The hands free feature over the cars microphone/stereo is unusable; leaving vehicle owners frustrated, and more likely to operate cellphones outside of standard hands free practices. I have been on numerous phone calls where people have told me that they were unable to hardly hear a thing I was saying. To remedy this problem I have 1. Tried speaking up (even yelling) 2. Turning off HVAC systems 3. Positioning my body closer to the microphone 4. Bringing the car to a slow or stop to reduce road noise. 5. Calling myself to see if I can hear myself. 6. Talked with someone who owns the same car (year/model/spec/trim) and they were hardly audible as they called me over their hands free system.

While I am driving my Toyota RAV4, the car will suddenly shut off and I can restart it and keep driving as the month is going on. The incidents are happening more often and now the dashboard is going black. I’ve had a couple of warning signs on the dashboard. I brought the car to Toyota and Pep Boys. Both of them checked the electrical systems and everything looks good according to their testing.

Severe, chronic rainwater intrusion through the vehicle's front cowl and body seams, resulting in catastrophic flooding of the passenger-side cabin floorboards. This issue occurs even when all sunroof drains are completely clear and functional. Toyota has issued Technical Service Bulletin 0055-24 acknowledging body seam and cowl leaks for this vehicle type. The passenger-side floorboard houses critical electrical wire harnesses and sits adjacent to the Supplemental Restraint System (SRS/Airbag) control module. Chronic standing water on the floorboard presents an immediate risk of electrical short-circuits, potentially causing sudden vehicle stalling, electrical component fires, or the uncommanded deployment/failure of the vehicle's airbag safety systems. Furthermore, the thick foam insulation beneath the carpet remains saturated, creating an immediate, toxic mold hazard within an enclosed passenger cabin, severely impacting occupant health and air quality. The manufacturer and dealership have left the vehicle unrepaired for over 30 days, refusing to remediate this safety hazard.

Malfunction: Mechanical failure of the Torque Converter Clutch. The vehicle exhibits a persistent transmission shudder during acceleration between 25–50 mph. Warning Signs: None. There were no dashboard warning lights or error messages prior to or during the failure. The only indicator was the physical shuddering of the vehicle. Safety Risk: The malfunction causes unpredictable hesitation when merging or changing lanes, creating a dangerous speed differential and risk of rear-end collision. Documentation: Comprehensive maintenance records are available to prove the vehicle was serviced according to the manufacturer’s schedule at a third-party facility.

On October 27, 2025, I was housesitting and watching a friends' son and dropped him off at school. I returned to their house, shut off the car, went into the house, started cleaning home in preparation of returning to my home. I was in downstairs master suite on opposite end of home when I heard a car horn. I went to the door, looked out and saw fire emitting from under the driver’s side wheel well, front of engine, from under the engine hood and half way up the windshield of the RAV4. I called [XXX] and informed the dispatcher that my car was on fire. The [XXX] fire department responded and extinguished the vehicle. I called my insurance company and also reported the incident to Toyota BEC, they assigned the incident Liability Case# XXX. I had also received 2 recall notifications from Toyota (NHTSA# 23V734). The car had been serviced by Grappone Toyota (now known as McGovern Toytota) in [XXX] on August 14, 2025 and the recall was documented as fixed on that date. The vehicle was a total loss and towed to a Coparts Salvage Yard in Candia NH. An inspection was performed by an inspector assigned by Toyota. I later received a letter from Toyota stating that the cause of the fire could not be determined and Toyota assumes no reponsibility whatsoever. While picking up my son from school, my 2018 Toyota RAV4 Hybrid suddenly experienced a complete steering failure. The steering wheel locked without warning and became impossible to move, making the vehicle inoperable and creating an immediate safety hazard. My son and I were stranded for over 7 hours in a public area. AAA was dispatched twice, but they were unable to resolve the issue. The vehicle ultimately had to be towed to a Toyota dealership. Toyota dealership diagnosed the car with a failed steering lock ECU, a critical safety component that should not malfunction under normal conditions. I was quoted over 2,000 for the repair. This issue aligns with multiple known recalls and technical service bulletins related to steering and electronic control failures in RAV4 Hybrid models, suggesting a potential manufacturer defect. This sudden steering failure placed me and my child at significant risk. There were no warning lights, no prior steering issues, and no indicators of malfunction before the event occurred. I am submitting this report because a steering lock ECU failure can lead to sudden loss of steering control, which poses a serious danger to vehicle occupants and others on the road.

XXX XXX XXX XXX XXX 11/13/2025 National Highway Traffic Safety Administration Office of Defects Investigation 1200 New Jersey Avenue SE Washington, DC 20590 Toyota Motor Sales, U.S.A., Inc. 19001 South Western Avenue Torrance, CA 90501 Subject: Complaint Regarding Torque Converter Shudder – 2018 Toyota RAV4 AWD To Whom It May Concern, I am submitting this complaint regarding a serious transmission defect in my 2018 Toyota RAV4 AWD, specifically a torque converter shudder that has been documented by Toyota technicians. The vehicle exhibits pronounced shuddering between 25–50 mph under light throttle, consistent with the torque converter failures previously acknowledged by Toyota in earlier RAV4 models (2013–2015). Toyota has extended warranty coverage for torque converter replacement in those earlier models, yet my 2018 RAV4—despite showing identical symptoms—has been excluded. The dealership has quoted me 9,000 for a full transmission replacement, even though the issue is well-documented and appears to be systemic across multiple model years. This situation raises serious concerns: - Toyota is aware of the defect but has limited coverage to select VIN ranges. - The cost of repair is excessive and disproportionate to the defect. - Consumers are left without recourse despite clear precedent for warranty extension. I respectfully request: 1. That NHTSA investigate whether the torque converter shudder in 2016–2018 RAV4 models constitutes a broader safety defect. 2. That Toyota extend its warranty program to include affected 2018 RAV4 vehicles. 3. That Toyota provide reimbursement or coverage for necessary repairs to restore safe vehicle operation. I am prepared to provide service records, dealership documentation, and correspondence confirming Toyota’s acknowledgment of the defect. Please advise on next steps and ensure this issue receives the attention it deserve INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

My vehicle has a recall for the battery. I took my car into Toyota in San Juan Capistrano. Car was purchased and owner’s manual states this vehicle uses a size 35 battery. When I returned to pick up my vehicle the service technician told me that “Toyota will only honor the recall on vehicles with a size 36 battery, and in order for them to rectify the recall was for me to purchase a size 36 battery. There is no mention of that on our recall notice nor is there anything stating that on the NTSA website. What can i do to get this fix this issue. I believe this problem is also the reason we have had to replace our battery 3 times since 2021and also the cause of other malfunctions in our vehicle

I purchased my 2018 Toyota RAV4 Limited Hybrid in June 2018. The automatic liftgate has been a consistent issue with this vehicle since January 2024. It either fails to close completely or takes an unusually long time to shut. I followed the official Toyota dealership videos and performed the recommended recalibration steps, but the problem remains unresolved. This is not just a matter of convenience. It presents a safety concern, particularly when loading items at night. Living in the Pacific Northwest, where it gets dark around 5 p.m., makes this even more concerning. I’m aware there is existing litigation related to this issue.

After receiving a recall notice from Toyota for the battery mount I scheduled my appointment on June 24, 2025. After spending $916.31 on maintenance products all of a sudden the dealership did not have the battery mount for my vehicle. Is this legal? Why would a dealership schedule a repair and not have the product to complete the recall? I was informed that I would be called when they had the part available and I still have not heard from the Toyota dealership. Are three months later considered a timely matter to correct the problem?

On September 3, 2025, I was driving down the I-10 in Biloxi, MS, at 6:45 a.m. It was around 75-80 degrees and the sun was just barely coming up. All of the sudden the glass in my sunroof completely shattered. There were no cars around me and it was an open portion of the road, so no rocks were flung up by nearby vehicles. Luckily, my kids were not with me, because a majority of the glass ended up in my backseat, ranging from huge chunks of about 3-4 inches in diameter to smaller shards being everywhere. I cut myself several times while trying to clean it up, so I can only imagine what it could have done to my kids! Luckily, I was able to keep my cool and not swerve off the road, as it was quite startling.

The vehicle's battery area caught fire. I was able to get out of the vehicle on time and call the fire department. Over 50% of the vehicle was consumed by the fire. Vehicle was my spouse's and she usually drives it alone with my [XXX] and she is [XXX]. Thankfully, I was driving it alone. NHTSA recall 23V734 came out on 11/2023 and they expected to have the fix by the end of 2024 but it never happened. The last vehicle maintenance by the dealer was in 02/15/2025. The vehicle has not been inspected yet but the insurance will make an appointment soon to do so. The preliminary findings of the fire department pinpointed the problem in the battery area. Prior to the fire, the vehicle shut down and the steering wheel locked. I turned it on and drove it out of the road and it did it again and when I came out of the vehicle, I saw smoke coming out of the hood and that's when I saw the fire. I took a video: for at least the 6th time since owning this vehicle a multi alert shows up on dash. “Brake Power Low, Visit your dealer”, “Antilock Brake System Malfunction, visit your dealer”, “Lame departure alert, visit your dealer”, “Pre-collision system malfunction, visit your dealer”, “AWD system malfunction, 2WD mode engaged, visit your dealer”, I have 2 other Rav 4s both of which have also shown this issue. A 2016 & a 2017. The 2017 this happened twice I believe. The 2016 at least twice. It’s currently on now in my 2018. Been on 2 days, but was off previous 2 days, but on 1 day before that.

Investigations
1 NHTSA investigation on record
Investigations are agency reviews, not findings that a vehicle is defective.
The Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE) to understand the contributing factors and frequency of vehicle fires originating from the battery region of the engine compartment in Model Year (MY) 2013-2018 Toyota RAV4 vehicles on 25 February 2021. At the time of opening, ODI attributed eleven allegations and additional EWR events to this PE. PE21005 focused on replacement batteries installed in the subject vehicles. No reported fire event identified in PE21005 involved the originally supplied 12V battery. The reported fires took place in vehicles known to contain aftermarket batteries or whose age exceeded the expected life of the original equipment battery. ODI identified battery dimensioning as it relates to the battery retention system (hold down bracket, radiator support bolt, J-hook, and battery tray) as the main factor set in the contact between the battery hold down bracket contacting the positive terminal of the 12V battery. The subject vehicles carried over the same battery tray and retention system from the prior generation (MY2006 – MY2012) of RAV4 vehicles. However, the subject vehicles were equipped with a dimensionally smaller battery (Group 35) than the prior generation (Group 24F). The smaller- length measurement in the Group 35 battery size in a battery tray originally designed to fit a Group 24F battery contributed to increased potential for battery movement which was exacerbated by the diversity in aftermarket battery configuration and irregularities intrinsic to battery replacement procedures. Battery movement coupled with the proximity of the B+ terminal on the battery and the grounded electrically conductive hold down bracket set the stage for a short to ground leading to an engine stall (if in operation) followed by a vehicle fire. This investigation was not able to classify the installation status, battery size, or cause of every allegation due to the fire event causing damage to the area, inaccessible vehicle, modification to the battery retention system after the event, and/or inconclusive photographic evidence. Toyota's investigation of the issue identified aftermarket battery catalogs that listed an improper battery replacement size (Group 26R) that would nevertheless fit into the subject vehicles whose battery retention system is incapable of reliably restraining a Group 26R battery. On 18 November 2021, Toyota initiated a consumer advisory campaign (21TG01) to the subject vehicle owners, notifying them of the correct size battery and offering a free inspection of the battery and retention hardware, a caution label to affix to the hold down bracket, and a discounted replacement 12V battery. This investigation with Toyota established three broad categories to characterize the battery retention system state of a given vehicle: "correct", "minor mis-installation", and "major mis-installation". - The "correct" terminology references the front bolt, bracket, and J-hook installed in a position not in contact with the positive battery terminal and the front bolt torqued to 17 Nm and the J-hook engaged with the tray hole and torqued to 4.9 Nm. - A "minor mis-installation" would describe the three retention components installed in the prescribed location, however not to the correct torque values. - The "major mis-installation" condition would constitute a retention component missing or disconnected from the system. Toyota's own testing used five exemplar batteries installed on a subject vehicle in each battery categorization resulted in: - Contact between the B+ terminal and hold down bracket in "major mis-installations" - Movement of the battery in "minor mis-installations" - No observable motion in "correct" installations. ODI conducted its own evaluation of the Toyota- specified replacement battery and 5 aftermarket Group 35 batteries. The six batteries tested conformed to the Group 35 Battery Council International (BCI) standard on overall dimensional tolerance and terminal location. That standard does not define the edge shape or material of battery casing. Physical interaction between the hold down bracket and the battery case and the friction between the casing material and the battery tray resulted in a large variability in movement forces required to incite movement when the battery was installed in the "correct" condition. Resulting in movement at an induced lateral load between 0.6 G and 1.91 G equivalent force across each battery. This force is higher than a typical vehicle could exert in a steady state turn and would only experience loads through brief shock or impulses. Multiple shock or impulse loads across time would contribute to a delay between the replacement battery installation and the fire event. Likelihood of a fire event increases as the installation method degraded from the "correct", to "minor mis-installation", and then to "major mis-installation". Protruding vent caps atop some of the batteries tested aided in restricting battery movement relative to the hold down. An absence of vent caps permitted additional movement. Following several discussions and vehicle inspections with ODI during this investigation, on 01 November 2023, Toyota filed a safety recall (NHTSA 23V-734) of 1,853,568 MY 2013 through MY 2018 Toyota RAV4 vehicles to reduce the risk of a vehicle fire with redesigned battery retention components.
