On May 13, 2026, while traveling at approximately 31 mph under normal driving conditions, a critical right-front suspension or steering component suddenly suffered a catastrophic structural failure on its own. This spontaneous mechanical failure caused the vehicle to immediately and violently veer to the right, rendering the steering ineffective and resulting in an unavoidable collision with two parked vehicles. The vehicle has been determined to be a total loss. Furthermore, during the crash event, the front passenger-side seat-mounted side airbag and side curtain airbags deployed despite the front passenger seat being completely unoccupied. Conversely, the driver's front steering wheel airbag failed to deploy completely during the impact. A corporate inspection by Toyota Motor North America/Engineering Analysis Associates confirmed severe, localized separation of the right front strut, knuckle, tie rod end, brake line, and ABS wiring. Toyota is currently utilizing its own interpretation of the Event Data Recorder (EDR) steering/speed metrics to deny component defect liability, attributing the structural suspension failure to post-crash forces rather than pre-crash causation. I am filing this report to log a potential structural component defect in the 2016 Lexus ES 350 front suspension/steering assembly, as well as an irregular Supplemental Restraint System (SRS) deployment pattern, which presents an unreasonable safety risk to drivers.

The brake actuator and/or brake booster pump assembly failed on my 2016 Lexus ES300h. This is a known issues for this vehicle's braking system and results in a sudden loss of power braking assistance. The failure requires increased pedal pressure and eventually brkaing failure, creating a significant safety hazard and increasing the risk of collision. The dashboard displays multiple warning lights, including Braking Power Low and Brake Override Malfunction. Lexus has acknowledged this defect through an extended warranty and consumer warranty program, but Lexus is alleging my vehicle (despite showing issues prior to 150,000 miles) is now over 150,000 miles and not eligible. I am reporting this because a critical safety component should not have a "timer" on it when it involves primary braking of the vehicle.

While driving at highway speeds on Interstate and without warning my standard size sunroof exploded. This event caused loud noise and shattered glass. I was not under overpass or around any other vehicles. This event caused immediate panic due to noise and not understanding what occurred until a visual inspection was conducted roadside. The vehicle has been inspected by dealership for repair resulting in $2800 repair utilizing insurance.

On 2/3/2026 i was sitting at a light and out of no where i heard a bang or what sounded like a explosion i get out of the car and my panoramic sunroof is completely shattered but it looked like it exploded from the inside out. My sunroof was closed the cover was shit and i have no idea what would have caused it. I asked the driver that was directly behind me at the light if they saw anything hit the car they said no nothing i dont understand how this happens and the dealership says they wont warranty it i dont know what to do

In April 2022, the timing cover seal on my 2016 Lexus ES 350 was replaced due to a leak. This was completed under warranty at approximately 84,785 miles. In February 2025, less than three years and roughly 56,000 miles later, I was informed by the same Lexus dealership during a routine service that the exact same issue—leaking from the timing cover—had reoccurred. There were no warning lights, engine performance issues, or visible signs of oil loss leading up to this. During my prior service appointment in December 2024, I was told the car was in excellent condition with no issues. I service my vehicle at every Lexus-recommended interval and maintain a full history of service records. The dealership has confirmed the new timing cover leak but has not been able to explain why the seal failed again. This is a major engine component that should not require a second repair within such a short time frame. I am now being asked to pay a significant portion of a nearly $5,000 repair despite a prior warranty fix, and despite having done nothing to cause or accelerate the failure. Lexus offered a partial goodwill credit, but no root cause has been identified. I am concerned that either poor workmanship or substandard parts contributed to the premature failure, and that it may happen again. Given the cost, critical nature of the part, and the lack of answers, this feels like more than just an isolated incident. I have complete service documentation and before-and-after video footage of the most recent repair. The vehicle is currently in my possession and available for inspection upon request. I purchased a Lexus because I believed in the brand’s commitment to long-term reliability and quality. That confidence has been shaken. I respectfully ask NHTSA to investigate this recurring failure and assess whether this is part of a larger issue.

The contact stated while at a stop sign, the brake pedal was depressed; however, the brakes failed to engage and the vehicle unintentionally accelerated which caused the front driver's side of the vehicle to crash into another vehicle. There was severe cosmetic damage however, no injuries was sustained. The air bags was deployed. No warning light was illuminated. The vehicle was towed to an independent mechanic where it was diagnosed that the vehicle was totaled. A police report was filed. The manufacturer was not notified of the failure. The failure mileage was approximately 131,000.

The contact owns a 2016 Lexus ES350 equipped with General Tires, Tire Line: ReliaTrek, Tire Size: 225/45/R18, DOT Number: VY4FWJ85. The contact stated that while driving 60 MPH, the vehicle was vibrating abnormally. The contact pulled over and called Roadside Assistance. The Roadside Assistance technician made the contact aware of a blowout of the driver's side rear tire. The contact was shown that the entire sidewall had detached from the tread. The spare tire was installed by Roadside Assistance. The contact stated that the tires had recently been purchased by the previous owner however, the contact was unsure of the failure mileage on the tires. The contact had not taken the vehicle to a dealer or independent mechanic. The failure was not diagnosed nor was the tire replaced. The manufacturer was not made aware of the failure. The approximate vehicle failure mileage was 100,000.

2016 Lexus ES350 with the 2nd row sunroof. While driving the car the front sunroof shattered without cause, sending glass pieces into the cabin creating a very dangerous situation. This appears to be a problem that has happened many times before as there are many people that have talked about this issue. Lexus denied fault.

Effective in 2022 all cellular carriers are upgrading to 5G. With this transition a majority of vehicles will no longer be able to use their vehicles emergency crash response systems. In my 2016 Lexus ES350 the Emergency SOS system utilizes 3G cellular service which will be turned off in 2022 making an automated call in the event of a crash impossible. The DOT needs to block this measure as owners of vehicles should not be forced to purchase a newer vehicle simply for emergency services.

The vehicle started experiencing the vaccum leak in the master brake cylinder/actuator/booster where the booster pump would run continually every 15 seconds causing the danger to brake failure. Dealer was contacted but refused to help. Eventhough toyota prius hybrids and newer and some other toyota that share the same design brake booster and master cylinder has been recalled previously. It is common failing component but dealer does not provide any assistance leaving vehicle owners pay thousands to replace abs brake components and bleed the system. Need your help!

At 1 p.m. July 5, 2021, I was driving on the expressway on a short trip from home (about 5 miles). Temperature outside was 83 degrees, inside car 70 degrees. No overpasses, nearby cars, or debris noted from roadway. We heard a loud explosion. Seeing no other vehicles, feeling nothing wrong with the tires, or broken mirrors, I cracked open the moon roof slightly and noticed shattered glass. Fortunately, we had the interior moon roof flap of the vehicle closed which prevented the shards from falling on us. There was no warning that this would happen. As it is a holiday, I am awaiting a response from both the insurance company and the dealership. You will note the "upward" explosion that would not be typical of an object coming from the road. We noted nothing from the road or sky (partly cloudy day).

BEGAN PUMPING ON BRAKES AND STILL NOTHING. STEERED CAR AWAY FROM BRAKING VEHICLES IN FRONT OF ME AND VEERED CAR INTO MEDIAN WALL TO AVOID REAR END COLLISION. . SEVERE DAMAGE TO CAR, BUT NO AIR BAG DEPLOYMENT AND AVOIDED HITTING ANY OTHER CARS. I'VE HAD ABS ON MULTIPLE TOYOTA AND LEXUS VEHICLES IN THE PAST, I HAVE NEVER EXPERIENCED ANYTHING LIKE THIS. THERE WAS NO BRAKING. I HAVE PICTURES OF THE CAR, BUT THE DAMAGE WAS DUE TO HITTING THE INTERIOR HIGHWAY WALL. THIS IS A CERTIFIED LEXUS VEHICLE THAT I PURCHASED JUST OVER ONE YEAR AGO (12/31/19). I JUST HAD MY SECOND SIX MONTH CHECK UP COVERED BY LEXUS UNDER WARRANTY. MILEAGE IS APPROXIMATELY 36K MILES.

THE RADAR CRUISE CONTROL WAS SET TO 75 MPH, AND I WAS DRIVING IN LEFTMOST PASSING LANE BEHIND A CAR GOING ABOUT 65 MPH, WHICH KEPT MY SPEED ALSO AT 65 MPH. THE CAR DIRECTLY IN FRONT OF ME MOVED TO AN OPEN LANE TO THE RIGHT, WITH CLEAR PATH IN FRONT OF MY VEHICLE TO ALLOW ACCELERATION UP TO 75 MPH. NOTE: IT WAS A CLEAR DAY, SLIGHTLY CLOUDY, ROAD WAS FLAT, NO CURVES. INSTEAD OF ACCELERATING MY VEHICLE SUDDENLY INSTEAD APPLIED THE BRAKES, THE ACCELERATOR PEDAL NOT RESPONSIVE TO ANYTHING I DID TO INCREASE OR MAINTAIN SPEED, WITH VEHICLE QUICKLY COMING TO A STOP AS I MOVED INTO THE GRASS MEDIAN AREA SEPARATING THE FREEWAY BEFORE CAR DIED IN THE PASSING LANE. COLLISIONS WAS BARELY AVOIDED AS PICKUP TRUCK BEHIND ME SLAMMED ON THEIR BRAKES AND VEERED INTO MEDIAN GRASS TO AVOID HITTING ME OR GETTING REAR ENDED THEMSELVES FROM BEHIND. MY CAR CAME TO A COMPLETE STOP EVEN THOUGH ENTIRE TIME I HAD ACCELERATOR DEPRESSED. ONCE STOPPED I EXITED THE CAR, TALKED TO THE OTHER DRIVER, WHO INFORMED ME MY BRAKE LIGHTS WERE ON DURING THE ENTIRE INCIDENT. I WENT BACK TO THE CAR WHICH WAS STILL RUNNING, CAR ONCE AGAIN RESPONDED TO ACCELERATOR BEING DEPRESSED, AND WE CONTINUED OUR JOURNEY. IT APPEARS THE PRE COLLISION SYSTEM WHICH THIS CAR HAD TURNED ON AT THE TIME IMPROPERLY ENGAGED, EVEN THOUGH NOTHING WAS IN FRONT TO HAVE CAUSED PROPER ACTIVATION. WE WERE LUCKY, WE COULD HAVE EASILY BEEN REAR ENDED BY A SEMI-TRUCK AS THEY HEAVILY TRAVEL THIS FREEWAY, OR POSSIBLY BEEN IN A LOCATION WHERE THERE WAS NOT A GOOD SHOULDER TO ALLOW THE QUICK STOP WITH MINIMAL IMPACT TO TRAFFIC GOING AT HIGH SPEED.

SAFELITE REPLACED IT. 6 DAYS LATER, DRIVING DOWN THE HIGHWAY AND IT CRACKED IN THE SAME PLACE STARTING FROM THE BLACK BORDER. NO ROCKS HIT IT. I HAVE BEEN DRIVING 35 YEARS AND NEVER REPLACED A WINDSHIELD. NOW 2 IN 6 DAYS. SOMETHING ISN'T RIGHT LEXUS.

I HAVE A LEXUS 300 ES HYBRID THAT ADVERTISES 39/40 MPG. I HAVE BEEN GETTING IN THE UPPER 28-31 MPG FOR 2 AND A HALF YEARS NOW .WHEN I FIRST RECEIVED THE VEHICLE IN THE FIRST MONTH I COMPLAINED ABOUT THIS GAS MILEAGE PROBLEM. THEY TOLD ME THE VEHICLE NEEDED TO BREAK IN FOR 500 MILES. AFTER THAT I CONTACTED LEXUS AGAIN, THEY SAID THE CAR NEEDED A 2000 MILE BREAK IN . I KEEP GETTING MORE AND MORE EXCUSES AND THE RUN AROUND. THEN THEY TOLD ME IT WAS MY DRIVING HABIT. I HAVE A CLASS A COMMERCIAL DRIVERS LICENSE AND HAVE BEEN DRIVING BIG RIGS FOR OVER 15 YEARS . I NOW HOW TO DRIVE AND I HAVE A VERY GOOD DRIVING RECORD WITH NO INCIDENTS TO DATE. SO THAT EXCUSE WAS ANOTHER PASS-THE-BUCK ROUTINE FROM LEXUS. I LET THIS GO FOR SUCH A LONG TIME, SINCE I WAS DISCOURAGED AND VERY DISAPPOINTED IN THE HORRIBLE CUSTOMER SERVICE AND ACCOUNTABILITY FROM LEXUS. THIS WAS JUST TOO MUCH TO UNDERSTAND. I HAD A LEXUS 200 CT PRE PAID LEASE THAT GAVE ME VERY GOOD MPG. I WANTED A LARGER VEHICLE SO AFTER SEEING THE MARONEY 39/40 MPG BOASTED ON THE LEXUS 300 ES HYBRID AND ON MANY NATIONAL ADVERTISEMENTS, I DECIDED TO TURN MY CT 200 IN SIX MONTHS EARLY AT A $2000 PRE PAYMENT LOSS TO ME. BECAUSE THE VEHICLE HAS NEVER GIVEN ME ANYTHING CLOSE TO THE MILEAGE CLAIMED , NEVER HAVE I HAD 35 + MILES ON THIS VEHICLE AND MOST OF THE TIME ALWAYS EXCLUSIVELY IN THE 29-31 MPG RANGE. THIS IS NOT A PROBLEM I CAUSED. THIS IS A PROBLEM LEXUS CAUSED. I HAVE BEEN CHEATED AND DECEIVED AND IGNORED, TOLD THERE IS NOTHING THEY CAN DO . TAKATA RECALL: I WAS CALLED BY LEXUS TO SAY THERE WAS A RECALL 2 MONTHS AGO. I MADE AN APPOINTMENT AND LEFT MY CAR THERE. THEY GAVE ME A LOANER. THEY CALLED AND APOLOGIZED THEY DIDN'T HAVE THE REPLACEMENT PART. THEY SAID THEY WOULD CALL ME WHEN THEY DID AND I RETURNED THE LOANER AND TOOK MY CAR BACK. THEN A MONTH LATER HEY CALLED AND SCHEDULED AN APPOINTMENT AGAIN FOR THE SAME REASON. I ASKED THEM TO CHECK IF THEY HAD THE PART, AND THEY CALLED ME BACK AN HOUR LATER SAYING 'OOPS WE STILL DON'T HAVE THE PART'. THIS IS NOT ACCEPTABLE. I AM DRIVING AN UNSAFE CAR FOR OVER 2 MONTHS AND AM WORRIED I AM AT RISK.

THIS CAR ALSO HAS HAD NUMEROUS RANDOM AND OCCASIONAL ELECTRICAL PROBLEMS, MIRRORS MOVING ON THEIR OWN, GARAGE OPENER NOT WORKING, SEAT MEMORY NOT WORKING, ETC. SHOP CAN'T DUPLICATE THEM AND THEREFORE CANNOT FIX. LEXUS DOES NOT RESPOND. WORRY IS THAT ENGINE CUT-OFF MIGHT HAPPEN AGAIN, AND ALSO WORRY THAT THERE IS AN OVERALL ELECTRICAL PROBLEM THAT AFFECTS MAJOR SYSTEMS, INCLUDING ENGINE OPERATION.

WHILE PLACING THE STICK SHIFT IN APPROPRIATE POSITION, THE TOP OF THE SHIFT KNOW BEGAN TO SPIN AROUND, CAUSING DRIVER TO LOSE GRIP OF STICK. THIS OCCURRED INITIALLY WITH VEHICLE STOPPED AND ATTEMPTING TO PUT VEHICLE IN PARK. UPON CONTACT WITH DEALERSHIP (LEN STOLER, LEXUS OF TOWSON) AND BRINGING THE CAR IN FOR INSPECTION, TWO SERVICE REPS AND A SERVICE TECHNICIAN AGREED THAT THE KNOB SHOULD NOT SPIN. THE TECHNICIAN STATED THAT IT WAS A POTENTIAL SAFETY ISSUE. AFTER DROPPING THE CAR OFF, THE DEALERSHIP CONTACTED ME TO STATE THAT THIS WAS NORMAL. IF SO, THIS IS A BAD DESIGN. WHY WOULD ANYONE WANT THE TOP PART OF THE STICK SHIF TO BE LOOSE AND SPIN AROUND? THE CHEAP DESIGN IS A DEFECT AND SHOULD BE CORRECTED.

I WAS DRIVING THE CAR ON THE FREEWAY WITH THE CRUISE CONTROL ON WHEN IT STARTED TO RAPIDLY DECELERATE AND PULL BACK TO A STANDSTILL. I PUSHED THE ACCELERATOR BUT NOTHING WOULD STOP THE DECELERATION. I HAD TO PULL OVER TO THE SIDE OF THE FREEWAY SINCE THE CAR EVENTUALLY STOPPED AND PUSHED THE BUTTON TO TURN OFF THE CRUISE CONTROL. AFTER A FEW MINUTES, I FELLT THAT I COULD REENTER TRAFFIC AND WAS ABLE TO DO SO BUT WITHOUT THE CRUISE COUNTROL ENGAGED. I TOOK IT INTO THE DEALER WHO INDICATED THAT THEY COULD FIND NOTHING. THIS WAS A VERY DANGEROUS SITUATION AND I COULD HAVE BEEN REAR ENDED OR GONE OFF THE ROAD. IF THIS HAD HAPPENED AT NIGHT THERE COULD HAVE BEEN VERY BAD CONSEQUENCES.

TELLS ME....... "WE DON'T HAVE THE PARTS AND WE WILL CALL YOU WHEN WE GET THE PARTS." I AM DRIVING A BRAND NEW CAR THAT DOES NOT COMPLY WITH A PORTION OF FMVSS 126, LOSS OF VEHICLE STABILITY CONTROL, INCREASING THE RISK OF A CRASH. IT IS NOW MID AUGUST 2016.
