“Inadvertent Automatic Emergency Braking
On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles. The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations. However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury. The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources. In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged. In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. “On Thursday, February 17, 2022, I drove home from work in Waycross, Georgia. I was driving my properly-maintained 2017 Honda CRV. I was wearing my seat belt, and the shoulder harness was properly secured across my chest. The day was clear, and at 4:03 p.m., I approached the intersection of Jenkins St, Tebeau St, Plant Ave, and Francis St. This intersection is in the downtown area; it is treated like a perpendicular intersection, although, as you can see from the attached photo, it is not, in fact, perpendicular. There are stop signs on Jenkins and Tebeau; travelers on Plant and Francis do not have to stop. Because of how Francis Street curves onto Plant, and because of the fact that Francis emerges from under an overpass, visibility is poor. I was on Francis St, which becomes Plant Ave headed east. A construction worker driving a 2016 Chevy Silverado K350 TK was fully stopped at the stop sign at the Jenkins St intersection. After stopping, he pulled into the intersection, T-boning my CRV and sending it into a series of spins. My head violently hit the window several times as the vehicle spun. When the CRV finally stopped, my first thought was, "Where are The airbags?" Side curtain airbags had deployed on the passenger side of the vehicle, but no airbags had deployed on the driver's side anywhere--or from the steering wheel. An ambulance took me to the local emergency room. I have a concussion. Had the airbags deployed as expected, that might not be the case. My safety was put at risk by the airbags' failure to deploy--basic physics requires that in a violent collision, the driver is protected after being propelled into the door and window. The CRV is in a Copart lot. I have requested that it be kept available for inspection. The CRV was deemed totaled; I do not know if anyone inspected the airbags. There were no prior warnings. During the instrument panel went berserk. Multiple error messages flashed simultaneously, despite the front end not being hit.”

AFTER THE CAR SCREENING YOU ARE IN CRASH I WAS A WAKE NOT SURE HOW IT STOPED AND WAS IN SHOCK FOR NOT SURE HOW LONG THEN I SAW A MESSAGE ON THE SCREEN SAYING CONTACTING HONDA BUT NOTHING HAPPEN. I TRY TO TOUCH IT BUT IT NOT WORK. I STOPED THE CAR MOVED THE KEY THEN TRY TO FIND MY TELEPHONE THANK FULLY MY CHARGER CORD WAS ON AND I PULLED IT FROM BEHIND SEAT I CALLED 911 THEN THE POLICE TOOK IT FROM THERE. I HAVE SO MUCH PAIN IN MY HEAD MOSTLY LEFT SIDE AND NEAR MY NECK I AM IN SO MUCH PAIN WHEN I AM IN THE SON MY VISION IS NOT OKEY. “I have had my crv suddenly brake on me on multiple occasions when absolutely no one is around to cause it. It’s created many of close calls and given me mild whip lash in the past. But yesterday it happened in a way much worse than ever bc I was on the interstate and it abruptly stopped (again no one in front of me or to the side) which jolted me and gave me some brutal whiplash and then it was locked up & wouldn’t go faster than 20mph!! I was in the left lane & I almost got hit by multiple people behind me & almost died! I had to drive like this for 3-5 minute (hazards on of course) until I could ‘safely’ pull over. Once i turned it off and turned it back on, it went back to accelerating normally. I have taken my car to the Honda dealership for every oil change I’ve gotten & this issue has never come up on their full vehicle inspection. I cannot emphasize enough how close I came to dying bc of this. I really hope a resolution or recall happens sooner than later bc I’m now learning there’s 1000’s of people online saying they’ve had similar experiences. It should not take years to address this when there are so many lives at stake. Please help. Sincerely, terrified to drive my car now (& that’s not good considering I drive for work)”

“The emergency braking was turned off by me a few years ago due to this happening before. Today while I was driving about 35 mph, my car did a hard brake pushing everything including myself to the front giving me whiplash. Good thing nobody was behind me or else they would’ve ran into me for sure. There was nothing even in front of me to even cause this to even happen. I will be calling the dealership about this tomorrow. There was no warning lamps, messages or anything else to indicate this was about to happen or should be happening.”

The records describe a possible crash, fire, injury, or loss of control.
“I WAS REAR ENDED ON THE HIGHWAY (101-N) BY A TRUCK GOING 20-30 MPH ABOVE THE SPEED LIMIT. DURING THE COLLISION, THE DRIVER AIRBAG DID NOT DEPLOY. MY SPEED LIMIT WAS 65-70 AND THE TRUCK SPEED LIMIT WAS MORE THAN MINE WHEN I TRY TO GO FASTER WAS STILL NOT ABLE TO OVIDED. AND IF I WALK MORE THEN 15 MINUTES I AM HAVE THE WORST PAIN IN MY FEET LEGS BACK HEAD AND I DO BELIEVE ALL MY BODY IS HURTING BUT SOME OF MORE THEN THE OTHER MY RIBS ARE BETTER BUT LEFT RIBS HURT WHEN I DRIVE MORE THEN 20 MINUTES. I HAD CONSTIPATION FOR THE LAST 5 WEEKS REALIZED LAST WEEK I CAN NOT SMELL WELL AND I REALLY HATE THAT PERSON FOR WHAT HE DID TO ME. I AM NOT SURE IF I WILL EVER BE ABLE TO RECOVER AL AND THAT SO NOT FAIR AND THE WORST THEY RUN A WAY LEFT ME THERE.”

To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.”
