COMPONENT: ELECTRICAL SYSTEM / TELEMATICS (DCM)The vehicle suffers from a documented manufacturing defect involving the Data Communication Module (DCM/Telematics system, Calibration N01D14B4644). The module enters an infinite transmission loop when the vehicle is turned off, causing a severe parasitic battery drain that repeatedly drops the 12V auxiliary battery below operational voltage.This creates a severe safety hazard, causing total electrical failure and leaving the driver unexpectedly stranded without warning, presenting an immediate safety risk in extreme weather or unsafe locations.An official Toyota Techstream diagnostic scan on 07/08/2026 confirmed 7 active DTC faults in the telematics system. Authorized dealerships are refusing to perform the mandatory firmware updates outlined in manufacturer service bulletins (T-SB-0089-21), opting instead to clear error logs temporarily without repairing the underlying defect. The manufacturer has failed to issue a safety recall despite widespread knowledge of this safety defect.

My 2022 Toyota RAV4 has experienced failures of the power door lock actuators on both front doors. Neither the driver's side nor the front passenger door consistently locks or unlocks using the key fob, interior power lock switch, or automatic locking system. This creates a significant safety concern because the doors may remain unlocked when the vehicle is parked, creating a security risk, or they may fail to unlock quickly during an emergency. In addition, occupants may believe the doors are secured when they are not. The problem has occurred on both front doors, indicating this is not an isolated component failure. The issue developed under normal use and was not caused by an accident, abuse, or modification to the vehicle. Multiple Toyota owners have reported similar failures involving door lock actuators in the same generation of vehicles, and there is pending litigation alleging Toyota has long been aware of this defect but has not issued a recall. I am submitting this complaint because I believe this defect presents a safety risk and warrants investigation by NHTSA

Our 3 yrs old + Toyota RAV4 Hybrid SUV with barely over 30,000 miles on it had 2 safety issues within 2 months of each other! First was the driver's side mirror- it was rattling, feeling like it might fall off! Seems common issue, perhaps due to cheap plastic parts inside. We had it fixed- Toyota Brand Engagement Center gave a 'Goodwill' gesture to cover us replacing it at Dealership, though it should never have happened on such a newer vehicle in the first place! Then not even 2 months later, a Warning flashed- "Pre Collision System Malfunction!" Also "Headlights" with same warning- "SEE DEALER NOW!" Toyota Dealer. They said Forward Recognition Camera had 'INTERNAL MALFUNCTION' and needed to be replaced. It was going to be quite expensive, but the camera part is not even available in foreseeable future as there's back order for hundreds and they're still in production! It seems this too is a common safety problem~ We can't understand why there's not a RECALL on this. Toyota took no responsibility for this issue, which is APPALLING as, like the first 'side driver's side mirror failure' was also a safety issue! The whole reason we bought this vehicle was for its safety features! Without the front facing camera that comes with the car in same compartment as rear view mirror resides, we now have NO Pre-Collision Warnings, NO Headlight safety feature, NO RSA (Road Sign Assist System). It's infuriating that Toyota, who obviously is making inferior safety products, does not offer to help out in any way. We're both [XXX] and entitled to these safety features as we bought the car both new and paid top dollar! Our service advisor at the Toyota Dealership agrees this is 'not normal', the car is too young to have this happen! He will attest to this & we have paperwork. It stated 'No immediate remedy'. After driving for approximately 30-minutes in a heavy rain i pulled over and the automatic engine shutoff feature activated while i was waiting to go inside for an appointment. When attempting to restart the car, the engine would start but then shut off showing a "low power steering" warning message on my dash. The problem persisted for approximately 15-20 minutes. Eventually after about 10 attempts to restart the engine, the car started back up and drove fine the rest of the day. I have heard of this happening to many other rav4s on the Autosafety.org website and it causes me concern, especially if this problems causes the car to stall in high speed traffic in the rain. It is my understanding that this maybe a problem with the Mass Airflow Sensor getting wet in heavy rainfall.

The panoramic moonroof on my 2022 Toyota RAV4 spontaneously shattered and exploded outward at approximately 2:09 PM on June 21, 2026 while my wife was driving and exiting the freeway. There was a loud bang, followed by glass shards ejecting primarily outward onto the roof with a pattern showing the glass failed/bent from the inside out. The outer glass panel broke into multiple large sections with a clean perimeter failure and no visible external impact point or road debris damage. This created an immediate safety hazard: loud distraction while driving at highway speeds, flying glass, and sudden exposure through the roof opening. No warning lights, messages, noises, or prior symptoms appeared. The moonroof had been used normally with no issues. The component (panoramic moonroof glass assembly) is available for inspection. I have a dealer appointment scheduled for tomorrow, June 22, 2026 at 9:45 AM for inspection. The vehicle has not yet been inspected by the dealer, manufacturer, insurance, or police. This matches numerous similar reports of spontaneous panoramic sunroof failures on RAV4 models.

I was beginning to slowly back out of a parking space in a super market parking lot. I had not put my foot on the accelerator because my car begins to slowly inch backwards once it is put into reverse. As the car began to inch backwards, I saw a car approaching down the parking lot driveway behind me. I immediately stepped on my brake, but instead of stopping, my car began to race backwards, accelerating at a fast speed. The car never responded when I put my foot on the brake, so I was unable to stop the car. This caused me to crash into the car driving past me, and that finally stopped the car. Because I was almost perpendicular to the car I hit, and the driver was unable to get out of his car, I then put my car into drive to move it out of the way. My car accelerated out of control, again, this time forward. Again, it did not respond when I put my foot on the brake. My out of control car was quickly heading toward a large metal fire gate. In complete desperation, I stomped very hard on the brake, pressing it to the floor as I was about to hit the gate, and the car finally stopped. In both incidents, my brakes completely failed to stop the car accelerator which was acting on its own. The problem has not been reproduced by Toyota, which says there is nothing wrong with the car. However the data report they gave us is suspect because the data shows no reverse movement of the car and labels the accident a rollover, which it was not. In addition, the inspector told my husband there were more tests he could do, but Toyota had not authorized him to do them. There is a police report, and a witness who observed the whole thing. She said that because of the way the car was moving, it looked like a hit and run accident with the driver trying to make a speedy getaway. There were no warning lights or messages displayed.

I am submitting this complaint to report a known and widespread safety defect involving the door lock actuators on my Toyota RAV4. My vehicle’s door actuators have failed at approximately 39,000 miles, just outside the 3‑year/36,000‑mile warranty. This failure prevents the doors from locking and unlocking properly, which poses a significant safety hazard while driving, especially in emergency situations. When I brought the issue to Beechmont Toyota, the dealership refused to repair the defect under goodwill or warranty consideration. I spoke with Stacy Meyer, Assistant Service Manager [XXX] ), who stated that the refusal was directed by Rodney Wetenkamp, Service Director. Despite the well‑documented nature of this defect, the dealership declined to assist in any way. This is not an isolated incident. There is an active class action lawsuit in Texas regarding defective Toyota RAV4 door lock actuators, demonstrating that Toyota is aware of the widespread nature of this problem. Toyota advised me to work through the dealership, stating that the dealer should escalate the issue through their internal channels. When the locks fail: Doors may not lock while driving, exposing occupants to risk Doors may not unlock in an emergency Children and passengers may be unable to exit the vehicle quickly The vehicle becomes vulnerable to theft and unauthorized entry Given the known defect, Toyota’s awareness of the issue, and the dealership’s refusal to escalate or repair the problem, I believe this represents a failure of manufacturer responsibility and a potential violation of safety standards. I was driving through pretty heavy rain for about 65 miles. When I got to my destination, I was idling in the car for a few minutes when the car started to shake. I turned the car off. Upon turning it back on the engine would start and the car would shake before getting a message “engine stopped. Power steering low.” After two hours, I was able to turn the car on and move a little bit but the car was lurching and the RPM needle was rapidly bouncing between 1,000 and 2,000.

The moon roof glass seems somehow dangerously tight or prone to exploding. This could endanger passengers and other vehicles. Out of seemingly nowhere the moon roof exploded while driving off the high way. I saw no flying object. We were not behind other vehicles that could have kicked something up. We hadn’t gone under a bridge. Glass began to crack and tinkle and cascade off the vehicle, possibly endangering other cars behind us. Also, the broken glass might have fallen in onto passengers. We slowed and exited the highway, and inspected the glass. There did seem to be one location where some object might have hit first, causing the shatter. But the evidence of a single impact point is not strong. We will never know exactly why the Moon roof exploded. I tapped the glass moon roof extensively and we drove home. There was no warning. The vehicle is available to inspect, but is getting repaired tomorrow. The repair shop has inspected it. The insurance people have seen the report from the body shop and will pay ask cost above my deductible. Total repair cost estimated at $5600 minimum at this time

Incident Detail: In April 2026, my 2022 Toyota RAV4 Prime (VIN: [XXX] ) suffered a critical failure where the charging cable became physically locked in the vehicle’s high-voltage (HV) charging port. I transported the vehicle to Ken Garff Cheyenne (authorized Toyota dealer), where management allowed non-certified, non-technical staff to perform unauthorized physical interventions on the HV port. No manufacturer-certified technician was provided for a safe diagnostic. ​Safety Hazard and Immediate Consequence: Following these unauthorized interventions, my wife (an educated automotive technician) determined the vehicle was in a hazardous state, risking thermal runaway or electrical fire. This hazard was so severe that the owner of the property where I was staying in Cheyenne—a [XXX] dependent senior—was too frightened to allow the vehicle to remain on the premises. Due to this immediate safety risk and the dealership's refusal to coordinate a safe tow, I had to have the vehicle towed to a neutral facility to protect the life and property of a vulnerable widow. ​Legal and Technical Context: I am a permanent resident of San Diego, California, and this vehicle is covered under the California Song-Beverly Act. The vehicle is also subject to Federal Recall 23V478 (DC-DC Converter Fire Risk). The dealership’s unauthorized tampering with the HV system, combined with ambient temperatures in Cheyenne often falling below the 41°F safety threshold for this system, created an unmitigated fire risk that the manufacturer has failed to address. ​Request: I am seeking a federal investigation into the dealer’s failure to follow High-Voltage safety protocols. I demand that Toyota Motor North America provide a Field Technical Specialist (FTS) to perform a safe diagnostic at the neutral facility where the car is now located, as the dealership’s actions rendered the vehicle a public safety hazard. The contact is an attorney concerned about a crash involving a 2022 Toyota Rav4. The attorney stated that the vehicle had a comma.ai device installed on the vehicle. The attorney stated that while 2 couples were occupying the vehicle and driving at night on an expressway, there was a police vehicle on the side of the road responding to a separate vehicle crash. The driver of the vehicle may have been asleep, along with the front passenger, while relying on the comma.ai device to operate the vehicle. The vehicle failed to engage the brakes in order to avoid crashing into the police vehicle, and as a result, the Toyota Rav4 crashed into the rear of the police vehicle. The vehicle speed was unknown at the time of the crash. The owner of the vehicle rear-ended a Louisiana state trooper. Air bags deployed in the front and on both sides of the vehicle. The vehicle was towed to a tow yard and then to a Progressive Insurance lot. No warning lights were noticed. All occupants were wearing seat belts. The driver of the vehicle sustained concussion injuries, and the front passenger occupant sustained arm and leg injuries. The two rear occupants succumbed to their injuries at the hospital. No other injuries were reported as the police officer was not occupying the vehicle during the impact. The comma.ai device failed to avoid the f vehicle in front. The manufacturer was not notified of the failure. The failure mileage was unknown.

After turning my car on and the car running for several minutes, the engine shut off on its own and it said “engine stopped steering power low”. This is the second time this has happened. The vehicle is available for inspection. My safety was put at risk because I am concerned about this happening while driving, or being stranded somewhere. I have not taken my vehicle in yet, but many other RAV4 owners have this issue from my internet searches. This has happened twice to me in the past two weeks. The vehicle has not been inspected. My infotainment screen went black while I was driving at night and it will not come back on. There was no warning. I took it to the dealer and they said that the system had failed. I saw on line that there was a recall for this very issue but my car is a '22 and not included in the recall, but I think it should be. This poses a safety hazard and I don't believe that this should happen under normal circumstances for a car of its age and relatively low mileage. I think this model year should be included in the recall and that Toyota should pay for the repair, which is quite expensive. The receipt on the diagnostic says the radio works, but it does not. It goes on when you push the power and immediately goes off.

We were traveling across country from Michigan to Arizona. We were 50 miles outside Tucson, AZ on [XXX]. Traveling 80 mph in a 75 mph zone, in passing lane and felt like car exploded and all airbags deployed and we came to a quick stop. No major injuries to either of us. One vehicle incident, did not collide with anything or hit any debris. It was a clear day, road conditions good, no noises, lights etc. My husband was able to keep from rolling and we came to a stop with many semi's etc flying by, very scary. When we were able to safely get out and call 911, there was significant drivers side front end damage around tire area and inside near axle. Looked like something broke inside car, sheered off bottom of tire. Police arrived and had to push vehicle on to shoulder of road. He assessed scene and thought it had to be a mechanicla failure as there was no other vehicle or debris on site. We did contact our insurance company and subsequently it was totaled. We also have Toyota investigating what the cause was if it was a faulty part or mechanical failure. We have not heard back but did receive the "recording of time of crash". I was driving home in the rain for about 75 minutes and when I arrived in my driveway I saw steam coming from the front grille area. It wasn't due to a high engine coolant temperature so I decided to carefully open the hood and check for the source. I saw water inside the bottom of the engine bay behind the radiator steaming from the heat of the engine. A few seconds later, the engine stalled and the following message appeared: "Engine Stopped, Steering Power Low" which came on with a check engine light, red battery light, and steering light. I used my scan tool and obtained the code: P0171, System Too Lean. After doing research, I've come up with a conclusion to the potential cause. Starting with the 2022 model year RAV4 non-hybrid (and onwards to the 2025 model year RAV4), the air intake inlet was redesigned from the prior one in the 2019-2021 RAV4 non-hybrid models, as seen in numerous comparison images online. This new design presents a dangerous fault. When driving in rainy conditions, especially at higher speeds with mist kicked up from cars in front, the air intake will take in water. The water will flow to the air filter and then reach the Mass Air Flow sensor (MAF). The MAF sensor monitors the amount of air entering the engine and sends signals to the car's computer to help determine how much fuel to send to the engine to maintain an ideal ratio for optimal efficiency. Considering that water is getting to the MAF sensor, it thinks that the water is excess air, so less fuel is sent to the engine as a result. Since I came to a stop in my driveway, the engine is now at its idle speed and only commands a small amount of fuel to remain running. Now, when combined with the water, that lower fuel flow is practically cut off, leading to the stall. This happened to me before when driving in the rain, but in more dangerous conditions in stop-and-go traffic. Toyota must follow up on T-TT-0763-24 because it has been well over a year since their initial investigation.

Driving up a slope of about 20-30 degrees gradient of a straight lane going into a private parking lot. Braked and decided to make a 3-point turn to get back out. There was no traffic in the lane. Put gear on reverse while on the slope with the vehicle pointing in the up direction of the slope. Vehicle suddenly surge backwards, accelerating. Vehicle sped down the slope and crossed a street. Could not stop. Vehicle was stopped only after placing gear into park from reverse, and after jumping curb across the street. Fortunately, there were no cross traffic in either direction on the street, otherwise vehicle could have been T-boned. Tire wall on the rear passenger side busted. AAA was called to replace tire with spare. There were no warnings on the dashboard or any symptoms anywhere else, although the vehicle had all the safety features.. The next morning, vehicle was taken to Toyota of Carlsbad. Left there for diagnosis and tires change. Vehicle is left with Toyota of Carlsbad until this issue is answered/solved. Told by service manager of Toyota of Carlsbad to call Toyota USA at [XXX] to report incident by customer, and request Toyota USA to send expert from Toyota to diagnose issue. Approximately six minutes later, I exited my home and approached the vehicle, which was not running. After unlocking and entering the vehicle, the interior temperature confirmed that the vehicle had not been running. I attempted to start the vehicle by pressing the brake pedal and pushing the start button. No dashboard lights illuminated, no warning messages appeared, and the vehicle was completely unresponsive. I attempted this process two additional times with the same result. After exiting the vehicle and closing the driver’s door, I attempted to lock the vehicle using the touch sensor on the driver-side door handle. The vehicle would not lock. I attempted this a second time and it still would not lock, indicating a complete loss of electrical power. This failure occurred in cold weather conditions and involved the hybrid battery and associated electrical system. The loss of power created a safety risk because the vehicle could not be secured and could have left me stranded in cold weather. Later that day, I contacted my insurance company for roadside assistance. A tow truck arrived and jump-started the vehicle using a portable jump box, which allowed the vehicle to start and the battery to recharge. A Toyota service appointment is scheduled to inspect the vehicle. The issue has not yet been confirmed by a dealer. The vehicle has not been inspected by the manufacturer or other representatives. Only the roadside assistance tow truck driver responded. The vehicle and battery are available for inspection upon request.

The Panoramic Sunroof Shattered on its Own out of nowhere. It just made loud noise. we thought it was tire, but car did not make move, looking above we saw the panoramic section of sunroof shattered. It was a rainy day and I had come home to pick up someone, didn’t turn my car off but had it in park. I went to reverse and pressed on gas, all of a sudden my engine started to aggressively shake/vibrate while showing on the dash that power steering was off, I couldn’t move my car anymore. At the car wash while slowly pulling (1mph) into a parking space to vacuum the vehicle the car unexpectedly accelerated. The brakes failed and the car continued to accelerate until forced into park and turning off the vehicle. During the acceleration there was a grinding noise. This acceleration caused the car to hit the front curb and cement pilings with poles supporting the roof and vacuum. There were no warnings or lights being presented. If not for the curb and being off the road anyone in the car would at the very least been injured and possibly hitting another auto. The dealer is claiming no none issues as described. The auto was towed to the body shop to inspect and repair. We were told by the body repair shop the front suspension failed. The auto was great prior to this incident. Afraid to drive for fear of a repeat especially due to others on the internet claiming the same issue has happened and often continued to have a repeat acceleration incident. No one has found resolution from Toyota. Approximately $7,000.00 worth of damage and our insurance has now gone up.

After driving for 35 minutes (in the rain), I pulled up to work and let someone out. While car was sitting there in park, it bucked in the back, I then put it into park and almost immediately, it shut off with this on the display "Engine Off, Steering Power Low, Do Not Drive". My lights and windshield wipers remained on. I tried to turn it back and and it shut off again. I waited a few minutes, turned it on and went to pull out onto main road and the car shut off again with the same message. Thankfully I was turning onto the main road so close to shoulder and able to keep my car pretty close to side of the road. I called the Toyota dealer who told me not to drive it and it would need to be towed. I towed it to the closest dealer and when they checked the codes (the next day), they said there were no codes and they could not replicate the problem. As a note, the rain was off and on, nothing that was worse than anything I have driven in previously. I drive main roads and did not go through any large puddles or anything similar. This is a major safety issue and I am terrified that it could happen while driving on a highway where I may not have time to get out of the way of other traffic (car literally stopped with no option of continuing to coast). I am also shocked that there were NO codes triggered as there was obviously some event and that the car is only one month outside of warranty.

While getting gas the car was running and I got a warning that said engine stopped power steering low. I started the car and everything was fine and was able to drive with no issues and haven't seen the warning. I know sometimes dealerships have reminders for maintenance and not sure if this was just a glitch. After reviewing forums this seems like a known issue on Toyota but the vehicle hasn't been recalled some users have said car was fine while others say battery or alternator. After driving for approximately 30-minutes in the rain I pulled over and the automatic engine shutoff feature activated while I was waiting to pick somebody up. When attempting to restart the car, the engine would not turn over and I got a low power steering warning message on my dash. The problem persisted for approximately 15-20 minutes. Eventually after about 15 attempts to restart the engine, the car started back up and drove fine the rest of the way home. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Rav4, Rav4 Hybrid, and Rav4 Prime vehicles. The front passenger seat may have been assembled with interference between internal parts that may cause the Occupant Classification System (OCS) sensor to incorrectly detect the occupant. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."

Risk: Incorrect detection of an occupant may result in improper air bag deployment during a crash, increasing the risk of injury.

Remedy: Dealers will inspect and repair the front passenger seat, as necessary, free of charge. Owner notification letters were mailed September 2, 2022. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 22TA08.
