The brake light, abs light, and traction control light is on. This was a known issue with Toyota Prius. They extended the warranty but failed to adequately notify user. I believe this is a serious safety issue and should be a recall as it affects the adequate stopping of a car, not just an extended warranty.

I am filing a formal safety complaint regarding a catastrophic brake component failure on my 2013 Toyota Prius, occurring at less than 83,000 miles. The vehicle’s Brake Master Cylinder/Actuator Assembly has suffered an internal hydraulic pressure leak, causing the electric booster pump to run continuously to prevent a total loss of braking assist. This is a well-documented manufacturing defect that directly compromises passenger safety, drastically increases stopping distances, and threatens a complete loss of power assist while driving. Toyota explicitly acknowledged this inherent design flaw through its Customer Support Program (ZJB). However, they limited coverage to a strict 10-year expiration date from the first date of use. Because my vehicle has exceptionally low mileage and was driven gently, the component took longer to degrade, causing it to fail just outside Toyota's arbitrary time limit. Limiting safety coverage based on time rather than mileage penalizes owners of low-mileage vehicles for an engineered factory defect.

On June 20, 2026 while driving the Toyota Prius I experienced a sudden brake failure. Initially I thought I might have pressed the accelerator instead of the brake. But then pressing the brake I found the car did not stop and the ABS brake, brake lights, and other lights lit up on the dash board. The car had only 61,150 on the odometer. I called on Toyota for their warranty coverage but they declined saying that the vehicle is no longer covered. This brake failure should have been under Recall. AAA auto service has pulled C1391 vehicle code - requiring removing and replacing brake booster pump assembly with master cylinder, etc.

There have been several reports of this matter and a ton of recalls due to the Brake Booster issues with all Prius makes and models from 2010 on. The recall was issued and ended before I purchased this vehicle and never fixed. The brake booster has recently began having trouble in my vehicle. I purchased it in February of 2024, the recalls all show they quit honoring them in 2023.

Year/Make/Model: 2013 Toyota Prius • Component: Brake Booster Assembly with Master Cylinder (Part #47070-47060) Description of the Defect: After approximately 30–40 minutes of driving, my dashboard illuminates with the Brake, ABS, and VSC warning lights simultaneously. The brake pedal becomes inconsistent and requires significantly more effort to stop the vehicle. This is a known internal fluid leak within the brake booster assembly that causes the accumulator pump to run continuously and ultimately reduces braking performance — a direct safety hazard. Why This Requires Federal Attention: This is not an isolated incident. Hundreds of 2013 Prius owners have reported identical symptoms, with associated diagnostic trouble codes C1391, C1252, C1256, and C1253. Toyota acknowledged this defect by issuing Technical Service Bulletin TSB-0089-20 and a Customer Support Program (ZJB) providing limited extended warranty coverage. However, that program was not well publicized . I was never informed as a vehicle owner . And it has now expired, leaving thousands of owners with a dangerous defect and repair bills costing thousands . A safety defect of this magnitude — one that can result in sudden, significant loss of braking ability — should not have an expiration date on its remedy. Toyota has not issued a mandatory recall, and owners continue to unknowingly drive vehicles with compromised brake systems. Requested Action: I respectfully urge NHTSA to: 1.Open a formal investigation into this defect across all affected 2013–2015 Toyota Prius model years 2.Require Toyota to issue a mandatory safety recall 3.Compel Toyota to cover repair costs for all affected owners regardless of mileage or age of vehicle

THE ABS brake system is now making a noise that is directly related to the failure of the brake pump booster and associated parts. I only know about this as I was able to look up other people that have heard a noise coming from it and I was able to directly compare my sound to video recordings of other priuses that make that noise just before the system fully fails. Mine has not yet completely failed and there are no warning lights yet, nor are there any "recalls" out for my VIN number. My car is also outside of the 10 year "extension" that toyota issued for he part, but is well under the 150,000 mile marker that toyota chose. It's at around 113,000 miles currently. I listed the date of the incident as the first date that I began hearing this noise. Please help.

The brake accumulator was replaced by a repair shop (ARC Auto Repair, Pasadena, CA), but the issue has recurred. The symptom is pressure-dependent: brake performance degrades in Eco drive mode as accumulator pressure bleeds down, and partially restores in Power mode when the system is forced to repressurize. This vehicle qualifies for a repair under T-SB-0024-19. However, this TSB expired in 2023 but the defect and responsibility shouldn't. The car didn't qualify for the repair before the TSB expired, but does now. Because the bulletin was issued and Toyota as acknowledged the repair is needed I have asked them to cover the repair as if it was prior to expiration date. Without the pricey repair, the car is unsafe to drive at no fault of the purchaser. The TSB covers the brake booster and brake booster pump and fail. Do lives have to be put at risk for Toyota to honor and repair the very thing they know is faulty? The car has under 115,000 miles and the issue just started in February. The dealership refuses to provide an itemized repair receipt. The ticket just outlines warranty information of the repair and the second are the techs notes provided via text.

While driving my 2013 Toyota Prius (110,691 miles), multiple warning lights suddenly illuminated on the dashboard, including the ABS, Brake, and Stability Control lights. I initially took the vehicle to an independent specialist (DC Line), but the mechanic there suggested I take it directly to Toyota because his staff recognized this as a known manufacturing defect that should be covered by the manufacturer. A Toyota dealership confirmed diagnostic code C1391, which indicates an internal high-pressure leak in the brake actuator and booster assembly. This is a critical safety defect that causes the brake pedal to become soft and unpredictable, significantly increasing stopping distances and disabling the ABS system. This poses a direct threat to my safety and others on the road. Toyota is fully aware of this defect through Customer Support Program ZJB, but they are refusing to cover the repair because of an arbitrary expiration date they set for 2023. I am currently 40,000 miles under the 150,000-mile limit Toyota established for this exact part failure, yet they are using the 2023 calendar date to deny coverage. Furthermore, I was never properly informed of this defect. Toyota claims they sent a single notification card in 2018, which I never received. I am now being forced to pay $3,000 out of pocket for a manufacturing flaw that even independent mechanics recognize as Toyota’s responsibility. It is unacceptable that a manufacturer can avoid responsibility for a dangerous braking failure by relying on a single postcard and an arbitrary 2023 deadline on a vehicle well under the mileage limit. This should be a mandatory safety recall, as the safety of a vehicle's braking system should not depend on a postcard or a calendar date.

I was driving and notice ABS light and other light came on, then noticed that when I tried to brake it felt weak, had to pump brake, then later noticed a leak on brake fluid. I took car to Toyota and they checked it and said brake booster and booster pump needed replacing and they wanted to charge me 3,000 dollars. I could not do the job so I checked around and the auto one in my area informed me that many Toyota Prius’s had a recall on that issue. I made an appointment with Parks Toyota in Deland, Florida to fix problem but when he got there they told me that my car was not on recall for that but was for battery EV fuse but couldn’t do because my engine light was off. I am having such a difficult time with this car. I cannot risk my life and that of others driving with this brake condition if I can’t have the manufacturer or who ever has to to take care of this issue. So far I was able to get a mechanic to do some adjustments so I can drive. I am being very careful and only use it as needed. I stopped looking for a part-time job because I need this fixed first. I was driving on Saturday January 10th, 2026, when out of nowhere all of my dash warning lights came on and the brakes felt spongy and unresponsive. I had to limp home with 20% of my brake capacity which was very scary. Did some research and it was my brake actuator that went out. Dealer quoted me $4,000 to fix! This is a very common issue with Priuses between 2005-2015 and it's a crime that the part has NOT been recalled. Please put out a recall for this part. Toyota has already had a class-action lawsuit so precedent has been set. I have no idea how I'm going to pay $4,000 to fix this, I'm recently unemployed. Please hold Toyota liable!

Crack in the Engine. I had been following the recommended Maintenance schedule, but I suddenly had issues where the engine would stall/stutter on activation (switching from electric to motor operation). The check engine light came on, and I immediately took it to the dealer who replaced the battery and spark plugs saying that was the issue. That solution did not even last a month, as I had sometimes severe shaking in the engine along with the stutter. I again brought it to the dealership who told me that the actual problem was a cracked head gasket and that the vehicle was "mechanically totaled". I was lucky that I live close to the dealership and worry what would have happened if I had to drive on it further.

On [XXX], we were driving from Coalinga, CA to Phoenix, AZ for Thanksgiving. As we began to descend into Los Angeles on [XXX], three warning indicators suddenly went on: the ABS warning light, the traction control light and the brake warning light. The brakes still worked but the brake assist was weakened. We pulled over as soon as possible at the nearest AutoZone. The employee performed a standard handheld OBD-II scan of the vehicle and did not detect any issues. We continued on our way through Los Angeles and to San Bernadino where we were spending the night. At one point the indicator lights went off for a few minutes but then came back on. We spent the night in San Bernadino, and the next day (11/26/2025), we took the car into the San Bernadino Toyota Dealership as we felt it was unsafe to keep driving. The dealer conducted a thorough full brake and hybrid system diagnostic and confirmed that the failure was inside the brake booster system. The repair estimate states: "Brake booster assembly with master cylinder and brake booster pump assembly have an internal leak." Thus, Toyota's own technician identified an internal hydraulic leak within the booster system, affecting the booster assembly, master cylinder, and booster pump assembly. The total repair estimate is $3,959.99. This is a major safety defect affecting sealed hydraulic components that are not part of routine wear-and-tear. There were 4 people in the car. Fortunately, the brake failure did not result in an accident, but it could have if we had kept driving on to our destination. We contacted the Toyota Brand Engagement Center to ask for Goodwill Assistance, but they were unable to help us. In addition to the repair costs, we have incurred rental car costs. My daughter was driving the 2013 Toyota Prius on the interstate in Chattanooga, TN. The brakes did not feel right and the car was very difficult to stop. (It did not brake at a normal distance) My daughter was able to get someone safely, and she had the car towed to the dealer. My daughter's life was endangered by this brake booster failure, but there is no recall listed on the vehicle. Also, when I Googled the issue, I found that it is a well-known and common problem in this year Prius. The brake booster failure is due to a Toyota design flaw, and Toyota is apparently being sued for it. Why is this issue not listed as a recall? I would not have allowed my daughter to drive around in a car for which the brakes are known to fail. How many accidents and deaths will be caused by this problem before Toyota stands behind their product and recalls affected vehicles? I had to pay $3200 for this brake repair, even though it is caused by Toyota's design flaw and endangered my family's life. My family owns four Toyotas, and I do think they make the most reliable vehicles on the road. I cannot understand why there is not a recall, and they are not standing behind their product.

There is a constant beeping noise in the car , which is a brake warning. I looked up the problem and just like many other Prius owners, there is a brake booster and brake accumulator issue and Toyota is not owning up to this problem. Many Toyota Prius owners are complaining of this loss of brakes issue. How long until NHTSA forces Toyota to do a proper recall about this very dangerous loss of brakes issue ??? They recalled a few vehicles, but the recall does not seem to cover many Prius vehicle owners that are complaining. Mine has 140 000 miles now, but this part is failing even on cars with 40 000 miles. Toyota knows very well that they have a big serious issue costing upwards of $4000 to fix. They are just refusing to recall this issue. NHTSA has to address this issue please. They are many complaints about this loss of brakes issue.

On this 2013 Toyota Prius 2, the brake actuator is starting to make more and more rhythmic noise. A mechanic just alerted me that the seal is leaking, requiring a costly repair - to which he said to check for a recall as he has seen many Priuses with the same issue. While no Toyota recall is present, I can see that the NHTSA is already investigating the issue for a potential recall. As this is a live example in the Los Angeles, CA area, I am happy to demonstrate this issue to a NHTSA office, if necessary and/or available.

My Prius has a brake actuator defect that is common among 2013 Priuses. Supposedly Toyota mailed a notification of this problem, but did not do a recall. The notification offered to fix the problem, but you had to know that your Prius had the problem. I did not know. Once I understood that my Prius had this problem, the Toyota repair window had closed. The Prius is dangerous to drive. I approached Toyota and the dealer that sold me the car, and Toyota refused to authorize the repair and so the dealership refused to fix at their expense. I spent thousands on unnecessary brake replacement because the problem was never the brakes but the brake actuator, which is a KNOWN PROBLEM! Yes, the problem is able to be discerned upon request and I have taken video and you can hear the noise. This problem puts the driver, passengers and other vehicles at risk. There are no warnings or lights showing this problem, but the noise is loud, grinding and obvious.

The hybrid braking systems on Toyota Prius' manufactured from 2010-2015 had a serious failure of the hybrid braking system that had additional long term warranties to repair, but never recalled in order to save the money from a total recall. The Sloan Toyota dealership in Glenside knew before I purchased the 2013 Toyota Prius I purchased that a serious brake system failure was very probable with my car but failed to tell me. If they did, i would not have purchased the car. I had the 5000 mile oil change in early August and three days later after the service and a clear 25 point inspection, and the system failed. I want full reimbursement or just pay for the repair.

The contact exited the vehicle and performed an inspection of the vehicle. While inspecting the vehicle, the contact became aware that the passenger’s side rear wheel was touching the frame of the wheel well. Additionally, the wheel was leaning inward. The dealer was made aware of the failure but provided no assistance. The vehicle was towed to an independent mechanic, where it was diagnosed with a fractured rear wheel assembly. The contact was informed that the rear wheel assembly needed to be replaced. The vehicle was being repaired. In addition, the contact was informed that the failure was a known failure. The contact was informed by the independent mechanic that two similar vehicles had been repaired with similar failures, within the week. The manufacturer was not made aware of the failure. The failure mileage was approximately 179,000.

when test driving the vehicle I asked about this buzzing/ whirring noise while sitting in the running but parked car. I was told these cars are hybrids, the electrical components make all sorts of whirring and humming noises and not to be concerned. Over the years I heard this noise, increasingly more and more, but I never knew it was something to pay attention to. Fast forward to July 2025. I noticed the car is inching forward while I’ve been stopped for a while at a red light almost like the brakes don’t want to hold. This has been happening so often that I have began to put the car in park while waiting for lights to turn green because it gets painful having to push the brake so far down. I take my car to a mechanic, explain all of the noises and the sensations and two mechanics Drive it but say it feels fine and there are no recalls. They say my front brake pads are low and that must be what I’m feeling. After replacing the brake pads nothing had changed. Now there is also a squeak while moving at a slow speed in stop and go traffic. 1 week later i bring it up while getting an oil change and ask if they know what it could be. They said brakes seemed fine. A couple weeks go by and yesterday while I was driving down the highway in heavy school traffic out of nowhere my car makes a clunk noise and all of these lights pop up on the dash. I roll the windows down and scream for my children to hold on because something was wrong. Luckily we were able to get to safety but I definitely had reduced breaking capabilities. I immediately take the car in and and find out my brake booster and accumulator has failed. service tech brings up the consumer program but my car is a 2013 and it is not eligible to be repaired bc it’s 10+ yrs. Even though Toyota admits to internal malfunctions of the Brake Booster Assembly, and my car is only at 76,000 mi it’s not being fixed. This is negligence on Toyotas part. Not fixing these faulty parts is endangering lives.

I was driving normally, about 30 mph in a residential area and the brakes all of sudden became soft. It was very scary and I didn’t know if it was safe to drive the vehicle. The brake, ABS, and traction control sign all came on at the same time. I scanned the car and it was a C1391 error code. The Toyota dealership said it is not covered under any recalls. I called Toyota and asked about the ZJB program that covered the repair but they said it expires. Toyota has a repair program for the defective part but no longer covers it. This issue is a known manufacturing issue and it’s dangerous. Toyota should cover it free of charge.

When I’ve hit a pothole or hit the breaks to hard by ABS, and traction control lights would come on and then turn off. The other day they all turned on and stayed on with the break light on as well. Breaks all seemed to work fine, just a little spongy. I brought it in to a shop and they estimated 3k to repair Remove, replace, calibrate, and bleed ABS accumulator assembly and brake master cylinder assembly. Test drive-New OEM Toyota Parts. These parts were recalled but only up to 10 years or 150k miles. I’m at 164k so ineligible for the recall but if this part completely failed while I was driving I could have been killed or caused someone serious harm.

Around May 2025, the brake booster and pump in the car stopped working. There was no clear warning lamp or indicator prior to it failing. While driving on the highway, the part stopped working and significantly changed the car's braking power. Rather than smoothly braking as it did before, the car's brakes have significantly less braking power. The defective part also affects the car's ABS system, making the car unsafe if a sudden stop were required as the brakes will lock up and the car will lose grip on the road and start skidding. Significantly longer braking distances is needed and it's not clear the car would be able to safely stop at a short distance in the event of an emergency. It's a miracle we didn't get into any accidents when the part suddenly failed. There is a significant risk that failure of this part will be the cause of a car accident when the car suddenly no longer brakes how the driver is used to. Toyota has previously offered an extended warranty on the part, but the part did not fail until shortly after the warranty expired. The car has been within my family since it was originally put into service and none of the owners were ever notified that the part could be replaced or even fail in such a manner. The problem has been confirmed by a dealer but they have only offered to fix it at a significant cost. The vehicle does not feel safe to drive. Warning lights only appeared after the part failed but they weren't particularly helpful in diagnosing the problem in the moment because 4 different lights turned on all at once.

Toyota recalled some cars and then decided to not own up to this issue which they know is affecting many pruis owners and costs between $3000 and $4500 for dealers to fix, therefore Toyota has just decided to refuse to do this repair and leave their customers high and dry with a known major safety brake issue affecting these most prius vehicles. Toyota needs to be forced to recall these cars and own up to this issue. Look at how many complaints Toyota has had on this brake booster issue and yet they are refusing to recall or even offer help to resolve this major safety issue. NHTSA needs to put pressure on Toyota to recall this brake issue affecting many prius owners as shown by the volume of complaints.

Toyota is recalling certain model year 2010 through 2014 Prius vehicles. In the affected vehicles, the Intelligent Power Module (IPM) inside the inverter module (a component of the hybrid system) contains transistors that may become damaged from high operating temperatures. If this occurs, various warning lamps will be illuminated on the instrument panel and the vehicle will have reduced power allowing it to only drive a short distance.

Risk: The vehicle may enter a fail-safe/limp-home mode that limits the drivability of the vehicle. The hybrid system could also shut down completely resulting in a vehicle stall, increasing the risk of a crash.

Remedy: Toyota will notify owners and dealers will update the software for both the motor/generator control electronic control unit (ECU) and the hybrid control ECU, free of charge. If an owner experiences a failure of the inverter before the vehicle receives updated software, the dealer will repair or replace the inverter assembly with a new one at no charge. The recall began on March 21, 2014. Owners may contact Toyota at 1-800-331-4331.

Brake Actuator Valve Wear

The Office of Defects Investigation (ODI) has received a petition dated September 19, 2023 from the owner of a 2012 Toyota Prius V requesting that ODI reevaluate its decision to deny DP19-004. In the letter, the petitioner reported experiencing an unexpected and complete loss of brake power while driving, accompanied by multiple dashboard warning lights. This petition is based on the petitioner's direct experience and assessment that ODI closed DP19-004 because its prompting petition was withdrawn by that petitioner. ODI will review the work conducted during DP19-004 and a review of any additional related complaints and crashes via a new defect petition The petition and related materials can be reviewed at NHTSA.gov under the following ODI number:11553736.

Pedestrian alert sounds

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

Brake Actuator Valve Wear

On September 19, 2019, the Office of Defects Investigation (ODI) received a defect petition from Mr. Roger Hogan requesting that the Agency investigate and recall certain Toyota Prius, Prius PHV, Camry Hybrid and Avalon Hybrid vehicles for a brake actuator valve wear condition covered under Toyota Customer Support Programs ZJB and ZKK (subject CSP?s). In support of his request, the petitioner identified 117 NHTSA complaints allegedly related to the subject CSP condition, including 60 reporting crashes. On September 30, 2019, ODI opened Defect Petition DP19-004 to evaluate the petitioner?s request for an investigation. In August 2020, the petitioner notified the Agency that he was withdrawing his petition. Because the petitioner has withdrawn the petition, we are denying the petition as moot and no further analysis of this petition is necessary.NHTSA is authorized to issue an order requiring notification and remedy of a defect if the Agency?s investigation shows a defect in the design, construction, or performance of a motor vehicle that presents an unreasonable risk to safety. 49 U.S.C. ?? 30102(a)(9), 30118. Because the petitioner has with withdrawn the petition, the petition is denied as moot. This action does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency notes that it is not required to receive a defect petition prior to opening a defect investigation and will take further action if warranted by future circumstances.
