The panoramic sun roof shade failed on my vehicle where instead of it retracting properly it drops down onto the second row passenger's. This posses a safety risk as it impedes completely the drivers ability to see through the rear view mirror by blocking off the view of the back glass. The issue happened without any notice or noise it just happened all of a sudden I noticed it as I was not able to look through rear view mirror. This also puts at risk others as it impedes the drivers field of view allowing the possibility of an accident of occurring. I like to drive with my sunroof open very often, so now that is a risk I will have all the time I open my sunroof where I will have my field of view blocked. I have called two ford dealership to try to get quotes to repair the issue. First I was shocked with the price for the repair, but I was also shocked when I was told that it is a very common issue with the panoramic sunroof where the shade drops down. I asked do people usually get it repaired or not and they told me that most people don’t fix it that they just decide to deal with it or not use the sunroof at all. One of the dealerships told me the cost of the repair was roughly above $3 thousand dollars and the other told me it would be around $2.8 thousand dollars. A lot of labor is required as they have to bring down the headliner and remove the sunroof glass panels to be able to get it repaired. They also were not able to guarantee that after the repair that it would not happen again as they told me it was a design flaw that it was just a matter of time before it happened again. I believe Ford should be hold accountable for this and provide a permanent fix to this issue that is very common with the panoramic sun roof that creates a safety hazard for vehicle occupants and others. Especially if after customers have to pay considerable amounts of money for repairs and the issue will happen again as it is a design flaw.

On XXX, in the parking lot of O'Reilly Auto Parts (store #2460, telephone number: [XXX]) [XXX], between 12:10 pm and 12:45 pm, my [XXX] daughter was seated in the rear driver's side seat of my parked 2022 Ford Explorer when the unbuckled seat belt unexpectedly became twisted around her body and would not release through normal operation. She was unable to free herself and the belt continued to tighten as we attempted to untangle her. She experienced difficulty hearing, breathing, lost sensation in her arms and nearly lost consciousness. Multiple adults attempted to release the seat belt using the buckle and by manipulating the belt mechanism, but the belt would not release. Because of the immediate risk to her safety, the seat belt had to be cut in order to free her. The failed component was the rear center seat belt assembly. The damaged seat belt is available for inspection upon request. This malfunction placed my daughter at risk of serious injury due to restricted breathing and the inability to exit the restraint. The vehicle could not be returned to normal operation because the seat belt had to be cut to release her. At the time of the incident, there were no warning lights, messages, or other indications that there was a problem with the seat belt system. UNKNOWN whether there were any prior symptoms before the failure. The incident was witnessed by individuals who assisted in freeing my daughter. Two O Reilly employees and my [XXX] daughter who was a passenger. They assisted during the emergency and can verify what occurred. The manufacturer and local dealership have been notified, and a customer service case has been opened. At the time of this report, the manufacturer has not inspected the vehicle or confirmed the cause of the failure. Im being told my car is not covered under the recalls from the Chicago Ford plan when I have the exact same DTC codes currently showing on my car as listed in the recall and SSM. The check engine light is on with DTCs P018C and P04F0. Please have the technician specifically reference Ford Special Service Message (SSM) 51247 and inspect connector C1198 on the driver's side frame rail for water intrusion, damaged terminals, or poor pin fit before replacing any parts. Also, please check my VIN for open Recall 22C02 (Fuel Filler Tube End Form) and perform the recall if applicable. If the P04F0 is related to the recall, please diagnose and repair accordingly. combination of P018C + P04F0 is still the reason to have connector C1198 inspected.

On [XXX], at approximately [XXX] PM, my 2022 Ford Explorer suddenly lost power while driving in the left lane of [XXX] in Emporia, VA, at 70 MPH (the posted speed limit), with my [XXX] daughter. My vehicle shuddered and started to decrease in speed. I was able to maneuver safely to the shoulder, and my Explorer became inoperable requiring roadside assistance. My daughter and I remained on [XXX] for several hours due to delayed tow service availability. My Explorer has been at Battlefield Ford in Culpeper, VA since June 19, 2026, and is available for inspection upon request. Battlefield has confirmed the low-pressure fuel pump failure, confirmed this is subject to Safety Recall 25S75 (NHTSA Recall 25V455), and there is no remedy available. When getting off the interstate at an exit, my husband pushed the brakes to stop and our vehicle completely shut off. The hazard lights would not work and it would not restart. We were at a very busy exit and we had our 2 children in the car with us. We could not get it jumped or anything and were 2.5 hours away from home. The fact that we couldn't even use hazards to warn other cars was scary, as it was a very busy exit. Our car was towed to the shop and we were just told that the battery mounted mega fuses had melted and that is what caused the car to completely shut down. This is not covered by warranty and we recently had our battery checked to make sure we did not need a new battery as we figured it was getting close to time. Apparently the fuses are not typically inspected. If this is the case, why would this not be covered by a warranty? This put us in a very dangerous situation. As it could have happened on the interstate if we would have needed to brake. The police did show up to help make sure cars went around us.

Unknown! sometimes the rear back up camera goes into a black screen and then once in drive the info screens stays black for about 30 seconds. the cars shifting is hard at times especially when going from slow to fast like for instance on a highway on ramp the shifting is very sluggish. the cupholder are a real hazard my cupholder bottomed out and my drink fell thru luckily there was no liquid in the cup otherwise it could have spilled into the electrical since it is right where the gear shift is. the the car not accelarating is dangerous when you trying to get on the highway. rear camera going blank sometimes and not being able to see whats behind you even when looking around you just can not see the blindspot which the camera could if it would not go black sometimes. if the car does not shift right when it is supposed to makes it really hard to go with flow of traffic on the highway. there never have been any warning lamps, message or anything else during these incidents and it does not happen all the time but when it does it is not safe

The reverse camera started pointing straight down one day so I took it in for a recall dealing with the screen and camera. Ther replaced the camera, and did some updating and when I got it back the camera is still pointing straight down. I called them immediately and told them my camera was still pointing straight down they asked me to bring it back. I was there 5 minutes and the service manager came to me and said the camera was pointing down when I brought it in and that the reason was probably because it was an aftermarket part, which it’s not it’s all factory, and that wasn’t in the recall. "Under FMVSS 111, S5.5, my vehicle is required to display a rearview image that covers a 10-foot by 20-foot zone directly behind the rear bumper. Because the camera is pointing at the ground, it fails to provide the 'clear and reasonably unobstructed view' mandated by federal law. This is a safety non-compliance issue that Ford is obligated to rectify to bring the vehicle back into a street-legal state." • "Federal safety standards for rear visibility do not have an expiration date for manufacturing defects. A mounting system that allows a safety-critical camera to fail and point at the ground is a defect in material and workmanship that creates a safety hazard."

Recently, my sun roof shade has started to sag and no longer retracts correctly. This is causing a loss of visibility out of the rear and side windows. It also is causing a distraction as it has lost it's tension. Additionally I have contacted two separate Ford dealerships, only for them to tell me that I have to bring the vehicle in and pay hundreds of dollars to "diagnose" that the shade is broken and that my extended warranty will not cover this cost even though it is a danger for me to drive my vehicle in this condition.

After multiple attempts to fold the third-row power seat in my 2022 Ford Explorer, the seat mechanism failed completely. The seat will no longer lock securely in either the upright or folded position. The seat frame appears to have failed and the seat constantly springs back and will not stay in place. The seat was being operated normally using the factory power folding function when the failure occurred. There was no unusual load or misuse of the seat prior to the failure. The issue was inspected and diagnosed by a Ford dealership, which confirmed that the third-row seat frame had failed and would require replacement of the seat frame assembly. The dealership quoted approximately 3,622 to repair the issue. Because the seat will not lock securely in the upright or folded position, this may present a safety concern for passengers using the third-row seating area. No warning lights or messages were displayed prior to the failure.

The panoramic sunroof sunshade in my 2022 Ford Explorer failed and now sags downward. The shade hangs low enough to partially block the rear window and interferes with visibility through the rearview mirror. After researching the issue, I found many other owners reporting the same failure with the sunroof sunshade mechanism or clips. This appears to be a common defect. The reduced rear visibility while driving creates a safety hazard when checking traffic behind the vehicle.

Head lamp warning alerted (refers driver to drivers manual, only to say see a dealer) but no issues, both lamps were working. Took vehicle to Ford service center and they could not find a problem, we figured this could be an electronic issue with the warning, but Head lamp warning became more frequent again no issues. Then while driving in the dark with headlights on the automatic function, head lamps would cycle rapidly between bright and normal when approaching another vehicle…Even turning them to manual it continued to cycle Researched the issue and apparently it’s a known issue with Ford. The Automatic headlamp module can get wet and short out… this is a 1200.00 repair… the headlamp is now out, and even if not on the automatic feature it still blinks when approaching oncoming traffic… These lights are bright an when approaching on coming traffic with bright lights flashing in your eyes that becomes a safety issue… Yes, it is going in for repair however if it’s a known issue with Ford shouldn't it be a recall?

Both catalytic converters have failed, with one have a catastrophic failure, resulting in pipe becoming disconnect from the exhaust system and dropped to pavement while vehicle was traveling at interstate speed. Pipe was facing forward when it dropped. Posed severe risk to myself and other vehicles. Disabled vehicle was towed to Ford dealership where it was confirmed both converters were cracked, with the one having broken apart. Noise and vibration condition was noted and started over a year prior, however, dealer was unable to diagnose it until the full failure made the source apparent.

My front passenger get side lighting component started blinking about 6 months ago. Now it’s completely out. I went to get a new bulb and they told me I need to buy the entire lighting unit it’s $2800 at the dealership. I joined a Ford group on Facebook and hundreds of people are having the same issue. Some models newer than mine, but most from 2020 - 205 models. I reached out to Ford they won’t cover the issue.

Component/System: Low-pressure fuel pump/fuel delivery system related to Recall 25S75. Vehicle is a 2022 Ford Explorer Hybrid. Description: My vehicle is subject to Recall 25S75 involving the low-pressure fuel pump that can cause loss of fuel pressure and engine stalling. The vehicle has stalled six times between about 15,000 and 35,000 miles. The issue is intermittent but ongoing. The vehicle is available for inspection. What failed or malfunctioned: The fuel delivery system appears to lose pressure consistent with Recall 25S75, leading to engine stalling and temporary inability to restart. Safety risk: One stall occurred while driving on a highway on 9/30/23 at about 15,000 miles. The engine lost power without warning and I had to coast to a gas station. Sudden loss of propulsion in traffic creates a serious crash risk. Other stalls happened in driveways or parking lots after the vehicle had been running. In several cases the vehicle would not restart for 20–60 minutes, which could be dangerous if it occurred in traffic or an unsafe location. Reproduction or dealer confirmation: Two incidents required towing to Ford dealerships. The vehicle has been taken to Ford dealers multiple times. Earlier incidents were not linked to a recall at the time. Now that Recall 25S75 has been issued, the symptoms match. Ford has not repaired it because the remedy is still under development and parts are unavailable. Inspections: The vehicle has been inspected by Ford dealerships and towed twice by AAA for diagnosis. No permanent fix has been made because the recall remedy is not yet available. Warnings or symptoms: Stalls often occurred after the vehicle had been running. The vehicle then would not restart for 20–60 minutes. In one event there were dashboard errors. There was not always a clear warning before a stall. Reason for complaint: The recall has been open since July 2025 with no remedy available. The vehicle continues to stall. I do not feel it is safe to operate.

The panoramic shade on my 2022 Ford Explorer failed causing an inability to see clearly. The dealership did replace the faulty motors on both sides free of charge. Afterwards when attempting to close the sunroof after opening, it wouldn't close. We had to force it shut due to rainy weather. After taking to the dealership for possible repair/replacement we've been told that both motors need to be replaced and the track is broken and needs to be replaced. Cost of 3500.00 to 3900.00. I have rarely used the sunroof because I'm afraid it won't work. Now I'm stuck with a huge repair on faulty Ford equipment that has been rarely used and can't be worn out unless you installed used equipment in the first place. Shame on Ford!

My 2022 Ford Explorer had Recall 23S55 performed on June 21, 2025 to address rear axle/differential fastener fracture. Despite the recall repair, the rear subframe-to-differential cover bolt sheared off while driving, causing the differential to drop and the driveshaft to be pulled from the differential. Grease leaked and the driveshaft was gouged against the subframe. This is the same failure mode the recall was intended to prevent and occurred after the recall was certified as complete. This failure happened while I was driving with my daughter and could have resulted in loss of control if it occurred at highway speed. Ford has only offered partial cost coverage instead of treating this as a failed recall remedy. I am concerned this represents an ineffective recall repair and an ongoing safety risk.

While traveling on I-26 east near North Carolina MM 51 in the left lane, the vehicle suddenly pulled uncontrollably to the left resulting in a sideswipe of the guardrail. The vehicle then swerved to the right stopping near the center of the interstate. My wife was able to move the vehicle to the right shoulder under its own power. The right front tire/wheel was laying almost flat attached to the vehicle only by the brake line. The entire wheel assembly was detached from the steering/suspension system. The vehicle was towed to a local collision center and inspected by my insurance adjuster. The vehicle was determined a total loss. There were no injuries or other vehicles involved. The NC Highway Patrol responded but did not complete an accident report.

The fuel level indication on my 2022 Ford Explorer behaves erratically and inaccurately. The issue occurs regardless of whether the vehicle is parked uphill, or downhill, just any time I’m on an incline, and it happens even when the fuel tank is full. The fuel gauge and/or “miles to empty” reading changes unexpectedly and does not reliably reflect the actual amount of fuel in the tank. This behavior is not limited to low fuel conditions. The problem appears consistently raising concerns about inaccurate fuel level reporting and potential fuel system malfunction. The vehicle has not displayed warning lights, and the issue is resolved by refueling, but changing parking orientation does not reset or cause the fuel gauge to show accurate reading. I have had one instance of needing to call roadside assistance as it falsely read I was out of fuel.

I own a 2022 Ford Explorer purchased in April 2025. Since purchasing the vehicle, I have experienced multiple defects affecting safety and normal use. The instrument cluster becomes foggy, making it difficult to clearly see my speed while driving. The infotainment screen intermittently malfunctions and affects the backup camera display, reducing rear visibility. I also experienced fuel contamination that was confirmed and repaired by the dealership. In addition, the rear HVAC air vents close and do not provide airflow to the back seats. The vehicle has been taken to the dealership four separate times for different issues. Despite these repair attempts, the problems have continued or new issues have appeared. Due to the ongoing defects and safety concerns, I do not feel safe or confident operating this vehicle.

On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family. If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles. In view of the recall action being taken by Ford, ODI is closing this Engineering Analysis. On July 17, 2023, the Office of Defect Investigation (ODI) opened this Recall Query (RQ) after receiving 2 consumer complaints alleging loss of vehicle forward power and/or loss of transmission torque of the rear wheels in model year (MY) 2020-2022 Ford Explorer vehicles (subject vehicles). The RQ was opened to focus on the effectiveness of the remedies of Recalls 22V-255 and 23V-199 related to the alleged problem. Recall 22V-255 involved replacing the bushing and axle cover in the following subject vehicles: 3.3L FHEV Police built before October 14, 2020, and 3.3L Gas Police vehicle built before January 31, 2021. For 2.3L RWD, 3.0L PHEV, 3.3LFHEV, and 3.0LST retail (non-police) vehicles, the recall provided an PCM software update to engage the Electronic Park Brake (EPB) when Park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury. Recall 23V-199 addressed 674 of the subject vehicles produced from February 2, 2019 to April 4, 2022, which received an incorrect remedy under Recall 22V-255. The affected vehicles had received a previous Powertrain Control Module (PCM) update which did not include an automatic EPB engagement when vehicle is shifted to park. On October 17, 2023, Ford filed a new safety recall (Recall 23V-675) covering 238,364 MY 2020-2022 Explorer vehicles with the following powertrains: 3.3L FHEV Police, 3.3L Gas Police, 3.0L Gas Police, 2.3L RWD, 3.0L PHEV, 3.3L FHEV, 3.0L RWD and 3.0L AWD, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. Ford stated that the cause of the bolt fracture is due to joint design not robust to peak axle input torques and manufacturing variability. The primary contributor is insufficient bearings for maximum joint loads. This can result in bearing deformation, increasing bending stress on the bolt, which may lead to fatigue failure. On January 19, 2023, Ford amended Recall 23V-675 to add 1,942 MY 2020-2022 Explorer vehicles with the following powertrain: 3.0L RWD Platinum and King Ranch variant, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. In addition to the safety recall, Ford will conduct a customer satisfaction program for the following MY 2020-2022 Explorer vehicles with the following powertrains: 2.3L AWD, 3.3L Gas Retail, 3.0L Gas AWD Platinum and King Ranch, produced from October 22, 2018 to July 6, 2022. Ford stated that the Explorer powerpack variants not covered under Recalled 23V-675 do not present an unreasonable risk to motor vehicle safety due to the combination of lower peak torque levels and torque management strategies to share inputs between front and rear axles, reducing occurrence of higher torque. According to Ford, this is also evidenced by the lower current and projected failure rates. In view of the recall action being taken by Ford, ODI is closing this RQ. NHTSA will continue to monitor complaints from the customer satisfaction program vehicles and reopen this issue, if necessary. The agency reserves the right to take additional action if warranted by future circumstances.
