In the past month 4 times I had a Software, Module malfunction. All lights in in the dash are on like a Christmas Tree. Believe is related to the PGM. When to the Honda dealer in Winter Haven explained what happened and show a short video and they said that because my Vin [XXX] have no recall I have to pay $89.99 for an inspection and other charges to fix the problem. I told the service department that my battery & alternator were Ok that many Honda vehicles had the same problems and that I believe is the PGM. When this happens I can’t turn the Suv off, the brakes started acting weird, can’t open the rear door and the engine accelerates. If this is not a safety hazard I don’t know what it is. I have an appointment tomorrow with the dealer but I think that pay for a manufacturer malfunction is no way of doing business INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

While operating my 2019 Honda Pilot, the vehicle experienced a sudden and complete failure of its advanced driver safety systems. Without warning, a cascade of simultaneous error messages illuminated on the dashboard, completely disabling the Collision Mitigation Braking System (CMBS), Road Departure Mitigation, Adaptive Cruise Control, and Lane Keeping Assist networks. This defect renders critical safety features completely inoperable and causes severe driver distraction due to persistent dashboard warnings flashing on the loop. This failure occurred when turning on the car. The sudden loss of safety systems while traveling at highway speeds poses an unreasonable risk of a crash, as the vehicle can no longer utilize automatic emergency braking or lane assistance features. The vehicle currently has about 88000 miles. The dealership has indicated the issue stems from a failure of the front-facing windshield camera sensor, matching the systematic defect patterns alleged in active federal class-action litigation against Honda. I am requesting that the NHTSA formally investigate this widespread camera component failure to mandate a safety recall.

While operating my 2019 Honda Pilot, the vehicle experienced a sudden mechanical failure of the factory moonroof assembly. While driving, the moonroof became permanently stuck open / began violently grinding and refused to close. This mechanical failure presents an immediate safety and security hazard. Because the glass cannot be closed, it exposes the interior cabin and driver to distracting external wind turbulence, weather, and road debris while traveling at highway speeds. Furthermore, the inability to secure the vehicle leaves it vulnerable to theft and environmental damage. The vehicle currently has 88000 miles.Inspection reveals that the failure stems from fragile internal components within the track assembly and drive motor. This structural failure requires a costly, full replacement of the track frame mechanism. I request that the NHTSA track these mechanical track failures to determine if a formal safety investigation or recall is warranted for the 2019 Pilot's moonroof assembly.

Additional Comments for Complaint #11750912:I am uploading photographic evidence of severe material delamination on my vehicle's front radar emblem shield (Part #71125-TG7-A51) [71125-TG7-A51].This manufacturing defect caused a critical safety failure: my vehicle recently executed a sudden, violent phantom braking event on the highway with absolutely no prior warning lights or error codes on the dashboard. The peeling layers and air bubbles on the emblem physically refracted the radar signals, creating a false target that locked the brakes and nearly caused a severe rear-end collision. While I was driving alone during this specific incident, this car is my primary family vehicle used daily to transport my wife and my three young children. I am now genuinely terrified to drive my family in this vehicle and must manually turn off the CMBS system every time I start the car to protect them. American Honda Corporate (Case #16495852) has formally refused to address this active braking hazard, deflecting the issue by focusing only on unrelated radio glitches.I request a federal safety investigation into Honda’s radar emblem material degradation, as it directly compromises collision-mitigation systems without triggering automated dashboard fault alerts, putting families and young children at immediate risk.

VEHICLE SAFETY COMPLAINT - RECALL REPAIR FAILURE AND CMBS MALFUNCTION I am filing a formal vehicle safety complaint regarding two critical defects on my vehicle (VIN: [XXX]) and the refusal of American Honda Motor Co. to address them under Corporate Case #XXX. 1. CRITICAL SAFETY HAZARD (PHANTOM BRAKING): The vehicle's Collision Mitigation Braking System (CMBS) / Automatic Emergency Braking (AEB) is experiencing severe and dangerous malfunctions. While driving at highway speeds through construction zones with absolutely zero obstacles, vehicles, or pedestrians ahead, the vehicle violently and unpredictably slams on the brakes by itself. This unprovoked emergency braking creates an immediate and extreme risk of a catastrophic rear-end collision on the highway. 2. FEDERAL SAFETY RECALL REPAIR FAILURE (CAMPAIGN ZEM): The vehicle's instrument cluster, infotainment display screen, backup camera, and audio system repeatedly go completely black and reset while driving, disabling all driver visibility of vehicle speed, safety alerts, backup assist, and vital indicators. While NHTSA records indicate that the ZEM Recall Campaign was previously logged as "completed" on this VIN, the defect is fully active. This constitutes an official federal recall repair failure. The original safety hazard remains unresolved. MANUFACTURER RESPONSE & REFUSAL: American Honda Corporate was formally notified of both issues under Case #XXX. Corporate responded by completely ignoring the severe phantom braking defect and refused to authorize a safety diagnostic. Furthermore, they used the administrative status of the closed ZEM recall to deny any responsibility for the ongoing screen, camera, and audio failure. American Honda is actively forcing me to drive a vehicle with a failed safety recall repair and a defective autonomous braking system unless I pay out-of-pocket diagnostic fees to the dealer (Dave Mungenast St. Louis Honda). I request an immediate federal investiga INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am submitting this complaint regarding my 2019 Honda Pilot after experiencing a failure of the fuel rail assembly that resulted in the check engine light illuminating and required repair at my own expense. My vehicle exhibited the same symptoms and was diagnosed with the same fuel rail issue that is described in Honda’s existing recall for certain 2019 Honda Pilot vehicles. However, when I contacted Honda, I was informed that my VIN was not included within the recall population, despite my vehicle experiencing the identical failure. The fuel rail failure caused the check engine light to illuminate and required replacement of the defective fuel rail. Because my vehicle was excluded from the recall, Honda declined to cover the repair, and I was forced to pay the full cost out of pocket. I believe this demonstrates that the defect is not limited to the VIN range identified in the recall. My vehicle experienced the exact condition addressed by Honda’s recall, yet I received no assistance solely because my VIN was not included. Owners of vehicles with the same defect should not be responsible for repair costs simply because their vehicle falls outside the current recall range. I am requesting that NHTSA investigate whether the recall population should be expanded to include additional 2019 Honda Pilot vehicles affected by this same fuel rail defect and that Honda reimburse owners who have paid for repairs related to this safety issue.

My 2019 Honda Pilot EX-L (perfectly maintained and with only 72K miles) just started displaying a "Braking System" failure warning light along with dash notifications that a number of critical safety systems are no longer functioning, including: forward collision warning, automatic emergency breaking, lane keeping assistance and departure warning, adaptive cruise control, and more. The car registers a failure code of "U3000-49" which I've learned is related to a bad front camera - a problem apparently plaguing so many Honda vehicles like mine that there is now a class action lawsuit (Jones, et al. v. American Honda Motor Co., Inc.) related to it. Given the lawsuit shows how pervasive this situation is, and the nature of the safety risks involved, I hope this issue will be elevated to a formal NHTSA Safety Recall along with reimbursement for repair expenses (mine will be about $1500).

I am filing this complaint because my 2019 Honda Pilot has developed a serious safety-related defect. When I started the vehicle, multiple warning lights remain on, including the LKAS, ACC, CMBS, and Brake System warnings. I haven't had it inspected by the dealer yet, as the closest dealer is 25 miles away down a curvy mountain road. I expect that it will be determined that the front-facing camera has failed. Because this camera supports several driver-assistance and warning systems, the failure has made those systems unreliable and prevents the vehicle from accurately alerting me to other potential problems. This creates a significant safety concern because, if another malfunction occurs, I may not be able to distinguish it from the existing warnings. The warning lights remain active unless the camera is replaced at an estimated cost of $2,600. I do not believe owners should be responsible for such an expensive repair when the failure affects critical safety and warning functions. This issue appears to be recurring among 2019 Honda Pilots, with similar complaints reported by other owners online and in social media forums. I respectfully request that Honda investigate this defect and provide an appropriate remedy, including repair assistance or coverage for the failed camera system.

Fuel started to pour from under the vehicle above the gas tank. The seal on the fuel pump was causing this when the vehicle was turned on. The fuel pump was on recall for this failure and the vehicle was towed to the Honda AutoNation Dealership in Mobile, AL. While there the franchise dealership executed Safety Recall 21-034 on my 2019 Honda Pilot; the fuel pump. The technician then sent us a video saying the fuel pump did not fix the problem as a rodent had chewed through the fuel line near the fuel pump above the gas tank. He did not show an image of this damage nor produce evidence of rodents. During the repair, the technician mechanically damaged the high-pressure fuel line. The line subsequently continues to leak gas under pressure, creating an immediate engine fire hazard. The dealer then falsified paperwork blaming rodents to escape liability. This is evident by the crescent shaped slice through the fuel line. I have video proof showing the difference in fuel leaks from the pump and then from the sliced fuel line.

I took my car in for an oil change into the dealership on 5/22. I was told that the timing belt would need to be replaced once I hit 100K miles. Two weeks later, I noticed my car rattling, I scheduled an appointment for Monday (the earliest I could get it into the dealer), on Saturday, the car wouldn't accelerate past 64mph, as I was getting on the highway, I pressed the gas and got it to 70mph. The engine light kicked on and the car started talking to me that the car was overheating and that I needed to pull over for 10-15 minutes. I did so and when I tried to start it, nothing happened. I then jumped it, got it started and it wouldn't accelerate past 12mph. I turned it off and had it towed to Honda. There, the mechanic told me that the rod had broken in the engine. I asked if it was the timing belt and he said "No, unfortunately, there is nothing you could've done to prevent this". I am submitting this complaint regarding a fuel injector failure and emissions system malfunction in my 2019 Honda Pilot. At the time of the failure, the vehicle had approximately 94,693 miles and had been routinely maintained. On [XXX], while traveling from Ohio to Pennsylvania with my family, the vehicle displayed an "Emission System Problem" warning and illuminated the check engine light. The vehicle was diagnosed by Faulkner Honda in Harrisburg, Pennsylvania. The dealership found DTC P0430 (Catalyst System Efficiency Below Threshold) and performed an air/fuel ratio test. According to the dealership, four of the six cylinders were running rich due to excessive fuel delivery. The dealership recommended replacement of the fuel injectors, and all six fuel injectors were replaced. The injector repair cost approximately $1,100. The dealership advised that possible causes included internal fuel injector failure or carbon buildup but could not provide a definitive explanation for the failure. Neither the dealership nor Honda could explain why my vehicle was excluded from Honda's fuel injector warranty extension despite exhibiting the same symptoms and requiring the same repair associated with that program. I was advised that the warranty extension applies only to certain VINs produced during specific manufacturing periods and facilities. However, my vehicle experienced the same failure pattern, including a rich-running condition, emissions-system warning, catalyst-efficiency diagnostic code, and replacement of the fuel injectors. While researching the issue, I reviewed NHTSA complaints involving 2019 Honda Pilot vehicles. Numerous complaints describe similar fuel injector failures, emissions-system warnings, catalyst-efficiency codes, and injector replacements. Based on the number of similar complaints, the affected vehicle population may extend beyond the VIN range currently covered by Honda's warranty extension. Vehicle: 2019 Honda Pilot VIN: [XXX] My 2019 Honda Pilot is experiencing an "Emissions System Problem" accompanied by Diagnostic Trouble Code (DTC) P219A (Air-Fuel Ratio Imbalance). This defect causes engine hesitation, rough idling, and a severe risk of sudden loss of motive power or engine stalling while driving at highway speeds, presenting a significant safety hazard to myself and other motorists. American Honda is fully aware of this manufacturing defect. They issued Service Bulletins 20-100 and 21-010 extending the warranty to 10 years/150,000 miles for this exact code and symptom on 2017–2019 Honda Pilots due to defective fuel injectors and faulty PCM software. However, Honda has arbitrarily excluded my specific VIN from this extension (case number Case # XXX). Honda Corporate has officially denied assistance, forcing me to drive a vehicle with a known fuel system defect that threatens sudden vehicle failure in traffic. I request that the NHTSA investigate Honda’s arbitrary exclusion of affected VINs for this acknowledged safety defect. Please find the details regarding the sudden, dangerous failure of my vehicle below. The itemized findings are pulled directly from the dealership’s multi-point inspection and diagnostic report. When was the first time (approximate date) you noticed the issue? The issue culminated in an immediate, hazardous crisis on XXX when the vehicle completely shut down while in operation and refused to restart. How often does the issue happen within a time frame (e.g., once a month)? This is a permanent, catastrophic mechanical failure. The engine is completely inoperable and the vehicle is entirely undriveable. What is the name & title of the person(s) assisting you at the dealer? I am being assisted by Victoria Dalessandro at Honda City Long Island in [XXX]. What is the reason/explanation provided by the dealer, as to what caused this issue? Following a formal engine teardown, the dealership confirmed a severe internal failure: a lower engine knock originating from the front connecting rod bearing. This internal component defect completely seized the powertrain, causing the vehicle to shut down and rendering the engine unrepairable. What repairs/services have been completed (if any)? Only the initial teardown and diagnostic inspection have been completed. I have a 2019 Honda Pilot Touring with an intermittent fault. At apparently random intervals, I'll get the following faults on start: Adaptive Cruise Control Problem Lane Keeping Assist Problem Collision Mitigation System Problem Road Departure Mitigation System Problem Auto High-Beam Problem If I drive the car a couple of miles, there's a 50-50 chance I'll get them the next time the car starts. It's not throwing any fault codes. Had the oil changed, battery checked, cleaned the "H" sign to make sure it wasnt anything dirty on the radar.

When at a complete stop, engine will stop running but AC and radio stay on (sometimes radio dies). I have to toggle between park or drive or neutral to get car to even power completely off, open door to kill electronics, and restart. This has happened in stop and go traffic and at stop lights. This week my car shifted into neutral while stopped at a light and I had to figure out how to power car off and restart, no idea what the magic combo ended up to be as I had a line of cars behind me trying to just get my car to go again.

My engine has been diagnosed as having connecting rod bearing knock, which is indicative of failing connecting rod bearings. Two independent mechanics have told me I need a new engine. I'm submitting this complaint as further evidence for the NHTSA investigation that Honda's Honda Pilot Connecting Rod Bearing Safety Recall had an insufficient of covered vehicles. At the time of purchase and as of today, my VIN is not part of Honda's recall, yet I have experienced connecting rod bearing failure, as have many others. There are several warning Lights including but not limited to ABS, lane assist, trailer assist. So far, my safety is not at risk, but I have a 2 year old grandchild that I drive around multiple times a week. I can’t afford this repair and feel as though I cannot drive this vehicle so I only drive it when absolutely necessary. The vehicle has 82,000 miles on it. Should the engine fail, it's likely that would cause an accident and injury or death. I originally had the vehicle diagnosed because I could hear the noise myself and it was not a normal noise.

Multiple malfunction warning lights came on stating the adaptive cruise control, emergency braking, lane keep assist, automatic headlights, etc. were not functioning. The Honda dealer attributed it to the forward facing multi-purpose camera. I was told they have seen the same issue before. It's a very expensive part to have replaced ($1846.50). While researching the problem it appears to be a very common problem with Honda and I'm sure the dealership is not forwarding this information to you. This is a safety issue and I'm sure people are driving around without fixing it because of the cost. The cost to fix this problem should be absorbed by Honda since it is definitely an engineering issue. Fortunately, I had an extended warranty on my car that covered most of the cost.

My engine lights turned on Friday, April 24th and I brought in my vehicle to a local dealership on Monday, April 27th. I was informed it was due to a faulty catalytic converter and it was replaced under warranty. The services was completed on Tuesday, April 29th and took my vehicle home. On May 7th, I turn on my engine and all the engine check lights came on. My car was jerky. I was scheduled to bring in my vehicle on May 12th to a local dealership. On May 13th, I received update that my engine had oil leakage and required a new engine. I believe that my 2019 Honda Pilot is among the thousands of Honda Pilot within the make year that are experiencing engine failure. The vehicle’s A/C system is experiencing a refrigerant leak due to a known issue with the compressor shaft seal. This issue appears to be associated with the transition to the newer R1234yf refrigerant, after which the shaft seal design was not adequately updated, leading to premature seal failure and refrigerant leakage. This is not an isolated case. Similar A/C compressor shaft seal leaks have been widely reported across multiple Honda models, including the Honda Civic, Accord, and CR-V. For several of these models, Honda has acknowledged the defect and issued an extended warranty program covering A/C compressor shaft seal failures. However, for the Honda Pilot, although Honda has issued a service bulletin describing the same shaft seal leak condition and repair procedure, no extended warranty coverage has been provided. The service bulletin for the Pilot describes the same failure mode and repair process as those covered under extended warranty programs for other models, indicating that Honda is aware of the defect and its root cause. Despite this acknowledgment, Pilot owners are required to bear the full cost of repairs, which can be significant, even though the underlying issue is consistent with those already covered in other vehicles. This issue also has potential safety implications. Loss of A/C function can impair the defrosting system, leading to windshield fogging and reduced visibility, especially in humid or cold conditions. Reduced visibility can directly impact driver safety. Given that Honda has already recognized this defect in multiple models and implemented extended warranty coverage, I believe similar coverage should be extended to the Honda Pilot to ensure fair treatment of customers and to address the associated safety concerns. I have attached the extended warranty documentation for the Honda Civic as well as the service bulletin for the Honda Pilot; they describe the same issue and repair procedure.

While driving on the highway, the vehicle experienced sudden engine failure at highway speeds, putting the safety of our son and others drivers at risk. The engine completely lost power. The diagnosing dealership confirmed failure of a connecting rod and bearings and internal engine damage with metal present throughout the engine, requiring full engine replacement at an estimated cost of over $17,000. The car is still at the South Pointe Honda dealership in Tulsa, OK. The diagnostic was also confirmed by a Honda certified mechanic at another independent shop. No others have inspected the vehicle. This appears consistent with reported issues involving connecting rod bearing failures in Honda V6 engines of similar model years. The failure occurred prematurely and without warning, raising concerns about reliability and potential safety risks if loss of power occurs during operation. There were no warning lamps, messages, or other symptoms of a problem prior to failure. The vehicle has been properly maintained and was not subject to abnormal use. I am submitting this complaint due to the severity of the failure, the high repair cost, and concern that similar failures may pose a safety risk to other drivers.

Fuel/Propulsion SystemApr 2026 · ODI 11731305Details +

An “Emissions System Problem” warning message appeared intermittently on the dashboard while driving. The warning light would sometimes turn off after restarting the vehicle, but the issue persisted in the vehicle’s stored diagnostic system. The vehicle was taken to an authorized dealer, where a diagnostic scan confirmed a P0430 code (catalyst system efficiency below threshold). The dealer identified the likely cause as failing fuel injectors and recommended replacement of the injector system. The issue was confirmed by the dealership and is reproducible through stored fault codes, even when the warning light is not actively displayed. This condition may impact engine performance, fuel mixture, and emissions system operation. In certain situations, this could lead to reduced engine performance or hesitation, which may affect the ability to safely accelerate or merge into traffic. The vehicle has been regularly maintained according to manufacturer recommendations. There were no significant warning signs prior to the initial dashboard alert other than the intermittent warning message. The failed components (fuel injectors) should be available for inspection upon request through the servicing dealership.

Investigations

4 NHTSA investigations on record

Investigations are agency reviews, not findings that a vehicle is defective.

Engine failure

The Office of Defects Investigation (ODI) has received 414 reports of connecting rod bearing failures in the 3.5L V6 engine used in the following vehicles: Model Years (MY) 2018-2020 Acura TLX, MY2016-2020 Acura MDX, MY2016-2020 Honda Pilot, MY2018-2020 Honda Odyssey, and MY2017-2019 Honda Ridgeline.  The subject engine failures were analyzed as part of a Recall Query (RQ24013).  It was determined that these engine failures are outside the scope of Honda Recall 23V-751. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751.  Based on analysis of data from RQ24013, NHTSA has closed that investigation. The significant number of reports of engine failure in vehicles outside the scope of RQ24013 presents a potential safety risk that warrants further investigation.  ODI is opening this Preliminary Evaluation (PE) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues.  To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

No Restart After Auto Start/Stop Engages

On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

Engine failure

On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation.  NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.
