To whom it may concern, On [XXX] at around [XXX] PST, I was taking my car to get a car wash at Quick Quack Car Wash located in [XXX] . While waiting in line for the other cars to go in, all of sudden I see a huge fire flame shoot out from under the hood. My car was on full fire extremely rapidly, in front of the multiple cars and near the entrance of this car wash, while this car was in fire I ran inside and asked the personnel for a fire extinguisher, to consume the flames coming out of the hood, as I was extremely concerned about the safety and personnel of this location. As soon as I got the extinguisher, I ran towards the car and started spraying powder everywhere I saw flames coming out from top of the hood and under the car, as flames and melted material dropped down from under the car. When I saw the flames got contained, I saw a water hose near the entrance in which I went and turn on and ran to continue pouring water into the engine bay compartment in order to make sure the fire was fully contained. All I could say is that I was very scared and extremely worried that this car could explode in front of this business with all the people working there. As soon I put the fire out, my friend help me push it out of the way into the much larger empty parking lot in order for the business to continue to conduct their business. My car has had 2 repairs for fuel pump relay issues. 2014 and 2020. recently, I noticed while driving my car will stall while accelerating on the highway. I had to pull over into emergency lane 3 times during 1 trip before, because the car stalled while driving. Just recently I noticed and took videos/pictures of the car going 40 mph but the RPM moving sporadically between 0-7000. It has always felt like a fuel pump issue but dodge servicers does not know whats going on. There was never a warning light until it stalled recently. Red lightning bolt popped up. I have pictures if that also.

I was in a blizzard a few days ago on the freeway & got caught in a ditch, but thankfully got out. I wasn’t going that fast & couldn’t due to not being able to see through the snow well so I can drive safely. In order not to crash because there were semi trucks around, I had to whip the steering wheel along with the weight of the car on the snow in order not to crash due to the car being big & it was sliding pretty heavy & somehow a couple things went out of wack after turning my steering wheel. Definitely a scary experience. It is very hard to turn out of no where, as well it’s rare at times when it turns just a little smoothly. Also, once when first pulling off, it makes a consecutive clicking noise coming from or by the tires. When I give it acceleration while turning, as well, it makes that clicking sound. Then there’s just something about the engine or the drive train that sounds off. I’m still learning as I go about fixing what needs to be done.

Chrysler has had a known issue with the ignition module nodes on its Dodge vehicles since 2013. However, they have failed to issue a recall on the 2011 and higher Dodge vehicles. My ignition module node is going out in my vehicle and the dealership will not recognize this as any kind of recall and is making me pay for the reprogramming and for the keys to be replaced in addition to a car rental. I am having to pay $800. This is a well-known and documented ongoing Nationwide safety issue. Dodge employees have received the attached bulletin making them aware of this known issue but has failed to initiate a recall. According to their attached safety recall report for previous Dodge years, The Ignition Node (WIN) Module on your vehicle may have unintentional movement of the Frequency Operated Button Ignition Key (FOBIK) from the “ON” to the “Accessory” position while driving. This could cause unintended engine shut off and increase the risk of a crash.

Car dies while driving at speed. Also will die at low speeds such as when turning or parking or waiting at a stop light. Sometimes have to wait before restart if dies at speed. If dies at low speed will start right back up. Dealer says no codes. Even when it died while on the techs computer he could not see a problem. These cars have been notorious for electrical problems due to Totally Integrated Power Module. Fuel relay recall did not solve the issue.

When slowing down the FWC warning comes on and the vehicle completely shuts down. I have to put it in park and restart it. This is very unsafe, as it happens at intersections, turnpikes, sometimes when turning causing BIG SAFETY ISSUES and we are lucky has not caused a crash yet.

While driving without notice transmission light flashes and the 2011 Dodge Durango will not shift out of gear. I have had this problem in the past and told dealer but they always say they have no codes and on test drive cant confirm the issue. The first time it happened was around the 50 000 mile mark I thought nothing of it back then as I restarted the car it went away. Over the years it go worst to the point that it would happen at least once or multiple times during my daily commute it I told the dealership about it but no codes and they said on a test run no problems. I stopped driving the car in 2019 because it was too dangerous to drive daily especially in traffic due to the car unable to shift gears left me stranded in the middle of the freeway many times waiting to restart car and car would behave normal until the transmission light comes on where i have to power off durango and restart where ever it happens freeway stop signs lights or where ever it decides to happen. I want to get this car running again but dont have the money to fix an issue that i had when it was still covered by the extended warranty and now i have to pay out of pocket because im over the milage so this car has been sitting and i feel as i been ripped off because of al the issues. Im end up dying in this thing if i take it out. Ive donr the recalls and now i see another recall for the same issue that they said they fixed is been reissued im certain all of the issues is tied to the computer unit it i already had one replaced in this car because when i took it for the alternator issue when the car just died with no warning they told me it was fried. All the recall issues that have been mandated are all the issues i had car stalling not starting battery dead and drained fuel relay power module no power how is this not a lemon. this car is a saftey issue on so many levels. Im now trying to get this car running and im still having the same issues i have 129000 miles on it now

The alternator suddenly failed with no warning lights or lamps indicating a problem. I was driving when the car suddenly stalled and I managed to get home . Heavy smoke was coming from under the hood and when I raised the hood heavy smoke was coming up out of the alternator and I could smell burning wires. I had it towed to Elder Dodge and they confirmed that the alternator was burnt out and it fried the PCM and other electrical components. The safety issue was it could've caught the whole engine on fire and it could have caused a wreck with traffic behind us. I contacted the FCA and sent all information that they requested for reimbursement but they refuse to reimburse me for a faulty alternator that short circuited and caught on fire. I am contacting you for help with this matter so they will reimburse me for damages done to my Durango.

With the initial incident with us as we were pulling into a roundabout & braking & turning to the left at some point and turning back to the right the engine stalls. All electrical was still working except for the engine and I smelled an odor of burning wire Seemingly coming from the center of the steering wheel. Shift into neutral hit the ignition and it restarts and everything‘s fine. After a couple times of this happening sporadically, we were notified there was a recall notice seemingly for this problem, but there was a wait while for the recall parts for the solution. With the subsequent occurrences there was no electrical smell, there’s no problem restarting it and there is no problem with any other electrical. Problem occurs with various levels of fuel. We had the recall work performed, and yet the problem still occurs, again, sporadically which makes it difficult to diagnose. The Dodge dealer service dept here, in Billings Montana, cannot seem to reproduce the problem. There are no error codes produced when it happens. It is becoming extremely dangerous. I wonder if anyone else has experienced that faint electrical smell like the first time coming possibly from the steering wheel. I think the recall was related to the fuel pump relay, but I think our problem has something to do with the wiring in the steering column. As I said, intermittently, the engine stalls upon braking with a slight turn to the left & then back to the right. It probably happens roughly once every three or four months and almost always any more pulling into a roundabout. Only once has it occurred other than entering a roundabout.

Durango was T-boned on driver's side. Durango was turning left into a parking lot from a county road. Tahoe was going 50-55+mph in the same lane and direction as Durango. Durango put on left turn signal, slowed down to ~10mph, and turned left towards parking lot. At the same time, Tahoe sped up behind Durango, Tahoe tried to pass Durango on the left. Tahoe collided with left side of Durango, pushing it 30+ feet into the ditch. Tahoe drivers airbag deployed. No airbags deployed in the Durango. The Driver of the Durango smashed head/face into door column, causing concussion, whiplash, contusion to forehead/eyebrow requiring 8 stitches.

There was a whirring noise associated with engine rpm. It appeared to be something driven by the serpentine belt. Within a few miles the battery charge light came on. I figured the noise was then the alternator. I stopped at a parts store to order one and when I came back out, smoke was billowing out of the hood. I opened the hood and it was coming from the alternator. It wasn't slowing down. I asked for some tools from the parts store to unhook the battery and then the smoke started subsiding. It appeared the alternator created a short and the battery was feeding the potential for fire. This all happen in the course of 4 miles. The day before I had parked this vehicle in a parking garage below a college building for a sporting event. This could have caught fire and potentially caused a lot of damage to other vehicles and building. I don't think I would have heard the issue at highway speeds either, and eventually the short caused the entire electrical system to fail (hooked battery up to put car in neutral to load onto tow truck). I currently have the alternator and ordered a replacement (with a core charge). The back of the alternator has some metal that had melted along with the back plastic housing. The bearings seem smooth. I noticed another recall with same vehicle and alternator but with electric hydraulic power steering with the alternator shorting to ground, but my vin is not affected by that.

There was an injury reported. The fire department determined that the fire was caused by an electrical fault. The vehicle was towed to a tow lot. The insurance provider had not determined if the vehicle would be a total loss. The manufacturer was not notified of the failure. The vehicle was not repaired. All current recalls have been performed on this vehicle. While making left and right hand turns after coasting down from a highway speed (55MPH or greater) the vehicle will stall. We will loose all power steering and brakes. No warning lights come on and everything appears normal except the engine is not running. Something in the fuel or electrical system is intermittently failing. I suspect it to be a "Dumb" device. Something that is told to turn on or off but there is no feedback to the computer to set a code when it fails. The vehicle is available for inspection at any time. This started occurring after one of the fuel pump relay recalls. It happens at random and is hard to duplicate, while we do have video of it, the dealer has been unable to duplicate it. The vehicle shows no codes, or anything wrong when it happens. Once the vehicle is stopped, shifted back into park, it will restart and run just fine. This has placed us in unsafe conditions in the middle of the opposing lane, in a turn, trying to restart the vehicle before another car comes around the turn. It is also hard to steer and stop a large SUV when power steering and brakes have been lost. There have been several close calls of getting the vehicle stopped prior to going off the road. The dealer has checked the vehicle over with no solution found for the issue. There has never been any warning lights associated with the vehicle stalling while driving. The vehicle does not know anything is wrong.

Car drove fine for a couple of months with some “minor” issues-lights flash speedometer drops- pull over turn car off- everything ok. Today full electric failure! EVERYTHING stated above happened at once, in addition to feeling like I could not accelerate.

Risk: The water inside could freeze and limit the braking ability of the vehicle, increasing the risk of a crash.

Remedy: Chrysler will notify owners, and dealers will add a water diverter shield to the booster after the booster has been tested to confirm it can hold an acceptable amount of vacuum pressure. If the booster inspection confirms an unacceptable loss of vacuum pressure, the booster will be replaced. Repairs will be made free of charge. The recall began on May 30, 2014. Active Head Rest Inadvertent Deployment

The Office of Defects Investigation (ODI) opened PE19014 on September 9, 2019, to investigate 128 consumer complaints alleging that the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge Durango vehicles.When ODI opened the investigation, ODI was aware of thirteen alleged injuries.A small number of Early Warning Reports identified driver and/or passenger front seat AHR inadvertent deployments without a rear impact event.Reported safety consequences resulting from the AHR deployments include concussions, headaches, soft tissue injuries, disorientation, nausea, vomiting and/or loss of vehicle control as a result of being surprised and distracted by being struck in the head by the inadvertent AHR deployment. On September 24, 2019, ODI sent an Information Request (IR) letter to Fiat Chrysler Automobiles (FCA).ODI requested certain detailed information from FCA related to the following subject vehicles: MY 2010-2014 Dodge Avenger 200, MY 2010-2014 Dodge Calber, MY 2010-2014 Chrysler Sebring, MY 2013-2015 Ram Tradesman, MY 2012 Ram Van, MY 2010-2019 Dodge Grand Caravan, MY 2010-2016 Chrysler Town and Country, MY 2010-2017 Jeep Compass, MY 2011-2020 Dodge Durango, MY 2011-2020 Jeep Grand Cherokee, MY 2010-2019 Dodge Journey, MY 2010-2012 Jeep Liberty, MY 2010-2011 Jeep Nitro, and MY 2010-2011 Jeep Patriot vehicles.On December 2, 2019, ODI received a response from FCA that included vehicle production data, consumer complaints, field reports, warranty claims, manufacturer actions, production changes, and other data related to the alleged defect from the start of production (2010) through September 24, 2019.On January 27, 2020, ODI received an update to the December 2, 2019, IR response from FCA.On September 22, 2020, ODI requested additional subject vehicle AHR information from FCA.ODI received FCA's response on October 21, 2020.ODI subsequently received an update to FCA's October 21, 2020, response on February 16, 2021.During the investigation, ODI requested updates to certain IR questions on two occasions.FCA provided updates to question #2 on March 5, 2021, and questions #2 and #5 on May 26, 2023. FCA’s IR response data identified approximately 7.5 million subject vehicles (with approximately 15 million AHR assemblies).Due to the large number of vehicles and failure reports, ODI focused its analysis on the FCA vehicle model with the highest failure rate to represent the worst-case scenario.For this purpose, ODI focused on MY 2012-2015 Jeep Grand Cherokee vehicles for its analysis.This population includes 710,890 vehicles that FCA manufactured between August 19, 2011, and August 8, 2015.ODI identified 2,839 alleged AHR failures within this limited population.ODI identified 263 alleged injuries that were caused by AHR failures in MY 2012-2015 Grand Cherokees vehicles.Of these alleged injuries, ODI categorized them as eighty-two (82) soft tissue (neck/whiplash, bruise) injuries, four (4) concussions, fifty-nine (59) headaches, and one hundred eighteen (118) without a specific injury description.Although complainants have alleged injuries, ODI has been unable to obtain evidence to substantiate or validate any injuries where medical treatment was required.Consumer outreach was conducted by ODI, where sixty-three (63) FCA complainants with alleged injuries were contacted.Twelve of the sixty-three FCA consumers responded and were interviewed by ODI.ODI requested medical records, however, only one response was provided. Attorneys representing the responding complainant provided a synopsis that indicated they had experienced prior neck-related injuries dating to the mid-1990s.ODI also contacted one hundred (100) consumers who submitted Vehicle Owner Questionnaires (VOQ) in an attempt to validate alleged injuries. Despite the efforts of ODI, the office has been unsuccessful at obtaining substantive documentation/evidence supporting complaints that allege serious injuries (where pre-existing medical conditions did not already exist). In addition to the low severity of alleged injuries caused by AHR deployments, there have been eleven alleged crashes and two alleged lane departures within the entire subject vehicle population that has been in the field up to sixteen years (with over 8,500 AHR inadvertent deployments).ODI was not able to validate that the alleged crashes were the result of inadvertent head rest deployments, and the crashes were minor in nature. ODI has conducted a thorough investigation of the alleged safety defect and with up to sixteen years of exposure for some vehicles and over 8,500 inadvertent AHR deployments, no serious crashes or injuries could be validated in the sub-population of focus.The lack of serious injuries validated by ODI with such large period of field exposure is consistent with thework done by NHTSA’s Human Injury Research Division and the Biodynamic Research Corporation, which support that an inadvertent AHR deployment is not likely to cause serious injury.In addition to the low risk of injury, FCA released an extended warranty program that covers inadvertent AHR deployment for ten years / unlimited mileage from the vehicle's in-service date. PE19014 is closed with the FCA extended warranty program.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the Agency reserves the right to take additional action if warranted by new circumstances. Post 14V-391 Headliner Fires

In response to this investigation, Fiat Chrysler Automobiles (FCA) is conducting a recall to address headliner fires that occur post 14V-391.On December 22, 2015 FCA notified the Office of Defects Investigation (ODI) of safety recall 15V-879 to remedy potential headliner fires occurring in 389,252 model year (MY) 2011-2013 Dodge Durango and Jeep Grand Cherokee vehicles manufactured from December 3, 2009 to September 1, 2012.Note this recall, which was scoped based on vehicle design level and field events, does not include all MY 2013 or any MY 2014 Durango and Grand Cherokee vehicles; ODI and FCA will continue to monitor the field experience for this population.ODI opened this investigation based on allegations of fires occurring in the headliner area after the remedy for recall 14V-391 had been conducted.Recall 14V-391 involved MY 2011-2014 Jeep Grand Cherokee and Dodge Durango vehicles manufactured by FCA from January 5, 2010 to December 11, 2013.The recall was influenced by ODI investigation EA14-001.During EA14-001, data provided by FCA indicated fires were caused by an electrical short in the vanity lamp wiring for either visor.Visors are mounted to the (metal) roof of the vehicle through the headliner with three screws. EA14-001 determined it was possible for the wiring to be pierced or abraded by one of the screws, or otherwise become electrically shorted either during initial vehicle assembly or subsequent headliner area repairs presenting a fire risk.Most of the fires occurred while the vehicle was being driven.The 14V-391 remedy consisted of installing a plastic guide way on each visor to route wiring away from the attachment screws and prevent electrical shorting.Additionally abrasion resistant tape was installed in key areas to protect the wiring integrity.In order to install the guide, the headliner had to be lowered and the existing visor and headliner wiring rerouted, a somewhat complex procedure that required sufficient service technician care and expertise.FCA modified the 14V-391 remedy multiple times to improve it.ODI has identified a total of 43 NHTSA complaints across affected vehicles indicating a fire occurring after the vehicle had been remedied under 14V-391.FCA reported 159 additional unique complaints of post-remedy fires. The fire events primarily occurred while driving however a few reported fires starting after the vehicle was parked. Totally Integrated Power Module Failure

In a letter dated August 21, 2014, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a defect investigation of alleged failures associated with the Totally Integrated Power Module (TIPM) installed in Chrysler SUV?s, trucks, and vans beginning in the 2007 model year. The petitioner alleges that TIPM defects may result in engine stall, airbag non-deployment, failure of fuel pump shutoff resulting in unintended acceleration, fire, and other symptoms. On September 25, 2014, the Office of Defects Investigation (ODI) opened DP14-004 to evaluate the petition for a grant or deny decision. ODI analyzed complaint data provided by CAS as well as complaints submitted to ODI from consumers. In total, there were 296 complaints submitted by the petitioner in the original petition and five supplements, including 271 complaints related to the subject vehicles equipped with TIPM-7. Approximately 3 percent of CAS complaints are related to vehicles equipped with TIPM-6 and ODI?s review of these complaints did not identify any safety defect trends. ODI conducted a detailed review of complaints narratives submitted by CAS and consumers including careful analysis of vehicle repair histories, warranty claims obtained from the manufacturer and any available Customer Assistance Inquiry reports (CAIR). Analysis of the field data indicated that MY2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles exhibited significantly higher complaint rates related to fuel pump relay (FPR) failures than other subject vehicles. In a September 3, 2014 letter to NHTSA, Chrysler submitted a Defect Information Report (DIR) identifying a defect in the FPR within the TIPM-7 which can result in a no start or stall condition in approximately 188,723 model year (MY) 2011 Jeep Grand Cherokee (WK) and Dodge Durango (WD) vehicles manufactured from January 5, 2010 through July 20, 2011 (14V-530).In a February 24, 2015 letter, Chrysler submitted a second DIR expanding the scope of the FPR defect condition to include an additional 338,216 MY 2012 through 2013 Jeep Grand Cherokee vehicles manufactured from September 17, 2010 through August 19, 2013 and MY 2012 through 2013 Dodge Durango vehicles manufactured from January 18, 2011 through August 19, 2013 (15V-115).ODI analysis of the CAS allegations of TIPM defects resulting in stall while driving, airbag non-deployment, unintended acceleration, fire and other faults identified a single defect condition related to 1 of over 60 different circuits in the TIPM assembly. The most common effect of this defect condition, related to the fuel pump relay, was a no-start concern, but it could also result in stall while driving (76 complaints). No valid evidence was presented in support of claims related to airbag non-deployment, unintended acceleration or fire resulting from TIPM faults and these claims were found to be wholly without merit based on review of the field data and design of the relevant systems and components.Except insofar as the petitioner's contentions relate to the defect condition addressed by the Chrysler recalls, the factual bases of the petitioner's contentions that any further investigation is necessary are unsupported. In our view, additional investigation is unlikely to result in a finding that a defect related to motor vehicle safety exists or a NHTSA order for the notification and remedy of a safety-related defect as alleged by the petitioner at the conclusion of the requested investigation. On July 1st, 2014 Chrysler Group LLC (Chrysler) notified the Office of Defects Investigation (ODI) that it would conduct a safety recall to remedy a wiring-related fire hazard on the headliner of approximately 661,888 model year (MY) 2011-2014 Jeep Grand Cherokee and Dodge Durango vehicles manufactured between January 5, 2010 and December 11, 2013 (see NHTSA Safety Recall 14V-391), with notifications to begin in August 2014. According to Chrysler, the fire is caused by an electrical short in the vanity lamp wiring for either one of the sun visors mounted on the vehicle. The sun visors are mounted to the roof of the vehicle through the headliner with three metal screws. It is possible for the sun visor wiring to be pierced by one of these screws either during initial vehicle assembly or later headliner area repairs which may cause an electrical short potentially resulting in fire.ODI opened an investigation involving MY 2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles based on consumer reports of headliners catching fire at the front of the passenger's compartment near the sun visors. Complainants reported failure symptoms that ranged from discoloration to open flame burning of the headliner and/or sun visor material. Complainants reported a burning plastic smell and smoke from the headliner, often occurring while the vehicle was in motion. Damage ranged from a small burn spot on the headliner to a near complete burn of the vehicle resulting in a total loss; sunroof damage/glass shattering occurred in some cases. Incidents are more likely to occur when the ignition key is in the on position.However, several vehicles continued to burn after the key was turned off, and key-off fires, while highly unlikely, cannot be fully eliminated as a potential problem. The earliest incidents chronologically were often associated with vehicle assembly or in-plant repair procedures. Chrysler revised assembly and in-plant repair procedures for the headliner which reduced the number of fields incidents.However, anytime a repair is made to the headliner of the vehicle there could still be a risk of shorting the wiring and creating a hazard.Dealership technicians will inspect the vehicles for damaged wiring caused by an improper service procedure and repair any damage. Also, all vehicles will have a plastic guideway installed on each sun visor that routes the wiring away from the attachment screws preventing the wiring from being shorted. All MY 2011-2014 Grand Cherokee and Dodge Durango built before December 11, 2013 are affected by the recall; vehicles manufactured after this date had the guideway installed during vehicle production.ODI received a total of 6 complaints as of June 2014, all of which reported a fire and two of which alleged related injuries. All 6 incidents occurred while the vehicle was being driven. In each case the driver smelled smoke and witnessed the headliner burning on either the driver or passenger side. Two complaints indicate smoke inhalation injuries to the driver. In response to ODI's information request letters of 8/28/2013 and 2/26/2014, Chrysler provided consumer and warranty data which was received by ODI on 10/7/2013 and 3/19/2014. Analysis of the Chrysler data indicated a total of 91 unique incidents related to the investigation of which about two thirds reported an open flame and smoke, the remaining showed only small burn marks with no visible flame. The Office of Defects Investigation (ODI) has identified 5 reports of vehicle headliners catching fire at the front of the vehicle near the sun visors on Jeep Grand Cherokees.Customers reported a range of fire conditions ranging from minor overheating to an open flame at the headliner and/or sun visor material while driving the vehicle. In some reports the fire spread to the front seats and/or door panels of the vehicle.In one report, the sunroof was damaged causing the glass to shatter.The cause of these fires is an electrical short in the vanity lighting wiring circuit that is routed to either one of the sun visors.The sun visors are mounted to the roof of the vehicle through the headliner with three screws.The sun visor wiring may be penetrated, or pierced by one of these screws either during initial vehicle assembly or subsequent headliner area repairs.The piercing causes an electrical short that could result in a fire.There is no dedicated fuse for the affected circuit so the electrical short can continue until the short clears or the vehicle is keyed off.The Dodge Durango uses the same headliner assembly, and Chrysler indicates there are similar headliner fire incidents affecting this model.
