I am filing a formal safety complaint regarding a severe braking and stability control failure that resulted in unpredictable, highly dangerous vehicle behavior. While driving, the vehicle displayed a cascade of critical system failures, throwing error codes APP_w009 (AEB disabled), DI_a222 (Stability control disabled), DI_a223 (Traction control disabled), ESP_a080, and ESP_a067 (ESP/ABS sensor faults). This concurrent breakdown caused a total loss of critical safety features: Anti-lock Braking (ABS), Automatic Emergency Braking, Traction Control, "Vehicle Hold," and "Hill Start Assist." Because the vehicle completely lost its ability to hold its position, it unexpectedly free-rolled backward on an incline when I anticipated it to remain stationary or move forward. This uncommanded movement created an immediate collision hazard. Of serious concern is that despite a total breakdown of vital braking and stability assists, the vehicle remained fully drivable and only advised me to "drive with caution." Allowing a car to remain operational under these severe fault conditions is an unacceptable risk. A driver expecting normal EV behavior (like vehicle hold and regenerative braking) is placed in immediate danger when the vehicle suddenly free-rolls without warning. The software should classify a multi-system ABS/ESP failure as a critical emergency requiring an immediate safe shutdown, rather than suggesting it is safe to continue driving.

While out on vacation in [XXX] on XXX at around 11 am I had just finished charging the Tesla at a supercharging station. I was headed to go eat breakfast at the intersection of [XXX] and XXX on the right lane. I stopped briefly prior to making a right onto [XXX]. Upon making the right turn the vehicle kept turning all the way right and making a sharp right turn instead of a soft right. When I noticed the sharp right turn I tried to turn the steering wheel towards the left but the steering wheel was locked and it wouldn't respond. The car drove me into a concrete barricade and caused a collision. My children were in the car and thankfully no one in my vehicle was seriously injured nor were there any pedestrians on the sidewalk near the barricade. I contacted Tesla for the repair and investigation into why the steering wheel locked. Tesla has not provided a loaner vehicle in the meantime despite my frequent requests. This incident is a serious and critical safety issue that may have caused deaths had I been traveling at a higher rate of speed or been on the highway heading back to [XXX]. The strange thing is that this vehicle is still under warranty with Tesla and had been working just fine without any issues since I got the vehicle in XXX. My Tesla's FSD turned off midway through a curve at highway speeds. I hit the guard rail and scratched the right side of the car. I could have run straight through the guardrail from the inertia or made the back car hit me due to the slowing down, though thankfully I took control of the vehicle to limit the damage. I visited a Tesla repair center, but they said they didn't address these types of issues and referred me to the Tesla.com/legal page and a body shop they work with. Obviously, every time FSD turns off there is a chime, but that sudden alert for an event that spanned 5 seconds, 2 for me to take control, failed to be enough.

After disengaging Full Self-Driving (FSD), a mandatory questionnaire pop-up now stays on screen until manually answered, instead of auto-dismissing after a few seconds as it previously did. This forces the driver to interact with the touchscreen immediately after taking back manual control — a moment when full attention should be on the road, not a pop-up. The prompt blocks part of the instrument/display screen until it is tapped, and in some cases requires a minimum 5-second voice response to dismiss (a previous workaround allowing a near-instant voice note to clear it was removed). The pop-up has also been observed persisting even after the vehicle is shifted into Park. This creates a distraction risk during an already safety-critical moment (immediately following a system disengagement, which by definition means FSD failed to handle the driving task). This behavior has persisted across at least 3 software updates. Multiple attempts to raise this with Tesla directly have not resulted in a fix or response. Other FSD users have reported the same complaint. No collision has occurred as a result of this specific issue, to my knowledge, but the design creates a foreseeable distraction hazard during takeover events, which are often already high-risk moments.

Tesla Full Self-Driving (Supervised) repeatedly ignored the “Avoid Toll Roads” navigation setting and attempted last-second lane changes into Express Toll Lanes at freeway speeds. This has occurred approximately 10 times. I was repeatedly forced to take over steering immediately to avoid entering the toll lane, creating a serious safety risk due to sudden maneuvers in heavy traffic. On several occasions, I could not safely intervene before the lane change was completed. Tesla Service Center inspected the vehicle, found no hardware or camera faults, and stated the behavior appears to be software-related.

On May 24, 2026, at approximately 8:28 AM, I activated Actually Smart Summon on my 2025 Tesla Model 3 using the Tesla mobile application to reverse the vehicle from my garage. Before this incident, the vehicle displayed no warnings, alerts, messages, or other indication of a problem, and I had previously usd Actually Smart Summon without similar behavior. Shortly after activation, the Tesla mobile application became unresponsive, and the vehicle did not stop despite my attempts to stop it through the app. I attempted to open the vehicle door during the incident but was unable to do so while the maneuver continued. Because I could no longer stop the vehicle through the app or access the vehicle, I placed my phone in my pocket so I could use both hands to try to stop the vehicle and prevent further damage. At that point, I was no longer pressing or holding the Actually Smart Summon control in the application. As the vehicle continued reversing, I repeatedly repositioned myself behind it and pushed against the rear of the vehicle with my body multiple times. Despite this physical intervention, the vehicle continued reversing and applying rearward force, forcing me to repeatedly move out of its path to avoid being pinned between the vehicle and my neighbor s garage. The vehicle pressed into my neighbor s garage with enough force that the impact transferred to the parked vehicle inside the garage. It did not stop upon the initial impact and continued pressing into the garage afterward. This sustained movement is clearly visible on the preserved TeslaCam footage and independent home-security recordings. I narrowly escaped being crushed between the vehicle and the garage while attempting to stop the vehicle and prevent further damage, sustaining a muscle strain in the process. The incident created a serious risk of bodily injury in addition to substantial property damage. I do not know which specific component or system failed. The passenger side front fold down visor/mirror failed. It is available for inspection upon request. The safety of myself, the driver, and the front passenger, were put at risk, because the design of the car allows the front passenger headset to be moved forward via the power seat adjustment to a point where it presses against the visor/mirror, and causes the glass in the mirror to shatter. If you never open the visor, you don't know that the glass is shattered. So when we are on the highway, and my passenger folds down the mirror to check her make up, small glass shards fly everywhere, requiring both the driver and passengers full attention on a busy highway. The dealer advised before even looking at the car that the problem would not be covered under warranty, and want to charge $200 for repair. I cancelled the appointment. Independent research shows this is a known issue caused by a design flaw with the passenger front seat being allowed to move into the zone of the visor/mirror. The vehicle has not been inspected due to their refusal to do so at their own cost. There was zero warning from delivery until now via any method whether it be a warning label, screen, or in the manual.

On May 16th, I felt the car vibration very much on the free way. I scheduled service on May 21st. Tesla technician found it was the interior foam detached problem. He said this happened on many cars. I asked for free replacement for two defect tires since the car was only drove one and half year about 17k miles. But he refused it because Tesla only replace it for free if the wear of tire is higher than 4 thread. He said mine is 3 thread and suggested me to buy new tires and replace them. He said Tesla normally would not remove the detached foam for customer any more. I did not agree. Finally, Tesla remove the detached foam and I paid for the service. Because this detached foam issue happens a lot, it is quality issue. Tesla or Michelin should repair and replace it for free.

Since leasing this car in April of 2025, it has been services 8 times totaling over 30 days of down time. The issues occured in order from oldest to newest/current: May of 2025 - Headliner trim falling down, unsuccessfully repaired by mobile technician June of 2025 - Windows stopped working. Center console door stopped shutting. Headliner still falling down. July of 2025 - Front wiring harness melted and took out the AC - the car didn't provide a single warning. The AC just stopped working. Passenger seat replaced for cosmetic defect August of 2025 - Continued issues with AC and turn signal buttons December of 2025 - Steering Wheel Rattle and Clunking, Suspension noises June of 2025 - Suspension still rough and making grinding noises, turn signal buttons still not functioning consistently, front passenger fold down mirror shattered because the passenger seat calibration allows it to slide into the mirror. Tesla has shown zero empathy towards any of these issues. A buyback was requested, but the offer would have left us without a car and with a huge pay off. We asked for help moving into another car, and we are ghosted.

This model lacks a well-tested turn signal lever, replacing it with arrows on the steering wheel. This method of activating turns signals is fundamentally unreliable and consequently dangerous. As an owner of the subject Tesla for just over one year, I find that I regularly fail to achieve correct activation, even with repeated attempts to do so. The steering wheel control may: Not activate at all, even with multiple attempts to activate the signal. I find no consistent patter as to why pressing the arrow or vicinity of the arrow fails to achieve activation. Worse, may activate in the opposite direction to the pending movement. This is the most dangerous situation, misleading other drivers with regard to the intended movement. It’s also not apparent to me that the opposite direction has been activated. The problem is made worse by: Lack of tactile feedback from the controls. The switches do not move perceptibly, nor is there any other feedback, such as a click. The ridge between left and right arrows doesn’t provide sufficient indication of correct or incorrect placement of the thumb on the controls. Pressing on the controls within the vicinity of the arrows may or may not result in activation at all or activate the opposite one. The position of the control varies depending on the steering wheel angle. The control may be very difficult to access, particularly where a sequence of turns in opposite directions must be executed. Specifically, it is essentially impossible to activate for the second movement while making the preceding turn. I understand that Tesla recognizes the problem and owner dissatisfaction in that it offers a retrofit with a conventional and well-tested turn signal lever for $600. Tesla must be compelled to recall these vehicles and retrofit them with a conventional turn signal lever or other effective control at no charge to owners.

Tesla is in a protracted battle with me over fees related to damaged property and illegal self-help repossession procedures. They have been formally notified that any trespass onto my property is not permitted because they said they sent armed repossession agents because of “angry customers” like me. I currently have a complaint for those issues filed with the Attorney General of Pennsylvania. Tesla’s recent attempts to repossess the vehicle were initiated approximately two hours after informing the Pennsylvania Attorney General’s office that Tesla had not contacted me in any way, that Tesla was not being factual. Tesla’s response to that was again to not contact me but to send me a form letter stating they intend to disable my vehicle remotely. I am a privacy-minded person, and part of choosing the vehicle was a modicum of privacy guaranteed by Tesla. Tesla publicly says they do not so this, I was told they do not so this in the course of buying the car, and recent news seems to state they say they can’t do it. They have yet to repair a windshield and cabin camera that seem to be damaged. In other words, they intend to disable and lock my car without my permission or knowledge, without verifying the contents of the car. In other words, if there happens to be an occupant at the time they illegally disable the car, they intend to kill them. I’m asking that my contact information be kept private because they have already threatened to use lethal force, make extremely reckless decisions regarding their procedures, and retaliated by initiating this process when I informed a government agency that they lied to them.

On March 14, 2026, at 20:54:21 UTC, while the Autopilot system was engaged, the vehicle malfunctioned by initiating a sudden, unprompted right-hand steering command within 0.3 seconds, resulting in a collision with another vehicle traveling in the adjacent right lane. Telemetry data from the event confirms the system failed to maintain the lane despite the Primary Steering Torque Sensor measuring 0.00 Nm, proving no driver input caused the maneuver. This unintended steering event at cruising speed created an immediate safety risk to myself and the occupants of the other vehicle. To date, the manufacturer (Tesla) has declined to perform a technical or remote diagnostic review, redirecting the case to a collision center instead. No warning lamps, audio chimes, or system messages occurred prior to the failure; the vehicle is currently available for inspection and raw CSV telemetry logs can be provided to investigators upon request.

Tesla Full Self-Driving (Supervised) was engaged at the time of the incident. The vehicle was traveling at low speed on a straight roadway when it unexpectedly initiated a sudden steering maneuver out of its lane toward a roadside structure (outdoor restaurant seating area), resulting in a collision. The driver did not provide steering input prior to or during the maneuver. There were no warning messages or alerts indicating a hazard or need for evasive action before the event. Vehicle telemetry data reviewed after the incident indicates that the steering system executed a rapid, large-angle deflection without corresponding driver input, consistent with a system-initiated maneuver. The system continued to apply throttle during the event. The incident created a direct safety risk to the vehicle occupants and nearby pedestrians due to unintended vehicle path deviation toward a fixed structure. Additional Context: When this car was in the repair shop for repairs, we were told that during the replacement of the dashboard/computer system, the technicians observed a persistent system code that would not clear. Tesla technical support was contacted, and the system was ultimately reinstalled to resolve the issue. While the direct connection between this code and the FSD malfunction is unknown, the persistent code may indicate preexisting software system instability. The vehicle has been inspected by repair professionals, and supporting data (telemetry and video) is preserved and available for review upon request. Cause of the malfunction is currently UNKNOWN, but evidence indicates a failure in the driver assistance system’s steering control.

I purchased a new 2025 Tesla Model 3. During normal driving conditions, the rear bumper detached without any collision or impact. The bumper became loose while the vehicle was moving, creating a potential safety hazard. Tesla Service inspected the vehicle but declined warranty coverage and refused to properly repair the issue. The bumper was only temporarily reattached and remains misaligned. Additionally, shortly after purchasing the vehicle, the seat belt assembly had a failure and had to be repaired by Tesla under warranty. While Tesla addressed that issue, it adds to my concern about the overall quality and reliability of a brand-new vehicle. Because the vehicle is new and the failure occurred during normal driving, I believe this may indicate a defect. I am requesting assistance in reviewing this situation. The vehicle presented an error that the audio system was offline, and that the horn, speakers, and internal safety sounds may not work. After driving on the PA turnpike and almost getting in an accident, this error causes the horn to not functioning at all. I was not able to alert the other driver due to this issue, and had to therefore slam on the breaks due to this issue which caused a safety risk to myself and other drivers on the road. This issue has been consistent since the error originally appeared a week ago. The horn does not function, as well as the reverse pedestrian safety sound that alerts pedestrians of the vehicle reversing. In the interior, no safety chimes, audio, turn signal clicks are audible and therefore are not able to be perceived. After putting in a service request and being told I was to be scheduled 2 weeks out and not provided a loaner, I called the dealership whom confirmed this, and stated they are not able to provide a loaner vehicle or move the appointment closer regardless of their being a potential safety issue. There are no other warning lights on in the vehicle, and the alert popped up out of no where, as well it does not mention the horn *does not* work, only warning that the horn may not work.

I was using the vehicle's Autopark feature to parallel park on a public street in a normal/spacious parking spot. I selected the parking spot on the center screen and started Autopark. During the maneuver, the vehicle reversed into the parked car behind me and caused a dent (low-speed crash). I was supervising the maneuver and did not press the accelerator or brake before impact. After the incident, I reviewed dashcam footage and vehicle telemetry. The rear vehicle was clearly visible in the rear camera view. Telemetry shows the vehicle had already shifted to Drive before impact, but it continued moving backward for about 1 second and then contacted the parked car behind. Telemetry also shows accelerator pedal position was 0 and brake was not applied during the impact sequence. This appears to be an automated parking/close-range obstacle stopping failure or delayed stopping response. No warning lamp or prior vehicle fault message was observed before the incident. The issue has not been reproduced since the incident. I have dashcam footage and telemetry logs documenting the event.

Tesla’s Obstruction of Repairs Tesla deleted my original service complaint and annotations regarding these delivery defects. The associated service appointment was cancelled without notice or rescheduling. No repairs were performed despite the issues being raised during the warranty period. On January 16, 2026, I noticed the vehicle's horn was extremely quiet and would not be audible to another driver. This puts the safety of the Tesla driver and other roadway users at risk because another driver or pedestrian would not hear the horn if the Tesla driver needed to get their attention. This problem occurred after receiving approximately 6-8 inches of snow and driving in the snow. It is possible the design of the Tesla allows for snow/rain to block the horn speaker causing it to not function correctly during and after snow/rain conditions. There were no warning lamps, messages, or other symptoms of the problem prior to the failure. I've submitted a service request via the Tesla app. The vehicle is available for inspection upon request.

I am pretty concerned with potential safety issue with the important steering-function while driving on highway and other roads. The car has only 804 miles as of Dec 26, 2025. The new car was picked up on May 31, 2025 with 15 miles. I have the FSD on monthly subscription. I have sent a message to Tesla. It has provided me with a service appointment at its West Bloomfield Township Center in Michigan on Jan 26, 2026 at 10:30 am. At the time of setting the appointment an estimate of about $450 was provided. It is concerning since the new car is under warranty. I am submitting this concerning safety related issue to NHTSA - to please investigate.

This is a new one but not new for me as this vehicle was the first under my own registration/not rental. Had passenger in car and with music down the passenger heard a very clear odd word which I can only assume was its desire, we declined. This is the umpteenth of many odd and offensive remarks that can cause odd distortion to within the immersive sound system and was what I previously attributed to rental vehicle blue tooths or dysfunctional family raising. Its offensive, distracting, and sometime low-level so unhealthy. Im awaiting the response from tesla but it should be documented as ive seen ISAC section on this website, and ive dealt with it since 2018 , and ended up having to put boxing gloves in my trunk for potentially violent followers trying to pair and talk obscene. Im not sure what but its been confirmed and im awaiting the reply from manufacturer.

I drove my car today after it snowed a few inches the night before and while I was driving, I tried to press the horn, but there was no noise coming from the horn. when I went back home I noted that the speaker used for the horn is located near the passenger side wheel well. The speaker was blocked due to snow accumulation. This is dangerous because that means whenever there’s any snow on the road and I try to press the horn in an emergency then I cannot I might get into an accident because of my horn failing.

The steering was hard to turn, and the fully supervised driving mode didn't work. This has happened twice, and both times the issue went away on its own after a few minutes. I performed a soft reset both times, but it did not resolve the issue. The first time, the steering was very heavy, i.e., hard to turn. The second time was less difficult, but in both cases, FSD (Full Self-Driving) did not function. General Guideline questions: 1. The steering system malfunctioned. The car is available for inspection. 2. The car was very difficult to drive. I was able to turn the wheel, but I won't let my wife drive it. 3. No. The nearest service center is 90 miles away, and so far, they want me to bring the car to them. I am still working to resolve this, as I believe this issue requires mobile service. 4. No inspections have been done. 5. Yes. An alert message on the main display stated: "Alerts - Steering & Braking - Steering assist reduced/Steering may require increased effort."

I am submitting this complaint to report a safety issue with the turn-signal buttons on my steering wheel of the 2024–2025 Tesla Model 3 Highland. The turn-signal buttons are located on the steering wheel surface instead of a traditional stalk. When the steering wheel is turned at any angle, the buttons are difficult to locate by feel, often requiring the driver to look down to activate them and take his eyes off the road. Trying to signal during turns, lane changes, or roundabouts is not intuitive, and I have nearly had several collisions. This design presents a potentially dangerous safety hazard and could increase the risk of collisions. Tesla should be required to recall and retrofit these vehicles with turn signal stalks on the steering wheel column.

I drove car for about 15 minutes and pulled into a parking space. There was no bumping or other incident. I left the vehicle for about 10 minutes and returned. The screen on my return showed 2 warnings, 1. Steering assist disabled, and 2. Collision avoidance system currently inoperable. Dealing with warning number 1: On starting to back out of parking space, i discovered there was no assist for steering, and a high degree of force was needed to turn the steering wheel and guide the car. This required considerable force. I tried a "system reboot" twice to no avail (this is done by holding 2 steering wheel buttons for about 10 seconds). I called Tesla for service and they towed to car to their service center. I do not know what steps were taken to diagnose or repair the problem. Warning number 2 disappeared from the screen when I started trying to back up..

Using the navigation system in self driving mode the turn signal indicators start 500 feet from the intended turn, regardless of speed, and not influenced by cross streets or parking lot driveways thay are present before the intended turn. This takes place regardless of upcoming cross streets or driveways, but based on distance only, giving drivers coming out of these cross streets and driveways an indication that i intend to turn sooner. This has caused several "close calls". Reference: "The California DMV driver handbook similarly instructs drivers to start signaling about 100 feet before a turn so that other road users have time to see and react to your intention. On freeways, the DMV further recommends signaling for at least five seconds before changing lanes, which is a related but separate guidance from the 100‑foot rule for turns." I consider this a serious safety issue.

I am reporting a safety issue involving my 2025 Tesla Model 3 Performance. My steering wheel began shaking while driving, and the vehicle felt unstable, especially at speed. I brought the vehicle to Tesla Service in Las Vegas. Their inspection found that the outer tie rod nut on the passenger front side was loose, which created steering play and caused the steering wheel shake (documented in Tesla invoice #XXX). A loose tie rod nut is a serious steering safety defect that can result in loss of steering control. Tesla performed a four-wheel alignment and tightened the components but stated the issue is “maintenance” and not covered under warranty, despite the vehicle having only 12,890 miles. Tesla also stated the steering rack had “no issues,” but they did not perform any repair on the steering system beyond tightening the loose tie rod nut and charging me for alignment. They also did not explain how a nearly new vehicle could develop a loose steering component. A loose outer tie rod nut should never occur on a new car unless there is a defect or improper assembly from the factory. This poses a significant safety hazard, especially at highway speeds. Tesla did not treat this as a steering safety defect and only attributed it to alignment. They declined warranty coverage and did not fully address the root cause. I am filing this complaint because steering components should not loosen on a 2025 model vehicle, and the situation could have resulted in loss of control or an accident. I want NHTSA to be aware in case this issue appears in other vehicles of the same model year.

The Tesla FSD performed in a dangerous way. It came into a left lane when a speeding truck was in the lane and nearly hit my car. This occurred on Nov 12 around 5:30 pm near floor and decor at Kolb and Broadway Intersection in Tucson AZ. I engaged FSD to return home in Oro Valley. First the car did not follow straight path going from the parking store lot to make a right trun and enter Kolb road. It made right turn and left turn and then right to Kolb road to turn right. It was a busy time in the evening. When an opening was found it entered Kolb road and then rapidly zig zagged to change lanes to try to enter left turn lane only on Kolb to turn on Broadway. In the left most lane, it did not detect a fast approaching truck. The truck honked, and I took control of the car and sped through the intersection to avoid collision.

The dealer determined that the tires were unbalanced. The vehicle was repaired, but the failure recurred. The vehicle was taken back to the Tesla Service Center, where the suspension torque was adjusted. The tires were rebalanced, but the failure persisted. The vehicle was taken to the Service Center, and it was determined that the failure was related to the front suspension. The vehicle was taken to the same Service Center, where it was diagnosed and determined that the steering column was the cause of the failure. The electric steering column and steering wheel were replaced. The manufacturer was made aware of the failure. The failure mileage was approximately 8,434.

I love my vehicle and its technology. However after installation of the latest (FSD V14) update, I observed issues I believe pose a risk to the safe operation of the vehicle. I have made dozens of reports for 30+ days to Tesla for these issues. 1. Driving Profiles and Speed The update features multiple driving profiles (e.g., "Sloth", "Chill", "Standard", and Mad Max). Only the "Sloth" profile drives at the “speed limit” (what their outdated map thinks speed limit is). All other profiles drive over the posted speed limit (sometimes significantly). The ability to manually adjust the speed limit setting has been removed or disabled in these profiles. Once the vehicle is in a profile, the system stays in its own selected speed. This behavior prevents the driver from easily controlling the maximum speed when using the system, undermining driver-intent. 2. Incorrect or Outdated Speed Limit Data The vehicle frequently displays incorrect speed limits: for example, school zones, reduced speed zones, temporary speed changes, work zones, and even a busy high way I drive everyday that’s been changed for 3 months now, are often not recognized or updated in the system. On several occasions over the past months, the system continued to use an outdated or incorrect posted speed, despite the road clearly being a lower limit. Because the system uses the erroneous speed data as the basis for its automated driving decision (and without manual override of speed in FSD), this situation increases risk-especially in areas where lower speed limits are enforced for safety (pedestrian zones, near schools). 3. Safety Risk Description The lack of manual speed adjustment under autonomous mode means the driver has less control over vehicle speed when using FSD, reducing the driver's ability to mitigate risk in a dynamic environment. I’m asking for Tesla to either make sure they have constant real time speed limit data, or allow again for manual speed adjusts when in FSD.

This has occurred while changing lanes on the freeway and when turning into shopping centers or side streets, leaving drivers behind me unaware of my intent and creating a safety hazard. The problem is worse when lane assist is active, as the vehicle resists the maneuver when the signal doesn’t engage, interpreting it as lane departure. The problem started happening at around 3,000 miles on the odometer and continues intermittently. I have not had Tesla service confirm the issue yet, but other owners have reported that replacement of the steering wheel does not resolve it, suggesting a systemic or software-related defect. No warning lamps or messages appear.

On [XXX] at approximately [XXX], I was driving northbound on [XXX] in Phoenix, Arizona in my 2025 Tesla Model 3 using Tesla's Full Self Driving (FSD) (Supervised) mode, when the incident occurred. I was paying close attention while driving in the left-most, non-HOV lane coming up to a bend just before the [XXX] overpass, when I noticed traffic ahead slowing down and coming to a stop. I monitored FSD as it went through the act of braking; however, it seemed to wait a little longer than I would have expected before it actually started to brake, and when it did, the FSD system had to brake more aggressively than I've previously experienced. As it neared the end of the braking event (while the car was still moving), the FSD system disengaged itself without any input from me (neither by manually applying the brake pedal, applying torque to the steering wheel, or pressing the FSD button). When the FSD system disengaged itself, my vehicle continued forward. Had I not been paying attention, my vehicle would have collided with the rear end of the vehicle in front of me. Since I was paying close attention, I immediately slammed on the brakes after noticing that FSD had disengaged itself. I have dashcam footage of this event from the TeslaCam system; however, it does not include audio or telemetry data. Even without that, you can see in the video that my car noses down while FSD applies the brakes, then noses up when FSD disengages itself, and then noses down again when I apply the brakes manually. (The dashcam footage exceeds the 20MB limit; however, I can provide it via other means upon request.) It should be noted that I am an experienced Tesla FSD user, as I used it for over a year in my prior vehicle (2018 Tesla Model 3, which had been upgraded with the HW3 FSD computer), and this is the first time I have ever experienced a near collision event while using FSD or an event in which FSD disengaged on its own. Tesla, Inc. (Tesla) is recalling certain 2024-2025 Model 3, Model S, 2023-2025 Model X, and Model Y vehicles. The computer circuit board may short, resulting in the loss of the rearview camera image. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Risk: A rearview camera that does not display an image reduces the driver's rear view, increasing the risk of a crash.

Remedy: Tesla released an over-the-air (OTA) software update, free of charge. Tesla will also identify any vehicles that experienced a circuit board failure, or stress that may lead to a circuit board failure, and replace the affected computers, free of charge. Owner notification letters were mailed March 7, 2025. Owners may contact Tesla customer service at 1-877-798-3752. Tesla's number for this recall is SB-25-00-001.

FSD Collisions in Reduced Roadway Visibility Conditions

The Office of Defects Investigation (ODI) is opening this Engineering Analysis to evaluate Tesla’s Full Self Driving Beta and Full Self Driving (Supervised) (collectively, FSD) degradation detection system. The focus of this investigation will be to assess the system’s ability, when encountering reduced roadway visibility conditions, to detect degradation and alert the driver with sufficient time to respond. ODI will evaluate the performance of FSD in degraded roadway conditions and the updates or modifications by Tesla to the degradation detection system, including the timing, purpose, and capabilities of the updates, and Tesla’s assessment of their safety impact. Tesla’s FSD is an advanced driver assistance system (ADAS) that relies exclusively on vision-based cameras and the related FSD software to detect and respond to the roadway ahead, projecting a path forward based on traffic control devices, vehicles, pedestrians, and the roadway itself. When Tesla began transitioning away from using both cameras and radars to an exclusively camera-based approach, known as Tesla Vision, in mid-2021, it developed and implemented a degradation detection system that it deployed by a software update to existing and new Tesla vehicles. On June 28, 2024, the day after Tesla submitted the SGO report of the November 28, 2023 fatal crash listed in this document, Tesla began developing an update to the degradation detection system. At this time, ODI does not have information on when the update was deployed and which vehicles have the updated system. ODI discussed individual incidents and its initial findings during the PE phase of its investigation with Tesla. As part of those discussions, Tesla’s post-incident analysis indicated that the update to the degradation detection system, had it been installed on the vehicles at the time, may have affected 3 of the 9 incidents identified by ODI. Tesla also described internal data and labeling limitations that prevented a uniform identification and analysis of crash events with the subject system engaged. ODI believes this limitation could have led to under-reporting of subject crashes over portions of the defined time-period. Available incident data raise concerns that Tesla’s degradation detection system, both as originally deployed and later updated, fails to detect and/or warn the driver appropriately under degraded visibility conditions such as glare and airborne obscurants. In the crashes that ODI has reviewed, the system did not detect common roadway conditions that impaired camera visibility and/or provide alerts when camera performance had deteriorated until immediately before the crash occurred. Review of Tesla’s responses revealed additional crashes that occurred in similar environments and where the system either did not detect a degraded state, and/or it did not present the driver with an alert with adequate time for the driver to react. In each of these crashes, FSD also lost track of or never detected a lead vehicle in its path. In upgrading PE24031 to an Engineering Analysis (EA), ODI will gather further information on the updated degradation detection system, including the status of updating vehicles and scope of compatible vehicles, the system’s visibility degradation detection capability, and alerts or warnings to the driver. Lastly, ODI will conduct analysis on six recent potentially related incidents. Traffic safety violations while Full Self Driving ("FSD") is engaged

The Office of Defects Investigation (“ODI”) is opening this Preliminary Evaluation (PE) to assess the scope, frequency, and potential safety consequences of FSD executing driving maneuvers that constitute traffic safety violations. This investigation concerns versions of FSD that Tesla has labeled as "FSD (Supervised)" and "FSD (Beta)." Tesla characterizes FSD as an SAE Level 2 partial automation system requiring a fully attentive driver who is engaged in the driving task at all times. Level 2 partial automation systems are designed to support and assist the driver in performing certain aspects of the driving task, requiring a driver to supervise and intervene as necessary.  The driver remains fully responsible at all times for driving the vehicle, including complying with applicable traffic laws. ODI’s investigation will therefore focus, in particular, on whether certain driving inputs within the control authority of FSD forestall the driver’s supervision when they are unexpectedly performed. ODI has identified a number of incidents in which the inputs to the dynamic driving task commanded by FSD induced vehicle behavior that violated traffic safety laws. Although reports of this nature span a variety of behaviors, the reports appear to most commonly involve two types of scenarios. The first type of scenario involves a vehicle operating with FSD proceeding into an intersection in violation of a red traffic signal. The second type of scenario involves FSD commanding a lane change into an opposing lane of traffic. With respect to the first type of scenario, ODI has identified 18 complaints and 1 media report alleging that a Tesla vehicle, operating at an intersection with FSD engaged, failed to remain stopped for the duration of a red traffic signal, failed to stop fully, or failed to accurately detect and display the correct traffic signal state in the vehicle interface. Some complainants also alleged that FSD did not provide warnings of the system's intended behavior as the vehicle was approaching a red traffic signal. ODI has identified six Standing General Order ("SGO") reports in which a Tesla vehicle, operating with FSD engaged, approached an intersection with a red traffic signal, continued to travel into the intersection against the red light and was subsequently involved in a crash with other motor vehicles in the intersection. Of these incidents, four crashes resulted in one or more reported injuries. At least some of the incidents appeared to involve FSD proceeding into the intersection after coming to a complete stop. ODI's pre-investigative work, including coordination with the Maryland Transportation Authority and State Police, indicated that the problem may be repeatable, given that multiple subject incidents occurred at the same intersection in Joppa, Maryland. NHTSA understands that Tesla has since taken action to address the issue at this intersection. With respect to the second type of scenario, ODI has identified 2 SGO reports, 18 complaints, and 2 media reports alleging that a Tesla vehicle, operating with FSD engaged, entered opposing lanes of travel during or following a turn, crossed double-yellow lane markings while proceeding straight, or attempted to turn onto a road in the wrong direction despite the presence of wrong-way road signs. Likewise, ODI has identified 4 SGO reports, 6 complaints, and 1 media report alleging that a Tesla vehicle, operating with FSD engaged, proceeded straight through an intersection in a turn-only lane or executed a turn at an intersection in a through lane despite the presence of lane markings or signals. Complaints also alleged that FSD did not provide warnings of the system's intended behavior. Some complaints alleged that more than one of these failures occurred and, as such, the numbers are not cumulative. Some of the reported incidents appeared to involve FSD executing a lane change into an opposing lane of travel with little notice to a driver or opportunity to intervene. ODI’s review will assess whether there was prior warning or adequate time for the driver to respond to the unexpected behavior or to safely supervise the automated driving task. This review will assess any warnings to the driver about the system's impending behavior; the time given to drivers to respond; the capability of FSD to detect, display to the driver, and respond appropriately to traffic signals; and the capability of FSD to detect and respond to lane markings and wrong-way signage. NHTSA's review will also consider any updates or modifications to the system(s) that may affect the performance of FSD with respect to obeying traffic safety laws and signals. This assessment will focus, in particular, on the types of traffic safety violations described above, as most reports identified thus far have centered around those behaviors. While the behaviors under investigation appear to occur most frequently at intersections, NHTSA’s investigation will encompass any other types of situations in which this behavior may arise, such as when traveling adjacent to a lane of opposing traffic or when approaching railroad crossings. If other evidence received during this investigation involve other types of traffic safety violations, those may be considered as part of this assessment as well. Compliance with Standing General Order 2021-01 Reporting Requirements

The Office of Defects Investigation (“ODI”) has identified numerous incident reports submitted by Tesla, Inc. (“Tesla”) in response to Standing General Order 2021-01 (the “SGO”), in which the reported crashes occurred several months or more before the dates of the reports. The majority of these reports involved crashes in which the Standing General Order in place at the time required a report to be submitted within one or five days of Tesla receiving notice of the crash. When the reports were submitted, Tesla submitted them in one of two ways. Many of the reports were submitted as part of a single batch, while others were submitted on a rolling basis. Preliminary engagement between ODI and Tesla on the issue indicates that the timing of the reports was due to an issue with Tesla’s data collection, which, according to Tesla, has now been fixed. NHTSA is opening this Audit Query, a standard process for reviewing compliance with legal requirements, to evaluate the cause of the potential delays in reporting, the scope of any such delays, and the mitigations that Tesla has developed to address them. As part of this review, NHTSA will assess whether any reports of prior incidents remain outstanding and whether the reports that were submitted include all of the required and available data. Actually Smart Summon sessions resulting in low-speed impacts.

On January 6, 2025, the Office of Defects Investigations (ODI) opened Preliminary Evaluation 24003 (PE24033) to investigate Actually Smart Summon (Summon) sessions resulting in crashes during active sessions. According to Tesla, Summon is a short-distance SAE Level 2 system, controlled by the user from a cell phone within a certain distance and intended for use in parking lots and on private property. ODI analyzed complaint data provided by Tesla as well as complaints submitted to ODI from consumers to identify Summon incidents resulting in crashes. ODI's analysis indicates that almost all Summon reported crashes involved minor property damage claims with no reported incidents involving a vulnerable road user, injury, fatality, or major property damage as indicated by an air bag deployment or vehicle tow away. Out of millions of Summon sessions, a fraction of 1% resulted in an incident. Almost all those incidents took place where, typically early in a Summon session, the system or person using the app failed to fully detect or respond appropriately to vehicle surroundings resulting in minor impacts. Incidents took place when app users did not have a complete 360-degree view of the surroundings in the app to assess situational awareness. This limited the app user’s ability to determine whether an impact was imminent during initial vehicle maneuvers such as reversing in close proximity to an obstacle or a curb. ODI found that the impacts most often occurred with parking gates, adjacently parked vehicles, and short parking bollards. During this investigation, ODI identified two Summon crashes related to camera blockages. In both crashes, Summon attempted to navigate a snowy parking lot with snow partially or fully obstructing the forward-facing cameras. Summon did not detect the camera blockage and the vehicles collided with unoccupied parked vehicles while navigating the parking lot. App users in both instances did not command a vehicle stop or pause despite the obstructed camera visible in the camera stream in the app. On January 15, 2025, Tesla released Over-the-Air (OTA) Software (SW) Update Action numbers 578998 and 579185 for vehicles in service to implement a camera blockage detection condition. Both OTAs improve camera blockage detection mechanisms. Additionally, on January 20, 2025, and January 30, 2025, Tesla identified additional system requirements associated with camera visibility checks and released OTA SW-578752 and SW-580322, respectively. These firmware updates reduce false negative camera blockage detections due to snow or condensation. In its investigation, ODI identified one Summon incident where the vehicle did not yield for a gate arm blocking a garage exit lane and the app user did not command a vehicle stop or pause, resulting in an impact. On February 6, 2025, Tesla deployed OTA SW-578839 to improve vehicle reaction to dynamic gates. This OTA update upgraded vehicle perception systems through a high-fidelity occupancy determination network, which uses data from vehicle sensory systems to improve reconstruction of field objects with high accuracy. On November 20, 2025, Tesla further improved vehicle performance by adding object detections from a separate neural network through OTA SW-580514. Owners of the affected vehicles received all six OTA SW updates. Tesla also released these SW updates to production vehicles.
