When in cruise, the car will merge itself into the next lane if you hold the turn signal partially. If you keep holding it or fully engage the turn signal (maybe you forgot to turn off you signal, or held it too long), the car will merge through the solid white line off the road into a ditch. This happened to me once, and I noticed it trying several other times.

I am reporting a recurring electrical and mechanical system failure on a 2024 Lexus RX 350h Luxury. On [XXX], the vehicle suffered a total power loss that rendered the electronic door latches unresponsive to all inputs. Because the occupants were a [XXX] and a [XXX] strapped into car seats, they were physically unable to reach the interior manual overrides. Critically, the mechanical backup key also failed to unlock the vehicle from the exterior, preventing immediate rescue. This created a life-threatening entrapment scenario. The doors only responded after multiple minutes of repeated manual attempts. Once entry was gained, the vehicle would not start and the headlights began blinking rapidly, indicating a total system communication failure. I had to use jumper cables to restore power. This is the second occurrence of this catastrophic failure, documented on Repair Order [XXX] at Lexus of Westminster. My 2024 Lexus RX 350h experienced a catastrophic 12V battery collapse (2.00V), disabling the electronic 'e-Latch' door system and rendering interior handles non-functional for standard use. While a manual override exists, it is not intuitive for emergency egress. Additionally, the electronic parking brake and shift-by-wire systems remained physically locked, preventing the vehicle from being moved to a safe location during the power failure. I am reporting this as a failure of primary safety and fail-safe protocols. Dealer did not consider this a a problem because he said I “needed to read the manual before I ever drove the car”

When coming from a stop, the car will not respond to the gas pedal being engaged OR it will begin to engage and then no response leaving the car rolling for a few seconds until a response is recieved and the car engages again. This has caused us to be put in harms way MANY times with oncoming traffic when making turns or in front of oncoming traffic in intersections. The dealer states this is not a known issue however it is all over the internet that hundreds of reports of this issue have been made. The dealer states no fix since they don't "have a problem". MAJOR safety issue. We have reported this many times to Lexus now with no resolve or help from them.

When accelerating at slow speed, the engine or turbo turns off which causes the nose of the car to take a hard nose dive and then turns back on within 1 second but results in a big loud sound coming from engine/transmission. This issue almost caused on accident because it does a hard engine break where I was almost rear ended but from other drivers perspective, it might seem that I’m break checking them. Showed car to Lexus but they said it is normal but it not normal and should be investigated as it can cause harm to everyone involved if accident were to occur. Situation occurred 3 times this week and it is entirely random and can’t replicate at will.

I went to reverse the car. Suddenly, the car's automatic emergency breaking system engaged. This occurred on 2 separate occasions. The first time, was in a parking lot during a sunny day. There was definitely nothing behind me, not even a person, but the camera saw a leaf or something small, and engaged the automatic emergency brake. The second time, it was raining in the evening, the sky was dark. I went to reverse (to complete a u-turn in an empty lot). Suddenly the automatic emergency braking engaged. In both instances, if I was reversing (as part of a u-turn on a busy street, this would have been very dangerous. Please investigate this. It was very scary.

The car started showing multiple error messages on the dash and then shut off while in operation, it would not restart without a service call and battery boost. This has happened multiple times. Online research shows this is a common issue with the hybrid models - the 12v battery doesn't get adequately charged, voltage drops and car ceases to work - even while mid drive on the road, this is extremely scary and dangerous.

Heard clicking noise coming from steering wheel. Car shimmied when breaking off of freeway. Newish car, only 20,611 miles. Took into dealership before anything went awry. Dealership said never heard of the steering problem before. Turned out it was a tie rod going bad and repair was covered by warranty. Break issue was also covered by warranty.

Was pulling into parking spot when car went haywire and started accelerating on its own. It would not stop until it has hit 5 cars and crashed into a cement wall. The brakes did NOT stop the acceleration. This occurred at the exact same time there was an incoming call which was not answered. The airbags did NOT deploy. The car appears to be totaled and has been towed to a State Farm affiliated auto yard. There were no indicator lights that something was wrong BUT the Lexus app showed 5 warnings that came in to my text messages same day. These 5 alerts are indicative of exactly what occurred.

When we purchased this car in June 2024 it did not have an engine revving issue. The engine, electrical or transmission maybe all of them have SERIOUS SAFETY ISSUES! When driving and you let off the gas pedal the engine starts revving very loudly, it scares the heck out of you because you don't know if you can stop! I have a serious mechanical background, there is always the possibility of the transmission not disengaging and forcing the car NOT to stop! This issue is VERY DANGEROUS AND SCARY! We get very nervous driving a car we paid almost 70K for! Yes reported to the dealer Mungenast Lexus of St. Louis, yes they have duplicated the issues. After keeping our car now on two different occasions for weeks at a time they went to Lexus engineers who are now trying to say its normal! if so, why didn't it start doing the revving until there were thousands of miles on the car! THIS IS A COVER UP! Not been inspected by the manufacturer yet, they only spoke with them. No reason for police or insurance yet. No, there has never been a check engine light appear. The only other thing the car did, after the first time of it revving like this it died when we were pulling out in traffic, it didn't die it actually shut off! This has only happened the one time, it shutting off, can it do it again you bet! These cars are dangerous, I hope nobody has to die to prove this!

Lexus RX Sunroof exploded on it's own. I just left my driveway and was the only vehicle on the road. There were no projectiles, the sunroof glass simply exploded and glass flew into the air. if I was driving at 60 MPH, the exploded glass would have become a projectile and could seriously hurt a bystander. They were no warnings. I filed a complaint with Lexus Reference # 250820001020 but their response was that this was not a mfg issue. I pointed out that they are two class action suits on Lexus, but they insist that this is not a recalled part. This is a safety issue with glass flying at 40 to 60 MPH in the open air.

While driving on the freeway the sun roof "exploded" shattering glass out onto the highway. The noise was loud and frightening. Upon pulling over I observed most of the sunroof was gone with fragments of glass remaining along the edges of the roof. I believe this to be a major safety issue. If the ceiling cover had been open, the passenger in the rear seat could have been seriously injured. No doubt flying glass is a danger to any cars following. The vehicle was inspected by the Lexus dealer who declined to cover the damage under warranty. My insurance (CSAA) agreed to pay the cost of replacement. The glass shop which did the work indicated this is a manufacturing defect and indicated they replace 5-6 sun roofs a week.

We live on dirt road in Kansas, and must drive 2.5 miles to get to paved road. This RX 500h Sport does NOT have proper gaskets around the doors. We can actually see dust rolling into the front and back doors as we drive. We have had numerousLexus RXs,& GXs over the years and never experienced a dust problem at this level. Again, we can actually see dust clouds entering the car as we drive down our daily 2.5 dirt road trips.

I experienced a sudden release of toxic fumes from the vehicle between the steering column and instrument panel area on June 29, 2025, while driving. The fumes struck my face, caused immediate bitter chemical-like taste and made me feel physically ill and extremely concerned for my safety. The fumes were strong enough to leave a lingering taste described as “bitter and green” with initial symptoms of headache, lightheadedness, nausea, vomiting green substances, eye irritations, nose and throat irritations. The local auto dealership inspected the vehicle, no mechanical defect was identified, and they noted that they cannot reproduced the event that led to the release of the toxic fumes. However, the vehicle manufacturer took possession of the vehicle for 1 week to conduit further inspections

From last month, I am receiving a lot of messages on my IPhone that multiple rear doors are open, please check the doors. Over night RX2024H draining the Battery and a lot of time it shows on screen red. Today again it showed only close to 20% charge left. I took the RX to Roseville Lexus and they perform 10K miles service and advise me that this RX350H has issues with the notices sending to owner's cell phones. They advised me to delete my account and logoff on my cell for Lexus App. Now I am driving on GUEST Mode and it still has the Battery issue it is draining over night or when parked in the garage. Chris Lawson at Roseville Lexus told me last month that RX350H has this issue and when battery is dead and car won't start and they called the Lexus service and they sent the AAA and AAA guy. Some of them got more issues and Lexus is not covering it under warranty. When, I called Chris that my batter shows RED on charge and he said, if I want them to check the car dealer will charge $219 for trouble shooting. Please look into it and Lexus should fix the issues in their cars or fix the issues in the applications. it is not safe for driving. I am writing to report a serious safety issue involving my Lexus RX350. While driving on the freeway with my [XXX] child, I suddenly heard a loud explosion-like sound coming from the roof. To my shock, I discovered that the sunroof glass had shattered into pieces. Fortunately, the sunroof cover was closed, preventing glass from entering the cabin and potentially causing severe injuries. When I contacted my Lexus dealership, they acknowledged that another GX had come in just the previous week with the exact same issue. They insisted that a rock was the cause. However, I find it extremely concerning that a small rock could cause such catastrophic failure, indicating that the glass material may be dangerously flimsy. This incident has left me deeply shaken, as it could have resulted in severe injury or even loss of control of the vehicle. I am afraid for my safety and that of my family, knowing that this could happen again. I urge NHTSA to investigate whether this is a defect in the sunroof design or material quality, as it poses a significant risk to drivers and passengers. When I start to coast to come to a stop the engine will rev, even though the Energy Flow screen shows the gas engine is disengaged and running only on EV. The engine will rev, even with me using the brake. But the EV light is off and the rpms are rising. Eventually the EV light will come on and the gas engine will cut off and the vehicle will switch to EV mode. My mpg has dropped to 36.2, so something is not right. When I was at the dealer for my 10K service, they told me this was the car "shedding" the extra energy, but the battery was not fully charged, so I'm perplexed and frustrated.

Below is the email that I sent to both the manufacturer and the dealership: I hope your Christmas Eve was better than mine. My son and I were driving up to the Poconos [XXX] to go skiing and I noticed the sonar detection light come on. When we were driving back from the Poconos, the transmission would not catch around the second or third gear. The RPMs would shoot up. The dash / warning lights lit up. I pulled over to the side of the road and I called Lexus dealership, service center, and road side assistance. However, due to the fact that it was [XXX], the offices were closed. I managed to make it to my parent’s home, but it was a very dangerous ride on [XXX]. The odometer stated that we were going 50 mph, but we were crawling and the RPMs were 4000. The RPMs at rest were always over 1000. Additionally at times, the gear would not shift into second or third and/or bottom out. Obviously the transmission is beyond malfunctioning. Below are pictures including: 1.) check your engine: A malfunction in the electric control of the engine, throttle or automatic transmission has been detected... 2.) a malfunction in the Intelligent Clearance, Sonar System has been detected 3.) traction control turned off 4.) Secondary Collision Break System malfunction, 5.) System Malfunction 6.) Drive-Start Control Malfunction. The vehicle has just over 3000 miles. I am beyond distraught. My phone number is [XXX] . The car comes with proactive driver assist. The system when activated will cause the car to unexpectedly turn the wheel and lunge usually left if the system suspects you are going off of track. It works even on unmarked 2 lane country roads. Very disconcerting and dangerous. My wife was then subsequently in a serious accident on a slippery and snowy night that totaled the car. We are not convinced that the proactive driver assist system was the cause of the accident but can't help but think it may have been a contributing factor to the accident. Her car swerved into the oncoming lane on a unmarked (snow covered) highway.

While driving on a local highway recently, my forearm brushed the automatic shift knob while reaching to place my phone on the wireless charging pad. The transmission shifted into neutral, resulting in an unexpected loss of speed. It took me a few seconds to realize what had happened - I shifted back into Drive, and resumed my trip. No accident resulted from this, but it has happened twice in the month that I have owned the car. I reported the issue to the dealership thinking my car had a problem - they said that this is the way the car is designed to function. I found a section in the Owner's Manual which confirms this - shifting from Drive to Neutral while stationary requires the knob to be held to the left for a couple seconds; for some reason, doing this while moving requires only a tap (see attached photo of the owner's manual). This seems like a design issue - the unexpected loss of power from bumping the shift knob to the left (no forward or backward motion is needed) could easily result in an accident. I don't understand why they specifically created a system where a "push and hold" is not required to shift into neutral when the vehicle is moving. I reported this issue to corporate Lexus today. They said they will consider this feedback in subsequent model designs, but I wonder whether a recall is appropriate. That's why I'm reporting the issue to you, as well.

Mileage 3445, vehicle age 3.5 months. Car located in home garage with doors unlocked. Car Locked itself making entry impossible. All systems refused to open with “clicker” or mechanical key. Possibly battery issue. Four (4) different tow truck drivers over 4 days could not gain entry. Safety hazard as a child or disabled person could have been locked inside. Tow driver could not “pop” the door lock. Despite pulling up on the inside handle, the door would not open. Towed to dealer. Thus far dealer has no solution. Dealer said they are contacting Lexus. No warning lights, etc. Vehicle is a plug-in hybrid. Drive Battery was at 100%.

We just pick up this car last week from the dealership here at San Antonio. TX. As we were driving there is a downpour/rain due to hurricane beryl, turn on the wiper and did not work, just got stuck in the middle of the windshield. i almost got hit on the road because i can see in front of me. Maybe it is also my fault by not checking this things before i drove off. At this moment the car is back at the dealership and my service advisor says that the motor is defective and they will order the parts.

I have purchased a new Lexus RX 350 2024 from South County Lexus in Mission Viejo in March 2024. Upon completion of the purchase, I was informed that I get only one key for the car as there is a shortage of the keys. I was told that it will take a few months to get a 2nd key. Now it is June 6, 2024. I called the Lexus dealership as well as corporate office. I was informed that I probably will get the key in August 2024. I find it absolutely unacceptable. It creates a safety hazard when a 2nd key is not available.

The contact owns a 2024 Lexus RX450. The contact stated that upon attempting to enter the vehicle, the vehicle failed to unlock with the key fob, prompting the contact to attempt to manually unlock the vehicle; however, the vehicle failed to unlock. The contact called Roadside Assistance and was determined that the battery was drained and was able to jumpstart the vehicle after using a Slim Jim to open the door. There was no warning light illuminated. The vehicle was taken to the dealer, who was unable to duplicate the failure. The contact was informed that leaving the key near the vehicle would result in inadvertent battery discharge. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 1,500. The VIN was not available.

Lexus is still not providing 2 key fobs on delivery. They are not listing the 2nd Key Program (23LJ01) as a campaign on their vehicle mobile software. The "digital key" is glitchy and not a safe and secure option - it seems to be a way for Lexus to periodically generate more profit at the expense of customer safety and security.

I started to notice that when in reverse or coming to a brake at low speeds, the brake pedal and car starts to vibrate/stutter. I have driven hybrids for the last 5 years and never had this issue before. It is very strange that the car vibrates when coming to a stop and it is on going. It happens and then it stops, but always starts up again.

The second-row center seat belt webbing may be damaged. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 209, "Seat Belt Assemblies."
Risk: A damaged seat belt may not adequately restrain an occupant, increasing the risk of injury during a crash.
Remedy: Dealers will inspect the seat belt webbing and replace the seat belt assembly if necessary, free of charge. Owner notification letters were mailed March 20, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TA03 and 25LA03.

Risk: A head restraint that is inadvertently removed and not reinstalled can increase the risk of injury during a rear-end crash.
