“The rear passenger side tire hit a pothole and it caused all of the airbags to deploy on April 6, 2026. The impact should not have set the airbags off. This has caused a severe rash/burn on my son's face and neck. The vehicle is now being totaled by my insurance company and forcing me to have to purchase a new vehicle and my insurance rates to go up. Three days later, April 9, 2026, a safety recall on the over-sensitive airbags was issued for this Odyssey. There were no other warnings leading up to this. Insurance has inspected the vehicle and deemed it a total loss.”

“In 2020 I was driving in Sequoia national forest and hit a small pothole/erosion hole in front of a gate at approximately 2-3mph. I slightly bottomed out the car. The airbags deployed. Mild abrasion friction burns on my wrist. My wife's airbags in the passenger seat also deployed, but since we had zero forward momentum they didn't even contact her. I was told by Honda that this is my fault and that their airbag system only deployes when it should, and ultimately just filled an insurance claim. I was extremely ignorant of how not road safe my car was, and drive it home. The car was towed for repairs. I just recently learned about this complain line, so I'm submitting now in early 2024.”

“My SRS light went on without being in an accident. Around the same time I noticed that there was a recall reported (NHTSA Recall Number 26V-227) which states that the vehicle may deploy airbags without cause by going over a pothole etc. I took my vehicle in to the Honda dealership and they confirmed that my SRS ECU needs to be replaced bc it detected an airbag deployment but since it already deployed they will not replace it under the recall. However, the recall states that they need to update the SRS ECU to prevent this exact scenario from happening without proper cause and if it already happened the SRS ECU will need to be replaced. The worst part is, that they cleared the recall through the database for my vehicle but did not actually fix any issues and the SRS system is still not functioning. I already made a complaint with Honda corporation over two months ago but so far they have not provided a resolution.”

EDR data verified this component failure by dealer technicians. ​ 2. Safety risk: Driver airbag spontaneously deployed at 41MPH with zero impact. Seven occupants suffered soft tissue injuries and PTSD; sudden deployment created severe risk of secondary crash. ​ 3. Dealer confirmation: Bay Ridge Honda (Brooklyn NY) diagnosed and confirmed the airbag clip defect via EDR readout and physical teardown, filed service records for this issue. ​ 4. Inspectors reviewed vehicle: Honda dealer techs, EDR forensic specialists, and GEICO adjuster all inspected the car and confirmed no pre-crash damage. ​ 5. Pre-fault warnings: No SRS warning lights, error codes or abnormal electrical symptoms appeared before the airbag deployed unexpectedly.”

“I have had 2 major issues, the idle stop feature has multiple times caused my car to stall. It has even happened on the highway during traffic where the car went into idle-stop mode and would not restart the engine. Last week it happened again on a major road and the only way to restart the car is shift to park in the middle of a busy 4 lane road, turn off the car completely and then attempt a restart. The other issue was with the rear sliding door, my 3 year old son closed the automatic sliding door by pressing the button and his hand was crushed/ stuck until I was able to reopen it. I have called Honda who told me I could have it looked at but it would cost me for the diagnostic and repair if needed due to the fact that it was one month past their 3 year warranty period.”

There was no collision or damage of any sort that led to this malfunction. The following systems no longer work: Adaptive Cruise control Collision mitigation system Lane keeping Assist Road departure mitigation system Blind spot info system Tire pressure monitor system Transmission system All of these failures started at exactly the same time. This severely compromises the ability to use this vehicle safely as these systems are all designed to prevent various safety hazards.”

“When vehicle is completely stopped at a traffic light there have been numerous occasions in which all electrical components fail and the vehicle turns off and fails to restart. This only occurs when the auto idle start is engaged. A few times I’ve had to completely jump the vehicle. I have replaced the battery and also had the car examined at the dealership. They stated they had previously updated the system, had heard of other instances of the same issue but since it wasn’t under recall and they couldn’t replicate the issue they couldn’t do anything. This has proved to be extremely dangerous because at rest, without being able to restart when the light turns green my family and I are at risk of being severely rear ended at high speeds.”

“I am the original owner of a 2020 Honda Odyssey Elite. On July 7, 2026, at approximately 76,127 miles, five safety-system warning messages suddenly appeared simultaneously upon startup: Adaptive Cruise Control Problem, Auto High Beam Problem, Road Departure Mitigation System Problem, Collision Mitigation System Problem, and Lane Keeping Assist System Problem. Restarting the vehicle several times did not clear the warnings. After the vehicle sat for approximately 30 minutes, the warnings disappeared. On July 8, after driving approximately 400 miles home, I parked the vehicle outside in approximately 110-degree heat. About two hours later, upon startup, the same five warnings reappeared simultaneously, along with the Brake System warning light. An authorized Honda dealer confirmed the multiple warnings and performed a DTC scan. Code U3000-49 was stored, indicating an internal failure of the multipurpose camera located behind the rearview mirror. Further testing confirmed that camera replacement was required. The dealer replaced and recalibrated the multipurpose camera at 76,567 miles. The repair cost was $1,631.80 for parts and $717.50 for labor, totaling $2,349.30. I am concerned that failure of a single internal camera component simultaneously disabled multiple driver-assistance and collision-avoidance safety systems. I believe NHTSA should investigate whether multipurpose camera failures associated with DTC U3000-49 represent a recurring safety defect in 2020 Honda Odyssey vehicles.”

“The front-facing Honda Sensing camera in my 2020 Honda Odyssey overheated in a 90deg climate. This triggered multiple dashboard warning lights: Adaptive Cruise Control (ACC) Problem, Lane Keeping Assist System (LKAS) Problem, Road Departure Mitigation (RDM) System Problem, Collision Mitigation Braking System (CMBS) Problem, BRAKE System Problem, and Auto High-Beam Problem. The Honda dealership quoted me $1850 to fix it and they said it was a well-known error. There were NO issues with my car prior to this problem. I keep up with all routine maintenance, oil changes, timing belt and water pump replacements, etc. Not only are these safety features inoperable, but these warning lights are very distracting to the driver, but worse than that- I would be unaware of other system failures since I have so many warnings displayed.”

“No Restart After Auto Start/Stop Engages On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. These complaints concern model year (MY) 2018-2022 Honda Odyssey vehicles manufactured by American Honda Motor Co. (Honda). Honda issued recall 26V227 on April 9, 2026, to address concerns of inadvertent side air bag deployment. In the recall filing, the company stated that “[a]s of April 2, 2026, Honda has had 130 warranty claims, 25 reports of an injury, and no reports of death related to this issue from January 24, 2017 – April 2, 2026.” Honda states that the Supplemental Restraint System (SRS) Electronic Control Unit (ECU) contains incorrect deployment parameters for the side thorax and side curtain air bags, which may cause inadvertent deployment when the vehicle encounters strong road impacts, such as driving over potholes, speed bumps, or road debris. Honda stated that the SRS control logic for the second and third rows of seating contained an insufficient deployment threshold margin, allowing signal inputs to be misinterpreted as side impacts and causing inadvertent deployment of the side thorax and side curtain air bags. Recall 26V227 states that dealers will reprogram or replace the SRS ECU with improved air bag deployment parameters. Consumers received notice of this recall in late May 2026. In view of the recall being conducted by Honda, ODI is closing this Preliminary Evaluation (PE). The Agency reserves the right to take additional action if warranted by future circumstances. “Engine failure On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation.  NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.”
