While driving on the Interstate, my transmission suddenly had a dramatic downshift. A few miles later as I was exiting the freeway, it went into a limp mode for about 3 miles. I barely could pull into a fast food parking lot immediately off the exit ramp. After letting the truck sit for about an hour, I restarted the truck. It seemed to drive fairly well and drove it to a parts store to see if any OBD code was apparent. It read PO722-00 (Output Shaft Speed Sensor Circuit No Signal). I had about 57 miles to home (this is Sunday 7/27/2025 at about 5pm). Made it home but am seriously concerned about allowing my wife driving this!

My F150 engine stalls while driving when I am at a1/4 tank or less in hot or cold weather in stop and go traffic, at a stop sign or red traffic signal. I am able to restart the engine after placing the transmission in neutral or park. I have reported this issue to my ford dealer, but they say there is nothing wrong with the vehicle. There is currently a recall (#25S75) for this issue in later model F150s, but my vehicle is not covered. The Ford hotline instructed me to report this issue to NHTSA.

I was going to pass someone and as I was speeding up the check engine light came on. I pulled over and there was nothing wrong with the truck. Months later there was a warning that came up on the dash it said something about shifting and i was to park the truck. I took it to a transmission shop, come to find out that the shifting sensor had went out. They cleared the check engine light and it came back on. Ran a diagnostic on it and it said there was like 8 different things that needs to be fixed. There was Catalyst System Efficiency Below Threshold, Air Distribution Damper Position Sensor- Circuit Short to Ground or Open, Bank 1 Air/Fuel Ration Imbalance, Sensor Circuit, Internal Control Module Torque Performance, Heater Control Circuit (Bank 1, Sensor 1). When I am at a certain speed the truck will vibrate (feels like you have ran over the rubber strips on the road), if I accelerate and the passing gear kicks in there is a strange sound coming from under the truck. I dontfell like this should be put off on me. I have had alot of people saying that they are having the same issues I am having.

While driving and this begins, it gets stuck in gear and gear selectors start flashing as it does not know what gear it is in. Lead frame problem in the 6r80 is what detect it is showing. Please help with getting this covered under a recall!

While driving at 65 mph the Orange drive selection indicator on the dashboard started flashing. In addition, the column listing the gears on the right side of the screen started flashing as well. Now there is no orange drive selection indicator light and no column listing the gears on the right side of the screen. I attempted to use the tiptronic shifting switch on the shifter and I was getting no response. When I brought the vehicle to a halt while still in drive, the vehicle moved into neutral. I had to shift the vehicle into "M" manual mode to get it to shift into a driving gear. The vehicle still drives and is able to automatically travel through the gears however I have no way of visually confirming if I'm in drive. I ran a diagnostic scan and the results were as followed "P0705 transmission range sensor a circuit (PRLDN input). When I place my vehicle in park the radiator fans turn on high until I shut the vehicle off. I'm at a gas station now, I can turn the truck on however I am unable to turn the engine back on. I can hear a clicking noise coming from the center console next to the shifter. After 2 min of this noise the orange drive selection light has returned on and is illuminating "P". I can turn the truck engine on, however when I shift I to "D" drive the orange selector light turns off. When I shift back to "P" park, the orange light returns. I shift to "D" press the accelerator and the engine RPM increases. However the vehicle does not move forward. When I shift to "M" the vehicle is in gear and begins to move forward. I still have no control over tiptronic shifting while in "M".

While driving, the vehicle has been going into limp mode at varying speeds on the roadway or while pulling onto the road. The problem lies within the components of the electric throttle body. The system malfunctions and locks the throttle body closed, thus putting the truck into limp mode. Shutting the vehicle off, waiting, and restarting temporarily resolves this issue UNTIL the PCM detects a fault in the electrical components of the throttle body and sends the truck into limp mode. When this happens, it also triggers a warning message regarding hill start assist. This is an extreme safety issue as pulling onto the roadway or loosing power in traffic and not being able to pull off can lead to collisions especially around blind curves or in heavy traffic. There also isn’t always a location to safely pull off the roadway, potentially leaving the vehicle and its operator in the roadway, increasing the risk of collision and injury

On 4 separate occasions approaching stops/stoplights the engine shuddered and died leaving me without power steering or power brakes. Three additional occasions while taking off from stops the engine did the same thing and dies suddenly causing the vehicles behind me to rear end one another. The electrical system has malfunctioned while driving, leaving me without blinkers, headlights or dash lights. The dealer as well as ford are aware of the problem and have issued reports regarding the faulty engines. This truck has been in the dealership multiple times without remedy from ford. There is no warning prior to the engine shuddering and cutting off. These issues have been reported since the first month of ownership. We purchased this vehicle new.

Went to Ziebart on 6/11/25 and they confirmed multiple hole/rust damage on frame which needed repair. They were perplexed as no problem was seen at my last yearly check of the rust protection in July of 2024. Also, no mention of a potential problem was found at my last vehicle inspection in December of 2024. How does this kind of damage occur in 6 months? I'm reading on-line that this is an issue with Ford F-150s, but no recall?

After passing a car on a two lane highway 120 miles from home a wrench light came on and the transmission got stuck in 5th gear. We pulled over, stopped the truck and then it got stuck in 3rd gear and then the check engine light came on. The truck then proceeded to on into limp mode. At that point with the transmission clunking we and the truck shutting down we didn't feel safe driving the vehicle to our weekend destination nor did we feel safe driving it home as we had no idea if the transmission would completely fail. These were two lanes roads with not much shoulder and steep cliffs along the road in many areas and it was very unpopulated so any number of things could have happened to put us at risk. We had the truck towed 120 mileshome and taken to the nearest Ford Dealership, which was Larry H Miller Ford in Mesa, Arizona. The wrench and check engine light did go off but the service center was able to find the codes when they checked and it was determined to be lead frame failure. They removed and replaced the lead frame. There was no warning or codes prior to this happening - only on the day it happened.

Truck Shifts from 6th gear to 1st gear at freeway speeds. This only takes place for a matter of seconds and then shift back to 6th gear. During down shift all throttle response is gone and then regains throttle as if it was resetting. My fear is its going to happen and lock up the truck causing a major accident. I recently had the trucks transmission serviced (Fluid, filter and flushed) thinking old fluid may be causing the issue. But sadly the problem is still there with nothing looking broke or out of place according to FORD. Then today i find my trucks year is apart of a Investigation opened into nearly 1.3 million Ford F-150 trucks over unexpected gear shifts [XXX] . This is a major safe concern not to mention very expensive repair,. Looks like ford is back to old ways, this similar Problem was in the 2012-14 model I believe and eventually ford recalled the vehicles. Now i ask what about the rest of us???? The contact stated that the transmission unexpectedly downshifted while driving at various speeds. In addition, the vehicle occasionally shut off while driving, with the "Transmission not in Park" message displayed on the instrument panel, while the gear shifter was in park(P). The vehicle was taken to an independent mechanic where it was discovered that the molded lead frame had caused the failure. The dealer and the manufacturer were notified of the failure and stated that the VIN was not under recall. The contact was then referred to the NHTSA Hotline to file a complaint. The vehicle was not repaired. The failure mileage was approximately 82,000.

I checked for stored codes and it was showing PO720 Out put speed sensor (lead frame in transmission).

Sudden and without any warning, complete catastrophic brake failure while stopped attempting to pay at the pay station to exit a parking garage. My truck was fully stopped, in drive with my foot on the footbrake pedal, and started moving forward and suddenly lurched thru the parking exit gate and damaging it and the front end of my truck. Fortunately, no one was crossing on the sidewalk in front my truck or they would have been injured or worse, or we would have T-boned by oncoming traffic on the street. There was brake fluid in several areas where I parked the vehicle on the street after moving from the parking garage. There was a "low brake fluid warning light" after having the truck towed to Haley Ford in Chester, VA. There were warnings prior to the accident. The truck had had a "The Works" service done at Haley Ford the week prior. Haley Ford claims the front brake lines were leaking and need to be replaced after their "diagnostic" inspection.

The door latches on some of the f150s, including mine, freeze under certain weather conditions. This presents the latches from opening. This can trap an individual inside in an emergency situation, or prevent someone from opening the door external to remove or address babies and children, which is an issue I experienced. Ford is aware of the problem, but will not repair unless the issue is replicated in an unrealistic environment. This leaves many owners at risk of a dangerous situation should the issue occur in a non-controlled environment. The inverse issue can also occur, where the door latches does not close properly and can cause the door to open during driving, and this has also been experienced. There are no warnings of the problem ahead of time.

Ford Motor Company (Ford) is recalling certain 2016-2017 Ford Expedition, Lincoln Navigator, and 2016-2018 F-150 vehicles equipped with 3.5L Ecoboost engines. The brake master cylinder may allow brake fluid to leak from the front wheel circuit into the brake booster.

Risk: A loss of brake fluid may result in a change in brake pedal travel, increased pedal effort and reduced front brake function, requiring greater effort and distance to stop the vehicle. These conditions can increase the risk of a crash.

Remedy: Dealers will replace the brake master cylinder. If the master cylinder is leaking, the brake booster will also be replaced. Repairs will be performed free of charge. These accessory door handle covers may cause the doors to unlatch in a side impact collision. Unintended Transmission Downshift and Rear Wheel Lock-up

On March 21, 2025, NHTSA’s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE25002) to investigate instances of unexpected transmission downshifting without driver input in model year (MY) 2015-2017 Ford F-150 vehicles (subject vehicles) manufactured by Ford Motor Company (Ford). The complaints allege that, without warning or driver input, the subject vehicles experienced a sudden and rapid deceleration often accompanied by temporary rear wheel lockup, seizure, or skidding, resulting in a loss of vehicle control that increases the risk of crash and injury to all motorists, including those not within the subject vehicles. The MY 2015 and MY 2016 F-150 vehicles were equipped with the “6R80” transmission, while the MY 2017 vehicles were equipped with either the “6R80” transmission or the “10R80” transmission. This investigation was limited to MY 2015-2017 Ford F-150 with the “6R80” transmission. ODI identified 329 Vehicle Owners Questionnaires (VOQs) related to this investigation and sixty percent of these consumers were interviewed to confirm the details of their allegations. Consumers reported that their vehicles’ transmission suddenly downshifted to a lower gear, often to 1st or 2nd gear, without driver input or advance warning. Consumers described the downshift events to be regular and repeated occurrences, with some consumers stating that they no longer drive the vehicle due to safety concerns. Forty-three percent of consumers reported experiencing at least one wheel lockup event, during which the rear tires locked, screeched, or skidded during the transmission downshift.  ODI also found that 114 of the 329 consumers reported having their vehicle’s molded lead frame or valve body assembly (which comes with a molded lead frame) replaced, 80 of which were confirmed through consumer submitted repair invoices. In its response to ODI’s Information Request, Ford provided an assessment of potential root cause of failure for the subject vehicles and how this failure is different from the failure associated with MY 2011-2014 Ford F-150 vehicles that was addressed by four safety recalls (16V-248, 19V-075, 19V-433, and 24V-444). Ford has identified that the defect in the recalled vehicle population involved lead frame supplier production issues resulting in signal loss from the Output Shaft Speed (OSS) sensor. Ford has identified that the alleged defect in the subject vehicle population involved degradation of electrical connections within the lead frame due to thermal cycling and vibration over extended time in service resulting in signal loss from the Transmission Range Sensor (TRS). Signal loss from the TRS can result in an unintended shift to neutral, unintended upshift, or unintended downshift which are regulated by a gear “shift map” based on the vehicle’s speed at the time of signal loss. According to Ford’s “shift map”, at speeds between 35-64 mph, the maximum allowable downshift would be to 2nd gear (i.e. a 6th to 2nd gear downshift event) which Ford has designated to represent the worst-case scenario for subject vehicles. Ford has also acknowledged that the worst-case scenario may also involve temporary wheel lockup. However, Ford has stated that the failure mechanism in the MY 2011-2014 F-150 vehicles—previously addressed through recalls—primarily involved OSS sensor signal loss that could result in 6th to 1st gear downshift events. This OSS-related failure mode is distinct from the TRS-related signal loss identified in the subject vehicle population. ODI has identified an additional potential safety defect associated with the alleged defect in the subject vehicles. Preliminary testing performed by NHTSA’s Vehicle Research Test Center (VRTC) showed that when the TRS experiences an intermittent signal loss, one possible outcome is a change in vehicle direction. Specifically, if a vehicle is operating in reverse up an incline when the TRS signal loss occurs, then the vehicle may shift into neutral causing the vehicle to change directions and roll forward. ODI is opening this Engineering Analysis (EA) to, among other things, perform component-level testing and vehicle testing, as well as review additional technical information to better understand the alleged defect that has been identified. ODI identified 329 Vehicle Owners Questionnaires (VOQs) related to this investigation and sixty percent of these consumers were interviewed to confirm the details of their allegations. Consumers reported that their vehicles’ transmission suddenly downshifted to a lower gear, often to 1st or 2nd gear, without driver input or advance warning. Consumers described the downshift events to be regular and repeated occurrences, with some consumers stating that they no longer drive the vehicle due to safety concerns. Forty-three percent of consumers reported experiencing at least one wheel lockup event, during which the rear tires locked, screeched, or skidded during the transmission downshift.  ODI also found that 114 of the 329 consumers reported having their vehicle’s molded lead frame or valve body assembly (which comes with a molded lead frame) replaced, 80 of which were confirmed through consumer submitted repair invoices. In its response to ODI’s Information Request, Ford provided an assessment of potential root cause of failure for the subject vehicles and how this failure is different from the failure associated with MY 2011-2014 Ford F-150 vehicles that was addressed by four safety recalls (16V-248, 19V-075, 19V-433, and 24V-444). Ford has identified that the defect in the recalled vehicle population involved lead frame supplier production issues resulting in signal loss from the Output Shaft Speed (OSS) sensor. Ford has identified that the alleged defect in the subject vehicle population involved degradation of electrical connections within the lead frame due to thermal cycling and vibration over extended time in service resulting in signal loss from the Transmission Range Sensor (TRS). Signal loss from the TRS can result in an unintended shift to neutral, unintended upshift, or unintended downshift which are regulated by a gear “shift map” based on the vehicle’s speed at the time of signal loss. According to Ford’s “shift map”, at speeds between 35-64 mph, the maximum allowable downshift would be to 2nd gear (i.e. a 6th to 2nd gear downshift event) which Ford has designated to represent the worst-case scenario for subject vehicles. Ford has also acknowledged that the worst-case scenario may also involve temporary wheel lockup. However, Ford has stated that the failure mechanism in the MY 2011-2014 F-150 vehicles—previously addressed through recalls—primarily involved OSS sensor signal loss that could result in 6th to 1st gear downshift events. This OSS-related failure mode is distinct from the TRS-related signal loss identified in the subject vehicle population. ODI has identified an additional potential safety defect associated with the alleged defect in the subject vehicles. Preliminary testing performed by NHTSA’s Vehicle Research Test Center (VRTC) showed that when the TRS experiences an intermittent signal loss, one possible outcome is a change in vehicle direction. Specifically, if a vehicle is operating in reverse up an incline when the TRS signal loss occurs, then the vehicle may shift into neutral causing the vehicle to change directions and roll forward. Based on NHTSA’s analysis of consumer data, information provided by Ford, and preliminary testing conducted by NHTSA’s VRTC, this Preliminary Evaluation is being upgraded to an Engineering Analysis (EA26001). During this Engineering Analysis, the agency plans to, among other things, perform component-level testing and vehicle testing, as well as review additional technical information to better understand the alleged defect that has been identified from this Preliminary Evaluation. Seatbelt Pretensioner Fire

On June 18, 2018, the Office of Defects Investigations (ODI) opened a Preliminary Evaluation (PE18-008) to investigate complaints alleging post-crash fires at the B pillar in the area of the seat belt pretensioners on MY 2014-2018 Ford F-150 Regular Cab, and Super Crew Cab Vehicles. On August 24, 2018, Ford Motor Company recalled (FSA 18S27 - NHTSA #18V568) the affected population. Ford's investigation found that some front seat belt pretensioners may generate sparks during deployment in excess of Ford's specification. Sparks in the pretensioner exhaust gases inside the lower portion of the B-pillar may subsequently ignite combustible components contained in the B-pillar, such as insulation or carpet, and may cause a subsequent vehicle fire. ODI also requested relevant information from other manufacturers to assess the potential scope of post-crash pretensioner fires. Ford will notify owners and require dealers to remove B-pillar insulation material and wiring harness tape, and install heat resistant tape, free of charge. Brake Master Cylinder Failure

On June 8, 2020, Ford Motor Company issued a safety recall (NHTSA Recall No. 20V-332) of 292,311 model year (MY) 2014-2017 Ford F-150 vehicles equipped with 3.5L Ecoboost engines and built from 9/1/2014 to 8/1/2016. Ford stated that the causes of brake fluid loss in the master cylinder are brake fluid contamination that can interfere with rear cup seal function, and rolling of the rearmost cup seal.Prior to total loss of the front brakes, depending on the amount of brake fluid loss in the master cylinder, the driver may receive an audible chime, message center alert and red brake warning indicator in the instrument cluster, and may experience a change in brake pedal travel and increased pedal effort. It should be noted that a small number of late-built MY 2014 and early-built MY 2017 F-150 vehicles are included in the recall along with the subject vehicles (MY 2015-2016).This Recall Query is closed based on the safety recall issued by Ford.
