The Office of Defects Investigation (ODI) has received 414 reports of connecting rod bearing failures in the 3.5L V6 engine used in the following vehicles: Model Years (MY) 2018-2020 Acura TLX, MY2016-2020 Acura MDX, MY2016-2020 Honda Pilot, MY2018-2020 Honda Odyssey, and MY2017-2019 Honda Ridgeline. The subject engine failures were analyzed as part of a Recall Query (RQ24013). It was determined that these engine failures are outside the scope of Honda Recall 23V-751. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751. Based on analysis of data from RQ24013, NHTSA has closed that investigation. The significant number of reports of engine failure in vehicles outside the scope of RQ24013 presents a potential safety risk that warrants further investigation. ODI is opening this Preliminary Evaluation (PE) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.”

“Engine failure
On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation. NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.”

“HONDA PILOT 2019 -DRIVING ON A HIGHWAY WITH MY WHOLE FAMILY WITH 3 KIDS AT THE BACK, AT A SPEED ~40MPH. THE LANE IN WHICH I WAS DRIVING (RIGHT) SLOWED DOWN, SO MOVED TO THE LEFT LANE. THAT LANE WAS ALSO SLOWING AS I CAN SEE THE CARS IN THE FRONT SLOWING DOWN AND STOPPING. MY ATTEMPTS TO STOP THE CAR PROVED FUTILE. THE CAR DID NOT STOP. 1) REPEATED PUMPING OF THE BREAKS DID NOT YIELD TO STOP THE CAR. HONDA PILOT COLLIDED WITH THE CAR IN THE FRONT. 2) THE CARS SYSTEM TO SLOW THE CAR WHILE APPROACHING A VEHICLE IN THE FRONT DID NOT FUNCTION. 3) AIRBAGS DID NOT DEPLOY AT ALL - NONE OF THE AIRBAGS DEPLOYED. - COLLISION CENTER WAS LITERALLY BAFFLING, OVER THIS - HAVE NO CLUE HOW THE AIRBAGS DID NOT DEPLOY WITH THIS KIND OF A COLLISION. I SERIOUSLY SUSPECT THAT THERE IS SOME THING SERIOUSLY FLAWED. SINCE I'M NOT AN AUTOMOBILE ENGINEER, I'M NOT SURE OF TECHNICALITY INVOLVED IN THIS. SOMETHING WENT WRONG TERRIBLY, THAT THE CAR DID NOT STOP ON BRAKING, CAR DID NOT SLOW DOWN, & THE AIRBAGS DID NOT DEPLOY. WHEN INFORMED ABOUT THIS TO HONDA, HONDA WAS REFERRING TO GEICO, THAT THEY WILL DO THE INVESTIGATION. GEICO SAYS THAT ITS NOT THEIR PROBLEM AND HONDA SHOULD DO THE INVESTIGATION. AS A COMMON MAN I WOULD LIKE TO HAVE THIS PROBLEM INVESTIGATED AND NECESSARY ACTION TO BE TAKEN. I SEE THEY SUBROGATE EACH OTHER, WHICH IS NOT CORRECT. I'M NOT SURE HOW MANY CARS ARE RUNNING IN THIS CONDITION ON THE ROAD. PLEASE DO TAKE NECESSARY STEPS TO CHECK ON THIS. I CAN GIVE ALL THE REQUIRED DOCUMENTS, PICTURES AND POLICE REPORTS IN THIS REGARD. I NEED ASSISTANCE IN INVESTIGATION OF THIS ACCIDENT AND FIND OUT WHY THE CAR DID NOT RESPOND. I SEE THAT THERE ARE OTHER RECALLS WITH REFERENCE TO HONDA PILOT 2019, THOUGH MY CAR IS NOT ON THE RECALL, I HAVE MY DOUBTS THAT THERE IS MALFUNCTIONING COMPONENT IN THE CAR, THAT HAS CAUSED THIS FLAW. HOPING TO HEAR FROM YOU SOON. THANK YOU.”

“I HAD A HEAD ON COLLISION IN MY 2019 HONDA PILOT ON A CITY STREET AT ABOUT 25-30MPH AUGUST 29, 2019. THE SENSOR WAS TRIGGERED AND THE AIRBAG SYSTEM "ATTEMPTED" TO INFLATE. MY HEAD SMASHED INTO THE STEERING WHEEL AND THE AIRBAG SYSTEM NEVER PROPERLY INFLATED. I JUST KEPT HEARING A SOUND LIKE IT WAS TRYING TO INFLATE BUT HAD A HOLE IN IT OR SOMETHING. THE MEDIC SAID THAT IF MY AIRBAG INFLATED PROPERLY, IT WOULD HAVE BEEN LIKE A PUNCH IN THE FACE AND BLOWN ME BACK. INSTEAD, MY HEAD SLAMMED THE WHEEL AND THE BAG NEVER GOT INFLATED NEAR CAPACITY. I CALLED HONDA AND REPORTED THE INCIDENT I WAS GIVEN HONDA CASE # 09867010. I WAS THEN CALLED A FEW DAYS LATER BY ADRIAN FROM AMERICAN HONDA WHO TOLD ME THERE WAS NO WAY THE AIRBAG FAILED, AND THAT IF IT DID, I HAD TO LET INSURANCE TELL HONDA AND I WAS TO TELL NO ONE ELSE. HE THEN TOLD ME Y MEMORY HAD TO BE WRONG AND IF I SAID OTHERWISE, I WAS A LIAR. I REPORTED THIS SO THAT NO ONE ELSE WOULD ENCOUNTER THIS SAFETY ISSUE AND HONDA WOULD HANDLE IT, BUT IT SEEMS THEY ARE DENYING THE POSSIBILITY. I HAD THE BODY SHOP SAVE THE AIRBAG AND RELATED COMPONENTS FOR POSSIBLE INSPECTION. THE INSURANCE COMPANY SAID THEY DID NOT CARE TO LOOK INTO THE MATTER AND THAT I SHOULD CONTACT HONDA OR AN ATTORNEY MYSELF IF I FELT SOMETHING FAILED.”

“In the past month 4 times I had a Software, Module malfunction. All lights in in the dash are on like a Christmas Tree. Believe is related to the PGM. When to the Honda dealer in Winter Haven explained what happened and show a short video and they said that because my Vin [XXX] have no recall I have to pay $89.99 for an inspection and other charges to fix the problem. I told the service department that my battery & alternator were Ok that many Honda vehicles had the same problems and that I believe is the PGM. When this happens I can’t turn the Suv off, the brakes started acting weird, can’t open the rear door and the engine accelerates. If this is not a safety hazard I don’t know what it is. I have an appointment tomorrow with the dealer but I think that pay for a manufacturer malfunction is no way of doing business INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)”

“VEHICLE SAFETY COMPLAINT - RECALL REPAIR FAILURE AND CMBS MALFUNCTION I am filing a formal vehicle safety complaint regarding two critical defects on my vehicle (VIN: [XXX]) and the refusal of American Honda Motor Co. to address them under Corporate Case #XXX. 1. CRITICAL SAFETY HAZARD (PHANTOM BRAKING): The vehicle's Collision Mitigation Braking System (CMBS) / Automatic Emergency Braking (AEB) is experiencing severe and dangerous malfunctions. While driving at highway speeds through construction zones with absolutely zero obstacles, vehicles, or pedestrians ahead, the vehicle violently and unpredictably slams on the brakes by itself. This unprovoked emergency braking creates an immediate and extreme risk of a catastrophic rear-end collision on the highway. 2. FEDERAL SAFETY RECALL REPAIR FAILURE (CAMPAIGN ZEM): The vehicle's instrument cluster, infotainment display screen, backup camera, and audio system repeatedly go completely black and reset while driving, disabling all driver visibility of vehicle speed, safety alerts, backup assist, and vital indicators. While NHTSA records indicate that the ZEM Recall Campaign was previously logged as "completed" on this VIN, the defect is fully active. This constitutes an official federal recall repair failure. The original safety hazard remains unresolved. MANUFACTURER RESPONSE & REFUSAL: American Honda Corporate was formally notified of both issues under Case #XXX. Corporate responded by completely ignoring the severe phantom braking defect and refused to authorize a safety diagnostic. Furthermore, they used the administrative status of the closed ZEM recall to deny any responsibility for the ongoing screen, camera, and audio failure. American Honda is actively forcing me to drive a vehicle with a failed safety recall repair and a defective autonomous braking system unless I pay out-of-pocket diagnostic fees to the dealer (Dave Mungenast St. Louis Honda). I request an immediate federal investiga INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)”

The records describe a possible crash, fire, injury, or loss of control.
“While operating my 2019 Honda Pilot, the vehicle experienced a sudden and complete failure of its advanced driver safety systems. Without warning, a cascade of simultaneous error messages illuminated on the dashboard, completely disabling the Collision Mitigation Braking System (CMBS), Road Departure Mitigation, Adaptive Cruise Control, and Lane Keeping Assist networks. This defect renders critical safety features completely inoperable and causes severe driver distraction due to persistent dashboard warnings flashing on the loop. This failure occurred when turning on the car. The sudden loss of safety systems while traveling at highway speeds poses an unreasonable risk of a crash, as the vehicle can no longer utilize automatic emergency braking or lane assistance features. The vehicle currently has about 88000 miles. The dealership has indicated the issue stems from a failure of the front-facing windshield camera sensor, matching the systematic defect patterns alleged in active federal class-action litigation against Honda. I am requesting that the NHTSA formally investigate this widespread camera component failure to mandate a safety recall.”

“Additional Comments for Complaint #11750912:I am uploading photographic evidence of severe material delamination on my vehicle's front radar emblem shield (Part #71125-TG7-A51) [71125-TG7-A51].This manufacturing defect caused a critical safety failure: my vehicle recently executed a sudden, violent phantom braking event on the highway with absolutely no prior warning lights or error codes on the dashboard. The peeling layers and air bubbles on the emblem physically refracted the radar signals, creating a false target that locked the brakes and nearly caused a severe rear-end collision. While I was driving alone during this specific incident, this car is my primary family vehicle used daily to transport my wife and my three young children. I am now genuinely terrified to drive my family in this vehicle and must manually turn off the CMBS system every time I start the car to protect them. American Honda Corporate (Case #16495852) has formally refused to address this active braking hazard, deflecting the issue by focusing only on unrelated radio glitches.I request a federal safety investigation into Honda’s radar emblem material degradation, as it directly compromises collision-mitigation systems without triggering automated dashboard fault alerts, putting families and young children at immediate risk.”
The records describe a possible crash, fire, injury, or loss of control.
“WHEN I STARTED MY 2019 HONDA PILOT TOURING CAR ON 7/13/26 MORNING, WHERE THE SCREEN WHERE THE SPEED USUALLY SHOWS SHOWED AMBER COLORED WARNINGS FOR HONDA SENSING FEATURES: ACC, LKAS, CMBS, ROAD DEPARTURE MITIGATION SYSTEM, AUTO HIGH BEAM. LKAS, CMBS AND BRAKE SYSTEM AMBER ICONS ARE ON. INFOTAINMENT DISPLAY SHOWS ACC AND CMBS HAS A PROBLEM. I TRIED CLEANING THE WINDSHIELD AND SENSORS IN THE FRONT BUT SAME WARNINGS STILL CONTINUED TO SHOW AT NEXT START UP AND DRIVE. LAST FEW DAYS I STARTED IT STILL POPS UP SO I TOOK IT TO INDEPENDENT SHOP YESTERDAY 7/15/26. THEY PULLED A U-CODE FOR MULTIPURPOSE CAMERA, TRIED TO CLEAN THE SENSORS BUT WARNINGS WOULD STILL SHOW SO IT'S A HARD CODE NOW. I HAD THEM DOCUMENT IT FOR ME. THESE ARE SAFETY FEATURES THAT CAME WITH THE CAR, SHOULD THEY BE FAILING THIS EARLY ON? I HAVE NOT DONE ANYTHING TO THERE, HAVEN'T BEEN IN ANY ACCIDENTS INVOLVING THAT AREA AND HAVEN'T HAD WINDSHIELD REPLACED EITHER. THEY WEREN'T ABLE TO FIX IT AS THEY DON'T HAVE THE CALIBRATION EQUIPMENT SO SAID WE'D NEED TO SEND TO HONDA DEALER.”
The records describe a possible crash, fire, injury, or loss of control.
“For about a year now the front facing camera throw 5 warnings, my cruise dont work, and it cost 2200 to fix. I follow a honda group on fb and apparently this is the main complaint. Something needs to be done bc it is very un sage to have all these safety features and they may r may not work”
The records describe a possible crash, fire, injury, or loss of control.
“I am filing this complaint because my 2019 Honda Pilot has developed a serious safety-related defect. When I started the vehicle, multiple warning lights remain on, including the LKAS, ACC, CMBS, and Brake System warnings. I haven't had it inspected by the dealer yet, as the closest dealer is 25 miles away down a curvy mountain road. I expect that it will be determined that the front-facing camera has failed. Because this camera supports several driver-assistance and warning systems, the failure has made those systems unreliable and prevents the vehicle from accurately alerting me to other potential problems. This creates a significant safety concern because, if another malfunction occurs, I may not be able to distinguish it from the existing warnings. The warning lights remain active unless the camera is replaced at an estimated cost of $2,600. I do not believe owners should be responsible for such an expensive repair when the failure affects critical safety and warning functions. This issue appears to be recurring among 2019 Honda Pilots, with similar complaints reported by other owners online and in social media forums. I respectfully request that Honda investigate this defect and provide an appropriate remedy, including repair assistance or coverage for the failed camera system.”

“Started vehicle and proceeded to drive forward in my driveway when smoke started coming in the vehicle. Noticed after driving that the smoke was coming in the vehicle from under the glove box on the passenger side by the floor mat. Within approximately two minutes a flame ignited in which the dashboard caught on re. The whole inside on the vehicle was destroyed & totaled the whole vehicle. I have about 25 more pictures of the car (after the re and during) that I couldnt send via this webform. If you need them I would be happy to email them to you directly.”

The records describe a possible crash, fire, injury, or loss of control.

“No Restart After Auto Start/Stop Engages
On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.”

“Engine failure
