigation, ODI sought to determine if the related defect allegation was limited in scope to the recalled population. After review of information request response materials from both FCA and BMW, NHTSA determined that sufficient information to identify a comprehensive recall population could not be produced by FCA and BMW.During the investigation FCA filed recalls 22V406, 22E048, 22V767, and 22E087 which include vehicles not identified in the initial subject population by ODI. ODI also received recall 21V586, involving loss of motive power due to failed CP4 fuel pumps on certain BMW manufactured vehicles. An information request letter response received from BMW indicated that failed pumps on their vehicles were caused by an interaction between pump internal components and US market diesel fuel, leading to increased slip and eventual particle-generating wear surface. NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
