Installed new Rainex wipers two days ago. Driving tonight in rain and the exterior of the windshield has a film on it that makes it difficult to see when the wipers cycle when near streetlights or when cars with headlighs on are approaching. Seems really dangerous.

I was pulling into a parking spot when my car suddenly accelerated to a very high speed. I traveled over a curb, on some pavement, through some bushes, knocked down a light pole and miraculously came to a stop. My foot was pumping the brakes to no avail. My entire car was damaged but miraculously no one was hurt. Car was towed to a mechanic who confirmed that the car accelerated with him as well. Front, back, underside, roof, tires and frames were all badly damaged.

The rear window retractable sun visor motor/gears have failed. When this happens, it leaves the sun visor in the up position in the rear window at all times. This makes it very, very, hard to see anything out the rear window at night, also pretty hard when it's raining or cloudy out. I understand this has happened to quite a lot of these cars and is a VERY expensive repair cost. I feel this is hazard that Toyota should bear the cost of fixing, because of the sheer amount of vehicles this has happened. Thank you for keeping us all informed and safe from these types of things!

The passenger side mirror failed. The mirror exploded into the passenger window with a loud sound while driving at 35 mph. No rock found impacting mirror, no bird found at impact point, no dead animal found at impact point. If passenger window had not been up then mirror remains would have entered the car cabin. The mirror has been replaced and is not available for inspection. The problem has not been reproduced or confirmed by a dealer or independent service center to my knowledge. The component has not been inspected by the manufacturer, police, insurance representatives or others. There were no warning lamps, messages or other symptoms of a problem prior to the failure. The problem occurred on March 22nd, 2022 at 10:30 AM eastern time zone approximately.

Multiple warning lights suddenly appeared on the dash while stopped at a traffic signal. (Photo attached) Note: the photo does not show the red "PCS" light which was flashing. I was able to get off the road safely although the brakes did not feel normal. Had the car towed to Capitol Toyota in San Jose the following morning for repair. They diagnosed the problem as a failure in the Master Cylinder Booster Unit and replaced it. Note: the attached invoice misstates the issue as "check engine light" which is incorrect.

My Avalon is trying to kill me. I promise I have done nothing wrong! When going down the highway at 85mph I have had people cut in front of me. When pushing on the brakes to avoid collision, the brakes lock up and do not release for about 5 seconds. Naturally, this is quite unnerving for myself, passengers as cars that scatter around me when the Avalon takes control --- slamming on the brakes --- and wont hand control back until it has caused quite a mess. This has happened three times now and no one can tell me what is wrong. I am told to get a brake job at the Toyota dealership even though the pads still have plenty of life.

FROM THE TIME FRAME OF 2017 TIL THE PRESENT, THE HEADLAMPS ON THE VEHICLE WOULD BURN OUT FOR NO APPARENT REASON. EACH TIME I REPLACE THEM, THEY WOULD LAST FOR ABOUT 4 TO 6 MONTHS AND THEN THEY WOULD BURN OUT AGAIN. SOMETIMES EITHER ONE WOULD BURN, LEFT OR RIGHT, AND TWICE I'VE HAD BOTH GO OUT AT THE SAME TIME. I DON'T UNDERSTAND WHAT IS CAUSING THE HEAD LAMPS TO BURN OUT. I'VE REPLACED THEM EACH TIME, AND IN COUNTING I HAVE REPLACED THEM AT LEAST 10 TIMES SINCE 2017. THIS IS TOTALLY UNUSUAL COMPARED TO MY OTHER TWO CARS WHERE I'VE NEVER ONCE HAD TO REPLACE THE HEADLAMPS ON THESE. IT HAPPENS WHEN THE VEHICLE IS BEING DRIVEN AND OTHER TIMES WHEN THE LIGHTS ARE TURNED ON BEFORE DRIVING. TODAY (MARCH 1, 2021) IS HAS HAPPENED AGAIN, AND THIS TIME THE DRIVER SIDE LAMP WENT OUT FOR NO APPARENT REASON.

I HAVE 95000 MILES ON MY TOYOTA AVALON LIMITED HYBRID. WHILE DRIVING DOWN THE ROAD IT JUST QUIT - DEAD STOP (VERY DANGEROUS) AND, HAD IT NOT BEEN A HYBRID, I WOULD NOT HAVE BEEN ABLE TO GET IT OFF OF THE INTERSTATE. TURNS OUT THAT THE FUEL PUMP STOPPED WORKING WITH NO WARNING. MY LAST AVALON WENT ALMOST 300,000 MILES ON THE SAME FUEL PUMP. THE PUMP, IN MY OPINION, IS FAULTY BUT OUT OF WARRANTY. TOYOTA IS CHARGING OVER $1,000 TO FIX IT. I TRIED TO GET TOYOTA MANAGEMENT CONTACT INFORMATION (DISTRICT MGR.) FROM THE DEALERSHIP BUT THEY WOULD NOT SUPPLY IT IN ORDER FOR ME TO DRAW ATTENTION TO TOYOTA OF THE PREMATURE FAILURE OF A KEY COMPONENT OF THE CAR. I CALLED TOYOTA HEADQUARTERS, GOT THE SAME RESPONSE AND FELT LIKE THEY COULD CARE LESS THAT THERE MAY BE A DEFICIENCY IN THEIR COMPONENTS. THIS IS A SERIOUS SAFETY ISSUE. I HAVE SEEN ONLINE OTHERS THAT HAVE HAD THIS EARLY FAILURE OF THE FUEL PUMP. TOYOTA NEEDS TO LOOK INTO THIS LIFE THREATENING PROBLEM AND THEY NEED TO ISSUE A TSB ON IT AND PERFORM THE WORK AT COST VS MAKING MONEY ON SOMETHING THAT COULD KILL ME AND MY FAMILY MEMBERS. I STILL CAN GET OVER THEIR LACK OF CONCERN.

CAR WAS PARKED OUTSIDE FOR COUPLE DAYS UNDER A HEAVY RAIN. NEXT DAY, WHEN IT WAS HOT AND SUNNY, I OPENED CAR'S DOOR AND INSIDE OF THE CAR WAS ALL HUMID AND STEAMY. THERE WAS A MUSTY SCENT AS WELL. AFTER CHECKING THE CAR I FOUND SOME WATER IN THE LEFT SIDE OF THE TRUNK. DEALER SERVICE DEPARTMENT AFTER TWO DAY DIAGNOSTICS TOLD ME THAT ONE OF SUNROOF DRAIN TUBES IS DETACHED AND THAT CAUSED A LEAK INSIDE THE CAR. I THINK IT PRESENTS BOTH DRIVING SAFETY AND HEALTH SAFETY ISSUES: - I WEAR GLASSES AND BECAUSE OF THE HUMID AND STEAMY ATMOSPHERE IN THE CAR - MY GLASSES KEPT FOGGING UP AND I HAD TO STOP COUPLE TIMES TO CLEAR THEM. CONDITION GOT BETTER, BUT DID NOT IMPROVE MUCH AFTER AIR CONDITIONING IN THE CAR RUN FOR SOME TIME. - HEALTH CONCERN IS RELATED TO A MOLD THAT CAN ACCUMULATE IN THE CAR BECAUSE OF THESE LEAKS. IT MIGHT CAUSE ALLERGIC REACTIONS OR ASTHMA. I THINK CAR MANUFACTURER SHOULD EITHER USE BETTER MATERIALS FOR THESE DRAINS, SO THEY DO NOT DETERIORATE WITH TIME OR CHANGE THE DESIGN OF SUNROOF DRAIN SYSTEM.

THIS HAS BEEN GOING ON SINCE THE CAR WAS BOUGHT NEW.

HEARD RATTLING NOISE IN STEERING WHEEL WHEN MAKING LEFT OR RIGHT TURNS. I REMOVED THE 2 PLASTIC COVERS ON EACH SIDE OF THE STEERING WHEEL AND THEN CONTINUED TO DRIVE IN HOPES THAT WHATEVER WAS LOOSE AND RATTLING WOULD FALL OUT. AFTER ABOUT A WEEK OF DRIVING, A SMALL ALUMINUM FASTENER (SCREW) FELL OUT OF THE STEERING APPARATUS. APPROXIMATELY 3 WEEKS LATER, NOTICED THE SAME RATTLING NOISE THAT'S OCCURRING WHEN TURNING THE STEERING WHEEL. SUSPECT ANOTHER FASTENER IS LOOSE INSIDE THE STEERING WHEEL ASSEMBLY. THIS FASTENER HAS YET TO FALL OUT OF THE STEERING WHEEL ASSEMBLY AND CONTINUES TO MOVE FREELY INSIDE THE STEERING WHEEL WHEN MAKING TURNS. I'M CONCERNED THAT THIS METAL FASTENER COULD CAUSE A SHORT WITHIN THE STEERING WHEEL CAUSING THE AIRBAG TO DEPLOY OR MAKE THE STEERING WHEEL MALFUNCTION. IS THIS A PROBLEM?

AUTO SUDDENLY BRAKED TO A STOP FOR NO APPARENT REASON. AUTO HAD BEEN REPAIRED FOR THE RECALL, 15V728000, FORWARD COLLISION AVOIDANCE: CRASH IMMINENT BRAKING. HAS OCCURRED TWICE IN THE LAST MONTH. HAVE SINCE DEACTIVITED THE PRE-COLLISION SYSTEM. BOTH OCCURRENCES HAPPENED WHILE ON CRUISE CONTROL, TRAVELING AT 55 MPH IN ONE CASE AND 60 MPH IN THE OTHER. BOTH WERE ON STRAIGHT, LEVEL HIGHWAYS WITH NO OBVIOUS CAUSE. THE 55 MPH CASE WAS WHILE CRUISE WAS SET AT 60 MPH BUT CRUISE CONTROL WAS CONTROLLING AT THE LOWER SPEED TO CONTROL THE DISTANCE FROM THE CAR AHEAD OF ME. THE CAR AHEAD DID NOT BRAKE OR SLOW DOWN. THERE WAS NO COLLISION IN EITHER CASE. MY DIAGNOSIS IS THAT THE REPAIR FOR THE ABOVE RECALL DID NOT CORRECT THE PROBLEM.

WHILE THE VEHICLE IS COASTING AT A STEADY SPEED ON A STRAIGHT ROAD (~30-40 MPH), A SLIGHT ROARING NOISE CAN BE HEARD FROM THE ENGINE BAY WHILE SITTING IN THE PASSENGER COMPARTMENT. THE DEALER, NORTHSIDE TOYOTA OF CHICAGO, INSPECTED THIS VEHICLE ON FRIDAY, NOVEMBER 4, 2016, AND DETERMINED THAT THE TRANSMISSION IS FAILING AND WILL NEED TO BE COMPLETELY REPLACED. HOWEVER, THEY CLAIM THAT, DUE TO A "COLLISION IMPACT" (A VEHICLE WHICH SCRAPED THE PASSENGER SIDE OF THE FRONT BUMPER AND WHICH MIRACULOUSLY DID NOT IMPACT EITHER THE ENGINE OR THE BODY FRAME), A LEAK HAS BEEN CAUSED FROM A "CLIP" THAT IS "MISSING," AND BASED ON THESE GROUNDS, THE DEALER IS REFUSING TO REPAIR THE TRANSMISSION. INSTEAD, THEY HAVE TOLD US WE NEED TO EITHER REPLACE THE TRANSMISSION OUT OF POCKET ($6000.00 - $7000.00) OR GO THROUGH OUR INSURANCE. THIS KIND OF PROBLEM MIGHT BE NORMAL FOR AN OLDER VEHICLE, BUT WHAT IS UNUSUAL ABOUT THIS CASE IS THAT THIS HYBRID AVALON IS ONLY 2 YEARS OLD AND BARELY CLOCKED 16,000 MILES. IT IS COMPLETELY UNACCEPTABLE FOR SUCH A MAJOR COMPONENT OF A NEARLY-NEW VEHICLE TO FAIL SO SOON AND THE DEALER IS WAVING IT OFF AS NOT A DEFECT.

Investigations
1 NHTSA investigation on record
Investigations are agency reviews, not findings that a vehicle is defective.
The Office of Defects Investigation (ODI) opened this investigation to determine if the failure of airbags to deploy during severe crashes, in certain vehicles, was the result of a safety related defect. During the investigation a complex failure was studied that can result in non-deployment of subject vehicle air bags and other restraint system devices in severe crash events. The subject vehicles may be equipped with an airbag control unit (ACU) for the supplemental restraint system (SRS) Electronic Control Unit (ECU) manufactured by ZF-TRW. The ECU receives signals from crash sensors mounted in the vehicle and deploys the vehicle air bags and seat belt pretensioners in accordance with manufacturer design specifications. The ECU in the subject vehicles contains a model DS84 application-specific integrated circuit (ASIC) which controls the communication of the crash sensor signal, firing commands (i.e., when to deploy the airbag(s) and/or pretensioners), and fault information (e.g., diagnostic trouble codes). In September 2016, FCA announced recall 16V-668 for certain model year (MY) 2010 to 2014 Chrysler, Dodge and Jeep products manufactured with the subject ZF-TRW ACU. In this recall, FCA discussed an EOS condition that resulted in a failure of the subject DS84 ASIC, which caused air bag non-deployment. FCA noted that the defect condition had only been observed in vehicles equipped with sensor harnessing routed across the front of the vehicle. Other FCA vehicles that also used the subject ACU, but were not equipped with cross-car harnessing, had not experienced EOS failures, despite similar time in service. During the course of this investigation, ODI sent two separate Information Request (IR) letters to six vehicle manufactures (including FCA, Hyundai, Honda, Kia, Mitsubishi, and Toyota) and one IR letter to ZF-TRW. These IR letters resulted in ODI receiving comprehensive data from these manufacturers and suppliers. Studies of this data found that the DS84 ASIC does not have sufficient protection against negative electrical transients or electrical overstress (“EOS”) that can be generated in certain severe crashes. An electrical transient occurs when the electrical power supplied to a circuit changes momentarily over a short duration of time. In these severe crash cases, the crash sensors and other powered wiring can be damaged and short circuited so as to create a negative electrical transient of sufficient intensity and duration (that are outside the vehicle manufacturer's specification) to damage the ASIC before the restraint device deployment signal is received by the SRS ECU. This damaged signal can lead to incomplete or nondeployment of the air bags and/or pretensioners. Airbag non-deployment and/or lack of pretensioner operation can increase the risk or severity of injury in a crash.A total of 8 fatalities and 14 injuries were associated with known EOS events. The common element in all investigated manufacturers vehicles is the SRS ECU containing a DS84 ASIC manufactured by ZF-TRW. The risk associated with the ASIC is equally shared among all OEMS involved in the investigation. The actual real-world risk can be mitigated by other factors which were assessed by ODI during this investigation. The first mitigating factor involves protections built into the ACU design which protect the DS84 ASIC from damage. There are multiple strategies and levels of protection employed by different OEMs that provide effective EOS mitigation. The two most common strategies at the ACU level are circuit protection diodes on the remote senor signal lines, and current limiting resistors that protect critical components. The second mitigating factor is found at the vehicle level and involves the location and routing of the wires leading from the crash sensors to the SRS ECU. If the wires are well protected in a crash and are not routed with other power wires carrying large currents, the risk for an EOS event is significantly reduced or eliminated. These design specific factors combine to produce a spectrum of risk for the vehicles equipped with ACUs using the DS84 ASIC. Given the many of years of field exposure, it is possible to divide the subject population into two groups; vehicles which have experienced EOS events, and vehicles which have not experienced EOS field events. Four of the six OEMs involved in this investigation have experienced EOS field events on at least one of their models equipped with a DS84 ASIC. All vehicle models (including the Toyota models identified in the Failure Report Summary of the opening resume for this investigation) with field events have been recalled. In an abundance of caution, ODI kept this investigation open five years to monitor field performance and did not identify any field events on vehicles not included in existing safety recalls. Given the spectrum of risk identified in this investigation and that all vehicles with a demonstrated unreasonable risk have been recalled, ODI is closing this investigation. ODI is closing this investigation with the following manufacturer safety recalls: 16V-668, 18E-043, 18V-137, 18V-363, and 20V-024. With the recall actions taken by the subject vehicle and equipment manufacturers, this investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exists on other model or model year vehicles outside of the recall scopes. The agency reserves the right to take further action if warranted by the circumstances.
