On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles. The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations. However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury. The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources. In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged. In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. “OUR BRAND NEW 2019 HONDA ACCORD, WITH ABOUT 1,000 MILES ON IT, WAS INVOLVED IN A FRONT END MOTOR VEHICLE COLLISION ON OCTOBER 13, 2019 AT 1:24 PM ON A CITY STREET WITH A SPEED LIMIT OF 45 MPH. WE STRUCK A 2001 F250 AT 45 MPH IN THE SIDE AFTER THE INDIVIDUAL FAILED TO YIELD AND STOPPED SIDEWAYS DIRECTLY IN OUR LANE OF TRAFFIC. MY HONDA RECEIVED SERIOUS FRONT END DAMAGE AND THE ENGINE COMPARTMENT WAS SMOKING, BUT NOT A SINGLE AIR BAG DEPLOYED DURING THIS ACCIDENT. THERE WERE THREE (3) ADULTS AND ONE (1) CHILD IN THE HONDA, THE CHILD AND TWO (2) ADULTS WERE SENT TO THE EMERGENCY ROOM VIA AMBULANCE. THERE WERE NO MAJOR INJURIES OTHER THAN SEVERE BRUISING AND SOME PULLED MUSCLES WHICH WERE CAUSED BY THE SEAT BELTS. THE FAILURE OF THE AIR BAG DEPLOYMENT DURING A HEAD ON COLLISION OF THIS PROPORTION IS VERY DISTURBING AND AS A RESULT WE HAVE LOST COMPLETE FAITH AND CONFIDENCE IN HONDA'S SAFETY FEATURES. WE ARE VERY THANKFUL THAT THE SEAT BELTS PREVENTED US FROM SLAMMING OUR HEADS INTO THE DASH AND WINDSHIELD, ESPECIALLY SINCE OUR AIR BAGS FAILED US.”

“While driving on a highway in snowy conditions, the vehicle experienced a brief loss of traction and slid across lanes. There was no confirmed collision with another vehicle, object, or obstacle. During this event, the passenger-side curtain airbag and passenger seat airbag deployed unexpectedly. There were no warning lights, messages, or prior symptoms indicating any issue with the airbag or safety systems before the incident. After the event, the vehicle was inspected by an authorized dealership. The inspection confirmed that the airbags had deployed; however, no diagnostic trouble codes (DTCs) were found in the system, and no system malfunction was identified. The dealer suggested that “impact seems” to be present based on visual observation, but no specific collision or impact event was confirmed. I expressed concerns about the safety implications of this incident and requested further diagnostic evaluation to determine the specific cause of the airbag deployment. However, no additional diagnostic analysis was performed, and no definitive explanation was provided regarding the triggering event. The cause of the airbag deployment is UNKNOWN. The system activated without a clearly identified impact or recorded fault, raising concern about whether the airbag system may deploy under loss-of-traction conditions without a qualifying collision. This unexpected airbag deployment created a potential safety risk, as it occurred during vehicle instability and could have further impaired driver control or visibility. The vehicle is available for inspection upon request.”

“The contact owns a 2019 Honda Accord. The contact stated that while driving at 45 MPH with activated cruise control, the vehicle independently came to a complete stop as a brake warning light flashed on the instrument panel. Due to the failure, the contact was hit from behind by another vehicle. The air bags did not deploy upon impact. The contact suffered whiplash as a result of the crash; however, no medical attention was given at the scene. The contact spoke with a doctor and received a prescription for her neck pain. A police report was filed at the scene. The contact was able to drive the vehicle home from the scene but later had the vehicle towed to an independent body shop. The contact notified the manufacturer about the failure; the manufacturer denied that the failure was caused by a mechanical failure and believed the failure was caused by a driver error. The vehicle had yet to be repaired. The failure mileage was approximately 25,000”

THE ENTIRE FRONT SIDE OF MY VEHICLE IS COMING OFF AND I WAS JERKED AROUND IN THE CAR, BUT NO AIRBAG IN THE BACKSEATS NOR DRIVER SEAT CAME OUT, AND MY DAUGHTERS CAR SEAT WAS FLIPPED TOWARDS THE WINDOW SO THANK GOD SHE WAS NOT IN THE VEHICLE WHEN THIS HAPPENED.”

“Service Bulletin - American Honda is extending the warranty on the A/C condenser to 10 years from the original date of purchase with unlimited miles. This warranty extension only covers vehicles that have a defective A/C condenser from the factory. The A/C condenser was not manufactured to specification. As a result, corrosion may develop in the form of tiny holes in the condenser tube walls that allow the refrigerant to leak out. This does not apply to any vehicle that has a leaking condenser due to foreign object damage. It also does not apply to any vehicle that has ever been declared a total loss or sold for salvage by a financial institution or insurer, or has a branded or similar title under any state's law.”

“I HAVE TRIED CONTACTING HONDA AND GOING TO THE DEALERSHIP MULTIPLE TIMES TO GET THIS TAKEN CARE OF BUT EVERYTHING IS PERFECT THEY SAY. JUST YESTERDAY THE CAR HIT THE BREAK ON IT'S OWN WHEN THE CAR ON THE OTHER SIDE WAS CLEARLY ON THE OTHER LANE THE CAR MADE A HORRIBLE SUDDEN STOP THAT CAUSED BRUISING TO MY NECK AND ALSO MY PASSENGER GOT HURT WITH EVERYTHING THAT FLUNG FROM THE BACKSEAT HE SAID THE TUG WAS TOO MUCH . WE THOUGHT SOMEONE HAD HIT ME . THIS ISN'T THE FIRST TIME THE CAR HAS DONE THIS I FEEL I GET CONSTANT TIRE POPS BECAUSE OF THIS CAR VEERS ME OFF INTO A LANE THAT ISN'T EVEN THERE. I HAVE WITNESSES THAT SAY I HAVE A LEMON CAR I ALWAYS SAY NO BUT THIS LAST INCIDENT WAS TOO MUCH FOR ME . ALSO THE CHARGER PORT NOT USB HAS NOT WORKED SINCE DAY ONE ANOTHER THING THEY SAID WAS WORKING AGAIN NOTHING . I WENT BACK AND THEY SAID I HAD NO WARRANTIES . I HAVE CALLED HONDA TRYING TO GET TO THE BOTTOM OF THIS AND GOING TO THE DEALER THEY SAID U SIGNED SO TAKE CARE OF IT . AT THIS POINT I AM IN THE PROCESS OF PURCHASING A NEW VEHICLE CASH AND I AM NOT EVEN 1% INTERSD IN KEEPING THIS CAR . IF I HAVE TO RETURN VOLUNTARILY I WILL HAVE TO . I HAVE BEEN PATIENT BUT IT'S ABSOLUTELY RIDICULOUS THAT I WAS PUT INTO THIS VEHICLE BEING A SUPPOSEDLY UPGRADE ? FROM MY 2017 HONDA COUPE V6 . THIS CAR NEEDS A RECALL ON LANE DEPARTURE , SENSORS , ELECTRICAL IT IS NOT A CAR I FEEL SAFE IN . I DO NOT TRUST TO LET IT DRIVE ON IT'S OWN AND FOR GOOD REASON . WHAT AM I PAYING FOR THEN ? I WOULD HAVE CRASHED INTO A CAR BY NOW IF I TRUSTED IT . I HAVE PROOF OF THE THREE TIRES I HAVE HAD TO GET IN THE PAST THEEE MONTHS AND I DON'T DRIVE MUCH AT ALL 9K MILES IN A WHOLE YEAR . PICS ON BOTTOM SHOW HOW THE DAMAGE COMES FROM THE SIDES DUE TO THE CAR GOING STRAIGHT THEN MAKING SUDDEN TURNS THAT HIT CURBS . THE SENSORS SEEM TO BE GETTING WORSE INSTEAD OF BETTER .”

I was about to pass a green light when my 2019 Honda accord sport started showing the brake signal on my dash. My car suddenly brakes and comes to a complete stop by itself in the middle of the road. There were no cars in front or on either side of me, no potholes or anything of that matter that would’ve caused this to happen. This caused a pick-up truck, to hit me from behind. The emergency brake light appeared at the exact same time as the car's emergency brakes applied. The driver of the other car confirmed there was nothing in front of me. The Honda dealer where I brought the car the next day stated there was nothing wrong with the camera or radar system. This failure caused an accident, and could have been much worse.”

The records describe a possible crash, fire, injury, or loss of control.
The records describe a possible crash, fire, injury, or loss of control.

“WHILE I WAS ON THE WAY HOME FROM WORK, I WAITED FOR THE BROKEN YELLOW LINE TO PASS 2 CARS IN FRONT OF ME, WHEN I WAS MIDWAY THROUGH PASSING BOTH OF THEM, THE SECOND CAR IN LINE WANTED TO PASS THE CAR IN FRONT AS WELL BUT DID NOT LOOK TO SEE THAT I WAS ALREADY PASSING AND RIGHT BESIDE HIM, AND JERKED OVER AND HIT THE SIDE OF ME WHICH CAUSED ME THEN TO SPIN OUT OF CONTROL AND LAND IN A RAVINE.

“On Thursday November 16th at around 8:00 am I was driving down La Cienega Blvd going around 30-40 mph. I am now suffering with severe neck and shoulder pain from this unexpected incident. My 1 year was also in the car with me. My car is now being inspected at the Honda dealership where it was purchased.”

“Travelling at approximately 47 mph, with nothing in front of me, the car's emergency braking system engaged completely unexpectedly, slamming my car to a stop.

“Inadvertent Automatic Emergency Braking
This does not apply to any vehicle that has a leaking condenser due to foreign object damage. It also does not apply to any vehicle that has ever been declared a total loss or sold for salvage by a financial institution or insurer, or has a branded or similar title under any state's law.”
