Stitching on the driver seat is tearing. Under normal wear. The vehicle is just out of the 3 year/36,000 warranty period. If the seat continues to degrade it will not be safe for future use as it worsens.

The car will occassionally lurch forward while braking. It's somewhat random but most often happens when going down a slight hill. My best guess is that there is an issue with the transition from regenerative braking to mechanical braking. It feels like the vehicle momentarily looses contact with the brakes and that the brake pedal has to be pushed down another inch or two to reengage. My concern is that if this happens at the wrong time that I will get in an accident. This has happened ever since I bought it. It's not very often, maybe once a week. Speeds have been variable from under 25 to highway speeds.

We brought our 2022 Toyota Sienna in for service January 2024 for the driver's seat. The seat would make really loud popping noises that sounded like something was loose when the seat moved forward and backward. We brought the vehicle in, and the service technician discovered that the driver's seat assembly was loose and cracking. The technician removed and replaced faulty components of the seat assembly under warranty. The seat starting having the same issues recently, and we are concerned of an on going safety issue given the instability of the driver's seat. The dealership informed us they will not repair the seat under warranty.

I was pulling out onto a main road, pressed the accelerator and "not much" happened. The car accelerated to maybe 5 mph and no faster. Took my foot off the accelerator and pushed it to the floor. No response. It was like the vehicle only ran on EV mode and did not turn the engine on. In a second or more returned to normal and could not reproduce what happened. On coming traffic had to break. There was enough room to pull out under normal situations.

I am reporting a recurring safety-related defect involving driver-assistance and steering-wheel controls on my 2022 Toyota Sienna Limited, VIN [XXX] ry 8, 2026, I brought the vehicle to Parkway Toyota in [XXX] bec [XXX] tion” warning appeared and cruise control stopped working. The dealer diagnosed the issue as related to the 12V battery, replaced the battery, and advised that the condition was resolved. The same malfunction later recurred. I returned to Parkway Toyota on March 18-19, 2026 for the same problem. I reported that the issue had reappeared after driving in cold conditions in Canada and that when the vehicle was warm there were no warning lights. The dealer diagnosed DTC U042381 (“invalid data received from instrument panel cluster”) and replaced the hybrid vehicle control ECU. I was told the repair was complete. Within about two days, the identical “LTA Malfunction” warning returned again and cruise control was again disabled. In addition, a broader failure then developed involving the steering-wheel-mounted controls: the steering-wheel buttons stopped responding, the instrument display became stuck on one screen and could not be changed, the cruise-control buttons stopped functioning, and the radio volume could no longer be changed from the steering wheel. This appears to be an intermittent but recurring defect affecting driver-assistance functions and vehicle controls while driving. The vehicle has now had two repair attempts for the same general condition, and the problem has not been resolved. Repairing dealer: Parkway Toyota, [XXX] . When purchased "As Is", an airbag deployment was disclosed but not that the front passenger side airbag had not been replaced. In December of 2025, an SRS Airbag warning appeared. Upon inspection, the mechanic discovered a "resistor" had been inserted in the SRS cirucuity to avoid disclosure of the absence of a replacement airbag. The replacement airbag was installed at my cost of $1,578.09 +8% Ohio Sales Tax. I contacted the selling dealer, Toyota of Bedford (OH), in early January 2026 but have not received a response. The other dealer disclosed in the Carfax (likely responsible for the omission of the airbag when the vehicle was repaired in July of 2022) is Suburban Toyota of Troy, MI.

While driving at about 40mph on a city street I complete lost steering, brakes and vehicle control. I suddenly started severely swerving/ spinning. I suddenly had no control over the vehicle. The vehicle threw a bunch of codes at once including: anti-lock brakes malfunction, pre-collision system malfunction, secondary collision brake system malfunction, braking power low, parking brake malfunction. I tried to turn off and restart the car and it did not reset any codes. I have brand new tires on the vehicle and just had an oil change and check up two weeks ago. There was no reason for this to happen other than a complete and sudden safety failure of apparently multiple systems. Vehicle is currently being inspected by local Toyota dealer.

The wiring harness connector for both lights has had water accumulate in the connector which causes the rear tail lights to fail. Both of my tail lights have failed, and the cause was corrosion inside the wiring harness connector. I had to replace the wiring harness and connectors on both sides, driver, and passenger And I had to pay out-of-pocket. The dealer said that this problem is common on 22 sienna's and I have seen other 22 sienna's with out their driver and passenger rear running lights burnt out.

For over a year now, I have been experiencing a random issue in my van with the brake system. It feels like a slippage when I hit the brakes to slow down and it will randomly feels like something slips and just give gives out and gets loose and like you are free falling until the brakes grab and actually stop. I have had it in the dealership twice to fix this issue. Last year they reprogrammed the brake actuator assembly control module, and it worked for a while, but then once again after several months, it started again and the second time I took it in they drove it around to try to get it to mimic the problem, but was not able to so Couldn’t do anything about it and just said to watch it and see if it became more frequent. They also said that once you have the actuator reset the computer shows updated and can’t redo it.The problem with this issue is that it is random and sometimes it’s less and sometimes more and I can’t give specific dates because of the randomness of the problem. I have never not had it to stop, but I am including my two invoices from Toyota of Richardson where I’ve had it seen as well as the research that I have looked up in different forums where other people are describing the very exact same problem with their vehicles. [XXX] . It is scary because you don’t know if it’s going to stop or not but so far it has. [XXX] . There is an issue, but don’t know how to permanently solve it or know for sure what it is. The forum does explain some different things because of the vehicle being hybrid, but who knows whether that’s it. Please look into this and advise because I do not wanna wait until this is a problem and I have a wreck because I am not able to have my brakes work Correctly. P.s. Brakes release briefly when slowing to a stop. While slowing to a stop with steady pressure on the brake pedal, the braking action will momentarily give out, resulting in the vehicle lurching forward beyond the anticipated stopping/slowing distance before the braking action re-engages. This has resulted in near collisions with cars, bikes, and pedestrians as the vehicle suddenly stops slowing without warning and unexpectedly increases stopping distance. The problem is intermittent and difficult to reproduce, but has occurred repeatedly over at least 6 months of ownership. No warning lamps, error messages, or other related symptoms from the vehicle either before or after occurrences of the problem. Has been taken to the dealership for an update to the vehicle but it didn’t fix it. There are multiple reports online and multiple lawyers talking about this.

Pre-Collision System Malfunction, Hybrid System Malfunction, AWD System Malfunction suddenly when restarting the vehicle. Towed to Toyota of Downtown LA dealership. Toyota of Downtown LA Service tells me that the CPU on my relative new Sienna 2022 was fried from jump start when Sienna starting-battery was low. It was done by a AAA professional, I doubt that is the reason for the fried CPU. I believe Toyota engineers would not design/allow the CPU vulnerable to being fried from a jump start. This is a horrific design flaw if it is this way. This happens now, just 6 months after the 3 year warranty coverage. Toyota of Downtown LA Service is telling me it is going to cost me $2900 to replace this CPU. I find this unacceptable.

The Entune 3.0 System governs navigation, calls etc. The system on my car has been replaced 4 times and still does not work. This is a life safety issue as the system grabs phone calls while driving without being able to answer. It also makes it frequently impossible to use hands free on the phone as it grabs calls. In order to avoid it grabbing calls I have to disable bluetooth on my phone which then causes my phone to not work in handsfree mode (ok google etc.) Furthermore the backup camera does not always work. Finally, a Toyota Service Manager told me in his oppinion 25% of the entune units are "bricked," non functional and need to be replaced. Toyota has fired according to him Panasonic as the supplier and now has a new provider.

I am writing to formally raise a serious and unresolved safety issue with my 2022 Toyota Sienna. The braking system on the vehicle behaves abnormally: when the brake pedal is pressed, the van does not stop immediately. In fact, it momentarily accelerates before decelerating, an extremely dangerous malfunction that has nearly led to multiple accidents. I have brought this issue to your attention several times: • In 2022, I contacted O'Regan's Toyota, the Halifax dealership, which has been unable to diagnose or repair the issue to date after several visits. • In 2023, I contacted Toyota Canada directly. I was referred to the dealership in Halifax. • The Halifax dealership has been unable to diagnose or repair the issue to date. • Despite the unresolved nature of this problem, the dealership has recently asked to charge me diagnostic fees for the same issue, one that remains unsolved and is clearly a manufacturer-level safety defect. This is not a minor inconvenience; it is a fundamental failure of the braking system and presents a critical safety hazard. The fact that this issue persists without a resolution, and that I am being asked to pay diagnostic fees for a problem that Toyota has not addressed, is unacceptable.

When decelerating, particularly on hills or into corners, there is an intermittent surge that occurs after the brake pedal has been depressed. It feels as though there is a release of the braking action, this is unsettling as if you are starting to approach a vehicle or pedestrian, this sudden "delay" of braking could result in contact with other objects. We have changed our driving behavior as a result of this. We reported this behavior at 5000 miles and there was not a solution until last year. We were not notified of this, and our vehicle is now out of warranty. Toyota has released as of last year a TSB related to programming and we are going to pay out of pocket $300 this week to get that TSB applied. This doesn't seem fair to consumers. You can see this is not an isolated problem here: [XXX] , a 2025 and 2024. In both of those vehicles, this behavior is not present. When braking, the vehicle "releases" and lurches forward at a faster rate, but then catches again and continues to brake as it was before. It does not flash or light up on the dash with any issues when this happens. We have tried to find how to specifically recreate this issue to show to the dealership, but it just happens randomly anywhere from 2-12 times a day. This is especially terrifying coming down steep hills or taking quick offramp with stopped traffic in front of you. It began about a year and a half ago and used to take place once every few weeks, it is now happening daily, with increased "release" strength and time. It is still only about 1-2 seconds of released brake pressure, but again is terrifying when behind the wheel and expecting your vehicle to continue to brake as normal while foot is on the pedal. The brake pedal does not depress further or move from its position when braking. The car is acting and performing just as it should be 'normally', except that the brakes give way for 1-2 seconds.

Unexpected sudden acceleration (when braking) as I was slowing down from exiting a highway, just as I was approaching full stop, it felt like the regenerative braking stopped grabbing momentarily and I violently lurched forward for what seemed like a feet or two. I thought it was a fluke back then and thought nothing of it. It just happened again today as I approached a stop sign - again as I neared complete stop with brake depressed it felt like the brake stopped working for a second and the car again lurched forward violently as if I had lost control for what seemed like a foot. Would have hit the car in front of me if there was one.

seat retractor failure- white plastic part off and visible , seat is making popping noise when seat is being adjusted. Noticed this when i picked up from dealer after oil change. I feel this is a driving safety concern. Larry H Miller American Toyota (Albuquerque, NM) service manager states it's safe to drive, however I feel this is a huge safety concern. The seat was in working order did not have this issue during drop off of service. However I believe this is a failure on Toyota engineering. I would like some guidance on how to get this repaired as they want me to pay $2,000.

On May 18, 2025, 4:00pm, my wife was applying brakes to stop for a stop sign at a T-intersection. Suddenly, the car lurched forward and accelerated uncontrollably. Despite her attempts to stop, her vehicle crossed four traffic lanes, vaulted over curb, nosedived into turf, and then crashed through brick wall into a fire station. My wife was unable to brake and stop the forward progression of the vehicle. My wife steered to avoid hitting trees but could not avoid the brick of the fire station wall. The airbags did NOT deploy. My wife sustained back injuries and bruising of the front torso. Her right foot is bruised from pressuring the brake pedal. The vehicle sustained heavy damage as did the fire station.

The dealer was unable to reproduce the problem. The dealer inspected the vehicle twice. They turned the rotors (unrelated to the issue) the first time I took it in. The second time I took it in, I made them apply the T-SB 0047-24 software update. The T-SB 0047-24 update says that some drivers might be "sensitive to the momentary transition from regenerative braking to hydraulic braking under certain driving conditions." The update to the software was designed to "improve the feeling" for the condition. The brakes are not slowing the vehicle predictably. There are no warning lights or errors given by the vehicle.

During braking the vehicle will lose braking force and “lurch” forward before regaining braking power. It usually happens at about 15-20mph decelerating. There are no warning lights associated with this. The dealer recommended we brake harder to prevent the issue. It still happens and the extra braking force leads to unnecessary wear on braking and suspension components. This caused us to have a near-mis accident while braking in traffic. It also causes quite a distraction during deceleration because it’s unpredictable and uncomfortable when it happens.

When coming to a stop at various speeds the car will sometimes surge during the braking process. It feels like either a brief acceleration or a release of the brakes. It then continues to brake as normal. It is an intermittent issue and can occur when coming to a slow stop or at faster stops. It started in November of 2023, and started happening only once a week or so. As time went on, it became more frequent until it was happening several times a day. By the summer of 2024, the Toyota dealership was finally able to replicate it, and they fixed it with a “computer update” as best I understood. The problem was fixed for a while but began again a few months later. The frequency at this time , early 2025, is every 1-2 days. I have returned multiple times to Toyota, and they have no fix since they have not been able to replicate it this time. I have pointed out to them that they did replicate it when they had previously fixed it. This issue presents a danger when the driver is coming to a stop. The first time it happened my reaction was to move my foot to the accelerator and I jumped a small curb. Although I am prepared now for this to happen, it is possible my instinctive reaction could happen again, resulting in a collision or injury. There are multiple forums and I see multiple complaints on this website, but I see no investigation cases. Please help me understand why this safety issue has not been investigated.

The complaint was the failure mode of a no start condition while traveling. The issue was related to partial or improper function of the DSM which caused a drain on the battery and battery cell damage requiring full replacement. The fix was a software update to prevent recurrence which cost several hundred dollars (according to the Toyota Service Bulletin or TSB-0089) along with approx $200 for diagnostic analysis. The TSB work would have been covered if we had not exceeded 36,000 miles. We had the vehicle for 3 years and 1 month. I do not understand the mileage impact to the coverage of a firmware update as this was related to a no start condition and could have occurred when we stopped at a rest stop area as we were traveling across country at the time. I am very disappointed with the failure and the impact of getting several jump starts for our vehicle as well as the couple days waiting to get scheduled into a dealership and 4 hours to get fixed.

Back seats do not lock down when folded. This almost caused my pet to get hurt when going over a speed bump sent them flying up. Seats should lock down when folded to prevent injury to pets. Also almost hurt by daughter in backseat when the single seat came up flying up and re-latched into up position from folded position hitting her arm in the process.

Vehicle will not start every single winter! Has been in the toyota repair shop multiple times. Told me it was possibly the kick sensors that are activated in the cold, stay on and then run battery dead. It is currently 20 degrees outside, but was parked in the garage all night, and STILL won't start in the morning! Completely unreliable. Nearest dealership is 1 hour away from me. Kick sensors were replaced last year, battery has been checked. Still happens. The vehicle is completely unusable and unreliable. Other issue- battery is in the back of the vehicle and you cant open the back door to access the battery to charge it! You have to get an external battery charger, charge front battery to open back door then jump back battery. If I was out and about I would have no way to do that. Absolutely unacceptable and a reoccurring problem. This needs recalled. Many others online report the same issue. Please respond.

When pulling out of a parking lot, the accelerator disables leaving the car unable to move in the middle of the rode for a few seconds. The happens intermittently and usually when there is traffic going both ways in the area. This has happened several times but has not been replicated with a dealership employee during test drive. After the accelerator disables, it will enable after a few seconds. It has left me sitting in the road with cars honking at me. Van is currently in the shop. Brake pedal was not deployed at the same time of acceleration.

On a regular basis when applying the brakes the brake pressure will release causing the vehicle to lunge forward or momentarily stop decelerating. This release of the brake pressure is only for a second but during that time the van will stop or slow decelerating and if you are slow when this happens you will feel the van lunge forward. It has even happened at a stop with the brake applied and the van would move slightly forward as the pressure released and then reapplied. It almost feels like a computer signal is being reset when this happens causing the momentary release of pressure.

When engaging the cruise control, the vehicle continued to accelerate even after I let off the button. During a trip with this vehicle on September 14, 2024, this vehicle did this twice. I have never driven another vehicle where this occurs. If this is supposed to be a "feature", it's unsafe and should not be legal. If it remains legal, there should at least be some kind of safety sticker.

I have a 2022 Toyota Sienna and have noticed that when I am braking (especially at low speeds while stopping at a STOP sign or a Red Light),the car "lurches" forward as if the car is accelerating a little bit. I have to slam the brakes to stop the vehicle. It feels really dangerous - it might hit a car in front or a kid in a neighborhood! I will reading through Toyota Sienna forums and found scores of similar incidents reported! Hope Toyota takes notice and fixes this.

Note: I have filed a complaint with the Florida Attorney General's Office Health Hazard. Mold Issue. Flooding Issue. Failing weather striping. We had a weather strip on the rear door get misplaced or is failing to stay on our 2022 Sienna. Toyota won't cover it. And we will have to make a claim per their recommendation, unsure. Third seat rusted; it was submerged in water. Didn't smell it or see it. 20k in damages. We never use it for anything except for groceries in the stroller. The worst decision I have made is bringing it to Toyota, because with it closed up, in the heat, the Van now has as all this mold. Before it was just wet in the 3rd seat compartment. It also had a Frog in it as large as a small hand. Improper storage. Toyota corporation also won't help. Don't be fooled and think this vehicle is imperfect. The new and previous generations have had this problem. They leak from the top and bottom of this door seal. Toyota seams to refuse to make a TSB. You do not have to look far online to find the reported issues. If it can happen to normal people like us doing normal things and many other people like us reporting it, it can and will happen to anyone. A Vehicle less than 3 years old should not be leaking. I have spoken online to other owners experiencing the same. But now since I see Toyota will not assist, and made it worse for us by improperly storing the Van and taking too long to respond. Now we cannot use it and it is a health hazard. Instead of helping me, they instruct me to file a insurance claim I would otherwise not have had to file, if I just air dried the Van. They should have acknowledged the problem and taken the opportunity to improve the design. Not create a mold situation for the customer and tell them to file a claim. This is a quality control issue. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Sienna vehicles equipped with seating for eight passengers. An incorrect seat belt webbing guide may have been installed on the outboard second-row seat belt assemblies.
Risk: During seat belt pre-tensioner deployment, the incorrect webbing guide may damage the seat belt webbing, causing it to tear and increasing the risk of injury during a crash.
Remedy: Dealers will replace the outboard second-row seat belt assemblies, free of charge. Owner notification letters were mailed January 10, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 21TB08 and 21TA08.

Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."
Risk: Driving with a deactivated stability control system increases the risk of a crash.
Remedy: Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.
