The contact stated while driving 15 MPH and making a left turn at night, the contact did not watch the opposite direction and another driver that was driving 40-50 MPH crashed into the passenger's side of the vehicle, between the rear passenger's side door and the rear passenger's side wheel. The vehicle was propelled 2-3 feet in the air and landed 10 feet away from the collision location. The contact stated that the other vehicle was propelled more than 100 feet away. The air bags failed to deploy. The contact's wife was occupying the front passenger's side seat while the contact's daughter was in the booster seat in the rear passenger's side seat. The contact exited the vehicle. The contact did not sustain any injury. The contact's wife sustained soreness in the right leg. The contact stated that he was able to take his daughter out of the vehicle; however, the rear passenger's side door was significantly difficult to open. The contact stated that his daughter had hit her forehead against the rear passenger's side window during the incident and was crying significantly. The contact's friend transported the daughter to the hospital, where several tests were performed. The contact was informed that no internal injuries were found and that the pain would subside. The contact stated that his daughter sustained emotional trauma after the incident. The vehicle was towed to the dealer, where it was deemed a total loss. The manufacturer was not notified of the failure. The failure mileage was 34,111.”

“Braking while pulling into a parking space. Instead of slowing down the car accelerated and went over the curb. Maintained pressure on the break but it did not slow the car. Instead the car kept accelerating down an embankment, across an empty street and towards a telephone pole. Turning the steering wheel to the left to avoid the telephone pole the car further descended into a ditch. The car had to be towed and is now deemed to be totaled. The brake- pads had been inspected during a routine maintenance check the day before this accident occurred and they were good. No other vehicle was involved. My personal injury was an abrasion and bruise where my seat belt did it's job. My muscles are sore. The side airbags did deploy after the car stopped. The floor mats were locked in place.”

“Steering suddenly locked up. Car steered to the right went off the road. Turned over several times and landed upside down. Causing serious injuries to myself and the driver. I was in hospital 3 weeks total with broken ribs , broken vertabrae and dislocated shoulder. Cuts on feet from the glass. Driver had cuts on head and dislocated shoulder. Prior to accident, I had reported (twice) to dealer that steering wheel felt loose. The second time, I had him write it down on report when I came in for oil change. So I have proof that I reported it. They checked steering and said they couldn't find anything wrong; but the problem was it didn't do it all the time. Sometimes it would take quite a few seconds before it would react if I would turn it a little left or right. Eventually, it did lock up and caused the horrible accident. The accident occured on November 26, 2020. I typed that date, but somehow this report changed it to Aug 6 and I can't seem to change it.”

The contact stated that while driving at an undisclosed speed, an orange steering symbol appeared on the instrument panel accompanied by the message: "Power Steering Malfunction, Steering Power Low, Visit Your Dealer." An initial inspection by an independent mechanic indicated that the electrical power steering and the rack and pinion assembly needed to be replaced. Subsequently, the vehicle was taken to a dealer, where diagnostic testing confirmed a defective power steering system. Following a software update performed by the dealer, the warning symbol transitioned to red, accompanied by an audible alarm and a significant increase in the physical force required to steer the vehicle. Restarting the vehicle temporarily reverted the warning light to orange and restored steering functionality. Upon investigation, the contact discovered NHTSA Campaign Number: 20V373000 (Steering); however, the VIN was not included in the recall. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline to report the failure. The vehicle was not repaired. The failure mileage was 105,857.”

Claim #060381946 I am experiencing neck, shoulder and rib pain right now, my 5 year old complain about right ankle pain and apparently hit his head on the right side.My 3 year old complain about neck pain yesterday. There was no sign showing any kind of failure on my dashboard. I asked my vehicle to be investigated formally by Toyota engineer asap or forensic expert. I asked my vehicle to be freeze right until proper airbag inspection have been conducted. I have picture of the car showing clearly that none of the airbags worked.”

“The contact owned a 2019 Toyota Rav4. The contact stated while driving 15 MPH and making a left turn at night, the contact did not watch the opposite direction and another driver that was driving 40-50 MPH crashed into the passenger's side of the vehicle, between the rear passenger's side door and the rear passenger's side wheel. The vehicle was propelled 2-3 feet in the air and landed 10 feet away from the collision location. The contact stated that the other vehicle was propelled more than 100 feet away. The air bags failed to deploy. The contact's wife was occupying the front passenger's side seat while the contact's daughter was in the booster seat in the rear passenger's side seat. The contact exited the vehicle. The contact did not sustain any injury. The contact's wife sustained soreness in the right leg. The contact stated that he was able to take his daughter out of the vehicle; however, the rear passenger's side door was significantly difficult to open. The contact stated that his daughter had hit her forehead against the rear passenger's side window during the incident and was crying significantly. The contact's friend transported the daughter to the hospital, where several tests were performed. The contact was informed that no internal injuries were found and that the pain would subside. The contact stated that his daughter sustained emotional trauma after the incident. The vehicle was towed to the dealer, where it was deemed a total loss. The manufacturer was not notified of the failure. The failure mileage was 34,111.”

“My wife was traveling 45 miles per hour and hit a deer and the airbag did not deploy. After review, the air bag is supposed to deploy at 20 miles per hour. I am especially worried because Toyota has issued Technical Service Bulletin, noting that the rail line leaks and can cause air bag failure. I have brought this to Toyota Corporate and Legal's attention and they have completely denied responsibility and implied that this isn't a serious issue. I asked to have the airbag inspected and they refused, saying we could only do that before repairs had been completed. “Braking while pulling into a parking space. Instead of slowing down the car accelerated and went over the curb. Maintained pressure on the break but it did not slow the car. Instead the car kept accelerating down an embankment, across an empty street and towards a telephone pole. Turning the steering wheel to the left to avoid the telephone pole the car further descended into a ditch. The car had to be towed and is now deemed to be totaled. The brake- pads had been inspected during a routine maintenance check the day before this accident occurred and they were good. No other vehicle was involved. My personal injury was an abrasion and bruise where my seat belt did it's job. My muscles are sore. The side airbags did deploy after the car stopped. The floor mats were locked in place.”

“Steering suddenly locked up. Car steered to the right went off the road. Turned over several times and landed upside down. Causing serious injuries to myself and the driver. I was in hospital 3 weeks total with broken ribs , broken vertabrae and dislocated shoulder. Cuts on feet from the glass. Driver had cuts on head and dislocated shoulder. Prior to accident, I had reported (twice) to dealer that steering wheel felt loose. The second time, I had him write it down on report when I came in for oil change. So I have proof that I reported it. They checked steering and said they couldn't find anything wrong; but the problem was it didn't do it all the time. Sometimes it would take quite a few seconds before it would react if I would turn it a little left or right. Eventually, it did lock up and caused the horrible accident. The accident occured on November 26, 2020. I typed that date, but somehow this report changed it to Aug 6 and I can't seem to change it.”

The contact stated that while driving at an undisclosed speed, an orange steering symbol appeared on the instrument panel accompanied by the message: "Power Steering Malfunction, Steering Power Low, Visit Your Dealer." An initial inspection by an independent mechanic indicated that the electrical power steering and the rack and pinion assembly needed to be replaced. Subsequently, the vehicle was taken to a dealer, where diagnostic testing confirmed a defective power steering system. Following a software update performed by the dealer, the warning symbol transitioned to red, accompanied by an audible alarm and a significant increase in the physical force required to steer the vehicle. Restarting the vehicle temporarily reverted the warning light to orange and restored steering functionality. Upon investigation, the contact discovered NHTSA Campaign Number: 20V373000 (Steering); however, the VIN was not included in the recall. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline to report the failure. The vehicle was not repaired. The failure mileage was 105,857.”
