The contact is an attorney who was representing a client who owns a 2010 GMC Sierra 1500. The contact stated that while the vehicle was being driven at an intersection, the front end of the vehicle crashed into the side of another vehicle, where it came to a stop. The contact stated that the passenger side dash air bag ruptured, related to a Takata Inflator. The driver's side air bag deployed as well. The vehicle speed was undisclosed. The vehicle was towed to an impound lot. A police report was filed. Both the driver and passenger were injured in the accident. The injuries sustained were lacerations to the passenger's leg and hearing trauma to both the driver and passenger. It was unknown if the occupants of the other vehicle were injured. With no warning truck just started jumping in neutral after take off from red light litterly almost getting hit from behind and on coming traffic and than I let off the gas pedal than it jerks back in gear and doesn’t want to stop and wants to jerk to keep moving like transmission won’t stop pulling had to turn truck off it is something electrical in transmission I can pull fuse and it resets than it’s normal till it decides to do it again I’ve got on line and notice there is a lot more of the same truck and year having same issue and people have complained also mechanical at dealer says they have had many complaints on same issue I need my truck fixed it gave no warning and is to dangerous to drive like it is

Took truck in to have brake light checked to prepare for a trip. Got a call the next day from auto service center that the frame off the truck is completely rusted and that it was unable to be lifted up for service. They stated that the vehicle was unsafe to drive and that any hard bump, speed bump or over correction could cause the frame to fall out.

The contact owns a 2010 GMC Sierra. The contact stated that while driving at 15 MPH going into an intersection, another vehicle going at approximately 5 MPH, crossed in front of the contact. There were no warning lights illuminated. As a result, the vehicle impacted the other driver on the driver-side front and back door, where it came to a stop. None of the air bags deployed. The contact suffered injuries to his lower back, legs, and a bump on the head with a bruise. The contact's aunt suffered injuries to her back and hip. The contact's uncle's back suffered injuries to his right or left arm; the contact was unsure. The contact and his family member had to have physical therapy due to injuries suffered by a chiropractor. It was unknown if the other passengers in the other vehicle sustained any injuries. There was no property damage. A police report was filed however, the contact does not remember the police report information. The vehicle was towed to a body shop and was repaired. The contact stated he then received an unknown NHTSA notification for the air bags. The contact stated that after the accident sensors on the vehicle malfunctioned, and it has the air bag warning lights were illuminated. The vehicle was taken to the dealer, where recall repairs were completed. The manufacturer was not made aware of the failure. The failure mileage was 110,000.

Over the last 3 years we have had ongoing issues with our truck going into limp mode. We have spent thousands of dollars at dealerships and mechanics trying to troubleshoot the issue. Several occasions while trying to merge onto the freeway the truck would slip into limp mode causing us to have other drivers slam on their breaks almost hitting us. We had 2 cars in January of 2023 veer off the side of the road. We pulled over and thankfully they missed hitting one another. We just couldn't speed up to get out of their way. We cant believe their hasnt been a safety recall on the TCM for these trucks. GMC started doing Service bulletins in 2013 about the TCM causing the limp mode issues. There are dedicated facebook groups with over a 1,000 followers who are experiencing the same issues with these TCM's. Someone is going to die if they havent already. Several collisions, involving the years affected, are being re-investigated to determine why the vehicle slowed abruptly causing a fatal accident. A class action lawsuit is gaining momentum because GM will not do anything about the TCM.

The transmission control module failed while towing my trailer. I had my family loaded in the truck and the failure happened in a busy intersection, nearly causing several accidents as I was unable to move the vehicle. Now I’m told that these TCM’s are on back order with no repair date known. My truck has already been down for a month now and I’m told this could be up to a year waiting for the part. I’m told that there are thousands of these on back order and GM is doing nothing to fix it. This is inexcusably dangerous. These modules have an unprecedented failure rate and give no warning on when they will fail as they are an electronic component. You could be left stranded in an extremely dangerous intersection like I was with my family with no way to move, or on some dark canyon road. This issue must be addressed! GM needs to take on the responsibility and accountability to ensure that these TCMs are replaced with a reliable part. Trucks from 2007-2015 are all susceptible to failure. That is a massive amount of vehicles! Help us get this problem fixed!

2010GMC SIERRA. CONSUMER WRITES IN REGARDS TO TAKATA AIRBAG SAFETY RECALL. THE CONSUMER STATED THE DEALER ADVISED THE DASHBOARD WOULD NEED TO BE REPLACED AT THE EXPENSE OF THE OWNER, DUE TO A CRACK. THE DEALER ADVISED THE DASHBOARD WOULD MORE THAN LIKELY DISINTERGRATE WHILE REPAIRING THE AIRBAG. THE CONSUMER STATED UPON RECIEVING THE INITIAL LETTER ABOUT THE AIRBAG, THERE WAS NO CRACK, BUT THE PARTS WERE UNAVAILABLE TO COMPLETE THE RECALL REPAIR. THE CONSUMER STATED IF THE RECALL HAD BEEN COMPLETED IN A TIMELY MANNER THE CRACK WOULD NOT HAVE YET FORMED. THE CONSUMER STATED THE MANUFACTURER SHOULD BE RESPONSIBLE FOR THE REPAIR IN IT ENTIRETY.

TWO TRACTOR-TRAILER TRUCKS GOING AT LEAST 70 MPH NARROWLY AVOIDED REAR-ENDING ME . ONLY THE LACK OF TRAFFIC ON THERE RIGHT SIDE PREVENTED A MAJOR DISASTER. MY DASHBOARD SHOWED A ENGINE POWER IS REDUCED/SERVICE TRACTION CONTROL/SERVICE SABILITRAK WARNINGS, AND THE ENGINE IDLED ERRATICALLY AS I MANEUVERED OFF THE TO THE SIDE OF THE HIGHWAY. THERE WAS NO THROTTLE RESPONSE. THIS ENGINEER DESIGN PUT THE SAFETY AND LIVES OF MANY AT RISK BY DUMPING ENGINE POWER TO WHAT IS THE EQUIVALENT OF SLAMMING ON THE BREAKS AT HIGH SPEED WITHOUT WARNING. IN APRIL 2018, APPROXIMATELY 8 MONTHS AGO, I HAD THE EXACT SAME PROBLEM WHILE DRIVING IN MY NEIGHBORHOOD ROAD. AN AUTHORIZED DEALER COMPLETED A $679 REPLACEMENT OF MY THROTTLE-BODY GASKET AND GAS PEDAL ASSEMBLY (SEE ATTACHED RECEIPT). THE TRUCK DROVE WITHOUT ISSUE UNTIL THIS INCIDENT. I DIDN'T THINK THIS WAS A SERIOUS SAFETY ISSUE BECAUSE IT DID NOT HAPPEN AT HIGH SPEED. NOW, I HAVE THE SAME PROBLEM OCCURRING AT A HIGH RATE OF SPEED ON THE HIGHWAY. FINALLY, THE DEALER EXPLAINS THAT THERE ARE MULTIPLE ISSUES THAT CAN CAUSE THIS MALFUNCTION AND THEY ARE NOW COMPLETING ANOTHER $600+ REPAIR, REPLACING MULTIPLE PARTS (REPAIR BILL NOT YET AVAILABLE). BASED ON THIS TROUBLESHOOTING AND REPAIR HISTORY, A CONSUMER IS NEVER GETTING A GUARANTEED PERMANENT FIX FOR THIS PROBLEM. General Motors LLC (GM) is recalling certain 2009-2014 Chevrolet Silverado HD and GMC Sierra HD, 2009 GMC Topkick 5000, 6000, 7000 and 8000 Series and Isuzu F Series, and 2009-2018 Chevrolet Express, and GMC Savana vehicles, equipped with certain Plastic-Handle Fire Extinguishers or Push Button 'Pindicator' Fire Extinguishers. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. Risk: In the event of a fire, if the fire extinguisher does not function properly, it can increase the risk of injury.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
