The contact was driving a company-owned 2017 Toyota Prius C. The contact stated that while driving at 65 MPH, another driver merging onto the highway attempted to cross multiple lanes of traffic, misjudged, and collided with the front passenger-side fender of the contact’s vehicle. As a result, the contact’s vehicle struck the left shoulder barrier, became airborne, and overturned three times before coming to rest in the middle of the roadway. Despite the severity of the crash, the air bags did not deploy. No warning lights were illuminated. The vehicle was towed to a storage facility and deemed a total loss by the employer’s insurance provider. A police report was filed, and injuries were reported. The contact was transported by EMTs to a local hospital and treated for multiple nondescript injuries, including a damaged right rotator cuff and body pain. The contact also underwent chest and torso X-rays due to concerns of possible internal bleeding. The manufacturer was not notified of the failure. The failure mileage was unknown.

TL* THE CONTACT OWNED A 2017 TOYOTA PRIUS C. THE CONTACT STATED WHILE DRIVING APPROXIMATELY 40 MPH, THE VEHICLE INADVERTENTLY ACCELERATED. THE CONTACT STATED WHILE THE BRAKE PEDAL WAS BEING DEPRESSED THE VEHICLE FAILED TO STOP. THE CONTACT THEN SWERVED TO THE RIGHT AND HIT A CURB AS THE VEHICLE WENT AIR BOURNE. THE FRONT OF THE VEHICLE CRASHED INTO A TREE AND SOME BRUSHES. THE CONTACT STATED THAT ALL FRONT, SIDE, AND REAR AIR BAGS DEPLOYED. THE CONTACT SUSTAINED SCRATCHES AND CUTS TO THE KNEES AND NECK INJURIES AS SHE WAS TRANSPORTED TO THE HOSPITAL BY AMBULANCE. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO A TOW LOT WHERE IT WAS DEEMED A TOTAL LOSS. THE DEALER WESTBURY TOYOTA (1121 OLD COUNTRY RD, WESTBURY, NY 11590, (516) 272-4499) WAS CONTACTED BUT THE FAILURE WAS NOT DIAGNOSED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE WERE A CASE WAS OPENED AND FIELD INVESTIGATORS WERE TO INSPECT THE VEHICLE. THE APPROXIMATE FAILURE MILEAGE WAS 21,000. *TT *AS *AS *TR CONSUMER STATED I'M IN TOUCH WITH MY PERSONAL CLAIMS MANAGER, MR. THOMAS OPATZ AT ESURANCE AND CUSTOMER EXPERIENCE AT TOYOTA WHO I SENT MY PERMISSION TO HAVE MY CAR INSPECTED BECAUSE I ENCOUNTERED A PROBLEM WITH MY ACCELERATOR WHICH APPARENTLY BECAME STUCK AND IN MY DESPERATE ATTEMPT TO STOP THE CAR BY APPLYING EXTREME PRESSURE TO MY BRAKES AND I COULD NOT STOP MY OUT OF CONTROL CAR SO I HAD TO SWERVE TO MY RIGHT TO AVOID CATASTROPHIC ACCIDENT. SEAT ADJUSTMENTS FAIL TO OVERCOME THE ISSUE. 5'3" WOMAN. I HAD A PRIUS 2006 AND IT WAS JUST FINE. THIS PRIUS SEAT IS SO UNCOMFORTABLE I MIGHT SELL IT.

SUMMARY: HAVE OWNED MY PRIUS PRIME FOR EIGHT MONTHS AND HAVE NEEDED TO REPAIR ONE WINDSHIELD CHIP, REPLACE MY WINDSHIELD ONCE, AND CURRENTLY HAVE ANOTHER CRACK THAT CANNOT BE REPAIRED. DETAILS: PURCHASED MY PRIUS PRIME DEC 10, 2017. ON DEC 20, 2017, GOT A CHIP IN MY WINDSHIELD WHILE DRIVING TO WORK ALONG THE LOCAL HIGHWAY AND WAS ABLE TO GET IT REPAIRED SAME-DAY. IN JUNE 2018, GOT ANOTHER CHIP IN MY WINDSHIELD AND THE CRACK SPREAD BEYOND REPAIR ALMOST IMMEDIATELY. STARTED THE PROCESS TO HAVE IT REPLACED AND IT TOOK APPROXIMATELY SIX WEEKS AND THREE APPOINTMENTS WITH SAFELITE BEFORE THEY HAD THE CORRECT WINDSHIELD WITH THE CORRECT PARTS. ACCORDING TO SAFELITE, TOYOTA HAD CHANGED THE DESIGN OF THE WINDSHIELD AND THE NEW DESIGN WAS ON BACKORDER. PAID MY $500 DEDUCTIBLE FOR THIS REPLACEMENT. ON AUGUST 8, 2018, JUST A FEW WEEKS AFTER REPLACING THE WINDSHIELD, I GOT ANOTHER CHIP THAT ALSO SPREAD BEYOND REPAIR IMMEDIATELY. AS OF THIS WRITING, I HAVE NOT YET REPLACED THE WINDSHIELD FOR THE SECOND TIME. AFTER DOING RESEARCH, I FOUND THERE IS A CLASS ACTION LAWSUIT, FILED IN MARCH 2018, RELATED TO THESE DEFECTIVE WINDSHIELDS. ATTACHED ARE RECEIPTS FROM SAFELITE SHOWING THE CHIP REPAIR, PAYMENT OF MY DEDUCTIBLE, AND PICTURES OF THE CURRENT CRACK.

SQUEAKING/CREAKING FROM REAR LEFT SIDE OF THE CAR CAN BE DANGEROUS IF COMING FROM THE WHEEL AREA OR SUSPENSION.

I HAVE 9150 MILES ON MY CAR. GOING 80MPH ON THE HIGHWAY MY RIGHT REAR TIRE BLEW OUT. THE TIRES ARE TOYO NANOENERGY A29. THE TIRE BLEW OUT ON THE SIDE OF THE TIRE FACING THE CAR. I DID NOT HIT ANYTHING. TOYOTA REFUSED TO PAY FOR THE TIRE EVEN THOUGH THEY ARE STILL UNDER WARRANTY. THEY EVEN CHARGED ME FOR LABOR.

THE LIGHTS ARE SO BRIGHT THAT IT BOTHERS THE DRIVER OF THE ONCOMING VEHICLE OR TRAFFIC. I FACE THAT ISSUE WHILE DRIVING AND CAN IMAGINE THE SAME WITH THE DRIVER OF THE ONCOMING VEHICLE. THERE ARE INSTANCES WHEN DRIVERS FLASH THEIR HEADLIGHTS ASKING TO USE LOW-BEAM WHILE I AM ALREADY USING THE LOW-BEAM LIGHT SETTING. LAST NIGHT 3/6/2018, AN IRRITATED DRIVERS JUST BRUSHED OFF MY CAR ON A COUNTY ROAD THINKING THAT I AM NOT RESPONDING TO HIS FLASHING LIGHT SIGNAL OR ASK TO USE LOW-BEAM ON MY CAR. I SEE EXCESSIVE BRIGHT LIGHTS ON NEWER CARS AS A SAFETY ISSUE WHILE DRIVING.

Gulf States Toyota, Inc. (Gulf States) is recalling certain 2017 4Runner , 86, Avalon, Camry, Camry Hybrid, Corolla, Corolla iM, Highlander, Highlander Hybrid, Prius, Prius C, RAV4, RAV4 Hybrid, Sienna and Yaris vehicles. The spare tire air pressure was not adjusted to the proper pressure as stated on the Tire Pressure Label.

Risk: A spare tire that is not inflated to the proper value may not perform as intended, increasing the risk of a crash.

Remedy: Gulf States will notify owners, and dealers will inspect the spare tires, adjusting their pressure as necessary, free of charge. The recall began June 15, 2017. Owners may contact Gulf States customer service at 1-800-444-1074. Toyota's number for this recall is 17R2.

Pedestrian alert sounds

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
