My 2012 Toyota Prius is experiencing brake actuator pump failure, presenting diagnostic trouble codes C1253 (brake actuator pump motor stuck/locked) and C1256 (brake actuator pump motor overcurrent). These codes trigger ABS, brake, and VSC warning lights and disable ABS and stability control. Toyota previously acknowledged this exact defect under Customer Support Program ZJB, which covered 2010-2015 Prius vehicles for brake booster and brake booster pump assembly failures triggered by these specific codes. I am the original owner of this vehicle, purchased new in 2012. I never received notification of CSP ZJB despite being the registered owner at the time notifications were distributed in 2019-2021. My current mileage is under 150,000 miles, within the secondary coverage mileage limit established by the program. Toyota has declined to provide goodwill coverage citing program expiration. This is a known safety defect that reduces braking effectiveness and disables ABS — it should have been addressed through a mandatory recall rather than a voluntary program with an expiration date that left original owners without recourse due to failed notification.

The brake booster and brake booster pump assembly (also referred to as the Accumulater or Accentuater) have failed in my vehicle the same as Toyota's known defect that was mentioned in their Customer Support Program 20TE07 (part numbers can be found within the filing of their CSP 20TE07). I am reporting that this issue appears to have affected more than their initial estimate of ~118,960 states impacted vehicles.l because Toyota is declaring my vehicle is outside of their CSP 20TE07 although their defect endangers my safety (and the safety of others) and attempts to put their defective vehicle brake parts as my financial burden. This issue should have been/should be a larger recall. Thank you.

2012 Prius with 174,344 miles in great condition, with regularly scheduled oil changes. ABS/Break Actuator failure on freeway with no warning other than a beep a few seconds before the breaks disappeared completely. Break pads changed 4 months ago and new wheels installed as well. This is a known problem with Toyota under the TSB002419 where Break booster actuator was put on a customer service program to be fixed by the dealerships, but has since expired since 2022. The cars are still known to have their breaks go out due to this malfunction but it is now not covered by Toyota and cost $4K-$5K to fix which is more than the market value of the car today. This needs to be fixed and regulated.

The vehicle produces a frequent buzzing noise from the brake system, especially at low speeds or when stopped. The noise occurs repeatedly when pressing the brake pedal, indicating the system is struggling to maintain hydraulic pressure. This condition raises serious safety concerns because it may reduce braking performance and increase stopping distance. Brake system failures present a significant risk to vehicle occupants and others on the road. After researching the issue, I learned this appears to be a common failure in Toyota Prius vehicles of similar model years, and Toyota previously issued extended warranty coverage for the brake booster/actuator assemblies. However, many vehicles now exceed the coverage limits despite the safety nature of the defect. I am submitting this complaint because a failing brake system is a critical safety hazard, and this issue appears widespread.

Vehicle check engine code C1391 (Antilock Brake System - abnormal leak in accumulator) came up on 2/5/2026. Recommended repair is to replace the Brake Booster Pump and Assembly, Brake Booster - over $1000 in parts alone. This WAS covered by an Extended Warranty by Toyota (10 years / 150K miles). I only have 120K miles at this time but am past the 10 year mark. However, it seems that this part will fail in every Prius given time and encourage NHTSA to monitor these failures and hold Toyota accountable to address post the 10 year / 150K mile mark.

Vehicle: 2012 Toyota Prius Mileage: Approximately 217,000 miles While driving normally in traffic, the vehicle experienced a sudden and unexpected brake system failure. Multiple warning lights illuminated simultaneously, including the red BRAKE warning light, ABS warning light, traction control light, and warning symbol (!). Shortly after, the vehicle began emitting an audible warning/beeping, and there was a sudden loss of power brake assist. The brake pedal behavior changed abruptly: the pedal became very soft initially, then became suddenly hard halfway through travel, This failure occurred while approaching another vehicle in traffic, and I narrowly avoided rear-ending the vehicle in front of me. The unexpected loss of brake assist created an immediate and dangerous situation, as the vehicle did not decelerate as expected when the brake pedal was applied. Due to the seriousness of the brake failure and safety risk, the vehicle was removed from service and towed. This incident represents a critical safety defect, as sudden loss of braking assist while driving poses a significant risk to vehicle occupants and other road users. I am reporting this incident to document the hazard and request investigation into the reliability and safety of the brake system design. I am requesting you to please issue a recall and reimburse me , as these parts are very expensive , it affected my financial situation, Thanks

My 2012 Toyota Prius plug in hybrid had just passed state inspections and was even looked at by a dealership a year ago with no reported issues. I brought up a whirring issue at the dealership when breaking but the dealership said that’s normal for a hybrid. In a matter of minutes while driving, the traction light, brake light, and ABS light came on as well as a groaning, whirring sound when breaking. Over time, the breaks failed and I could not stop the car, almost causing a wreck because I could not stop without putting all my body weight on the break pedal. When taking it to the Toyota dealership for a whole diagnostic, they said it was a break actuator and break booster failure that is known to happen with Prius’s. They quoted me $3000 for a repair and then marked my breaks as yellow not red on their sheet implying I could still drive it despite the dangerous situation of driving without functioning breaks. When looking into it, this seems to be a known and common problem with Prius’s due to a faulty manufactured part. Codes were c1391, wc1256, and c1252. I believe this part should be recalled and Toyota should fix these faulty parts, especially considering they inspected my car and did not mention it as an issue despite it being a known issue. I got a second opinion from Mavis Tires and they concurred with the diagnosis but could not fix it.

While driving, the vehicle experienced a sudden brake system malfunction without prior warning or gradual degradation. The vehicle was being operated under normal driving, road, and weather conditions. No recent brake service, repairs, or aftermarket modifications had been performed prior to the failure. Multiple warning lights illuminated on the dashboard, including the ABS warning light, brake warning light, and traction control warning light. Shortly after the warning lights appeared, the vehicle began emitting continuous warning beeps. Audible buzzing or pumping noises were also present from the brake system during the malfunction. During this event, brake performance was reduced and braking felt inconsistent. The brake pedal became hard at times and spongy at other times, requiring increased effort to slow the vehicle and resulting in unpredictable braking response. This created a safety concern while the vehicle was in motion. The driver was forced to alter driving behavior and exercise extreme caution due to reduced braking confidence and unpredictable pedal feel. The malfunction has occurred intermittently during normal driving. The condition temporarily resolves on its own and later reoccurs without corrective action. The suspected failed component is the brake actuator / ABS actuator assembly. A diagnostic scan retrieved fault codes C1252 (Brake Booster Pump Motor ON Time Abnormally Long), C1253 (Pump Motor Relay Malfunction), and C1256 (Accumulator Low Pressure), which are consistent with brake actuator malfunction. The failure occurred at approximately 154,000 miles. This vehicle model and generation have a documented history of premature brake actuator failures with similar safety-related symptoms reported by other owners. The condition places the driver and others at risk due to unpredictable braking behavior, reduced braking effectiveness, and repeated warning alarms while driving. Warning lamps and audible alerts continue intermittently.

While driving on a city road below the speed limit, braking was compromised leading to loss of traction and failure of the ABS system. During the incident, the brake, ABS, and engine warning lights all turned on and a collision was only avoided due to pumping of the brakes. Further investigation by a mechanic revealed a failure of the brake actuator system. Failure of this system has increased the stopping distance of the vehicle considerably and makes braking unreliable and unpredictable.

2012 Toyota Prius. The brake system malfunctioned and my daughter was almost in a wreck. The ABS, traction control, and brake warning lights on the bashboard all illuminated at the same time. The brake pedal feels soft and is less responsive requiring more effort to stop the vehicle. The brake actuator assembly is making a hissing noise. Ran a ABS test using a scan tool and received a C1391 error code. The Code points to a leak within the brake actuator system. Upon research I have found that this is a very common and costly repair that Toyota has known about for many years. Toyota issued a customer support program related to this issue that ran for 10 years or 150,000 miles from the vehicle's date of first use. This car, being a 2012, is beyond the support program. I contacted Toyota, hoping they would do the right thing and cover the repair. I was told that they would not cover the repair or supply the parts to cover the repair. I told the Rep. for Toyota that I did not feel like it was fair for Toyota to continue to put millions of drivers at risk for a product that they clearly realize for years now is defective and could hurt or kill someone. The code and the braking issue did not appear until after the customer support program ended and therefore we had no warning or way to anticipate this braking issue or the danger to my daughter and anyone else riding with her in the car or on the roadway near her travel routes. I traveled 300 miles, rented a Uhaul car tow trailer and brought the car back home. My mechanic confirmed the code C1391 and the fact that the brake actuator has failed. The mechanic has ordered the parts (the same defective parts) to replace the broken ones. Toyota is still selling the same part as a replacement part clearly knowing that it is not safe and no warning came with the replacement part explaining the potential for a braking issue. Based on internet research this same part is in many 2005 to 2013 Toyota and Lexus

While pulling out of a parallel parking spot going slightly uphill, I braked as I looked around the back of a double parked truck. I saw a car coming, braked hard, and suddenly my car surged forward across the street and hit a parked car. I looked down and saw that my foot was squarely on the brake pedal pressing down as hard as I could. The parked car was unoccupied, and I was uninjured. There were no warning lamps, messages, or other symptoms of the problem. I turned my car into the dealer for repairs and a fix to the sudden unexpected acceleration. They could offer no fix until a Toyota representative examined the car. On August 14, by an emailed letter, Toyota claimed it found no defects and it must be the result of driver error, specifically, my mistakenly hitting the accelerator. This is gaslighting denialism. (a) The EDR data sent to me does not indicate that I hit the accelerator unless that data is in the page of hexadecimal numbers. (b) Toyota's claim that I hit the breaks at "-0.2 seconds prior to the collision" is physically impossible. No one can move their foot from the accelerator to the brake that fast. (c) Toyota's claim is contrary to what I experienced and observed.

Brake Actuator and ABS Accumalator failed. It is a common failure reported by multiple owners on these cars. I was on the high way going 70 when it failed and lost any brake boost I had putting me in a very dangerous situation where I narrowly avoided an accident. Upon scanning the car's codes I got code C1391 which I looked up and saw a bunch of other people who were also struggling with this issue.

I am writing to express a complaint regarding the issues I have experienced with the brake system of my Toyota Prius C, 2012 model. For the past several months, I have noticed that the brakes exhibit strange noises, soft pedal sensation, reduced braking efficiency, were driving and the ABS, traction, brake, and that other light turned on and the brakes became slow these issues have been observed on multiple occasions and pose a safety risk to myself, other drivers, and pedestrians. I have taken the vehicle for inspection, but the problems persist or have not been definitively resolved. I believe these faults may be related to defects in the braking system, and therefore I request a thorough inspection and necessary repairs at no cost, considering that the manufacturer could be responsible for manufacturing defects.

The brakes on this Prius started to make weird noises. Because it is a hybrid, I wasn't sure if it was normal. After searching online, it is a known issue. The car has just under 76,000 miles on it. It might be worth $7000 on a good day. Toyota wants to replace two brake parts, numbered 47050-52020 and 04006-74152 to a tune of $2700. According to a Toyota bulletin, this job should take 2.3 hours. For that amount of work, my local Toyota shop wants almost $2000 in labor. My car was seen by the local shop (being on an island, it is literally the only Toyota shop that this car can be seen) when the "customer support program" 21TE01 was in place and they did nothing. Now, the time window has disappeared, even though we only have half of the maximum number of miles to get the repair. Toyota should be ashamed of themselves.

I was driving down the interstate with my cruise control on when all of a sudden the cruise control disabled and warning lights came up on my dash board. It still drove fine for several weeks. On Friday December 6th, I was going down the road and had to press the brake slightly for a car turning. When I pressed the brake, the car went into a spin. It spun a couple of rotations. Luckily I was on a wide part of the highway and no oncoming cars were approaching. Toyota had a recall for the same code and parts for the 2010 Prius but did not include the 2012 Prius. They just had an alert which I did not receive because the notice went to the former owner. However, I did take my car in for oil change and diagnostic but they did not tell me that there had been a problem with these parts. They want almost $4,000 to repair it. Why would they not include ALL vehicles with this same part. If it fail on one, it will fail on another. I think Toyota should make them all good. Please make them issue a recall for all of these parts. I took my car to a mechanic but because of special tools needed, you have to take your car to the dealership for repairs. When I called the dealership, I was told I needed a break booster pump and a brake master cylinder for a total of $3, which is ridiculous on a $7,000 car.

The ABS system put us at risk by failing prematurely, and traction control and ABS functions seemingly stopped working while driving on the road. 3. Yes, dealership confirmed the issue with a full diagnostic, and the part has a customer support program bulletin/TSB out for this specific issue. 4. inspected by manufacturer dealership service center. 5. Lights such as the parking brake, ABS, and (!) lit up while driving and the failure occurred at the same time.

While my daughter was driving brake pedal faded and rear brakes locked up causing her to lose initial control of car . Steering felt tight . Many warning lights , check hybrid system , brake lights , steering warning lights , along with abs lights came on . Took car to Toyota dealer to diagnose it needs brake booster and master cylinder module . Service consultant said known issue with TSB but it had expired . Today 05/25/2024 noticed a slight camber on the right rear wheel. Jacked up the car and removed the right rear to take a closer look and realized the rear axle/subframe was splitting in half along the weld point. If I hadn't caught this today and my daughter was driving it I'm confident she or others would have been killed or severely injured since most of her driving is highway related. The part that is blowing my mind is the vehicle was at the dealership twice in the last month and no one noticed when on a lift. The car is now permanently on jack stands till it can be repaired. I'm not sure it will even hold together to get onto a tow truck. I will not attempt driving do to safety specially others. I witnessed a tire go through a vehicle resulting in the death of someone.

I have had a 2012 Prius since 2016. The entire time I've had it, it occasionally "hydroplanes" but without any water. It happens in wet or dry (but mostly dry) conditions. Usually at slower speeds so, though it makes me very uncomfortable, it doesn't happen with any regularity. I've taken it multiple times to my mechanic who always says "Prius' just do that", which is not reassuring. I know there was a recall and I was told my car did not fall under that umbrella. On Wednesday of this week, 10/25/23, I finally had an accident because of this. The brake lockup had been happening more frequently in the past week and in this instance I saw the car in front of me suddenly slow down - I had about 2/3 of a cars distance between me and the car ahead. I hit the brakes and nothing happened. I was pushing down on them long enough to think, "Ok, this is it. They aren't responding at all and I'm going to hit this car in front of me." I did and it crushed the passenger side of the car. Luckily I wasn't hurt. There were no warnings from the car at all that this was going to happen, as there never are. I took it in for possible repair today (the mechanic is not sure but thinks it may be a total loss). This car did everything they describe in the recall cases and I want to know if there's anyway to be repaid from the company for this as there was nothing I could do to stop this accident? Why wasn't my car included in the recall?

The contact owns a 2012 Toyota Prius Plug-In Hybrid. The contact stated while driving approximately 70 MPH, the stabili-trak, ABS, and a several unknown warning lights were illuminated. The contact stated there were also audible alarms being sounded. The contact depressed the brake pedal and the brake pedal failed to respond. The contact stated that she was able to coast the vehicle off the highway and shifted into park(P) and turned off the vehicle. The contact turned off and turned on the vehicle and attempted to depress the brake pedal; however, the brake pedal was abnormally firm. The contact had the vehicle towed to a local dealer, where it was diagnosed and determined that the brake booster pump assembly needed to be replaced. The vehicle had not been repaired. The manufacturer had been informed of the failure. The failure mileage was approximately 130,000.

Last week my 2012 Toyota Prius-C had four lights come on (ABS, Brake, Swerve, and one other). I took it to my local mechanic who informed me that it was my master cylinder brake that needed to be replaced and would cost me $3,300. This issue started roughly 10-12 months ago, when I heard my brakes making a noise that sounded very unusual, almost like a hydraulic issue. I learned that there was a Toyota initiated CSP. Toyota realized there was an issue with the 2012-2014 brake system that affected over 100,000 vehicles. Those affected by CSP's are supposedly notified by mail. Needless to say, I never received such notice. This aforementioned CSP expired in November 2022 that my car would have qualified for, had the problem been diagnosed prior. However, when I brought my car in on August 8th, 2022 for my hybrid battery, a diagnostic was done on my brake system with no indication that my brake system was in trouble. I really can't fault the mechanics at the local Toyota, I took it to a second brake specialist a few months after and they too could not identify the problem. The problem was so bad, Toyota discontinued the brake system. Toyota said it would need to be broken 100% with an error message to qualify prior to the November 2022 deadline. After being facetious and saying I should have broken the part myself to qualify, she said that is what I would have needed to qualify.

Without prior warning brake pedal goes to floor before minimally engaging braking system at a very reduced rate causing me to swerve and narrowly avoid collisions on interstate. There is no pressure feeling in the brake pedal although brake fluid level is normal and inspection of brake pads indicated significant pad available. No DTC light is coming on and OBD scan tool indicates no brake error. Brakes are unable to stop car to avoid collision, so car is dangerous and undrivable.

Brakes failed while I was slowing down for a speed trap; if I had been stopping for traffic I would have been in somebody's back seat. There was already a class action lawsuit about this that Toyota settled out of court. We were supposed to receive notice about the extended warranty related to this but did not; additionally the dealership lied to us on two different occasions about our brakes being fine after they started behaving differently. If the dealership hadn't lied in both 2020, and 2022, my life wouldn't have been in jeopardy, and we wouldn't be paying thousands of dollars to fix a defective part. Why are you putting so many people's lives at risk by not forcing Toyota to issue a recall? All their hybrid braking systems over a decade of models have this issue. The hybrid specialist mechanic we ended up with said he sees several cars a week with this issue.

My letter to Toyota: I'm writing this in regard to an inverter issue my 2012 Prius C recently experienced. While driving last month my car completely shut down, no warning and thankfully cars avoided hitting me and I was able to get from the middle lane to be able to coast to a side street in which I had to be towed. It turned out it was an inverter issue which had to be replaced. Granted my vehicle has less than 135k even though it is a 2012. I have done research and found models of Prius had the same exact issue(s) which was part of a warranty enhancement and I truly think my vehicle should be included. The brake pressure pump comes on every 2 seconds. The warning lights on the dash are on for brakes and abs. The car still stops but it reacts slower to pedal pressure. Dealer says it needs master cylinder and actuator pump for 3000$ at least. It is available for inspection, has 155,000 miles now, but pump has been hyperactive for about 12 mo. May be a hazard now, may fail completely ,I don't know. But too expensive to me to fix. Has only been inspected by autonation Toyota, not police etc. Pump was overactive for a year but right after anautomatic car wash the lights came on an the brake pedal got mushy. Then I called dealer. Apparently a common problem that was a recall for 2010 and silent recall after for later model gen 3s.

I was driving on the expressway and experienced a sudden loss of power. The internal combustion engine suddenly stopped and did not deliver any power to the wheels. I immediately received a message on the system that said 'Check Hybrid System Stop Vehicle in a Safe Place'. The electric motor still continued to provide some power with the available battery charge. I was able to make it to the shoulder of the road but it was challenging to get to the shoulder with no power or warning and high speed traffic. My wife and I were in the vehicle. The vehicle had no engine or error codes until the failure on the expressway. The vehicle has the code P0A1A and P0A94. This is consistent with the error codes in the NHTSA 19V491000 recall and Toyota Safety Recalls E0E and F0R.

I bought a first generation Toyota Prius PHEV in 2012. Apparently a Customer Support Program notice was sent out two years ago which I never received. For the last year or so my Prius has made a hissing sound when I stepped on the brakes unless I push extra hard on the pedal. Didn’t think anything of it until last month several lights went off. I took it to Big O Tires and they added brake fluid and the lights went away. Next month it went off again, I took it back and they suggested taking it to a dealer. I take it to Tustin Toyota they diagnose it as a Brake Booster and Pump problem. My issue is that I never received a mailer or email like I did for previous recalls. I did transfer ownership to my wife a while back, so I don’t know if that DMV administrative change impacted the mailer. It is a safety concern as my wife drives our two kids and the brake had signs but we did not take it in to the dealer until the failure lights lit up 11 months after the expiration. Is there a chance that a Customer Support Program becomes a Recall if NHTSA receives enough complaints?

a warning light came in on my dashboard brakes regarding the Brake pump accumulator. It feels like my brakes are not working right, not stopping like they always did.I was scared I almost hit someone in front of me at a red light because after applying the appropriate notes due on the brake pedal I noticed a delay in the car stopping. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2014 Toyota Prius and 2012-2014 Toyota Prius V vehicles. Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.
Risk: If the hybrid system shuts down unexpectedly, it can increase the risk of a crash.
Remedy: Toyota will notify owners, and dealers will update the software for the motor/generator control electronic control unit (ECU) and the hybrid control ECU as necessary, free of charge. The recall began January 22, 2019. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is J0V. Note: The software updates in this recall are needed even if the vehicle had the software previously updated under recalls 14V-053 or 15V-449.

Southeast Toyota Distributors, LLC (SET) is recalling certain 2005-2011 4Runner, Highlander, Sequoia, and Sienna, 2005-2010 Avalon and Tundra, 2006-2008 Camry Solara, 2006-2010 Highlander hybrid, 2006-2011 Rav4 and Tacoma, 2007-2010 FJ Cruiser, 2007-2011 Camry hybrid, 2009-2010 Venza, 2010-2012 Prius, 1988-1990 and 2005-2010 Camry, and 1989 and 2009-2010 Corolla vehicles equipped with aftermarket accessory seat heaters with a copper strand heating element. The electrical wiring in the seat heaters may be damaged when the seat cushion is compressed.
Risk: If damaged, the copper strand heating element may short circuit, increasing the risk of a fire.
Remedy: SET will notify owners, and dealers will disconnect the seat heaters, free of charge, and refund the purchase price of the seat heater accessory. The recall began on July 14, 2016. Owners may contact SET customer service at 1-866-405-4226. SET number for this recall is SET16B.

Toyota is recalling certain model year 2010 through 2014 Prius vehicles. In the affected vehicles, the Intelligent Power Module (IPM) inside the inverter module (a component of the hybrid system) contains transistors that may become damaged from high operating temperatures. If this occurs, various warning lamps will be illuminated on the instrument panel and the vehicle will have reduced power allowing it to only drive a short distance.
Risk: The vehicle may enter a fail-safe/limp-home mode that limits the drivability of the vehicle. The hybrid system could also shut down completely resulting in a vehicle stall, increasing the risk of a crash.
Remedy: Toyota will notify owners and dealers will update the software for both the motor/generator control electronic control unit (ECU) and the hybrid control ECU, free of charge. If an owner experiences a failure of the inverter before the vehicle receives updated software, the dealer will repair or replace the inverter assembly with a new one at no charge. The recall began on March 21, 2014. Owners may contact Toyota at 1-800-331-4331.

These vehicles were sold with labels that were outside the allowable one percent of accuracy of actual weight added. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 110, "Tire Selection and Rims."
Risk: An inaccurate label could lead to owners overloading their vehicles and tires. An overloaded vehicle can result in a tire failure which may result in a vehicle crash, personal injury, or property damage.
Remedy: Southeast Toyota will notify owners and provide a corrected label with instructions concerning its installation. Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.
Risk: If the front passenger seat occupant sensing system is out of calibration, the front passenger airbags may not deploy or they may deploy inappropriately for the passenger's size and position. This could increase the risk of personal injury during the event of a vehicle crash necessitating airbag deployment.
Remedy: Southeast Toyota will notify owners, and dealers will test the sensitivity of the occupant detection sensors, and recalibrate them as necessary. The recall began on March 21, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.

A weld in the curtain shield air bag inflator may crack causing one or both portions of the inflator to eject into the passenger cabin.
Risk: If either portion of the curtain shield air bag inflator ejects into the passenger cabin, there is an increased risk of injury to vehicle occupants.
Remedy: Toyota will notify owners, and dealers will install retention brackets to the curtain shield air bags, free of charge. The recall began on October 17, 2016. Owners may contact Toyota Customer Service Department at 1-800-331-4331. Toyota's number for this recall is Toyota G0U, and Lexus GLJ.

The Office of Defects Investigation (ODI) has received a petition dated September 19, 2023 from the owner of a 2012 Toyota Prius V requesting that ODI reevaluate its decision to deny DP19-004. In the letter, the petitioner reported experiencing an unexpected and complete loss of brake power while driving, accompanied by multiple dashboard warning lights. This petition is based on the petitioner's direct experience and assessment that ODI closed DP19-004 because its prompting petition was withdrawn by that petitioner. ODI will review the work conducted during DP19-004 and a review of any additional related complaints and crashes via a new defect petition The petition and related materials can be reviewed at NHTSA.gov under the following ODI number:11553736.
NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
On September 19, 2019, the Office of Defects Investigation (ODI) received a defect petition from Mr. Roger Hogan requesting that the Agency investigate and recall certain Toyota Prius, Prius PHV, Camry Hybrid and Avalon Hybrid vehicles for a brake actuator valve wear condition covered under Toyota Customer Support Programs ZJB and ZKK (subject CSP?s). In support of his request, the petitioner identified 117 NHTSA complaints allegedly related to the subject CSP condition, including 60 reporting crashes. On September 30, 2019, ODI opened Defect Petition DP19-004 to evaluate the petitioner?s request for an investigation. In August 2020, the petitioner notified the Agency that he was withdrawing his petition. Because the petitioner has withdrawn the petition, we are denying the petition as moot and no further analysis of this petition is necessary.NHTSA is authorized to issue an order requiring notification and remedy of a defect if the Agency?s investigation shows a defect in the design, construction, or performance of a motor vehicle that presents an unreasonable risk to safety. 49 U.S.C. ?? 30102(a)(9), 30118. Because the petitioner has with withdrawn the petition, the petition is denied as moot. This action does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency notes that it is not required to receive a defect petition prior to opening a defect investigation and will take further action if warranted by future circumstances.
