“Inadvertent Automatic Emergency Braking On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles.  The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations.  However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury.  The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources.  In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged.  In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. “Driving at a speed of approximately 30-35 mph over a railroad track, the automatic emergency braking system was erroneously triggered. The vehicle came to an extremely abrupt stop in the middle of the railroad track. The vehicle behind us proceeded to rear-ended us. This is the second time the vehicle has stopped abruptly at a railroad track. The warning messages only appeared at the exact same moment the car stopped, not providing any warning as to what was about to happen. I'm awaiting an appointment for the car to be seen by the dealership for the AEB system, but because this caused a collision I filed a claim on my insurance and the car had to be repaired. This was a safety issue for multiple reasons, including that it caused injury for both the driver and the passenger. It also was a safety issue because it occurred on a railroad track and if the car had not been drivable, or if we had been incapacitated, we could have been killed by the train.”

“On September 6, 2023 at approximately 5:30 pm, I was driving approximately 30 mph when suddenly my car stopped abruptly for no reason. Consequently although with seat belt on, my head and neck were thrown forward and back. There was no obstacle in my path...the nearest vehicle in front of me was about one city block away. I had an immediate headache and my neck was hurt also. This pain lasted several hours. I had my grandson and his friend in my vehicle at the time and they were wearing their seatbelts and fortunately they were not injured.I made an appointment with the dealer and went the next day They adjusted this system and told me these things have occurred possibly because of a shadow or something else off to the side of the road. There was nothing in either case.”

“On Aug 15, 2023 i was turning left at an intersection with no cars in front of me and no cars in oncoming traffic. i was going 20-25mph and slowing down a little bit to turn when i drove over a standard metal plate on the road and my 2018 Honda crv came to a complete abrupt and loud , painful stop. “I was driving on a clear flat surface street and all of a sudden the brake warning started to flash and then my vehicle shook a little and then automatically stopped. There were no cars in front of me. Fortunately the car behind me was able to swerve to prevent a catastrophic accident. The car behind was a large utility van with metal in the front - he would have completely smashed my car. My son was in the backseat and slightly injured. “On 01/08/2025, Time: 8:10 PM EST – Automatic Emergency Breaks applied though there are no objects, people or vehicles in front of the vehicle. My wife, [XXX] kid and I were heading back to home from COSTCO, Mathews NC. Suddenly, the Automatic Emergency Break applied without any warnings on the dashboard. Because of the sudden break, my wife got injured, she hit her knee to the back air vent. I must take a long breath for a min and move the car to the parking lot and check back with my kid and wife. There was a little distance from my car to the other car behind me. So, luckily, we avoided the collision. Otherwise, there would be a major collision. This is the third time we were run to this issue at the same spot. I am keep observing the warning messages earlier while driving on the highway and but stopping all the sudden without any warnings make us very scary. I was still doubting the security measures and investigation process being held at the HONDA. Please consider the fact that, “Human Lives matter and security is a fundamental right.” and resolve the issue at the earliest. “Rear Differential Seizure On November 29, 2022, the Office of Defects Investigation (ODI) opened PE 22-011 after reviewing field reports alleging rear differential seizure in 2018-2022MY Honda CR-V and HR-V vehicles equipped with all wheel drive. To date, ODI has not received any consumer complaints. Some of the field reports received alleged the vehicle’s wheels locked up with no warning while the vehicle was in motion. These vehicles were towed to a dealership. Examination of these vehicles showed the driveshaft had broken and the differential unit was completely damaged. The field reports cited a lack of oil due to damaged oil seals as the cause. Honda provided data including consumer complaints, field reports, and warranty claims they received relating to the loss of oil retention and resulting differential damage. Honda’s data cited 1201 incidents with unique vehicle identification numbers (VINs). The rear differential oil seals  prevent the oil from leaking out and protect internal components from contaminants. A loss of oil lubrication results in failure of the vehicle differential.  Honda stated that the oil seal’s lip contact with the driveshaft may become unstable due to intermittent changes in internal pressure. Their review of field report and warranty data indicated that these failures occurred early in the vehicle’s life thus the failures were covered by the manufacturer’s warranty. Honda stated that in November 2021 they changed the seal specifications and improved the packaging process of the parts for transport.  Honda also enacted an enhanced visual inspection process both before and after the oil seal installation on the assembly line. Honda stated that in the event of differential failure, the operator can control the vehicle without stability loss. After the drive shafts break due to differential failure, the operator will hear noises and the vehicle will still function as a 2WD vehicle preventing complete loss of motive power. Based on the following factors, ODI is closing this investigation: ·       Information regarding field failures does not indicate hazardous outcomes ·       The failures are occurring early in the life of the vehicle ·       Repairs are covered under the manufacturer’s warranty ·       The vehicle can continue functioning as a 2WD without loss of stability control in the event of failure The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the agency reserves the right to take additional action if warranted by new circumstances.”

“The contact owned a 2018 Honda CR-V. The contact stated that his wife was driving approximately 60 mph, when a Chevy Blazer crashed into the vehicle. The front passenger door was detached during the impact. There were no warning lights illuminated. The air bags did not deploy. A police report was filed. There was no reported fire. The passenger occupant sustained head injuries, and medical attention was provided. The driver and two rear occupants had no injuries, but medical attention was provided at the hospital. The vehicle was towed to a towing lot. “Driving at a speed of approximately 30-35 mph over a railroad track, the automatic emergency braking system was erroneously triggered. The vehicle came to an extremely abrupt stop in the middle of the railroad track. The vehicle behind us proceeded to rear-ended us. This is the second time the vehicle has stopped abruptly at a railroad track. The warning messages only appeared at the exact same moment the car stopped, not providing any warning as to what was about to happen. I'm awaiting an appointment for the car to be seen by the dealership for the AEB system, but because this caused a collision I filed a claim on my insurance and the car had to be repaired. This was a safety issue for multiple reasons, including that it caused injury for both the driver and the passenger. It also was a safety issue because it occurred on a railroad track and if the car had not been drivable, or if we had been incapacitated, we could have been killed by the train.”

“On September 6, 2023 at approximately 5:30 pm, I was driving approximately 30 mph when suddenly my car stopped abruptly for no reason. Consequently although with seat belt on, my head and neck were thrown forward and back. There was no obstacle in my path...the nearest vehicle in front of me was about one city block away. I had an immediate headache and my neck was hurt also. This pain lasted several hours. I had my grandson and his friend in my vehicle at the time and they were wearing their seatbelts and fortunately they were not injured.I made an appointment with the dealer and went the next day They adjusted this system and told me these things have occurred possibly because of a shadow or something else off to the side of the road. There was nothing in either case.”

“On Aug 15, 2023 i was turning left at an intersection with no cars in front of me and no cars in oncoming traffic. i was going 20-25mph and slowing down a little bit to turn when i drove over a standard metal plate on the road and my 2018 Honda crv came to a complete abrupt and loud , painful stop. “I was driving on a clear flat surface street and all of a sudden the brake warning started to flash and then my vehicle shook a little and then automatically stopped. There were no cars in front of me. Fortunately the car behind me was able to swerve to prevent a catastrophic accident. The car behind was a large utility van with metal in the front - he would have completely smashed my car. My son was in the backseat and slightly injured. In the past, this brake warning has come on numerous times without any cars in front but has not stop automatically. This was the first time. I'm scared to drive the car now. “COMPUTER BREAK SYSTEM AND APPLIED BRAKES FAILED TO STOP OUR HONDA CRV 2018 AS WE WERE EXITING THE SANTA FE OPERA IN A SLOW MOVING LINE OF CARS DOWN AN INCLINE. “On 01/08/2025, Time: 8:10 PM EST – Automatic Emergency Breaks applied though there are no objects, people or vehicles in front of the vehicle. My wife, [XXX] kid and I were heading back to home from COSTCO, Mathews NC. Suddenly, the Automatic Emergency Break applied without any warnings on the dashboard. Because of the sudden break, my wife got injured, she hit her knee to the back air vent. I must take a long breath for a min and move the car to the parking lot and check back with my kid and wife. There was a little distance from my car to the other car behind me. So, luckily, we avoided the collision. Otherwise, there would be a major collision. This is the third time we were run to this issue at the same spot. I am keep observing the warning messages earlier while driving on the highway and but stopping all the sudden without any warnings make us very scary. I was still doubting the security measures and investigation process being held at the HONDA. Please consider the fact that, “Human Lives matter and security is a fundamental right.” and resolve the issue at the earliest. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations.  However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury.  The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources.  In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged.  In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. “Rear Differential Seizure On November 29, 2022, the Office of Defects Investigation (ODI) opened PE 22-011 after reviewing field reports alleging rear differential seizure in 2018-2022MY Honda CR-V and HR-V vehicles equipped with all wheel drive. To date, ODI has not received any consumer complaints. Some of the field reports received alleged the vehicle’s wheels locked up with no warning while the vehicle was in motion. These vehicles were towed to a dealership. Examination of these vehicles showed the driveshaft had broken and the differential unit was completely damaged. The field reports cited a lack of oil due to damaged oil seals as the cause. Honda provided data including consumer complaints, field reports, and warranty claims they received relating to the loss of oil retention and resulting differential damage. Honda’s data cited 1201 incidents with unique vehicle identification numbers (VINs). The rear differential oil seals  prevent the oil from leaking out and protect internal components from contaminants. A loss of oil lubrication results in failure of the vehicle differential.  Honda stated that the oil seal’s lip contact with the driveshaft may become unstable due to intermittent changes in internal pressure. Their review of field report and warranty data indicated that these failures occurred early in the vehicle’s life thus the failures were covered by the manufacturer’s warranty. Honda stated that in November 2021 they changed the seal specifications and improved the packaging process of the parts for transport.  Honda also enacted an enhanced visual inspection process both before and after the oil seal installation on the assembly line. Honda stated that in the event of differential failure, the operator can control the vehicle without stability loss. After the drive shafts break due to differential failure, the operator will hear noises and the vehicle will still function as a 2WD vehicle preventing complete loss of motive power. Based on the following factors, ODI is closing this investigation: ·       Information regarding field failures does not indicate hazardous outcomes ·       The failures are occurring early in the life of the vehicle ·       Repairs are covered under the manufacturer’s warranty ·       The vehicle can continue functioning as a 2WD without loss of stability control in the event of failure The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the agency reserves the right to take additional action if warranted by new circumstances.”

The magnet that controls the torque sensor output signal for the electronic power steering system may not be properly secured, allowing the magnet to become dislodged. During a full lock turn, the dislodged magnet may cause steering assist to be applied in the opposite direction.
