I am submitting this complaint to report an unresolved safety recall and the inability to obtain a timely inspection or repair, which I believe places me and my family at increased risk of a crash. My vehicle is a 2015 Ford Transit Connect. The vehicle is subject to Ford Safety Recall 25S89 and NHTSA Recall 25V572, concerning a failure of the rearview camera system. The rearview camera on my vehicle frequently displays a message stating “Rear View Camera Unavailable, Contact Ford Dealership” and then goes to a black screen. This results in a complete loss of rear visibility while backing up, increasing the risk of a collision, especially involving pedestrians or other vehicles. I received the safety recall notice in September 2025. Since then, I have made numerous attempts to have the vehicle inspected and repaired. Despite repeated contact with my local dealership, has informed me that they cannot perform the inspection or recall repair because they do not have an SSCC repair procedure available and therefore cannot proceed. I have also contacted Ford Motor Company customer service by phone multiple times, but these calls have not resulted in a resolution or a clear path to getting the vehicle repaired. As a result, I am left operating a vehicle with a known safety defect and no access to the recall remedy. I am deeply concerned for my safety and the safety of my family. Operating this vehicle without a reliable rearview camera significantly increases the chance of a crash, and I believe the continued delay in providing a remedy defeats the purpose of a safety recall. I respectfully request that NHTSA document this complaint and take any appropriate action to ensure that Ford Motor Company provides a timely inspection and repair for this safety defect.

The rear passenger side sliding door fell off the track and fell off the vehicle, while the door was being closed. I originally reported difficulty to the dealership shortly after purchasing the vehicle around 2016. I asked the dealership on multiple occasions to fix the door sliding issue. I was told there was nothing they could do to fix it. The door was always difficult to open and close. 6/25/24, the door fell off the vehicle while it was being closed.

The detent designed to hold the passenger side sliding door open has malfunctioned due to what the dealership describes as a manufacturing defect commonly found in the 2015 Ford Transit Connect XLT/LWB. Dealer states dealer is unable to correct. When parked on an incline, the sliding door can violently slide forward with a significant potential for injury to a passenger, especially a young child, entering or exiting the vehicle. Dealer suggests taking the vehicle to a body shop to correct this defect at owner's expense.

Service BrakesMar 2023 · ODI 11509886Details +Close −
2015 Ford transit connect 150: while driving on the freeway 60-65 mph, the brakes seem to lock up without depressing the pedal, slowing to 40 mph and shaking uncontrollably. putting on the brake to exit the flow of traffic, braking caused the vehicle to shake even more finally by pumping the accelerator, the speed increased again. finally getting off of the freeway the brake pedal went to the floor. i had to pump the brake pedal until it finally reacted. less than a week later, getting on the freeway almost the exact thing happened. luckily i was a bit more prepared when i felt the shaking. today, driving the van back to our warehouse approx 15 miles on regular streets (not freeway driving) the brakes again went to the floor. master cylinder not leaking, brakes seem to be in good shape...but something is wrong we brought the vehicle to Sawgrass Ford in Sunrise Florida. they had it for over a month. they were unable to determine the problem and only made suggestions to completely change out all the brake components for $5-$6,000.00 but also said that they could not guarantee that this would fix the problem. the brakes were serviced approximately 6-7 months prior to these incidents I called Corporate Ford office. was told that the 2016 ford transit connect had Brake Hydrolic Control Unit recalls, but the 2015 doesn't have the recall listed. I also looking on manywebsites to verify any other2015/2016 problems with the brakes "locking up" or vehicle going into safe mode (another possibility) there were numerous complaints and concerns regarding 2016 & even 2017 situations very similar to what i went through. ford told me that model DV6Z2C405D BRAKE HYDROLIC CONTROL UNIT did not have an official recall. but that Ford stopped using that model and started using model EK4Z2C405A which was recalled and then replaced the recall with FK4Z2C405C. Seems like there may have been a problem with the 2015 model which caused the to change and with the 2016 model as well

The fuse associated with the rearview camera of this vehicle has consistently blown over the last few years leaving the backup camera inoperable with no image on the dashboard display. The lack of a functioning backup camera results in the loss of a view directly behind the vehicle which could result in an accident. A warning stating that "rear view camera is unavailable" and a dark display occur when the fuse is blown. The rearview camera fuse blows often typically only after a few vehicle trips. After diagnosing the blown fuse issue, chafing of a wiring harness was found behind a grommet where wires pass between the vehicle body and the rear liftgate. The damaged wiring harness is in the vehicle and available for inspection. This issue has also been documented by others as was discovered by searching for information on the internet.

THIS WEEK TOOK THE VAN TO THE SAME MECHANIC AFTER HEARING THE SAME ROARING IN THE FRONT, REPLACED BEARINGS IN THE RIGHT FRONT WHEEL. VAN HAS ONLY 30000 MILES. DOESNT SEEM REASONABLE THAT THIS SHOULD HAPPEN TO BOTH WHEELS, INSTALLKED INCORRECTLY OR NOT LUBRICATED CORRECTLY AT THE FACTORY?

WAL-MART ALPHARETTA, GA INSTALLED THE WRONG TIRES ON THE FRONT OF MY 2015 FORD TRANSIT CONNECT IN AUGUST 2017. STARKE, FL WAL-MART COULD NOT BALANCE THE TIRES DUE TO WRONG LOAD LIMIT HAVING BEEN INSTALLED, BUBBLE ON DRIVER'S SIDE THAT COULD BLOW AT ANY MINUTE & TIRES HAVING BEEN COUNTER-BALANCED WHEN INSTALLED. I HAVE CONTACTED ALPHARETTA WAL-MART, PROVIDED ALL NEEDED INFO, ASKED THAT THEY MAKE ARRANGEMENTS WITH STARKE, FL WAL-MART TO INSTALL 2 NEW PROPER LOAD TIRES AT NO COST TO ME. ASSISTANT MANAGER TONY WAS TO CONTACT ME THE FOLLOWING DAY AFTER WAL-MART WAS ADVISED OF THE PROBLEM. I ATTEMPTED 3 TIMES TO FAX THE INFO BUT THEIR FAX MACHINE WOULD NOT WORK. I MAILED ALL DOCUMENTATION TO ASSURE THEIR NOTIFICATION. I HAVE NOT HEARD FROM ANYONE. I AM AFRAID TO DRIVE. IF AN ACCIDENT OCCURS, THIS WILL IMMEDIATELY BECOME A MUCH LARGER PROBLEM THAN IT IS NOW. PLEASE DO SOMETHING TO MAKE WAL-MART CORRECT THIS PROBLEM RIGHT AWAY.

AUTOMAX FORD KILLEEN INSTALLED RECALL 16S30 ON 6-16-17. ON 7-22-18, DOOR LATCH FAILED IN MANNER DESCRIBED BY RECALL. TOOK VAN INTO AUTOMAX ON 7-25, ON 7-31 WAS TOLD RECALL PART HAD FAILED, ON 8-1 TOLD I WOULD NEED TO PAY FOR REPAIR. I THOUGHT PAYING FOR DUPLICATE FAILURE OF RECALL ISSUE WAS WRONG AND CONTACTED FORD NATIONAL OFFICE FOR HELP. FORD CORP. RESPONSE WAS FAVORABLE BUT NOT DEFINITELY COVERING COST UNTIL DEALER RESPONSE. ON 8-6 RECEIVED FINAL APPROVAL THAT FORD CORP. WOULD PAY DEALER. ON 8-8 REPAIR PICKED UP CAR AFTER REPAIR COMPLETED, ALSO PERSONALLY PAID FOR 2ND PART FOR PASSENGER SIDE (SINCE IT WAS LIKELY TO FAIL TOO IF DRIVER SIDE RECALL PART FAILED & BOTH WERE REPLACED DURING 2017 REPAIR). DURING PICKUP, NOTICED THAT FAILED PART THAT HAD BEEN REMOVED HAD A 07.10.2014 MFG DATE STICKER. SINCE RECALL WASN'T ISSUED UNTIL 2016, THIS PART COULD NOT BE A FAILED RECALL PART BUT WAS ACTUALLY AN ORIGINAL PART FROM MY VANS MFG IN LATE 2014. DEALER SUGGESTED THIS WAS NOT THE CASE BUT WOULD INVESTIGATE AND ALLOWED ME TO RETURN HOME WITH ONLY DRIVERS SIDE REPAIRED AND PASSENGER SIDE UNTOUCHED. RETURNED HOME AND PULLED DOOR TRIM ON PASSENGER SIDE, FOUND SEAL INSIDE DOOR UNTOUCHED AND DATED 2014 ALSO. COULD NOT HAVE PERFORMED RECALL IN 2017 WITHOUT BREAKING SEAL. RETURNED 8-9 WITH PROOF AND FINALLY GOT PASSENGER SIDE REPAIRED USING PART I PAID FOR. PROMISED REFUND BUT NOT RECEIVED UNTIL 8-21 AND AMOUNT SLIGHTLY LESS THAN PAID DUE TO TAX NOT INCLUDED. EXPRESSED CONCERN THAT OTHER CUSTOMERS MAY ALSO HAVE VANS WITH RECALL DOCUMENTED BUT NOT ACTUALLY REPAIRED. ASSURED DEALER WOULD TAKE ACTION, BUT NEEDED TIME TO DETERMINE HOW TO FIX FOR OTHERS AND LET ME KNOW. ON 10-4 FOLLOWED UP AND DEALER STATED THERE WAS NO PLAN AND SINCE MY VEHICLE WAS FIXED, DANGER TO OTHERS WAS NOT MY WORRY. SUMMARY: DEALER DID NOT PUT IN RECALL PARTS, CHARGED FOR FIX, AND MAY HAVE DONE IT TO OTHERS.

Investigations
2 NHTSA investigations on record
Investigations are agency reviews, not findings that a vehicle is defective.
In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below. On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge. On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). According to Ford, spring tab failure rates for vehicles covered by this action are about half that of the 20V-177 vehicles. Under this action, dealers will replace all door latches, free of charge, if either; a) the vehicle exhibits a related latch concern, or b) if the vehicle owner expresses a concern about the latches in their vehicles. Details can be found under NHTSA TSB No. 10190990, a copy of which is available in this investigative file available at NHTSA.gov.On June 8, 2020, Ford submitted Safety Recall 20V-331 (Ford 20S30) addressing vehicles previously remedied under NHTSA Recalls 15V-246, 16V-643 and 17V-210, where the repair may not have been properly completed, specifically not all of the door latches were replaced, or replaced correctly. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). The three ODI injuries were all relatively minor. Ford?s reported injury incidences include one (unconfirmed) report of an ejection, three minor injuries with the others involving undisclosed injuries. Ford?s crashes involved two (unconfirmed) crashes alleging the incident door contacted another vehicle and three involving doors striking other property. The recalled vehicles include MY 2011-2015 Ford Fiesta, 2013-2016 Ford Fusion and Lincoln MKZ, 2015 Ford Mustang and Lincoln MKC, 2013-2015 Ford Escape and Ford C-MAX, 2012-2015 Ford Focus and 2014-2016 Ford Transit Connect.Ford's actions address ODI's concerns at this time. ODI will take further action if warranted by new information. The reference (OD
In January, the Office of Defects Investigation (ODI) opened this Preliminary Evaluation based on consumers' allegations of doors failing to latch and/or inadvertently opening on model year (MY) 2012-2013 Ford Focus vehicles (subject vehicles).ODI has received 464 unique reports related to the alleged problem, with over 200 claiming that the door(s) opened inadvertently while the vehicle was in motion. One consumer alleged an injury that occurred while driving when a door inadvertently opened and then closed on a child's arm.Another alleged that a door that inadvertently opened struck a parked vehicle. In response to ODI's information request, Ford Motor Company (Ford) provided 500 unique reports related to the alleged problem on the subject vehicles.Ford reported no injury or accident claims.Ford also submitted 1,513 warranty claims related to door latch failures. Many customers indicated that the affected door could not be latched at all and some of them decided to restrain the door by other means in order to operate the vehicle. Some customers also reported door latch failures occurring on multiple doors and at different times.Additionally, Ford stated that MY 2014-2015 Ford Focus, 2013-2015 Ford Escape, 2013-2015 Ford C-Max, 2014-2016 Ford Transit Connect, 2015 Ford Mustang, and 2015 Lincoln MKC, all use the same door latch as the 2012-2013 Ford Focus. On September 6, 2016, Ford notified ODI that it would conduct a safety recall (16V-643) to remedy the door latches in the MY 2012-2015 Ford Focus and other Ford models mentioned above (total of approximately 2 million vehicles).According to Ford, a component within the door latch may break preventing the door from latching.Ford dealers will replace all four door latches in the recalled vehicles with a new more robust latch design that is currently in production. The recall action initiated by the manufacturer appears to address the safety risks.Accordingly, this investigation is closed. The ODI reports cited above can be reviewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the identification (ODI) numbers found in the attached list.
