“on 3/6/2020 I was in a serious car accident in the vehicle. which required surgeries for not only myself but also other drivers on the road. The driver air bags did not deploy, the turck was totaled and had body damage around the entire vehicle. There was a blood stain from my head hitting the driver door window but no air bag deployment. The gas pedal was not attached when reviewing the photos taken. The collision detection did not detect. we have asked Ford Motor Company to come and look at the car as it has been in a protective hold but the response from FMC was that due to covid they would not send anyone and we were left on out own. There were no warning messages, lamps, lights or indicators prior to the collision.”

THE VEHICLE WAS T-BONED ON THE FRONT DRIVER'S SIDE DOOR WHILE DRIVING AT 55 MPH AS A RESULT THE VEHICLE WENT DOWN INTO A DITCH AND CONTINUED TO SLIDE INTO BRANCHES AND BUSHES. THE CONTACT STATED THAT ALL AIR BAGS DEPLOYED EXCEPT FOR THE DRIVER'S SIDE FRONTAL AIR BAG. THE CONTACT ALSO MENTIONED THAT THE DRIVER'S SIDE SEAT HAD ALSO SEPARATED FROM THE SEAT FRAMING. THE WHOLE DRIVER'S SIDE WAS SMASHED IN WITH THE ROOF DAMAGED INCLUDING THE SIDE AND BACK WINDOWS. THE FRAME WAS ALSO DAMAGED. THE CONTACT WAS TRANSPORTED TO THE HOSPITAL BY AMBULANCE FOR NECK INJURIES. THE OTHER DRIVER AND HER PASSENGER WERE AIR LIFTED AND TRANSPORTED BY AMBULANCE TO THE HOSPITAL AS WELL. A POLICE REPORT WAS FILED. THE INSURANCE COMPANY WAS STILL INVESTIGATING AND HAD NOT YET OFFICIALLY TOTALED THE VEHICLE. THE VEHICLE WAS TOWED TO A TOW YARD. THE DEALER DEERY BROTHERS FORD LINCOLN (2343 MORMON TREK BLVD, IOWA CITY, IA 52246 319-337-4600) WAS CONTACTED IN REGARDS TO THE AIR BAG FAILURE HOWEVER, OFFERED NO ASSISTANCE BUT REFERRED THE CONTACT TO THE INSURANCE COMPANY. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURES. THE FAILURE MILEAGE WAS 5,000.”

“I drive a 2019 Ford F-150 with the 3.5L engine and 10 speed transmission. I have received a warning for low fuel pressure, trouble starting, and the truck has cut off once after turning over. It never fails to crank, however, it may take 5 or so seconds of cranking to get it to start. I have had the check engine light come on one time over the last couple of months of the issues starting; the code it showed was a P00C6. I have not brought the vehicle in for service as it just started a couple months ago when it got hot outside and there was a recent recall that did not include this VIN but matches the issue exactly. The warning states to not drive the vehicle and to get it towed if fuel is not low. The recall states there is not a current fix for the issue and I need my vehicle. This is a daily problem, mostly after the vehicle sits for a couple of hours. I am also having an issue with the cam phaser rattle upon startup and rattling while driving. I have noticed some power reduction and it has progressively gotten worse over time. I am at 135,000 miles currently and have experienced this issue for the last 50,000 miles or so.”

“On [XXX] with an empty vehicle besides I myself the driver, going south bound on [XXX] just north of twin falls Idaho I had cruise control set to 81 MPH when I felt a slow to the vehicle then noticed an uncontrolled/unprompted shift and loss of power indicated on the dash cluster jumping from 10-9-7-10-5-3-8-9 gears then transmission temperature went from 176 degrees to 205 degrees for the remainder of the trip. And it did not shift higher than 9th gear while cruise control was engaged even at lower speeds. The transmission then began to experience harder shifting, and gained a shutter like a clutch pack not engaging properly form 1-3rd gear, and sporadic shifting under light normal driving conditions at random intervals and remains to now have a normal operating temp of 204 degrees under normal driving conditions. Took it to be diagnosed at a ford dealer and it was the CDF drum that lead to a a cascade of issues leading to the transmission needing replacement. This resulted in a safety concern when pulling out into traffic and the vehicle would stop moving. As the problem worsened shift points got harder and the truck computer would show that it was shifting from a high gear to first gear at highway speeds. A reputable transmission shop inspected the vehicle and removed the transmission. They confirmed that a sleeve or bushing in the CDF drum had moved and disrupted oil flow to the clutch packs.”

“Windshield trim molding Item detachment On January 27, 2023, the Office of Defects Investigation (ODI) opened PE23-001 to investigate allegations of A-pillar trim panel detachment on model year (MY) 2011-2019 Ford Explorer vehicles. At the time of opening, the office had received 164 Vehicle Owner Questionnaires (VOQs) reporting this failure. ODI has received 671 VOQs regarding this failure, including 1 alleged crash and 2 alleged injuries. Some consumers have reported that the failure occurred while they were driving at highway speeds, with the detached trim panel entering the path of travel of the vehicle behind them, requiring the driver of the following vehicle to take evasive action. It is noteworthy that motorists who experience this type of road hazard are less likely to accurately identify the subject vehicle the item detached from and report the event to the NHTSA. In its April 17, 2023 response to ODI’s information request, Ford provided 175 consumer complaints (including field reports) and 14,162 warranty claims related to A-pillar trim panel detachment. Ford relayed that the subject vehicles utilize a plastic A-pillar bracket, which is bolted to the vehicle structure during assembly. The A-pillar trim panel is attached to the bracket utilizing 7 plastic clips. Contributory factors which have been identified by Ford as leading to the subject defect include incomplete installation at the assembly plant, inadequate part stack up, damage to fasteners during installation, and failure to follow proper repair procedure during windshield replacements or other services. On January 19, 2024, Ford filed safety recall 24V-031 for MY 2011-2019 Ford Explorer vehicles to address the subject defect. In the Part 573 Safety Recall Report, Ford identified that a detached A-pillar trim panel can create a road hazard for other road users, increasing the risk of a crash. The recall remedy includes an inspection of the existing A-pillar trim panel and, if necessary, the replacement of the component. The remedy A-pillar trim panels will be installed with an additional adhesive for robustness and utilize an inspection procedure to ensure full engagement of all the retention clips during installation. In view of the recall action being taken by Ford, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances. “on 3/6/2020 I was in a serious car accident in the vehicle. which required surgeries for not only myself but also other drivers on the road. The driver air bags did not deploy, the turck was totaled and had body damage around the entire vehicle. There was a blood stain from my head hitting the driver door window but no air bag deployment. The gas pedal was not attached when reviewing the photos taken. The collision detection did not detect. we have asked Ford Motor Company to come and look at the car as it has been in a protective hold but the response from FMC was that due to covid they would not send anyone and we were left on out own. There were no warning messages, lamps, lights or indicators prior to the collision.”

“I drive a 2019 Ford F-150 with the 3.5L engine and 10 speed transmission. I have received a warning for low fuel pressure, trouble starting, and the truck has cut off once after turning over. It never fails to crank, however, it may take 5 or so seconds of cranking to get it to start. I have had the check engine light come on one time over the last couple of months of the issues starting; the code it showed was a P00C6. I have not brought the vehicle in for service as it just started a couple months ago when it got hot outside and there was a recent recall that did not include this VIN but matches the issue exactly. The warning states to not drive the vehicle and to get it towed if fuel is not low. The recall states there is not a current fix for the issue and I need my vehicle. This is a daily problem, mostly after the vehicle sits for a couple of hours. I am also having an issue with the cam phaser rattle upon startup and rattling while driving. I have noticed some power reduction and it has progressively gotten worse over time. I am at 135,000 miles currently and have experienced this issue for the last 50,000 miles or so.”

“While driving on a straight roadway the right rear wheel hub broke off of the rear axle. This caused the truck to turn to the left out of control. The truck crossed over the center line while the entire wheel/tire separated from the wheel well of the truck. The wheel/tire traveled another 50 yards off the roadway into a field. The truck struck a hill on the other side of the road deploying the side airbags due to impact with a road sign. The wheel hub/axle was inspected by a mechanic and the insurance company. “On [XXX] with an empty vehicle besides I myself the driver, going south bound on [XXX] just north of twin falls Idaho I had cruise control set to 81 MPH when I felt a slow to the vehicle then noticed an uncontrolled/unprompted shift and loss of power indicated on the dash cluster jumping from 10-9-7-10-5-3-8-9 gears then transmission temperature went from 176 degrees to 205 degrees for the remainder of the trip. I proceeded to pull over and let the transmission rest for 45 mins then proceeded with my trip after not seeing any exterior damage or leaks. And it did not shift higher than 9th gear while cruise control was engaged even at lower speeds. The transmission then began to experience harder shifting, and gained a shutter like a clutch pack not engaging properly form 1-3rd gear, and sporadic shifting under light normal driving conditions at random intervals and remains to now have a normal operating temp of 204 degrees under normal driving conditions. Took it to be diagnosed at a ford dealer and it was the CDF drum that lead to a a cascade of issues leading to the transmission needing replacement. This resulted in a safety concern when pulling out into traffic and the vehicle would stop moving. As the problem worsened shift points got harder and the truck computer would show that it was shifting from a high gear to first gear at highway speeds. “Windshield trim molding Item detachment On January 27, 2023, the Office of Defects Investigation (ODI) opened PE23-001 to investigate allegations of A-pillar trim panel detachment on model year (MY) 2011-2019 Ford Explorer vehicles. At the time of opening, the office had received 164 Vehicle Owner Questionnaires (VOQs) reporting this failure. ODI has received 671 VOQs regarding this failure, including 1 alleged crash and 2 alleged injuries. Some consumers have reported that the failure occurred while they were driving at highway speeds, with the detached trim panel entering the path of travel of the vehicle behind them, requiring the driver of the following vehicle to take evasive action. It is noteworthy that motorists who experience this type of road hazard are less likely to accurately identify the subject vehicle the item detached from and report the event to the NHTSA. In its April 17, 2023 response to ODI’s information request, Ford provided 175 consumer complaints (including field reports) and 14,162 warranty claims related to A-pillar trim panel detachment. Ford relayed that the subject vehicles utilize a plastic A-pillar bracket, which is bolted to the vehicle structure during assembly. The A-pillar trim panel is attached to the bracket utilizing 7 plastic clips. Contributory factors which have been identified by Ford as leading to the subject defect include incomplete installation at the assembly plant, inadequate part stack up, damage to fasteners during installation, and failure to follow proper repair procedure during windshield replacements or other services. On January 19, 2024, Ford filed safety recall 24V-031 for MY 2011-2019 Ford Explorer vehicles to address the subject defect. In the Part 573 Safety Recall Report, Ford identified that a detached A-pillar trim panel can create a road hazard for other road users, increasing the risk of a crash. The recall remedy includes an inspection of the existing A-pillar trim panel and, if necessary, the replacement of the component. The remedy A-pillar trim panels will be installed with an additional adhesive for robustness and utilize an inspection procedure to ensure full engagement of all the retention clips during installation. In view of the recall action being taken by Ford, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances.
