I am reporting several safety concerns based on approximately one year of experience using Tesla Full Self-Driving (FSD). Primary Event: Between February 23 and February 28, 2026, while traveling northbound on the NJ Turnpike with FSD engaged, the vehicle accelerated to approximately 95 mph in a 65 mph. The acceleration appeared unexpected and excessive for the traffic conditions. I disengaged FSD and immediately took control of the vehicle to reduce speed and continued driving manually.No collision occurred, but I considered the event a safety risk because the vehicle exceeded the posted speed limit by a substantial margin. Additional Observations: 1. On multiple occasions, FSD appeared not to recognize or comply with “NO TURN ON RED” traffic signs.2. Situations where overhead or elevated traffic signs appeared not to be recognized by the system.3. On one occasion, the vehicle navigated toward railroad tracks where the roadway and tracks intersected at an approximate 45-degree angle. The vehicle behavior caused concern.4. When driving directly into bright sunlight, particularly when the sun is low and shining directly into the windshield, FSD performance appears degraded or confused. Human drivers typically use sun visors and other visual cues in these conditions. The vehicle’s driving behavior appeared less predictable during such events. 5. Lane changes are attempted across areas marked by continuous lane where lane change would not normally be expected. Additional Concern: A significant regulatory issue may involve situations where drivers intervene immediately before a collision. In real-world driving, drivers often instinctively take control when they perceive an imminent hazard. Therefore, when evaluating crashes involving FSD, I believe investigators should review vehicle data for period immediately preceding a collision, including several seconds before any disengagement, rather than focusing solely on whether FSD was engaged at the exact moment of impact.

As I was driving the Tesla Model X, I received a steering assist reduced error on the dash. There was no immediate noticeability, but after about 5 minutes, it felt like there was ZERO power steering, with no additional warnings in the car. With zero power steering, car required extreme forces required to get to the shoulder. If attempting any kind of turn, car would require forces in excess of 80lbs of pressure to execute. Once safely in a parking lot, I put the car into SERVICE mode to check the errors. There was an EPAS error, where it said Torque Sensor 1 not receiving, along with a Torque Sensor 2 not receiving. This has occurred to me 3 times, in the 3 years I've owned the car. Other than "Steering assist reduced", there is no other warning that the car's EPAS steering system reverts to just manual forces required to steer a car. I do not trust my wife to drive this car, as she would not have the strength necessary to get the car to a safe location. If this happened while turning, there's a strong possibility the car would not turn at all, a huge safety factor.

The steering wheel locks periodically and I have to get out and re enter my car. On long drives it works pretty fine and does not do this much on highways only once but in hotter weathers it seems to be much much worse. This happened once before the warranty but I did not think much of it. In one instance the steering wheel was so tight that it hurt my wrist while getting off the road

When I brought the vehicle to the manufacturer's official service center to have the defective safety latch inspected and repaired, they confirmed the behavior but refused to fix the underlying hardware. Instead, the service center officially documented on my repair invoice that this latch failure is "frequently present and deemed a characteristic of the vehicle." A malfunctioning, binding, or misaligned front trunk latch poses a severe safety risk. If the primary latch is failing to release properly, it indicates a defect in the retention mechanism that could lead to the hood unexpectedly opening, flying up, and obstructing the driver's vision at highway speeds (a known issue that has caused recalls in the past). The manufacturer's refusal to repair a failing primary safety latch, and instead documenting it as a normal "characteristic," leaves a critical safety component in a compromised and dangerous state. There were no warning lights prior to this failure; it is a physical, mechanical defect that the manufacturer has acknowledged in writing but refuses to remedy.

The failure involves the rear suspension system, specifically suspected rear control arm / suspension linkage play, which caused severe misalignment (rear toe out of specification). The vehicle is available for inspection upon request. This defect created a significant safety risk. The abnormal toe condition caused rapid and uneven tire wear, with both rear tires becoming nearly completely bald within less than 7,000 miles. This condition increases the risk of tire blowout, loss of traction, and potential loss of control at highway speeds. The issue has been confirmed by an independent service center (Les Schwab), which documented suspension play in the rear control arm area and alignment out of specification (rear toe approximately +1.20° total). The alignment could not be maintained, indicating an underlying mechanical issue. Tesla inspected the vehicle but stated that no suspension components required replacement and attributed the issue only to alignment. However, alignment does not go out of specification without a mechanical cause. Despite multiple requests, Tesla refused to provide detailed documentation regarding the inspection or repair of the suspension system. They replaced only one rear tire and did not address the root cause. The other rear tire had already worn completely bald. After approximately 5,386 miles, the replacement tire has again worn down close to the minimum safety limit, demonstrating that the underlying issue was not corrected and continues to affect vehicle safety. There were no warning lights or system messages prior to the failure. The first symptoms were abnormal tire wear, increased energy consumption, and unstable driving behavior at highway speeds, which began shortly after vehicle purchase. This appears to be a repeat safety-related defect that has not been properly diagnosed or repaired.

The electric power assist steering system on my 2022 Tesla Model X began displaying a "Steering Assist Reduced / Steering May Require Increased Effort" warning intermittently beginning in approximately May 2024, shortly after I purchased the vehicle. This warning appeared more times than I can count over the following months. In most instances the warning appeared but the steering continued to function normally. On approximately March 2, 2026, the warning appeared and for the first time the steering became extremely difficult to turn, requiring significantly increased physical effort to control the vehicle. The first physical failure occurred while parking at low speed in reverse. The steering failure also occurred subsequently while driving at highway speeds. In both instances I stopped the vehicle, exited, waited briefly, and upon re-entering the vehicle the warning cleared and steering returned to normal. This same sequence repeated the following day. The vehicle was brought to a Tesla authorized service center on March 13, 2026. Tesla's technician confirmed the steering assist reduction as reported and determined the steering gear required full replacement. The failed part, steering gear left hand drive part number 1070801-00-F, was replaced. Only full hardware replacement resolved the issue permanently. Prior power cycling provided only temporary relief. My safety was directly at risk. I am a full-time rideshare driver operating at highway speeds. The failed component has been replaced and is in Tesla's possession. Tesla is charging the full repair cost out of pocket despite this matching a documented pattern of identical failures reported to NHTSA by multiple Tesla owners.

On January 31, 2026, at 10:30 PM, I was driving from Fort Lauderdale to my home in Boca Raton, Florida. I was using FSD supervised mode. Close to my home, a big red image suddenly appeared, the car started accelerating fast, and I had no control over the car or its steering wheel. The car hit several electrical posts and only stopped after the airbags engaged. My wife was in the passenger seat. The front of the car was totally damaged, and I will have to total it.

I am writing to formally document a life-threatening mechanical failure involving my 2022 Model X on January 30, 2026. While my wife was operating the vehicle, it suffered a simultaneous and catastrophic collapse of all primary safety systems. During operation, the vehicle experienced a total loss of steering and braking control. This was accompanied by a rapid cascade of critical system alerts on the primary display, including “ABS Malfunction,” “Stability Control Disabled,” and “Steering Assist Reduced/Disabled.” Most critically, the vehicle exhibited Sudden Unintended Acceleration (SUA), increasing speed in direct opposition to the driver’s physical application of the brake pedal. This resulted in a near-miss where the driver narrowly avoided a construction crew.

The turn signal buttons intermittently fail to work. Many presses of the tuen signal buttons do nothing. When it stops working it stops for several minutes and then will start to work again. The problem is happening more and more frequently. The yoke has been replaced 3 times already for the same issue and after about a year just started happening again.

It shows a passenger when the seat is empty and activates passenger seatbelt warnings incorrectly. Recently the vehicle displayed: “Front passenger safety restraint system issue: Service required (RCM2_a064).” Tesla attempted to repair an electrical harness in the seat, but the problem returned. A Tesla technician documented that the seat may still need a new occupancy sensor. Because this affects the Occupant Classification System and airbag deployment logic, it raises potential FMVSS 208 compliance concerns. 2. FSD / GPS Safety Hazard (5 Unsuccessful Repair Attempts): Across five service visits, Tesla attempted to fix GPS drift and unsafe FSD routing. The vehicle still attempts incorrect turns, performs premature lane changes, and misinterprets intersections. One recent incident involved FSD attempting a left turn one intersection early, nearly causing a collision. Tesla replaced the GPS antenna, recalibrated cameras, and replaced the computer, but the unsafe behavior continues. These two defects—incorrect airbag/OCS behavior and hazardous automated-driving/navigation behavior—create a significant safety risk to occupants and other drivers. Tesla repaired this defects under the 'Basic Vehicle Limited Warranty,' confirming it was a factory assembly error. This defect created an imminent risk of a high-voltage short to the steering column, which could result in driver electrocution or total vehicle power loss at speed. 2. Passenger Entrapment: The vehicle has a documented history of Rear Falcon Wing Door failure (7 repair attempts from April 2023 to May 2025). The doors frequently fail to latch or open. On July 8, 2025, the Left Rear Door failed to open due to a sensor fault. 3. Combined Hazard: If the incorrectly routed High Voltage harness causes a vehicle power failure, the rear doors' electronic latches will fail. Because the doors have a history of mechanical jamming, passengers in the rear seats would be trapped in a vehicle with a compromised High Voltage system.

I had my two Teslas parked in the garage. The model X was charging at about 1:30 AM. We heard explosions coming out of the garage. Went to check only to see my model X in flames. We evacuated and called 911 eventually a fire investigation report was made Showing the fire started within the vehicle outward. The battery pack was on fire. We had lithium contamination all over the house and this fire endangered five lives. This incident has been two years ago and until now Tesla refuses to acknowledge the faulty battery and is refusing to do anything about it

WE WERE CHARGED 300+ DOLLARS FOR THEM TO FIND THEB EXCAT PIN/ WIRE THAT CAME LOOSE. WE ALSO HAVE A 2020 TESLA MODEL 3 STANDER RANGE, WE RECENTLY HAD THE SAME ISSUE AND THE WARNING LIGHT IS CURRENTLY ON AND GOES AWAY IF WE MOVE THE PASSANGER SEAT. This vehicle suffered a flat tire on the left rear at 20,000 miles. It was discovered that the tire had worn out on the inside prematurely due to a faulty negative camber alignment. The other tire on the right rear was examined and found to be worn down to the treads on the inside as well. I informed Tesla and they said this is normal for the make and model. You cannot see the inside of these tires during a normal walk around inspection. Tesla says that the problem cannot be fixed. I could have suffered a tire failure at high speed which could have been catastrophic. Tesla just says its unfortunate and you have to live with this issue. i consider this to be a very unsafe issue that Tesla should address at their expense. This car is under warranty and Tesla refused to repair the car.Tesla did not divulge this issue to me prior to purchasing the vehicle.

After the fifth time, they suddenly tells me that I got into a hole in the road and that is why it's shaking, also, the car had a message on the screen saying that the air pressure can't lift the vehicle, and it made a huge noise, I had to call a tow truck to take it to Tesla center. NOW, they are telling me that a mouse ate the hose, and that will cost me $9000. I can't pay that much money, and it's not OK. To Whom It May Concern, I am writing to inform you of a recurring issue I am experiencing with the power steering in my 2022 Model X Plaid. This problem began on [XXX] and has progressively worsened. Initially, the power steering would function normally for approximately 30 minutes of driving. It would work fine when starting in the morning or when the car was cool. However, after driving about 15 miles, the power steering would completely fail, and a warning message would appear indicating that extra effort was required. Recently, the power steering now fails every time I drive. Based on my research, this appears to be a common problem with Tesla vehicles, and I understand a recall has been issued for some model years, although not for the year of my vehicle. I intend to have this issue addressed and repaired through Tesla service. While driving my Model X 2022 I noticed the warning "Steering assist reduced, Steering may require increased effort". I was then at a red light and I started to make a left turn and I could BARELY turn the wheel it was like it was almost locked up. Luckily I had enough strength and I was driving into a parking lot where I needed to have my car towed. Tesla service just said there was moisture in the steering wheel connector and they just cleaned it up and connected it. They test drove it and said it's fine. 4 days later I was driving it on the highway and I got the message again and I had my daughter in the car and was terrified. Luckily the steering did not lock up and just drove it home. I have messaged Tesla to see next steps, but when I look at the forums this has been happening A LOT! This is a huge safety concern and I don't feel Tesla is properly handling it based on all the reviews I have read...

On September, 21 2024 I scheduled an appointment with the Tesla Service Center for the “Front Row Curtain Airbag” recall to be performed. October 30th, I took my 2022 Model X Plaid to the Tesla Service Center for the recall work to be performed. During the repair, their service technician BROKE-OFF an anchor nut that secures the airbag to the A-pillar. The advisor said in order to “fix” it they would need to take the car to a body shop to have a “new” anchor nut welded onto the A-pillar. I was told this repair would require cutting open the A-pillar to access the location where the nut should be welded. Cutting the A-pillar will compromise the structural integrity of the vehicle's roof. And retrofitting a new mounting point for the side curtain airbag will compromise the effectiveness of the airbag in the event of an accident. The manufacturer's airbag recall "fix" does not involve cutting open the A-pillar to weld in a new mounting point for the vehicle's airbag. The notification stated "electrical system unable to support functions." My safety was put at risk and others. I was traveling with my 11 month old and other cars could have hit my car if they were not paying attention when coming through the intersection. The car was towed to the Tesla dealership. I was stopped at an intersection in the left turn lane heading southeast at a busy intersection. Left turn signal was on and within 3 minutes of getting to the intersection the vehicle experiences an uncommanded full power acceleration into a very busy intersection. To avoid oncoming traffic, I veered to the right while breaking which did not stop the vehicle. Avoiding a traffic pole and continuing into a business empty parking lot which usually is not empty due to loitering in the area, and avoiding the building the vehicle continued accelerating at a high rate of speed and was stopped only because it hit a metal pole which was hidden by a large bush on the side of the business. If the pole would have not stopped the vehicle it would have continued into oncoming traffic in the north bound lane. Air bags were deployed. The only visible injury I received was burns from the airbag on my right arm and sever busing on my abdomen which has taken 4 months to resolve. The police were on the scene immediately, my insurance company representatives have seen the videos produce by myself and Tesla sent to me via email. The insurance had put a hold on the vehicle which was towed to Carparts. Tesla has never contacted me or my insurance company that I know of besides my request by email for the additional data. I have driven a Tesla model S for 7 yrs prior to purchasing the Model X. The Model X had an incident once that it stopped on its own while I was driving on an interstate. I veered to the right to get off the highway and thankful no one was behind me. There wasn't any indication that anything was wrong and may have been something corrected by an automatic update which dealt with exiting right off a highway which I was not doing but this incident never happened again.

Many newer models of Teslas do not include a horn in the middle of the steering wheel. Instead there is a small button on the left side which is impossible to find to warn others of a possible collision! The driver gets distracted trying to find the horn and makes the situation even worse by not reacting quick enough. They released a new version of the wheel with a horn but those who are out of warranty have to pay a large amount for a basic yet hugs safety oversight. They should be replaced on with recall free of charge.

The contact owns a 2022 Tesla Model X. The contact stated that while the vehicle was parked, the driver’s side rear passenger’s seat belt failed to release the child occupying the seat. The contact stated that after five minutes the seat belt was released. The contact then stated that while driving at approximately 20 MPH, with the front passenger and the driver’s side rear seats occupied, the restraint system fault for each passenger was displayed. Additionally, the contact stated that on several occasions while driving at various speeds, the messages appeared intermittently while both seats were unoccupied. The vehicle was taken to a dealer where it was diagnosed that a seat belt module needed to be replaced. The vehicle was repaired but the failure persisted. The manufacturer was notified of the failure. The failure mileage was approximately 2,000.

The passenger side back door does not open consistently leading to loss of egress. Although the other doors do function regularly, removing a child from a car seat is nearly impossible when accessing from another door. The door also struggles to close consistently, and has closed on my son in the past. The inconsistency of a functioning falcon wing door is a safety risk and it continues to worsen despite many attempts to repair the sensors. The Tesla service center has attempted to repair the door and sensors 4 times without success. The most recent service appointment they stated the door was not opening correctly because it was getting stuck in “indoor mode” sensing something over the top of the car. They replaced the sensor but the problem has not improved. I have notified Tesla of the ongoing issue multiple times without any response, even via certified mail with proof of delivery. There are no warning lamps, beeps or any notifications showing an error in door function.

The yoke steering wheel only had a small horn button on the right side, only accessible by right thumb. If you are in the middle of a turn that requires more than a quarter turn, then your hand cannot possible be in the right place to use the horn. The lack of a horn in the center of the steering (like every other car) is unsafe. On many occasions I have found myself needing the horn but unable to press it fast enough and have had to take other evasive actions to avoid other vehicles.

Center steering wheel horn is not included on these vehicles, which should be considered a significant safety issue as we are trained from a young age to hit the center of the steering wheel/yoke in the event of an imminent accident. I was recently driving when another vehicle ran a stop sign, and I was barely able to avoid them; however, when hitting the "horn" to alert the other driver, it didn't work as the horn button is not in a typical location. This should be considered a significant safety risk. Elon Musk himself indicated a software update would fix this issue; however, it has not. Tesla now indicates it requires a new wheel/yoke and has not yet made it available or covered under warranty.

A couple of days ago, when I was dropping off my child at school, I parked in a spot that required parallel parking to save time as I was in a rush. After reversing into the spot, I just needed to move forward a bit by 1-2 feet. I naturally slide the gear shift up on the screen. Unfortunately, it didn't slide into place, and the car was still in reverse (R), but I didn't realize it. I was already very close to the car behind me. When I pressed the accelerator, the car continued to back up. Luckily, I slammed the brake, avoiding a collision. Just wanted to share that even after a year and a half of driving a Model X, mistakes can still happen. After reading what happened to [XXX] , I posted what happened to me on a social platform and someone else shared they had a similar experience that needed up with a crash. So I decided to report mine and encouraged them to do the same. I like the minimalist design but I need to stay alive to enjoy it. As Tesla is moving away from stalkless design in more models, I hope they can do something to make the current shift more noticeable. The new stockless button controls on the Tesla are entirely unsafe. The buttons themselves are extremely unreliable and don't always respond, and the placement on the steering wheel requires you to take your eyes off the road in order to locate the correct button. I was recently driving on the freeway with my wife and kids in the car, it had been raining earlier and the roads were wet. A car in the opposite fast lane hit a puddle which sent wave of water over the center divider. It covered our windshield to the point where I couldn't see. With previous cars, including previous Teslas, it was easy to quickly tap the stalk and engage the wipers, but now with this new button system I had to look down at the steering wheel to find the button to quickly engage the wipers. From the surprisingly blinding wave and the time it took to clear the windshield, I veered into the next lane and almost hit the car next to me. It was an eye-opening experience with a car I've really enjoyed owning. The buttons are terrible, and a deviation from a universal, internationally accepted system that was not broke, did not require fixing, and has created a safety hazard.

I continue to experience accelerated severe rear tire wear since my original purchase of this car new in February 2022 on both rear tires (275/45R20). The car has had the alignment specifications confirmed by Tesla. Yet I have had to replace two sets of rear tires in less than 36,000 miles due to the inside edge of both tires being worn through the rubber and into the steel cords of the tires. This last set of tires went ~14,000 miles and still had 5/32" tread depth remaining across all treads. This substantial wear is not readily observed by the vehicle operator without crawling under the car. The wear on these tires is indicative of excessive negative camber settings of the suspension. However, TESLA states the rear suspension alignment is within camber specifications at -2.13 to -2.06 degrees. The tires appear road worthy during a casual walk around of the vehicle. I have been fortunate these tires have not experienced a blow out, which at highway speed could have resulted in loss of control of the vehicle. I have retained both removed rear tires. Tesla Model X all doors are 100% automatic which means that they cannot be manually opened from the outside if the car software is not working. I have been facing a lot of issues with the Falcon doors and to my surprise a lot of other Tesla X users on Reddit face the same issue. So, when you press the open button on your app to open the rear falcon doors it might not open at all and just make a beep sound. It's hard to replicate this issue but I shot a video showing how it happens. This is not the first time that it had happened with my car. I have had this happen multiple times to my car. Every time the service center would simply change the door sensor which would fix the issue but temporarily. I think it a big safety issue as I have 2 toddlers who occupy the rear row in their car seats. I cannot get them out of the car when such issue arises. I have done a workaround to make it work which takes at least 2 minutes to work properly. A lot can go wrong in 2 minutes with kids. This issue happens very randomly and there is no prior warning. Earlier tesla told me that it had to do something with the climate. If it's too hot outside then the door sensors might get fried up, which I think is not completely true. I have heard different explanations from their different technician. Well yesterday when it happened, it was 34F and windy. I was outside of the car in this cold weather trying to get my daughter out of the car and figuring out how to open these damn falcon doors.

Ever since a recent software update, when turn signal is activated, right fender camera feed appears on screen but is completely black instead of showing a visual of the right blind spot. Due to this other features such as Autopilot, Automatic Emergency Braking, and likely most of the ADAS functions are also disabled. Due to the vehicle's poor natural visibility, there is a high reliance on the camera systems functioning properly. Not having a blind spot camera endangers turns and lane changes, especially if one has become accustomed to their usage. With most if not all the ADAS features disabled, we are returning to a 1990's level of auto safety. The problem remains unresolved despite a recent 2 week stint in Tesla service. Since this is most likely a software issue (possibly a regression introduced with the recent rear camera recall fix) there is no recourse except for Tesla to issue a bug fix. The car was returned to me without resolution. I'm not the only party experiencing this problem. It seems the only constant is it's a problem with the fender cameras however whether it's the right or left side is variable. FSD Collisions in Reduced Roadway Visibility Conditions

The Office of Defects Investigation (ODI) is opening this Engineering Analysis to evaluate Tesla’s Full Self Driving Beta and Full Self Driving (Supervised) (collectively, FSD) degradation detection system. The focus of this investigation will be to assess the system’s ability, when encountering reduced roadway visibility conditions, to detect degradation and alert the driver with sufficient time to respond. ODI will evaluate the performance of FSD in degraded roadway conditions and the updates or modifications by Tesla to the degradation detection system, including the timing, purpose, and capabilities of the updates, and Tesla’s assessment of their safety impact. Tesla’s FSD is an advanced driver assistance system (ADAS) that relies exclusively on vision-based cameras and the related FSD software to detect and respond to the roadway ahead, projecting a path forward based on traffic control devices, vehicles, pedestrians, and the roadway itself. When Tesla began transitioning away from using both cameras and radars to an exclusively camera-based approach, known as Tesla Vision, in mid-2021, it developed and implemented a degradation detection system that it deployed by a software update to existing and new Tesla vehicles. On June 28, 2024, the day after Tesla submitted the SGO report of the November 28, 2023 fatal crash listed in this document, Tesla began developing an update to the degradation detection system. At this time, ODI does not have information on when the update was deployed and which vehicles have the updated system. ODI discussed individual incidents and its initial findings during the PE phase of its investigation with Tesla. As part of those discussions, Tesla’s post-incident analysis indicated that the update to the degradation detection system, had it been installed on the vehicles at the time, may have affected 3 of the 9 incidents identified by ODI. Tesla also described internal data and labeling limitations that prevented a uniform identification and analysis of crash events with the subject system engaged. ODI believes this limitation could have led to under-reporting of subject crashes over portions of the defined time-period. Available incident data raise concerns that Tesla’s degradation detection system, both as originally deployed and later updated, fails to detect and/or warn the driver appropriately under degraded visibility conditions such as glare and airborne obscurants. In the crashes that ODI has reviewed, the system did not detect common roadway conditions that impaired camera visibility and/or provide alerts when camera performance had deteriorated until immediately before the crash occurred. Review of Tesla’s responses revealed additional crashes that occurred in similar environments and where the system either did not detect a degraded state, and/or it did not present the driver with an alert with adequate time for the driver to react. In each of these crashes, FSD also lost track of or never detected a lead vehicle in its path. In upgrading PE24031 to an Engineering Analysis (EA), ODI will gather further information on the updated degradation detection system, including the status of updating vehicles and scope of compatible vehicles, the system’s visibility degradation detection capability, and alerts or warnings to the driver. Lastly, ODI will conduct analysis on six recent potentially related incidents. Traffic safety violations while Full Self Driving ("FSD") is engaged

The Office of Defects Investigation (“ODI”) is opening this Preliminary Evaluation (PE) to assess the scope, frequency, and potential safety consequences of FSD executing driving maneuvers that constitute traffic safety violations. This investigation concerns versions of FSD that Tesla has labeled as "FSD (Supervised)" and "FSD (Beta)." Tesla characterizes FSD as an SAE Level 2 partial automation system requiring a fully attentive driver who is engaged in the driving task at all times. Level 2 partial automation systems are designed to support and assist the driver in performing certain aspects of the driving task, requiring a driver to supervise and intervene as necessary.  The driver remains fully responsible at all times for driving the vehicle, including complying with applicable traffic laws. ODI’s investigation will therefore focus, in particular, on whether certain driving inputs within the control authority of FSD forestall the driver’s supervision when they are unexpectedly performed. ODI has identified a number of incidents in which the inputs to the dynamic driving task commanded by FSD induced vehicle behavior that violated traffic safety laws. Although reports of this nature span a variety of behaviors, the reports appear to most commonly involve two types of scenarios. The first type of scenario involves a vehicle operating with FSD proceeding into an intersection in violation of a red traffic signal. The second type of scenario involves FSD commanding a lane change into an opposing lane of traffic. With respect to the first type of scenario, ODI has identified 18 complaints and 1 media report alleging that a Tesla vehicle, operating at an intersection with FSD engaged, failed to remain stopped for the duration of a red traffic signal, failed to stop fully, or failed to accurately detect and display the correct traffic signal state in the vehicle interface. Some complainants also alleged that FSD did not provide warnings of the system's intended behavior as the vehicle was approaching a red traffic signal. ODI has identified six Standing General Order ("SGO") reports in which a Tesla vehicle, operating with FSD engaged, approached an intersection with a red traffic signal, continued to travel into the intersection against the red light and was subsequently involved in a crash with other motor vehicles in the intersection. Of these incidents, four crashes resulted in one or more reported injuries. At least some of the incidents appeared to involve FSD proceeding into the intersection after coming to a complete stop. ODI's pre-investigative work, including coordination with the Maryland Transportation Authority and State Police, indicated that the problem may be repeatable, given that multiple subject incidents occurred at the same intersection in Joppa, Maryland. NHTSA understands that Tesla has since taken action to address the issue at this intersection. With respect to the second type of scenario, ODI has identified 2 SGO reports, 18 complaints, and 2 media reports alleging that a Tesla vehicle, operating with FSD engaged, entered opposing lanes of travel during or following a turn, crossed double-yellow lane markings while proceeding straight, or attempted to turn onto a road in the wrong direction despite the presence of wrong-way road signs. Likewise, ODI has identified 4 SGO reports, 6 complaints, and 1 media report alleging that a Tesla vehicle, operating with FSD engaged, proceeded straight through an intersection in a turn-only lane or executed a turn at an intersection in a through lane despite the presence of lane markings or signals. Complaints also alleged that FSD did not provide warnings of the system's intended behavior. Some complaints alleged that more than one of these failures occurred and, as such, the numbers are not cumulative. Some of the reported incidents appeared to involve FSD executing a lane change into an opposing lane of travel with little notice to a driver or opportunity to intervene. ODI’s review will assess whether there was prior warning or adequate time for the driver to respond to the unexpected behavior or to safely supervise the automated driving task. This review will assess any warnings to the driver about the system's impending behavior; the time given to drivers to respond; the capability of FSD to detect, display to the driver, and respond appropriately to traffic signals; and the capability of FSD to detect and respond to lane markings and wrong-way signage. NHTSA's review will also consider any updates or modifications to the system(s) that may affect the performance of FSD with respect to obeying traffic safety laws and signals. This assessment will focus, in particular, on the types of traffic safety violations described above, as most reports identified thus far have centered around those behaviors. While the behaviors under investigation appear to occur most frequently at intersections, NHTSA’s investigation will encompass any other types of situations in which this behavior may arise, such as when traveling adjacent to a lane of opposing traffic or when approaching railroad crossings. If other evidence received during this investigation involve other types of traffic safety violations, those may be considered as part of this assessment as well. Compliance with Standing General Order 2021-01 Reporting Requirements

The Office of Defects Investigation (“ODI”) has identified numerous incident reports submitted by Tesla, Inc. (“Tesla”) in response to Standing General Order 2021-01 (the “SGO”), in which the reported crashes occurred several months or more before the dates of the reports. The majority of these reports involved crashes in which the Standing General Order in place at the time required a report to be submitted within one or five days of Tesla receiving notice of the crash. When the reports were submitted, Tesla submitted them in one of two ways. Many of the reports were submitted as part of a single batch, while others were submitted on a rolling basis. Preliminary engagement between ODI and Tesla on the issue indicates that the timing of the reports was due to an issue with Tesla’s data collection, which, according to Tesla, has now been fixed. NHTSA is opening this Audit Query, a standard process for reviewing compliance with legal requirements, to evaluate the cause of the potential delays in reporting, the scope of any such delays, and the mitigations that Tesla has developed to address them. As part of this review, NHTSA will assess whether any reports of prior incidents remain outstanding and whether the reports that were submitted include all of the required and available data. Actually Smart Summon sessions resulting in low-speed impacts.

On January 6, 2025, the Office of Defects Investigations (ODI) opened Preliminary Evaluation 24003 (PE24033) to investigate Actually Smart Summon (Summon) sessions resulting in crashes during active sessions. According to Tesla, Summon is a short-distance SAE Level 2 system, controlled by the user from a cell phone within a certain distance and intended for use in parking lots and on private property. ODI analyzed complaint data provided by Tesla as well as complaints submitted to ODI from consumers to identify Summon incidents resulting in crashes. ODI's analysis indicates that almost all Summon reported crashes involved minor property damage claims with no reported incidents involving a vulnerable road user, injury, fatality, or major property damage as indicated by an air bag deployment or vehicle tow away. Out of millions of Summon sessions, a fraction of 1% resulted in an incident. Almost all those incidents took place where, typically early in a Summon session, the system or person using the app failed to fully detect or respond appropriately to vehicle surroundings resulting in minor impacts. Incidents took place when app users did not have a complete 360-degree view of the surroundings in the app to assess situational awareness. This limited the app user’s ability to determine whether an impact was imminent during initial vehicle maneuvers such as reversing in close proximity to an obstacle or a curb. ODI found that the impacts most often occurred with parking gates, adjacently parked vehicles, and short parking bollards. During this investigation, ODI identified two Summon crashes related to camera blockages. In both crashes, Summon attempted to navigate a snowy parking lot with snow partially or fully obstructing the forward-facing cameras. Summon did not detect the camera blockage and the vehicles collided with unoccupied parked vehicles while navigating the parking lot. App users in both instances did not command a vehicle stop or pause despite the obstructed camera visible in the camera stream in the app. On January 15, 2025, Tesla released Over-the-Air (OTA) Software (SW) Update Action numbers 578998 and 579185 for vehicles in service to implement a camera blockage detection condition. Both OTAs improve camera blockage detection mechanisms. Additionally, on January 20, 2025, and January 30, 2025, Tesla identified additional system requirements associated with camera visibility checks and released OTA SW-578752 and SW-580322, respectively. These firmware updates reduce false negative camera blockage detections due to snow or condensation. In its investigation, ODI identified one Summon incident where the vehicle did not yield for a gate arm blocking a garage exit lane and the app user did not command a vehicle stop or pause, resulting in an impact. On February 6, 2025, Tesla deployed OTA SW-578839 to improve vehicle reaction to dynamic gates. This OTA update upgraded vehicle perception systems through a high-fidelity occupancy determination network, which uses data from vehicle sensory systems to improve reconstruction of field objects with high accuracy. On November 20, 2025, Tesla further improved vehicle performance by adding object detections from a separate neural network through OTA SW-580514. Owners of the affected vehicles received all six OTA SW updates. Tesla also released these SW updates to production vehicles. FSD Collisions in Reduced Roadway Visibility Conditions

On October 17, 2024, NHTSA’s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE24031) of Tesla’s Full Self Driving Beta and Full Self Driving (Supervised) (collectively, FSD) to assess: the ability of the FSD system to detect and respond appropriately to reduced roadway visibility conditions; whether any other FSD crashes had occurred under degraded roadway visibility conditions that are similar in nature to the four SGO-reported crashes identified in the opening document, and if so, the contributing circumstances for each of those crashes; and any updates or modifications by Tesla to the FSD system that may affect the performance of FSD in degraded roadway visibility conditions, including the timing, purpose, and capabilities of any such updates, and Tesla’s assessment of their safety impact. Tesla’s FSD is an advanced driver assistance system (ADAS) that relies exclusively on vision-based cameras and the related FSD software to detect and respond to the roadway ahead, projecting a path forward based on traffic control devices, vehicles, pedestrians, and the roadway itself. During this phase of the investigation, ODI reviewed the materials provided by Tesla detailing any actions taken, or changes, modifications, and updates made that may relate to the alleged defect. ODI reviewed information related to Tesla’s transition to a vision-only perception system, and its deployment of the strategy in vehicles. ODI’s findings include information on limitations of the vision-only perception system and updates made in response to known subject crashes. When Tesla began transitioning away from using both cameras and radars to an exclusively camera-based approach, known as Tesla Vision, in mid-2021, it developed and implemented a degradation detection system that it deployed by a software update to existing and new Tesla vehicles. On June 28, 2024, the day after Tesla submitted the SGO report of the November 28, 2023 fatal crash listed in this document, Tesla began developing an update to the degradation detection system. ODI reviewed certain public statements, including those made during an April 2025 earnings call, in which Tesla stated that it had developed a breakthrough “direct photon-counting” capability that eliminates the degradation that the camera-based system experiences when glare is encountered. At this time, ODI does not have information on when the update was deployed and which vehicles have the updated system. ODI has discussed individual incidents and its initial findings with Tesla. Based on Tesla’s post-incident analysis, the update to the degradation detection system, had it been installed on the vehicles at the time, may have affected 3 of the 9 incidents identified by ODI. Review of Tesla’s responses revealed additional crashes that occurred in similar environments and where the system either did not detect a degraded state, and/or it did not present the driver with an alert with adequate time for the driver to react. In each of these crashes, FSD also lost track of or never detected a lead vehicle in its path. Tesla also described internal data and labeling limitations that prevented a uniform identification and analysis of crash events with the subject system engaged. ODI believes this limitation could have led to under-reporting of subject crashes over portions of the defined time-period. Available incident data raise concerns that Tesla’s FSD system fails to detect and/or warn the driver appropriately under degraded visibility conditions such as glare and airborne obscurants where the camera-based system performance degrades significantly. In the crashes that ODI has reviewed, the FSD system did not detect common roadway conditions that impaired its visibility and/or provide alerts when camera performance had deteriorated until immediately before the crash occurred. ODI is upgrading this investigation (PE24031) to Engineering Analysis (EA) 26002 to further evaluate this matter. Recall 23V838 Remedy Effectiveness

The Office of Defects Investigation (ODI) is opening a Recall Query to assess the remedy adequacy of Recall 23V838. On December 12, 2023, Tesla filed a Defect Information Report (Recall 23V838) applicable to all Tesla models produced and equipped with any version of its Autopilot system, which Tesla described as an SAE Level 2 (L2) Advanced Driver Assistance System (ADAS). Autopilot is the simultaneous engagement of Tesla’s Traffic-Aware Cruise Control (TACC) and Autosteer. In describing the safety defect, Tesla’s Defect Information Report (DIR) explained that “the prominence and scope of the system’s controls may be insufficient to prevent driver misuse,” and Tesla committed to the deployment of a multipart remedy aimed at improving system and engagement controls and reducing mode confusion. EA22002 (upgraded from PE21020) was opened to investigate whether Tesla’s Autopilot contained a defect that created an unreasonable risk to motor vehicle safety and involved extensive crash analysis, human factors analysis, vehicle evaluations, and assessment of vehicle control authority and driver engagement technologies. The work conducted in these investigations aligns with Tesla’s conclusion in its 23V838 recall filing. During EA22002, ODI identified at least 13 crashes involving one or more fatalities and many more involving serious injuries in which foreseeable driver misuse of the system played an apparent role. Tesla filed Recall 23V838 to address concerns regarding the Autopilot system investigated in EA22002. Following deployment of the remedy in Recall 23V838, ODI identified concerns due to post-remedy crash events and results from preliminary NHTSA tests of remedied vehicles. Also, Tesla has stated that a portion of the remedy both requires the owner to opt in and allows a driver to readily reverse it. Tesla has also deployed non-remedy updates to address issues that appear related to ODI’s concerns under EA22002. This investigation will consider why these updates were not a part of the recall or otherwise determined to remedy a defect that poses an unreasonable safety risk. ODI is therefore opening this Recall Query investigation to further evaluate the adequacy of the remedy for recall 23V838.

Sudden Unintended Acceleration

The Office of Defects Investigation (ODI) received a petition requesting that ODI reevaluate its decision to deny DP20-001 on the basis that intermittent high electrical current demands on the vehicles' 12VDC systems may have caused some or all of the incidents examined by ODI in DP20-001. The petitioner bases this information on a review of open-source research and the DP20-001 denial. Defect Petition Interlock Request

On March 21, 2023, the Office of Defects Investigation (ODI) received a petition requesting a “recall of all Tesla cars” produced from 2013 to the date on which the petition was filed due to what Petitioner considers to be the increased likelihood of pedal misapplication. Attached to the Petition is a paper authored by Petitioner. According to Petitioner, the differences in the operator controls between the subject Tesla vehicles and internal combustion engine powered vehicles promote driver pedal misapplication, leading to sudden unintended acceleration (SUA) incidents. To fix this alleged defect, Petitioner argues that the subject vehicles should be equipped with measures that require: (i) occasional removal of the driver’s foot from the pedals; and (ii) application of the brake pedal before fully stopping the vehicle. ODI evaluated the allegations by, among other things, reviewing the Petition and supporting technical paper, and analyzing Tesla’s response to ODI’s Information Request. ODI is denying this Petition. ODI has not found evidence that warrants the opening of a safety defect investigation into the Tesla vehicles as described in the Petition. The use of regenerative braking controlled by the accelerator pedal, or one-pedal driving, is common across most light vehicle manufacturers of electric vehicles and Tesla vehicles are not unique in this respect. Further, ODI identified only a handful of collisions potentially within the scope of the alleged defect; and corresponding vehicle data demonstrated that the subject vehicles responded appropriately to control inputs by their drivers. Moreover, ODI is unaware of any evidence to suggest that Petitioner’s proposed interlock would have prevented alleged SUA events as apparently described in the Petition and supporting materials. Accordingly, the Agency is denying the petition. As with all potential motor vehicle safety risks, NHTSA will continue to review any new information or incidents as they are submitted to the Agency.

Front seat belt anchor detachment

On March 24, 2023, the Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE) after receiving 2 consumer complaints alleging the front seat belts had suddenly detached from the outboard lap pretensioner anchor while in motion in model year (MY) 2022-2023 Tesla Model X vehicles. In its June 19, 2023 response to ODI’s May 4, 2023 Information Request letter of the PE, Tesla reported that its review of field repair data identified 12 additional warranty claims of the subject condition in the affected vehicles. On July 14, 2023, Tesla filed a safety recall (NHTSA No. 23V-488) of 15,869 MY 2021 - 2023 Tesla Model S and Model X vehicles. The recall remedy is an inspection of the 1st row seat belts for proper connection to the pretensioners. Service personnel will resecure the pretensioners or, if necessary, replace the seat belt retractor assembly. In its recall filing, Tesla stated that effective June 22, 2023, it had updated procedures requiring a first-row seat belt to be disconnected from- and reconnected to- its pretensioner anchor to include additional visual aids to assist technicians and require a pull test-step to confirm that the seat belt was reconnected to the pretensioner anchor to specification. In view of the recall action and process updates taken by Tesla, ODI is closing this PE. The agency reserves the right to take additional action if warranted by future circumstances. Pedestrian alert sounds

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

Autopilot System Driver Controls

The Office of Defects Investigation (ODI) upgraded PE21020 to EA22002 on June 8, 2022, to extend work and deepen the PE21020 crash analysis, to supplement that analysis with additional data, and to perform vehicle evaluations to understand how Tesla’s Autopilot system may exacerbate human factors or behavioral safety risks by undermining the effectiveness of the driver’s supervision. To support this work, ODI collected additional crash information and assessed vehicle control authority, driver engagement technologies, and related human factors considerations associated with partial automation via analysis of peer vehicle data and hands-on vehicle evaluation, assessments from NHTSA human factors subject matter experts, and reviews of related publications dedicated to partial driving automation. Autopilot is the simultaneous use of the features that Tesla calls Traffic-Aware Cruise Control (TACC) and Autosteer. TACC is a type of adaptive cruise control that, like traditional cruise control, maintains a set speed but also slows or accelerates as necessary to maintain the vehicle’s following distance from a vehicle in front. As designed, Autosteer detects lane markings and the presence of other nearby vehicles and objects to keep the vehicle in its driving lane. Autopilot is characterized by Tesla as an SAE Level 2 (“L2 system”) partial driving automation system that provides driver assistance through steering, propulsion, and braking within a specified driving environment under direct supervision of the driver. L2 systems should be designed to support the driver’s need to monitor the system in response to the constantly changing driving environment and, if necessary, take over the dynamic driving task. To ensure sufficient driver engagement, vehicles with L2 systems should employ driver engagement systems and usage controls that are appropriate and sufficient for the L2 system design and driver expectations. ODI completed an analysis of 956 crashes reported up to August 30, 2023. In approximately half (489) of those crashes, ODI found: 1.) that there was insufficient data to make an assessment; 2.) the other vehicle was at fault; 3.) Autopilot was found to not be in use; or 4.) the crash was otherwise unrelated to EA22002. Of the remaining 467 crashes, ODI identified trends resulting in three categories: collisions in which the frontal plane of the Tesla struck another vehicle or obstacle with adequate time for an attentive driver to respond to avoid or mitigate the crash (211), roadway departures where Autosteerwas inadvertently disengaged by the driver’s inputs (111), and roadway departures in low traction conditions such as wet roadways (145). ODI observed this pattern across all Tesla models and hardware versions. Crash and human factors assessment showed that Autopilot controls did not sufficiently ensure driver attention and appropriate use. At the same time, peer analysis and vehicle evaluations established that Autopilot invited greater driver confidence via its higher control authority and ease of engagement. This mismatch of weak usage controls and high control authority was evident in these crash categories, which included indications of driver disengagement from the driving task. This mismatch was also evident in roadway departures when the system was engaged in low traction conditions outside of Tesla’s recommendations.Additional information regarding NHTSA’s crash analysis is available in the EA22002 file. ODI reviewed these findings with Tesla during several conversations in Quarter 4 of 2023. On December 12, 2023, Tesla filed a Defect Information Report (DIR) (Recall 23V838) applicable to all Tesla models produced and equipped with any version of its Autopilot system. Tesla’s DIR described the functionality of this system, stated that the prominence and scope of the system’s controls may be insufficient to prevent driver misuse, and described a remedy to improve the effectiveness of driver warnings and to reduce mode confusion. ODI completed an extensive body of work via PE21020 and EA22002, which showed evidence that Tesla’s weak driver engagement system was not appropriate for Autopilot’s permissive operating capabilities. This mismatch resulted in a critical safety gap between drivers’ expectations of the L2 system’s operating capabilities and the system’s true capabilities. This gap led to foreseeable misuse and avoidable crashes. During EA220002, ODI identified at least 13 crashes involving one or more fatalities and many more involving serious injuries, in which foreseeable driver misuse of the system played an apparent role. ODI’s analysis conducted during this investigation, which aligns with Tesla’s conclusion in its Defect Information Report, indicated that in certain circumstances, Autopilot’s system controls and warnings were insufficient for a driver assistance system that requires constant supervision by a human driver. Given Tesla’s recall (23V838) of all vehicles equipped with Autopilot for insufficient controls to prevent misuse, ODI is closing EA22002. Concurrent with that closing, ODI has opened a Recall Query (RQ24009) to assess the effectiveness of the 23V838 remedy. Autopilot & First Responder Scenes

On August 13, 2021, NHTSA?s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE21-020) to assess the performance of Tesla?s Autopilot system (a system characterized by Tesla as an SAE Level 2 driving automation system designed to support and assist the driver in performing the driving task) available in Tesla vehicles. The investigation opening was motivated by an accumulation of crashes in which Tesla vehicles, operating with Autopilot engaged, struck stationary in-road or roadside first responder vehicles tending to pre-existing collision scenes.
