The contact owned a 2012 Toyota Yaris The contact stated that while slowing down and coming to a stoplight, a second vehicle traveling 54 mph crashed into the rear of the vehicle. During the crash, both the driver-side and passenger-side seat belts malfunctioned and did not secure the occupants. The driver sustained head and torso injuries and the front passenger who was the driver's wife sustained broken ribs, and head and spine injuries. Medical attention was received. The vehicle was destroyed. A police report was taken at the scene and the vehicle was towed away. The local dealer and manufacturer were not notified of the failure. The failure mileage was 76,200.

I was driving a 2012 yaris when I was in an accident that totaled the vehicles. I was hospitalized for 3 weeks which I believe the airbags had played a major role in the seriousness of the injuries I sustained but not sure how to prove that they failed to protect me and possible they made things worse with the scar across my forehead from being sliced open needing numerous staples as well as shattered pelvis. Is there any way by looking at the pictures can you tell me if the air bags did their job

WAS DRIVING ON THE FREEWAY AT 70 MPH AND HAD TO BRAKE FOR SLOWER TRAFFIC AHEAD. WHEN I DEPRESSED THE BRAKE PEDAL, THE ABS SYSTEM ENGAGED, BRAKING THE LEFT SIDE OF THE CAR, THEN THE RIGHT ALMOST SENDING ME OFF THE ROAD. AFTER THIS HAD HAPPENED, THE SPEEDOMETER HAD STARTED JUMPING TO DIFFERENT SPEEDS VERY RAPIDLY AND I WAS UNABLE TO TELL MY CORRECT SPEED. THE ANTILOCK BRAKES HAVE CONTINUED TO ENGAGE RANDOMLY WHEN THEY ARE NOT NEEDED, CAUSE LOSS OF BRAKING CONTROL AND THE OVERALL ABILITY TO COME TO A STOP. THE SPEEDOMETER ALSO JUMPS AROUND EVERY NOW AND THEN. THE ABS LIGHT WAS ONLY TRIGGERED ONCE, AND THEN TURNED OFF.

I AM ON VACATION HEADED TO SOUTH DAKOTA FROM CALIFORNIA AND THIS PUTS MYSELF, PASSENGER, AND OTHER DRIVERS ON THE HIGHWAY IN DANGER IF IT COMPLETELY DETACHES!

I PUT IT BACK CORRECTLY JUST AS IT IS SUPPOSED TO GO, BUT IT DID NOT FIX THE ISSUE. THE ENTIRE LATCHING MECHANISM HAS FAILED. MY 2007 YARIS HAD A RECALL FOR THIS SAME ISSUE FOR THE DRIVER SEAT. THE CHASSIS FOR ALL MODEL YEAR YARIS' ARE THE SAME. I BELIEVE THE SEAT MECHANISMS WE'RE CONTINUED TO BE BUILT WITH A KNOWN ISSUE. MY CAR ONLY HAS 63,000 MILES AND THIS SEAT IS BARELY USED.

SEVERAL MONTHS HAVE GONE BY, AND I HAVEN'T RECEIVED A CALL. MY NEAREST DEALERSHIP, BREDEMANN TOYOTA @ (847) 655-1435 HAS NOT CALL ME BACK FOR 3 MONTHS OR SO. I RECEIVED THIS LETTER BACK IN JANUARY, AND IT IS NEARLY JULY. ALMOST HALF A YEAR HAS GONE BY, AND MY SAFETY HAS BEEN AT RISK EVERY SINGLE DAY. I AM ONLY GETTING 20-21 MPG (COMBINED, BUT MOSTLY CITY), WHICH IS WHAT I GOT WITH MY OLD DATSUN THAT I JUST SOLD. THIS IS FALSE ADVERTISING. I CALLED CARMAX AND THEY SAID THOSE NUMBERS DON'T COME FROM THEM, BUT FROM TOYOTA. SO I CALLED TOYOTA AND THEY SAID I WOULD HAVE TO TAKE IT TO A TOYOTA DEALER TO SEE IF SOMETHING WAS DEFECTIVE. BUT AS WE ALL KNOW, DEALERS CHARGE RIDICULOUS PRICES. I WANTED TO TAKE IT TO MY MECHANIC, BUT TOYOTA SAID THAT THEY DON'T RECOGNIZE OUTSIDE MECHANICS. THE MILEAGE HAD SO MUCH TO DO WITH MY PURCHASING THIS CAR THAT THIS REALLY MAKES ME MAD. WHEN I RE-CHECKED THE 2012 YARIS MPG ON CARMAX, IT SHOWED THEM AS MAKING 32/38 MPG (THESE WERE AUTOMATICS, MINE IS MANUAL, BUT I WOULD THINK MANUAL WOULD BE EVEN BETTER MILEAGE THAN AUTOMATIC?). ANYWAY THEY DIDN'T HAVE ANY MANUAL 2012'S LISTED. IF IT WERE OFF A MILE OR 2 OR EVEN 3 THEN OKAY, THERE ARE VARIANCES, BUT TO BE OFF A GOOD 7-8 MILES/GALLON IS WAY TOO MUCH TO NOT SCREAM "FALSE ADVERTISING". I WAS STUCK IN THE INTERSECTION WITH THE CAR STUCK IN PARK I KEPT PRESSING THE BRAKE, TO SHIFT TO NEUTRAL BUT THE CAR WOULDN'T CHANGE GEARS OR EVEN TURN ON AT THIS POINT. I CONTINUED TO SMOKE AND I SMELLED FIRE SO I GRABBED ALL OF MY THINGS AND EXITED THE VEHICLE. IT WAS RAINING AND A GOOD SAMARITAN POPPED THE HOOD AND THE HARD COLD RAIN PUT OUT THE FIRE. On January 9, 2018, Toyota Motor Engineering & Manufacturing (Toyota) notified NHTSA that they are recalling 14,585 model year 2010-2012 Yaris vehicles ever registered in the states of Alaska, Arizona, Arkansas, Colorado, Connecticut, Delaware, District of Columbia, Idaho, Illinois, Indiana, Iowa, Kansas, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, Montana, Nebraska, Nevada, New Hampshire, New Jersey, New Mexico, New York, North Carolina, North Dakota, Ohio, Oklahoma, Oregon, Pennsylvania, Rhode Island, South Dakota, Tennessee, Utah, Vermont, Virginia, Washington, West Virginia, Wisconsin, and Wyoming. As of January 30, 2018, Toyota is additionally recalling 3,347 model year 2010 Pontiac Vibe vehicles located in states listed above. Also included in this recall filing is 98,194 model year 2003-2008 Toyota Corolla Matrix vehicles, located throughout the U.S., that previously received a “like-for-like” interim air bag inflator replacement. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these inflators may explode due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.

Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

Remedy: Toyota will notify owners, and dealers will replace the passenger frontal air bag assemblies with an alternate inflator, free of charge. The recall began on January 29, 2018. The affected vehicles may have been manufactured with a roof headliner that does not provide the proper occupant protection in the event of a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) No. 201, " Occupant Protection in Interior Impact."

Risk: If the vehicles are equipped with a headliner that does not meet the impact requirements, there is an increased risk of occupantl injury in the event of a crash.

Remedy: Toyota will notify owners, and dealers will replace the headliner, free of charge. The recall began on June 22, 2015. Owners may contact Toyota customer service at 1-800-331-4331.

Toyota is recalling certain model year 2012 Yaris vehicles manufactured August 21, 2011, through August 31, 2011. A relay in the Power Steering Control Module of the electronic power steering system could experience a short circuit due to insufficient distance between terminals in the relay and moisture contamination of relay coil windings. A short circuit in the relay could disable power steering assistance.

Risk: If the power steering assistance shuts off, the driver may have difficulty steering the vehicle at low speeds, increasing the risk of a crash.

Remedy: Toyota will notify owners, and dealers will replace the power steering control module. The recall began on July 31, 2013. Owners may contact Toyota at 1-800-331-4331.

The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.

Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

Remedy: Toyota will notify owners. Depending on the vehicle model, dealers will replace the front driver inflator, front passenger airbag inflator, the airbag assembly, or the front passenger airbag sub-assembly and inflator. The recall began November 20, 2019. Owners may contact Toyota customer service at 1-888-270-9371 or Lexus customer service at 1-800-255-3987. Toyota's numbers for this recall are J0A, J0B, and J0C. Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
