While barely moving, (about 2 mph), in a parking lot & preparing to make a sharp left turn, knuckle of driver's side wheel assembly suddenly broke, causing the wheel assembly to collapse, wheel top tilted inward and bottom tilted outward, causing major damage to upper and lower control arms, tie rod and significant body damage to two body parts at the wheel opening. Mechanic reported that there had been very excessive ‘play’ before the collapse At 86,000 miles, this is a very low-mileage for this degree of wear and complete failure, on a full-size, PPV SUV, with many heavy-duty features, designed to tolerate high stress usage. FORTUATELY, the car was barely rolling. If this had happened 5 minutes later, I would have been driving 65 mph and this would have caused a catastrophic wreck, could have involved several other cars, and could have injured or even killed many people. I am now very concerned about the condition of the knuckle on the passenger side. It could be developing microscopic cracks that would only be observable with X-ray. ESPECIALLY with this defect being on a very popular police model, Highway Patrol PPV vehicle, more failures will endanger many people.

The vehicle was involved in a "T-Bone" collision where two vehicles drove into the driver side of my vehicle. I was not at fault and yes, the vehicle is available for inspection upon request. In February of 2022 I took the vehicle to Felix Chevrolet to have the airbag recall repaired. In late January 2023, the "T-Bone" collision occurred. My elderly mother (front passenger) and my two sons (13 and 14 rear passengers) were also in the vehicle. ZERO airbags deployed in my vehicle. The vehicle has been parked at my residence since the incident. The vehicle or component has NOT been inspected by the manufacturer, police, insurance representatives or others since the incident. I noticed the dash cracking after the recall was performed in 2022. I contacted Felix Chevrolet and spoke the Asst Manager Sergio who told me that GM is NOT replacing the cracked dash boards and if I could take a photo and text it to him--which I did.

My dash is very cracked. I do keep it covered, but it doesn’t make the issue disappear. Upon research I have seen that this is a very common issue and that legal action was taken in 2018. It causes a safety concern.

Developed cracks in dash starting in 2018 and progressively got worse. Rattles when you drive. Driver door pull has also broken in several places and has pinched my hand multiple times. Dealer is aware this is a common problem but will need to replace whole door panel and dash at my expense. Transmission went at 81,000. Chevy, you can do better. I was warned not to buy this, but I took my chances. It feels very lemony sour now.

We took our vehicle to the Chevrolet dealer: Don Mealey Chevrolet 17185 State Road 50 Clermont, Fl. 34711 (352)394-6176. They said they would not replace the defective air bag part unless we agreed that they would destroy our dash by completing the recalled part replacement. We purchased the vehicle brand new and have taken excellent care of it and the dash is in excellent condition, how can they refuse to do the required FREE recall unless we agree that if they destroy our dash. Please help us get this important safety recall completed. My name is [XXX]. I’m sure if Chevrolet is doing this to us there are other Consumers that they are doing this to that will not speak up. My family’s safety depends on the safety equipment that they placed in the vehicle that we purchased in good faith. I wanted to try this option prior to engagement of a Consumer Attorney. took car to dealer for nys inspection* {recall must be done to pass} service advisors work on commision, so i get a $5500 quote on "" seriously needed work"" yet my concerns were overlooked. & unauthorized work yet Recalls are to be done free so there no commison, so they upsale you and put off the takata airbags as"" on order""= profit over safety. now im on the hook for $6000 or they keep the car then sell as "certified pre- owned" this is part of a serious flaw within the automotive industry. my car is a decommisioned nj police k9 car. so it was a fleet car + very well maintained. the service advisor is grossly over billing and padding the work order. i had to put a stop work order as a disabled person who needs his car to be mobile and i bought a used car to avoid financing and large financial obligations that i can not afford. i even got corperate involved { but they are all chevy team leaders}

IT IS BEGINNING TO CRACK IN OTHER PLACES. THE SHARP EDGES HAVE CUT MY FINGER BEFORE WHILE JUST WIPING THE DASH. THE SHARP EDGES COULD ALSO CUT INTO ELECTRICAL WIRES DOWN THE ROAD. THIS VEHICLE HAS ALWAYS BEEN TAKEN CARE OF AND STORED INDOORS AND THIS IS UNACCEPTABLE TO HAVE SHARP CRACKED EDGES LIKE THIS FROM NORMAL USE.

MY VEHICLE IS GARAGE KEPT, SO I KNOW THE HEAT FROM THE SUN DID NOT CAUSE MY DASH TO CRACK IN 3 SEPARATE PLACES. ABOUT A MONTH AGO I RECEIVED A RECALL ON MY VEHICLE DUE TO FAULTY AIRBAGS. SO I DECIDED TO EDUCATE MYSELF ON THE RECALL AND AFTER READING HUNDREDS OF COMPLAINTS REGARDING THEIR VEHICLE'S DASH CRACKING AS WELL.. LETS SEE, FAULTY AIR BAGS ALONG WITH CRACKED DASHES WHERE THE AIR BAGS ARE LOCATED GUESS ITS NOT A COINCIDENCE. GUESS IT WON'T BE A COINCIDENCE WHEN THE FAULTY AIR BAG GOES OFF AND THE CRACKED PLASTIC DASH GOES FLYING AND SEVERALLY INJURES OR CAUSES DEATH TO A LOVED ONE. MY DASH IS CRACKED AND BROKEN TO THE POINT IT MAY COLLAPSE IN THE MIDDLE. I HAVE CRACKS ON BOTH SIDES OF THE PASSENGER AIRBAG AND ANOTHER ONE ABOVE THE SPEEDOMETER THAT RUNS TO THE MIDDLE OF THE DASH. IT IS CRACKED SO BADLY ACROSS THE LENGTH OF THE DASH THAT IT HANGS LOOSELY AND RATTLES WHILE DRIVING. I HAVE USED A SUNLIGHT VISOR SINCE BUYING THE VEHICLE AND I CAN ASSURE YOU THIS DAMAGE IS NOT MY FAULT. I HAVE LOOKED AT REPLACING THE DASH MYSELF BUT CANNOT AFFORD TO DO SO RIGHT NOW AND I AM AFRAID IT IS GOING TO COLLAPSE. THIS IS A "MOM" RIDE, I CARRY MY KIDS IN THIS VEHICLE. APRIL OF 2016 THE DASHBOARD ON MY 2012 TAHOE HAD CRACK TO SHOW UP AT MY AIRBAG. I TOOK IT TO MY DEALERSHIP TO ASK IF THIS IS A HAZARD. THEY AGREE THAT THE LOCATION MADE IT A HAZARD. THEY ALSO TOLD ME THAT I WAS NOT THE FIRST TO COME TO THEM WITH THIS SAME PROBLEM IN THE EXACT SAME AREA. THE DEALERSHIP SAID THAT THERE WAS A LAWSUIT IN PLACE TO HAVE THE DASHBOARD REPLACE BY GMC. THEY THEN TOLD ME THAT OTHER CRACKS WOULD FORM OVER TIME BECAUSE THAT IS WHAT IS HAPPENING TO OTHER VEHICLE OWNERS WITH THIS SAME VEHICLE. UNFORTUNATELY, THEY WERE CORRECT. A FEW MONTHS LATER ANOTHER CRACK SHOWED UP ON MY DRIVER SIDE. I TOOK IT BACK TO DEALERSHIP AND THEY ONCE AGAIN SAID THAT THIS IS EXACTLY WHAT IS HAPPENING TO EVERYONE ELSE. I ASKED IF THERE WAS ANYTHING THEY COULD DO FOR ME. THEY COULD REPLACE IT BUT THEN SAID, THAT GMC SHOULD REALLY STEP UP AND MAKE THIS A RECALL. AS I RESEARCH THIS ONLINE, I FOUND WHERE THIS IS HAPPENING TO A LOT OF PEOPLE THAT HAVE 2012 GMC TAHOE. THIS IS HAZARD TO ANYONE THAT IS IN THE CAR. ACCORDING TO THE EXPERTS AT MY DEALERSHIP, THE CRACK AT THE AIRBAG COULD GET CUT OR MESS IT UP FROM DEPLOYING. PLUS IF I WAS TO GET INTO A WRECK THE PIECES COULD EASILY BREAK OFF AND CAUSE MORE PHYSICAL HARM. WE HAVE SPENT A LOT ON BUYING THIS TOP OF THE LINE 2012 GMC LTZ TAHOE. WE UNDERSTAND THAT OVER TIME DIFFERENT PARTS OF THE VEHICLE WILL NEED TO BE REPLACE LIKE RUBBER SEALS AROUND THE DOOR, DOOR HANDLES & OTHER THINGS THAT COME WITH NORMAL WEAR AND TEAR. HOWEVER, REPLACING A DASHBOARD SHOULD NOT BE ON THIS LIST. ESPECIALLY WHEN THIS IS HAPPENING TO THOUSANDS OF OTHER VEHICLES. THE CRACKS ARE HAPPENING IN THE EXACT SAME SPOT IN BOTH PLACES. THE CRACKS ARE GETTING WORSE. I REALLY WOULD HOPE THAT GMC WOULD STEP UP & DO THE RIGHT THING TO PLACE A RECALL ON THE DASHBOARD BEFORE SOMEONE GETS INJURED. IF THEY CARE ABOUT PEOPLES LIVES, THERE SHOULD NOT BE A PROBLEM.

General Motors, LLC (GM) is recalling certain 2012 Cadillac Escalade, Escalade ESV, Escalade EXT, Chevrolet Avalanche, Silverado 1500, Silverado 2500/3500, Suburban, Tahoe, GMC Sierra 1500, Sierra 2500/3500, Yukon, and Yukon XL originally sold, or ever registered, in the states of Alabama, California, Florida, Georgia, Hawaii, Louisiana, Mississippi, South Carolina, Texas, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands or "Zone A." These vehicles are equipped with non-desiccated passenger frontal inflators containing phase-stabilized ammonium nitrate (PSAN) propellant. These inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, high temperatures, and high temperature cycling.
Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants, resulting in serious injury or death.
Remedy: GM will notify owners, and dealers will replace the front passenger air bag inflator with an alternate one, free of charge. Interim letters notifying owners of the safety risk were mailed on February 22, 2021. Second notification letters will be mailed once the remedy is available. Owner notification letters were mailed on May 11, 2021. General Motors LLC (GM) is recalling certain model year 2011-2012 Cadillac Escalade, Escalade ESV, Escalade EXT, Chevrolet Avalanche, Silverado HD, Silverado LD, Suburban, Tahoe, GMC Sierra LD, Sierra HD, Yukon, and Yukon XL vehicles. In the affected vehicles, the ignition lock actuator may bind, making turning the key difficult or causing the ignition to get stuck in the "Start" position.
Risk: If stuck in the "Start" position, the ignition may suddenly snap back into the "Accessory" position, causing a loss of engine, steering, and braking power, increasing the risk of a vehicle crash. If the vehicle is in a crash, the air bags may not deploy, increasing the risk of occupant injury.
Remedy: GM will notify owners, and dealers will inspect and replace the ignition lock housing, as necessary, free of charge. The recall began on February 12, 2015. Owners may contact GM customer service at 1-800-458-8006 (Cadillac), 1-800-222-1020 (Chevrolet), or 1-800-462-8782 (GMC). GM's number for this recall is 14696 for the original equipment, and 14912 for the service replacement parts.

Investigations are agency reviews, not findings that a vehicle is defective.
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
