On 7/19/26, I was driving my 2022 Ford Bronco Wildtrak and attempting to turn left across oncoming traffic. When I pressed the accelerator to complete the turn, the vehicle hesitated and failed to accelerate as expected. An approaching vehicle had to decelerate abruptly and honk to avoid a collision. My Bronco is subject to NHTSA Recall 25V-455/Ford Campaign 25S75 concerning loss of fuel pressure and fuel flow caused by low-pressure fuel-pump failure. I am concerned that the hesitation was related to the recalled fuel-pump condition. There were no injuries or collision, but the loss of expected acceleration created an immediate safety risk. The recall remedy is not currently available. I am requesting that NHTSA document and evaluate this incident.

And Ford Dealership said they at least a dozen or more around his dealership with the same issue. And online I have been told of 100s. Have the same thing happening to their Broncos the years 2021 to 2024 even ones sit on lots for with the same problems.

35 miles out of [XXX] on Monday, XXX our Bronco loss fuel flow in the fast lane of [XXX] and stalled while going 70mph we were surrounded by trucks also going 70mph, unexpectedly mid morning. No warning . Almost got rear ended and had to safely drift over to reach the side of the road. Very terrifying with cars going very fast behind us. We attempted to restart the Bronco and it started a few times and sputtered out. I immediately called the closest Ford dealership Gaudin and they dispatched a tow truck. The temperature was very hot and we waited for 1.5 hrs in the desert sun. The dealer provided a rental but I was not allowed to leave NV nor go more than 50 miles, they confirmed that it was a bad fuel pump. I did not receive the car back until Friday 6/19/26. This incident occurred on my first day of VC planned in [XXX]. My whole vacation was ruined, unexpected expenses, and the stress of having to drive with a defective part when it’s fixed is a problem. We have to drive back to LA and I can’t miss work. We took the vehicle to Gaupin in LA to have it serviced before this trip even an oil change on 6/11/26. They assured us that all was well, only 2 recalls showed up- rear camera and a seat adjustment bolt . Why didn’t the dealer tell us about the fuel pump and the dangers. The vin number has a listed “1 un-repaired recall - fuel pump dated 7/2025. Manufacturer #XXX, NHTSA# 25V455. No notice has come through the mail and then it states Recall incomplete in red. If it was known that the fuel pump could fail, how are owners allowed to drive around without a complete recall, and without a fix. The dealer said they are going to announce on Jun 29. Well in our case that’s a little too late. Am I supposed to drive with a defective fuel system??? They replaced the fuel pump with another new pump that will have the same issue, not a fix. On June 19 later that evening after fueling it up, a service engine light came on with exhaust system issues. While operating the vehicle under load, the vehicle suffered a sudden and severe loss of engine performance, followed immediately by loud metallic banging from the engine bay. The vehicle lost all power and was immediately disabled (LIMP MODE), presenting an imminent highway safety hazard.The Physical Damage: Subsequent mechanical teardown revealed deep vertical cylinder wall scoring, heavy raw fuel residue in the engine oil pan, and a blown head gasket. Relationship to Active Safety Defect: This catastrophic block failure perfectly mirrors the progressive failure profile of NHTSA Campaign 25V455 / Ford Safety Recall 25S75 (Low-Pressure Fuel Pump Failure). Internal component degradation in the fuel tank delivery module dropped fuel pressure to the engine. Under turbo boost, this starvation induced an extreme, instantaneous lean-out condition. The resulting low-speed pre-ignition (LSPI) exploded and shattered the internal piston rings (the metallic banging), leading to mechanical wall gouging, unburnt fuel wash into the lower crankcase, and a secondary blown head gasket. Request for Action: Ford's current VIN parameters for Recall 25S75 have not yet captured my specific vehicle despite it matching the exact production window, component setup, and catastrophic failure sequence. I am requesting that the NHTSA log this case as clear evidence of consequential engine destruction originating from the 25V455 pump defect, and evaluate whether Ford's recall scope must be expanded to protect other consumers from highway engine failures.

The Incident: While operating the vehicle under high load, the driver received an explicit "Service Engine Soon" powertrain malfunction alert stating: "The powertrain control module lost communication with the vehicle active grill shutter module for a period of time." Immediately following this warning, the engine experienced a severe loss of power, followed by loud metallic banging from the engine bay. The vehicle stalled completely and lost all motive power, creating an imminent highway safety hazard.The Physical Damage: Subsequent mechanical teardown by the dealership revealed terminal block destruction, including deep vertical cylinder wall scoring, heavy unburnt liquid gasoline residue in the engine oil pan, and a blown head gasket.Relationship to Active Safety Defect: This catastrophic engine failure mirrors the progressive failure profile of NHTSA Campaign 25V455 / Ford Safety Recall 25S75 (Low-Pressure Fuel Pump Failure). In this EcoBoost architecture, the Active Grill Shutter (AGS) module shares critical voltage paths, grounding blocks, and wiring looms with the engine management and fuel delivery systems. The sudden network communication loss is diagnostic proof of a severe voltage drop or electrical short caused by the high-friction fuel pump degradation detailed in Recall 25S75. This circuit failure instantly starved the engine of fuel under turbo boost, inducing an extreme lean-out condition. The resulting pre-ignition forces violently shattered the internal piston rings (the loud banging), which then gouged the cylinder walls, wiped out compression, fouled the crankcase with fuel residue, and blew the head gasket.Request for Action: Ford's current VIN parameters for Recall 25S75 have not yet captured my specific vehicle despite it matching the exact production window, component setup, and catastrophic failure sequence. While driving on an interstate it was determined that the engine walls over compressed and blew out the sides of the motor of my 2022 Ford Bronco. The Bronco has 67,000 miles on it which shows a clear manufacturer defect. It is available to be inspected as well to confirm. My wife’s and unborn child’s safety was put at risk by losing acceleration and losing control of the vehicle briefly as it began to shake from the engine misfiring. This easily could have caused a serious wreck had more traffic been present at the time. The dealership has not been able to confirm this on other cars. The engine has been inspected by a Ford Dealership. There were no warning lamps or messages prior to the failure and only appeared shortly after the engine over-compressed and began to shake.

They also noted a sticking valve in the main control on my 2022 Ford Bronco 2.7L V6 when taken in to dealership to evaluate vehichle under warranty due to ongoing intermittent thump/clunking noise when coming to a stop and accelerating from a stop as well as when backing up from a stop and while driving as well, intermittent loud metal slapping sound at speeds over 60 mph, intermittent jerking of vehichle when cycling through gears, unexpected downshift at high speeds. After differential replacement and transmission overhaul due to excessive wear while under warranty, my vehichle continues to experience the same symptoms and problems and feel this is a safety issue.

I took my vehicle to the local Ford dealership for the remedy for recall Ford 25S01 on Feb 2, 2026. During the inspection, it was found that the part affected was damaged and the dealership sent photos to Ford. After that, I haven't received any guidance from Ford or the dealership. Since the invoice I received indicated that there was a weld crack in one of the absorbers, I assume that the recall should repair this as a safety concern, but I haven't received any proactive communication from Ford or the dealership. I have contacted the dealership twice since February but I have not received any callback. I am concerned that this is a safety issue and Ford hasn't done anything to correct this since they found out about the issue in February.

The instrument cluster panel goes black and stays blacked out for driving from startup to shutdown. This does not occur consistently, but it occurs often and unpredictably and is occurring more and more often. Attempted to have dealership fix and they want to replace the instrument cluster panel. This is a safety issue for basic functionality of the vehicle. Ford has had this issue identified during other years of Bronco production but appears unwilling to fix it in all of them.

There has been an SSM release by Ford (SSM 51415) acknowledging that 2021-2022 Bronco vehicles flat towed in neutral may experience inconsistent or unintended ABS (anti-lock brake) module activation while being towed. The ABS module exits sleep mode unexpectedly due to road vibration input during towing, which can cause brake drag or lockup while the vehicle is being flat towed behind a motorhome. This is a safety concern as unexpected braking of the towed vehicle can affect the towing vehicle's stability and control at highway speeds. The vehicle is being operated exactly as documented in the Ford owner's manual, which explicitly supports neutral flat towing. Ford has developed a software update to address this defect but is declining to cover it under warranty or goodwill, instead classifying it as non-covered maintenance. The issue has been confirmed by an authorized Ford dealership who identified SSM 51415 as applicable. A community workaround requiring removal of fuses 28 and 29 to disable the ABS pump has emerged among owners, indicating this is a widespread known issue affecting safety during a manufacturer-approved towing method. Ford should issue a TSB or recall and cover the software correction at no cost to owners.

Noticed that my hardtop was cracking around the rear windows. This is a know issue with Ford and is being replaced under warranty. Unfortunately, my vehicle is outside of the original 3 year 36,000 mile warranty period by just over 6,000 miles that would qualify for replacement. The dealer quoted over $10,000.00 to replace the entire top since all roof panels appear to be affected. This is almost 1/4 the cost of my vehicle on the used market and is unacceptable for a know issue that would normally be replaced under warranty.

My 2022 Ford Bronco experienced recall 25S75 where fuel pump failed while driving on the freeway outside LA near San Bernardino. Recall was communicated in July of 2025 and no fix available. How long has Ford been building cars? There were no warnings or symptoms prior to failure. This was extremely dangerous for car to stall in the middle of the freeway, not to mention it was 98 degrees outside. I was able to tow my Bronco to a Ford dealership where they confirmed and fixed the recall fuel pump issue.

Truck began scrolling warnings that pre-collision braking, hill start assist, forward collision, adaptive cruise control, and more were all not working. Alerts came out of no where while driving on highway and continued every 30 min to hour of drive time. Cruise control was disabled with this failure. Diagnosed with faulty steering-rack. This has been recalled on other 2022 Ford Broncos, but for some reason our was not included in this recall. This lot was clearly impacted as well. The truck is used for daily driving, does not go off-roading and was not involved in any collision or incident that triggered this, simply driving down the highway. Please let Ford know to expand the recall, our car is not safe to drive and is not inoperable unless we pay for the repair or the recall is expanded. Thank you for your assistance!

On May 7, 2026 while driving approximately 18 mph, my Ford Bronco 2022 Outerbanks experienced a sudden loss of normal braking assist. The brake pedal became stuck in the upright position requiring excessive force to stop the vehicle. When the brake pedal went down the truck rolled to the curb and I put it in park. There was a warning sign on the dash that flashed and went away. I could not see what it was. Luckily there were no bystanders or other cars on the road at the time. I was near an elementary school at the time. This would have caused a crash or worse if there were pedestrians. I turned the truck off and called the tow. At the dealership they diagnosed and confirmed an electronic brake booster (EBB) failure. This EBB failure mirrors a recent recall from Ford affecting 2025 Broncos. The dealership has refused warranty because I am 6 months outside the warranty period even though I only have 29,960 miles. This is not normal wear and tear. The 2025 recall # is 25V488. This EBB recall may also affect previous years such as mine and should be investigated.

I was operating the vehicle on the highway and the steering locked up. I had no indication that this was going to happen. It would not turn and almost caused an accident. I was able to break the steering do to my strength. I was able to limp truck to dealer. Upon inspection they confirmed a faulty / locked up steering shaft. This was not covered by ford. The shaft was replaced and functional now. I have made several attempts to communicate with corporate ford about this safety matter and haven’t been heard from. They have had shaft issues with their f150 trucks also. This issue is a direct result from poor manufacturing and is a real safety issue. This should never happen with a vehicle that is only a few years old with not that many miles.I have saved the part for inspection and looking for some assistance. Thanks you.

Vehicle brake pedal is very hard to press, unable to push in until the car engine is running. When driving the brake pedal seems it has more pressure and cannot push it to normal level. On the highway it came up brake safety warning when I was in 4Wheel Auto. The pedal stiffness has been checked and confirmed by AutoNation Service. They stated no Warranty or Recall Notice for this issue. The issue is commonly reported by broncos with 2.3L and is determined to be the Brake Booster Vacuum Line. Statements share similar brake pedal stiffness when engine stopped, and engaged stiffness when in 4 wheel mode or Cruise Control, causing driver to put immense pressure on pedal to have the car brake. Before I pay for the parts and long service this clear safety concern should be looked at.

My vehicle is experiencing potential electrical issues which appear to be similar to the same 12-volt battery management failures that are currently recalled in the Bronco Sport models. The "Charging System Service Now" alarm continues to pop up, sometimes every 30 seconds. I am also not able to use my communication and navigation system, and I am not sure if this is related to the same electrical system malfunctions. Furthermore, on three recent occasions, my vehicle stalled while I was pulling out of a street which was on a slight incline, another while I was on a hill in a parking lot, and another while I was reversing and putting my car into 1st gear to drive. During all of these occasions, my vehicle stalled and did not automatically re-start which had me blocking traffic while I turned my vehicle back on. All of these issues have affected the safety of myself and my passengers due to the unexpected stalling as well as electrical power loss, including loss of navigation/communications. It has also resulted in me getting pulled over by law enforcement twice due to swerving while I was looking at the screen when it went black and the other because I was speeding because my Charging System Service alarm went off and I was also unable to determine the speed limit of that road without having my navigation screen working. I believe I have contacted the Ford Dealership regarding the specific loss of the communication and navigation issue and was told they did not have the parts/software available at this time. I have taken my vehicle to an Advance Auto Parts store when the "Charging System Service Now" first appeared and was told it was potentially the alternator. I will have to locate the documentation at a later time.

I would like to update my previous safety concern since both concerns are related to separate recall issues. This concern is related to my vehicle experiencing potential electrical issues which appear to be similar to the same 12-volt battery management failures that are currently recalled in the Bronco Sport models. I have not able to use my communication and navigation system for months now which has caused continuous issues which first started with my gps taking me to a completely different location on several occasions. This has caused me to get lost, be late, and be distracted while trying to fix the navigation error. These electrical issues have affected the safety of myself and my passengers due to the potential for complete electrical power loss, including loss of navigation/communications. It has also resulted in me getting pulled over by law enforcement twice: Once, where I received a warning for failing to use my turn signal when exiting which was the result of my entire screen going black while I was using my GPS and panicked. The second time was for speeding: 1. I was distracted because the Charging System Service alarm popped up on my dashboard. 2. My navigation screen was not working which I what I used to monitor the speed limit and maintain correct speed when the sign was not visible. 3. According to research, this may have affected the accuracy of the speed gun since I did not agree with the speed the officer detected. Research into this issue did, in fact, indicate that electromagnetic interference caused by the electrical faults and alternator issues can produce broadband noise or EMI which may overload or confuse the sensitivity of the radar gun, causing a false speed reading. Yes, the Ford Dealership was contacted, and I was told they did not have the parts/software available to fix the electrical issue. A mechanic also indicated issues with the alternator.

Low-pressure fuel pump failure

On July 29, 2024, the Office of Defects Investigation (ODI) opened PE24019 to investigate complaints of low-pressure fuel pump failures in MY 2021 Ford Bronco vehicles. Some consumers reported that the failure occurred without warning while driving, resulting in a complete loss of motive power with no ability to restart the engine. Ford determined that several other vehicle models contain low-pressure fuel pumps substantially similar to the 2021 Ford Bronco, including the 2022-2023 Ford Bronco, 2018-2023 Ford F-150, 2022-2023 Ford Expedition, 2019-2023 Lincoln Navigator, 2020-2023 Ford F-250-F-550, 2024 Ford Super Duty Chassis Cab, 2024-2025 Ford Ranger, 2020-2023 Ford Explorer, 2020-2023 Lincoln Aviator, and 2021-2025 Ford Mustang. ODI's investigation found that the affected vehicles may lose fuel pressure and flow from the fuel delivery module (consisting of a low-pressure fuel pump, jet pump, reservoir, and other parts) due to a low-pressure pump failure. This can cause the engine to stall while driving. The root cause is reduced internal clearance in the fuel pump, increasing friction and sensitivity to vapor lock. Fuel blockage from contamination inside the jet pump also contributed to the failure. Ford stated that the contamination source was unknown. On July 7, 2025, Ford filed recall 25V-455 for certain 2021-2023 Ford F-550 SD/F-450 SD/F-350 SD/F-250 SD, 2021-2022 Lincoln Navigator, 2021-2022 Ford Mustang, 2021-2022 Ford F-150, 2021-2023 Ford Explorer, 2022 Ford Expedition, and 2021-2023 Ford Bronco vehicles produced between July 1, 2021, and July 30, 2022. The recall also included certain 2021-2023 Lincoln Aviator produced between July 1, 2021, and December 21, 2022. Ford explained in its recall filingthat the production dates used for the recall were based on changes in the manufacturing process. Specifically, Ford determined that in June 2021, the supplier modified the jet pump manufacturing process, producing fuel pump chambers with a lower-than-specified internal clearance. In July 2022, the supplier implemented corrective actions to the manufacturing process by adding two additional vacuum pumps to reduce contamination during the production of the jet pump. ODI has identified approximately 1,000 reports involving vehicles covered in this investigation but excluded from recall 25V-455. However, even with such reports, these vehicles experienced comparatively lower failure rates than the vehicles that are subject to Recall 25V-455. Ford has explained that these vehicles were excluded from the recall population because they did not experience the manufacturing changes that prompted the recall. Specifically, according to Ford, vehicles built before July 1, 2021 (the recall production start date), were not affected by the supplier’s change to the jet pump manufacturing process. Even after closing this investigation, ODI will continue to monitor complaints for vehicles excluded from recall 25V-455 to consider whether they exhibit the same risks as the vehicles encompassed by the recall. The recall remedy is currently under development. In view of Ford’s recall action, ODI is closing this Preliminary Evaluation, but will continue to monitor the issue as a remedy is developed and implemented. Loss of Motive Power

On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family.  If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles. In view of the recall action being taken by Ford, ODI is closing this Engineering Analysis.
