My car does not always respond when the brakes are tapped. They are soft/squishy at times and I have to push the pedal all the way to the floor for the car to stop. This issue has been occuring off & on for approximately 3 years though it's been more frequent since June of 2026. I have never seen any warning or failure light/notification any of the times this has happened. The issue was determined to be a faulty master cylinder by the service department at Colley Ford in Glendora, CA.

Our 2012 Ford Fusion had a sudden and complete loss of power steering & “Power Steering Assist Fault’ flashed on dash. The steering immediately became extremely difficult to operate, requiring both hands and excessive physical force just to turn the wheel. It felt less like driving a passenger car and more like wrestling a fully loaded dump truck with flat tires. This created a serious safety concern because the failure occurred without warning and fundamentally affected our ability to steer the vehicle safely. Fortunately, my wife was not driving at the time. The amount of force suddenly required to turn the wheel could easily be overwhelming, particularly during an emergency maneuver, in traffic, or while entering an intersection. After the failure occurred, we discovered vehicle had previously been subject to recall 15V340000 and Ford Safety Recall 15S18, both involving the exact electric Power Steering Control Module (“PSCM”) that has now failed. Under that recall, a Ford dealer performed only a software update on 10/17/2016. We opened a case with Ford, brought the vehicle to a local dealer & paid for a diagnostic inspection. Using Ford IDS equipment, dealer confirmed the condition and documented ‘NO COMMUNICATION WITH PSCM’. The dealer report quoted “Steering Gear Assembly - Rack & Pinion Assembly Replace: $3,215.81.” The diagnosis was reported directly to Ford under the active case number. Despite the recall history and confirmed failure of the same steering system covered by the prior recall, Ford denied assistance because the earlier software reprogramming had already been completed. Our position is that this is clearly a hardware failure involving the recalled electric power steering system itself — the precise condition the recall was intended to address. Ford’s own recall communications dated May 17, 2019 specifically instructed owners to retain recall documentation in the event this exact situation later occurred.

The Power Steering Assist Fault notice keeps popping up on the dashboard screen. When this happens, the power steering is inoperable leaving me stranded. I've had my mechanic fix it several times (after the Ford Recall on steering didn't properly repair the issue) costing an insane price. It's very unsafe to be on the road & have the power steering go out. It is unsafe not only for me & my family but also for the vehicles traveling around my vehicle. It causes mental distress & anxiety as I never know when I'm going to be left stranded. This time, I'm stranded at the laundromat, on a Sunday. Not knowing how I'm going to get home or to work tomorrow. My receipts for repairs are at home. Until Ford makes this right, I'll never see the Ford brand as a reliable manufacturer nor will I ever purchase or recommend the Ford brand.

I am writing to formally report that the safety recall repair on my 2012 Ford Fusion has failed to resolve the dangerous loss of power steering. My vehicle underwent the 15S18 recall update only at Tom's Ford back in 2015. On January 20 2026, the steering failed again while driving, creating an immediate safety risk. The dealer has failed to provide a permanent remedy at Jim Hudson Ford in Lexington SC. As safety recalls do not expire, I request a permanent fix of the steering gear replacement within 14 days of this letter, per NHTSA guidelines. If this is not resolved, I will pursue further legal action.

I am writing to formally file a complaint and request resolution regarding a serious safety issue involving my 2012 Ford Fusion SEL (VIN: [XXX] ). I have been a loyal Ford customer for over 20 years and have owned only Ford vehicles. Unfortunately, my recent experience with Ford Customer Care and my local dealership has been extremely disappointing and concerning. A few weeks ago, my 2012 Ford Fusion suddenly lost power steering while my young son was driving home from school. This created a terrifying and dangerous situation that could have resulted in a serious accident or injury. Thankfully, he was able to safely pull over, and no one was hurt. However, this was clearly a major safety failure. After this incident, I contacted Ford Customer Care and my local Ford dealership to seek assistance. I was informed that the Customer Satisfaction Program related to the EPAS steering system had expired for my vehicle and that Ford would not provide further support. However, this directly contradicts the information shown in my Ford app. The Ford app clearly lists Customer Satisfaction Program Campaign 15N01 – “EPAS Steering Effort” and instructs me to schedule a free service with a dealer. I later learned that an update was performed in 2015, but the EPAS steering system itself was never replaced—only updated—and that the program was considered a one-time offer. It is extremely upsetting to now face costly repairs for a known safety-related issue that was clearly not resolved correctly the first time. The fact that my Ford app continues to show this campaign as available, while Ford Customer Care claims it cannot be seen in their system and provides no further explanation, is unacceptable. As a long-time Ford customer, I expect transparency, accountability, and support—especially when it comes to safety-related defects. My vehicle all of a sudden showed a "power steering assist fault," luckily it was parked. This is a problem with the 2012 Ford Fusion and I called Koon's Ford in Baltimore to see if this would be covered since my vehicle has never had steering issues before. They said yes, but after a couple days they called to tell me that Ford denied the claim since it is outside of the ten year warranty window. My car does not have a lot of miles for being over ten years old. I understand the warranty is in place for a reason but this is a dangerous manufacturer issue. I could have gotten seriously hurt if it happened on the road so it is upsetting to hear that Ford refuses to cover it. I will try to call customer relations to see if there could be another verdict. It is concerning that the manufacturer does not have an extended warranty for such a dangerous issue and I would like to make it known.

The power steering of the vehicle was lost along with illumination of “power steering sensor assist fault” and “service advance trac.” The difficulty of steering made it very dangerous on the road. The problem has been diagnosed and confirmed as code 15N01 (formerly safety recall 15S18). The 15N01 is an extended program under safety recall 15N01. I am issuing this complaint as the 15N01 program has an expiration of 150,000 miles and/or date of 10 years since purchase. My car had the original safety recall 15S18 done in 2015 but now is not qualified for the steering gear (15N01). This shows that this program shouldn’t have any limits as these steering gears were shown defective from the factory. Safety recalls never die and what they did as a quick fix for those cars in 2015 that were not showing those DTC’s was not an adequate repair . They should have addressed the steering gear issue instead of just the reprogramming of the PSCM.

One morning, as I started the car in the parking lot to head to work, I noticed the power steering was not functioning. A warning light appeared indicating a problem with the power steering system. I contacted a mobile mechanic to inspect the issue. After examining the vehicle, he confirmed the power steering had failed and advised me to take the car to a dealership for repairs. I arranged for a towing service to transport my vehicle to Interstate Ford in Dacono, Colorado. The dealership diagnosed the issue as a failure in the power steering gear and estimated the repair cost to be approximately $3,500. Concerned about the nature of the failure, I researched online and discovered that Ford had issued a recall (NHTSA Campaign ID: 15V340000) in June 2015 affecting 393,623 vehicles, including certain 2012 Ford Fusion models, due to similar power steering issues. However, when I contacted Ford, I was informed that my vehicle’s VIN is not included in the recall. I believe this may be an oversight, as the failure in my vehicle matches the conditions described in the recall. A sudden loss of power steering while driving could have led to a serious accident. I respectfully request that Ford re-evaluate the recall criteria and consider including my vehicle’s VIN, as well as potentially others that may be at risk of the same issue. I am filing a complaint regarding the ineffectiveness of Ford Recall 15S18 / NHTSA 15V-340 on my 2012 Ford Fusion (VIN: [XXX] ). On [XXX], immediately after starting the vehicle (prior to moving), the Electric Power Assisted Steering (EPAS) system failed completely and a steering-assist warning illuminated. This occurred when a female driver attempted to operate the vehicle and could not drive it safely without power assist. Modern vehicles like the Fusion are not designed to be driven without EPAS, and as NHTSA has noted in prior recall actions, loss of assist requires significantly greater effort and poses a safety risk. Diagnostics showed U3000:49-08, confirming internal EPAS rack failure. The vehicle required full replacement of the steering rack at Lilliston Ford (Vineland, NJ), costing me $2,460.65 on July 18, 2025 (invoice attached). Following this repair, new diagnostic codes appeared (B10B9:14-08 and U0423:68-08), coinciding with complete A/C failure. These faults were not present before the EPAS repair. I purchased a blower motor on July 23, 2025 for $101.96 to attempt a fix. I requested reimbursement from Ford on July 29, 2025 and followed up on August 19, 2025. As of this filing, Ford has not responded. Because the recall remedy failed to prevent recurrence, and new electrical issues arose immediately after the EPAS repair, I request that NHTSA review whether Recall 15S18 has been effective and ensure Ford honors reimbursement obligations. Attached: repair invoice, diagnostic reports, recall proof, photo, blower motor receipt, and prior correspondence to ford can be provided if necessary. The issue was diagnosed by Gillis Auto Center and confirmed again by Mullinax Ford in 2025. Ford refused to cover the repair, citing expired coverage, despite both dealerships confirming it is the same failure addressed by the original recall. I believe this is a safety defect that was not permanently remedied by the original recall repair. I am requesting NHTSA to review whether other vehicles are also experiencing repeat failures. We looked up safety recalls and found Safety Recall 15S18 and 15N01 to repair the power steering due to a manufacturer defect. The dealer charged us $250 to "assess" the issue and then refused to recall the vehicle or make the repairs. It claimed that the car was too old and out of warranty. But this is not a warranty repair it is a safety recall issue for a defective power steering system. Our car has error codes U3000:49, U3000:72, and U3000:96. They are DTCs in the PSCM.

Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. Power Steering Failure

On September 28, 2016, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE16-011) to investigate claims of power steering assist loss on model year (MY) 2010 Ford Fusion vehicles.  During the PE, ODI received 718 unique claims related to the alleged problem with 598 reports indicating the power steering assist failed while driving at various speeds with no warning.  Ford Motor Company (Ford) provided 1,851 unique reports and 1,720 warranty claims related to the alleged problem.  It should be noted that as the result of investigation PE14-030, Ford conducted a safety recall (15V-340) ofMY 2011-2012 Ford Fusion and other vehicles using the same electric power assist steering unit to rectify incidents of loss of power steering assist. On November 20, 2017, PE16-011 was upgraded to this Engineering Analysis (EA17-004) to further assess the scope, frequency, and consequence of this failure.  This was based on an analysis of the subject vehicle complaint data that had indicated an increased likelihood of failure as the vehicles age.  At the time, the rate of occurrence of this failure was comparable to previous investigations involving loss of power steering assist including the previously mentioned PE14-030. During the EA, ODI’s analysis of the alleged defect in the subject vehicles identified the following:  testing showed minor steering effort changes between “assist” and “no assist”, no root cause identified, a declining failure trend, the ramp-down or reduced assist provided when a DTC is set, a lower failure rate than the recalled Fusion vehicles, and the high age of the vehicles. A safety-related defect trend has not been identified.  Accordingly, this Engineering Analysis is closed.  See the attached Engineering Analysis Closing Report for details.  The closing of this investigation does not constitute a finding by NHTSA that a safety defect does not exist, and ODI will take further actions if warranted by future circumstances.  To review the reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.

Electric power steering failure

The Office of Defects Investigation (ODI) opened a preliminary evaluation to investigate complaints of the loss of power steering while driving. The subject vehicles are equipped with a rack mounted Electric Power Assisted Steering (EPAS) unit.Some of the complaints alleged that the steering assist failed and resulted in increased steering efforts that contributed to a loss of control and in some cases a crash.The EPAS system was manufactured by TRW Automotive for Ford and was designated as Generation I.These units were first produced and put into vehicle production in July 2008 beginning with the model year (MY) 2010 Ford Fusion and Mercury Milan.The system is comprised of a mechanical steering rack-and-pinion gear with torque sensor, electric assist motor, motor position sensor and a power steering control module (PSCM). The PSCM is self-monitoring and designed to detect faults within the system and can store diagnostic trouble codes (DTCs) if a problem is detected.There are 29 potential DTCs the system can detect.Many of these allow the system to go into reduced-assist steering mode by using the default vehicle speed signal should the live speed signal be lost, if voltage to EPAS system falls below 10.7 or goes above 17 volts, the temperature at the PSCM goes above 105C (221F), or if high friction is detected.Some DTCs will cause the system to go into manual steering mode (no assist). On July 1, 2015, Ford filed a Defect Information Report (DIR) identifying a safety related defect in TRW GEN I EPAS systems installed in some of the subject vehicles (NHTSA Recall 15V-340, Ford No. 15S18).The DIR indicated that intermittent electrical connections caused by quality issues associated with conformal coat contamination and ribbon cable pin misalignment within the steering gear motor may lead to a loss of the motor position sensor signal, resulting in a sudden loss of power assist while driving.The recall covers approximately 393,623 vehicles built from September 2010 through February 2012:2011-12 Fusion built SEP 2010 - FEB 2012; 2011-12 Lincoln MKZ built SEP 2010 - FEB 2012; 2011 Milan built SEP 2010 - DEC 2010; 2011-13 Taurus built SEP 2010 - FEB 2012; 2011-13 Flex with 3.5L GTDI engine built SEP 2010 - FEB 2012; and 2011-13 MKS, MKT with 3.5L GTDI engine built SEP 2010 - FEB 2012.The repair consists of updating the PSCM software to prevent a loss of steering assist while driving. The revised software will provide audible and visual warnings to the driver. If one or more of the following DTCs are stored, C1B00-62, C200B-2F, C200B-61, C200B-62, C200C-2F, C200D-49, U0300-00, U2011-49, U2011-61, U3000-46, U3000-49, U3000-61, U3000-72, U3000-96, P07AE-09, the dealer will replace the EPAS system at no charge to the owner.ODI's analysis of failure data for this PE identified a total of 2,584 complaints to ODI and Ford and 9,967 Ford warranty claims related to the alleged defect.The recalled vehicles account for 1,634 of the complaints and 7,311 of the warranty claims.ODI's analysis identified 40 complaints alleging crashes that may be related to loss of steering assist while driving, 20 of which involved vehicles included in Ford's recall.The information available for the alleged crashes were not sufficient to assess specific causes of the incidents or to verify EPAS malfunctions in each case.Most of the crashes were low-speed impacts with curbs or roadside objects resulting in wheel or minor body damage.None of the injury claims indicated medical attention was required.In addition, statistical modeling of the failure data by NHTSA's National Center for Statistics and Analysis projected 10-year failur

Electronic Throttle Body Malfunction

On February 21, 2013, the Office of Defects Investigations (ODI) opened Preliminary Evaluation PE13-003 to investigate allegations of electronic throttle body (ETB) failures resulting in sudden reduction of engine power in model year (MY) 2009-2013 Ford Escape, Fusion, Mariner and Milan vehicles. During this investigation, Ford identified a condition in subject vehicles equipped with 2.5L and 3.0L engines that may result in a sudden reduction of engine power.According to Ford, the ETB internal motor contacts may develop a high resistance material buildup condition on the commutator, resulting in intermittent electrical connectivity and reduced engine power. When this condition occurs, the Malfunction Indicator Lamp (MIL) or Wrench light will illuminate and the vehicle may enter a limited limp home mode.Ford?s trade name for the feature is Failure Mode Effects Management (FMEM) mode.In this mode, engine power and vehicle speed are reduced, while full function of the power steering, power braking, lighting, and climate control systems are maintained.ODI?s complaint analysis indicate that the predominant failure mode involved reduced motive power associated with the limited limp home mode with engine speeds limited to approximately 900 RPM. Analysis of warranty claims provided by Ford identified 59,807 claims related to ETB replacements and approximately 50 percent of claims are associated with diagnostic trouble codes (DTC) P2111, Throttle Body Stuck Open, and P2112, Throttle Body Stuck Closed.Ford described several factors where the ETB motor may fail resulting in DTCs P2111 or P2112 but the failure is not an existing stuck open or closed ETB valve position.According to Ford, the ETB control strategy provides the driver with three FMEM modes that allow varying degrees of vehicle mobility depending on the severity of the fault detected.DTCs associated with stuck open or closed throttle valves are designated the highest failure severity resulting in engine speeds limited to high idle corresponding to the limited limp home mode.Vehicles are not likely to unexpectedly stall as a result of this condition, but drivers may characterize the reduced functionality as a stall, even though their vehicle may still has motive capability.Other FMEM limp modes may result in reduced engine performance but will maintain vehicle speed above 20mph. During this investigation, Ford and its suppliers, Delphi and Igarashi, updated the powertrain control module (PCM) software to include a throttle body motor cleaning cycle during key-on and modified the ETB internal motor components design, surface finish and material composition to improve durability. Additionally, Ford developed a remedy procedure and issued a special Customer Satisfaction Program (CSP) 13N03 extending the ETB warranty coverage and instructing dealers to update the powertrain calibration to improve vehicle performance in the event that intermittent electrical connectivity of the throttle body motor contacts occurs. The program extends the coverage for up to 10 years of service or 150,000 miles from the warranty start date of the vehicle, all vehicles are eligible for the program through January 31, 2015 regardless of mileage.Owners of the affected vehicles will be contacted by mail to take their vehicle to a Ford dealer who will reprogram the PCM to the latest calibration.
