[XXX] - I was on the way to work on a clear and sunny morning. After stopping at a traffic light, vehicles started moving forward through the intersection. Traffic was building and the lady in front of me stopped very quickly. When I pressed the the brake pedal to stop behind her nothing happened. The brakes did not brake! The pedal went all the way to the floor. It didn't even try to slow down. I just rolled right into the back of her. The front end of my 2014 Nissan LEAF sustained significant damage and the rear bumper of her SUV sustained minor damage. Both of us had minor neck/back pain after. I am still recovering from spine surgery and I'm concerned about any long term issues that may be caused from this accident. We were in a very busy area of Atlanta and it is amazing that no one was seriously hurt. After calling 911 and being on hold for close to 30 minutes State Farm advised it was okay to swap info without an official police report. I don't want this accident to count against my accident free/good driver discount with my insurance. There were no warning lights on my dash or audible brake sounds. I have had similar problems in the past with the brakes not engaging. It has been to the dealership at least 4 times for brake related issues in the 10 years that I have had this car. The dealership and 1 independent garage have inspected my brake system, including fluid, pads and rotors. Everything is in good condition. In the past there was a correlation between the 12V battery power and the brakes not working. It has not been possible to recreate the problem on demand. I've reported this very serious safety issue to Nissan Consumer Affairs. They deny responsibility, but will not provide it in writing. A basic google search shows that I am not the only LEAF owner that has this problem. It is parked in my driveway and is considered unsafe to drive unless it is repaired per the dealership. I am glad that no one else in my family was injured because of this. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Complete braking system failure. After putting the car into reverse, only the emergency brake and parking system were functional. Once taken out of park, the vehicle was effectively free-rolling and nearly hit a structure. While in-gear, the brake sensation was that of having ABS actively working, and the brake needed to be smashed to the floor to get any functional braking. The fearful part was the car was still applying powertrain effort, without any accelerator input. Yellow Brake Warning Light, Red BRAKE light, VDC Warning Light, Red/Yellow Master Warning Light were all illuminated. This had not been brought up with the dealer, but a terrifying failure mode of electronic braking systems.

The power steering has intermittently been de-activating. There is a warning light that comes on (yellow triangle with an exclamation mark). When this happens it causes the steering wheel to feel like it’s pulls in a direction. After we suddenly lose power steering with no warning we’re left to drive a car with no power steering until it suddenly re-engages. There has been no rhyme or reason to when this happens. The issue has been diagnosed at a factory dealership. The car has not sustained any accident (ever) in the +/- 4 years we’ve owned it

Brake Failure - ABS noise, there is zero brake action. Pedal goes all the way to the floor and nothing happens. Car makes an ABS noise but there is zero brake action. I reported this to Nissan dealer twice at first, they couldn't find a problem now their saying it maybe the ABS Control Unit. They want to replace the ABS Control Unit at $2,200 but does not guarantee that's the problems. The Nissan dealer states that the car is not safe to drive. even if I pay them $2,200 to replace the ABS control unit.

My car suddenly accelerated out of my control and my car was totaled in a supermarket parking lot. Amazingly no serious injuries though two other cars were damaged by this event. No warnings and no time to act. I demanded that Nissan do an inspection. The email I got from them this morning said they found nothing wrong with the vehicle BUT that the accelerator was at 100% at the time of the accident. I was parked and getting out of my car when this occurred but Nissan did not think it suspicious that my car accelerated to 100% without any reason for me to accelerate and my foot not on the power pedal. Their letter to me claimed no responsibility for the malfunction. In fact. They are claiming no malfunction at all. I have requested in writing to get all the data they collected and although they had previously agreed to provide it to me. They are now backpedaling on that now that I’ve posed legitimate questions. This happened TWICE in a 15 month period and the last accident destroyed my car. Everyone was eager to blame me for the first accident that did thousands of dollars damage, which I know for a fact I was parked in my own driveway and not even driving. It was scary and stressful and still is. The only thing I can feel good about is that the car, my beautiful LEAF, is off the road forever and will not be dangerous any longer to me or anyone else.

I WAS DRIVING MY 2014 NISSAN LEAF SL AT A LOW SPEED (5-10 MPH) WHEN THE BRAKES HAD A SUDDEN AND UNANTICIPATED TOTAL BRAKE FAILURE. WHILE PULLING UP TO A STOPLIGHT TO MAKE A RIGHT TURN, I WAS UNABLE TO STOP THE CAR FOR APPROXIMATELY 10 FEET BEYOND WHERE I HAD INTENDED. FORTUNATELY, THERE WERE NO OTHER VEHICLES OR PEDESTRIANS IN MY PATH. THIS EVENT HAPPENED IN DOWNTOWN SALT LAKE CITY. FROM THE POINT OF FAILURE, I WAS ABLE TO PREDICTABLY STOP THE CAR WITH SOME EXERTION. BUT CLEARLY, A FUNDAMENTAL MAJOR COMPONENT OF THE VEHICLE FAILED. IT WAS NOT PART OF THE BRAKES THEMSELVES, OR THE ROTORS, OR WEAR. I DROVE THE VEHICLE DIRECTLY TO KEN GARFF NISSAN, WHICH WAS APPROXIMATELY 5 BLOCKS AWAY. THEY DIAGNOSED THE PROBLEM AND REPORTED THAT IT WAS A FAILURE OF A MAJOR ELECTRICAL COMPONENT THAT PROVIDES BRAKING TO THE BRAKE SYSTEM. I CONTACTED NISSAN ABOUT THE ISSUE, AND REPORTED TO THEM THAT AFTER SEARCHING THE INTERNET I HAD SEEN OTHER REPORTS FROM INDIVIDUALS ABOUT SIMILAR PROBLEMS WITH THEIR NISSAN LEAF BETWEEN THE MODEL YEARS 2013-2015. THERE WERE REPORTS OF THIS PROBLEM AFFECTING MORE THAN 40 VEHICLES. NISSAN USA EXPLAINED THAT THEY DIDN'T HAVE ANY RECALLS FOR THAT ISSUE, AND THAT I WOULD SIMPLY HAVE TO PAY FOR THE REPAIR. HOWEVER, I FELT THAT THIS EXPERIENCE, BECAUSE IT WAS SO DANGEROUS, AND SO SUDDEN, AND WASN'T A RESULT OF POOR MAINTENANCE OR AN ERROR OF ANY SORT, IT SHOULD BE ESCALATED AND EVALUATED FOR RECALL. NISSAN USA SHOWED NO INTEREST OR INTENTION OF MAKING FURTHER INQUIRY INTO THE PROBLEM, SO I FELT I SHOULD PUSH FOR A RECALL. MY CASE NUMBER WITH NISSAN USA WAS ORIGINALLY ASSIGNED AS #436-319-65, AND A NEW CASE NUMBER WAS ASSIGNED AS #436-368-82. THE INDIVIDUALS ASSISTING IN THE CASES WERE NAMED PAMELA AND NANCY, RESPECTIVELY.

AFTER COMING FROM WORK (54 MILE BACK AND FORTH) ON 11/4/2020, PARKED 2014 LEAF SV AT THE DRIVEWAY FOR 40 MINUTES. THEN WENT BACK IN, POWERED THE CAR AND SHIFTED TO REVERSE. THE CAR BACKED-UP FAST BECAUSE THE BRAKE SEEMED NOT WORKING. SO I HAVE TO STEPPED ON THE PEDAL HARDER. I KNOW SOME THING IS WRONG BECAUSE WHEN YOU STEP ON THE BRAKES A NOISE A CAN BE HEARD AT THE FRONT. I CHECKED AND IT SEEMED LIKE ITS ACTUATING SOMETHING BUT NOTHING IS HAPPENING. THE PEDAL IS DEEP. THE DASHBOARD BRAKE LIGHT IS ON AND YELLOW WARNING LIGHT. I TURNED IT OFF AND AFTER 1 HOUR I RECHECKED AND IT GOT BACK TO NORMAL OPERATION. BUT THIS WAS REPEATED SEVERAL TIMES DURING COLD WEATHER. UPON STARTING THE CAR IN THE PARKING LOT, THE BRAKE PRESSURE DID NOT RELEASE AS USUAL WHEN STARTING THE CAR. WHEN DRIVEN IN THE PARKING LOT AT LOW SPEEDS, THE BRAKE DOES NOT FUNCTION AS IT SHOULD. THERE IS SO MUCH PRESSURE THAT I CAN ONLY PRESS THE PEDAL DOWN A LIMITED AMOUNT TO MAKE THE CAR STOP. AFTER A FEW DAYS OF DRIVING THE CAR LESS THAN 10 MILES TOTAL, THE PROBLEM HAS WORSENED AND THERE IS LESS ROOM TO PRESS THE PEDAL DOWN WHILE BRAKING. I TOOK THE CAR TO GET THE BRAKES INSPECTED AND WAS TOLD THAT EVERYTHING IS FUNCTIONING AS IT SHOULD. I AM WORRIED THE ISSUE WILL WORSEN TO THE POINT THAT THE BRAKES WILL NOT BE ABLE TO BE PRESSED DOWN AT ALL.

WHEN THE CAR WAS TURNED ON IT WOULD MAKE A LOUD HUMMING NOISE. WHEN YOU PUT THE CAR IN REVERSE IT WOULD ROLL BACK, ONCE YOU TAP ON THE BREAKS TO STOP IT WOULD KEEP ROLLING BACK. ONLY WAY IT WOULD STOP WAS BY SLAMMING ON THE BREAKS. SAME THING WOULD HAPPEN WHEN GOING FORWARD. MY NISSAN LEAF HAS HAD MAJOR ISSUES WITH THE WINDSHIELD WIPER MOTOR. IF THERE IS ANY AMOUNT OF ACCUMULATED SNOW, EVEN A SMALL AMOUNT, AN AMOUNT ON OTHER CARS I HAVE DRIVEN DOES NOT STOP THE WIPERS OR BREAK THE WIPER'S MOTOR. IN LESS THAN ONE YEAR MY WIPER'S MOTOR HAS BROKEN THREE TIMES. THE LAST TIME THE MOTOR BROKE I WAS COMING HOME IN A SNOW STORM. I COULDN'T SEE BECAUSE THE WIPERS QUIT WORKING. I TRIED TO SLOWLY PULL OVER BECAUSE I WAS ON A BUSY STREET. I HIT A PEDESTRIAN BECAUSE I COULD NOT SEE AND WAS AFRAID TO STOP ON A BUSY ROAD. NISSAN REPAIR SHOP BASICALLY TOLD ME THERE CAN BE NO SNOW ON YOUR WINDSHIELD OR THE WIPERS MOTOR WILL BREAK. THERE WAS VERY LITTLE ACCUMULATED SNOW ON THE WIND SHIELD. AS I SAID, I HAVE NEVER HAD THIS PROBLEM WITH ANY OTHER CAR. I FEEL LIKE WINDSHIELD WIPERS SHOULD BE ABLE TO HANDLE AN 1/8 OF AN INCH OF SNOW WITHOUT BREAKING THE MOTOR. YOU COULD BE DRIVING AND SNOW COULD DROP FROM A TREE ONTO YOUR CAR. *TR

CHILD LOCKS ISSUE: MY FAMILY AND I WERE GOING TO A RESTAURANT, AND, UPON ARRIVAL, MY MOTHER WAS UNABLE TO EXIT. I REALIZED THAT THE CHILD LOCK DEVICE LEVER IN OUR 2014 NISSAN LEAF WAS OUT OF THE CHILD-UNLOCKED, OR NORMAL-EGRESS, POSITION. I ALSO REALIZED THAT THIS WAS A SERIOUS SAFETY CONCERN. I INVESTIGATED THE DOOR CHILD-LOCK MANUAL LEVER ON THE LEAF AND INVENTED A DEVICE TO KEEP THE LEVER POSITION IN THE CHILD-UNLOCKED CONDITION UNTIL WE DESIRED TO REPOSITION IT (WHEN TRAVELING WITH SMALL CHILDREN). I ALSO INVENTED THE ALTERNATE DEVICE, THAT IS, TO MAINTAIN THE CHILD-LOCKED POSITION. THIS DEVICE WAS DESIGNED TO RESTRAIN THE LEVER AND INDICATE THE CHILD-LOCK CONDITION AT A GLANCE; THIS IS WHY THE LEVER KNOB IS VISIBLE WHEN THE OBJECT IS INSTALLED AND COLORS WERE USED TO AFFIRM THE STATUS. FOR OUR NISSAN LEAF, I USED A GREEN RUBBER INSERT FOR THE CHILD-UNLOCKED POSITION, AND A RED RUBBER INSERT FOR THE CHILD-LOCKED POSITION. THIS INVENTION, ALONG WITH SEVERAL VARIATIONS TO ACCOMMODATE DIFFERENT ACTUATORS AND INDICATION PARADIGMS FOR INDICATING STATUS, WAS RECENTLY SUBMITTED TO THE US PATENT OFFICE, AND IS THUS ?US PATENT PENDING.? THERE IS AN AIR BAG LIGHT THAT ILLUMINATES WHEN THERE IS WEIGHT ON THE PASSENGER SEAT BUT THE AIR BAG WILL NOT BE DEPLOYED, MAYBE BECAUSE THE VEHICLE THINKS THERE IS A DOG, SMALL CHILD, OR INANIMATE OBJECT SITTING IN THE SEAT. THE TROUBLE IS THAT THE "AIR BAG DISABLED LIGHT" OCCASIONALLY TURNS ON WHEN MY TEENAGE SONS SIT IN THE PASSENGER SEAT. THE DEALER SAYS IT IS BECAUSE MY SONS DO NOT ALWAYS SIT SQUARELY TO THE BACK OF THE SEAT; THAT IS, THEY OCCASIONALLY SLOUCH IN THE SEAT. I HAVE VIDEOS OF THIS BEHAVIOR WHICH I CAN SEND TO YOU, SO YOU CAN DETERMINE IT IS IS SOMETHING YOU WOULD LIKE TO LOOK INTO. NISSAN'S RESPONSE IS AKIN TO STEVE JOBS SAYING OF THE IPHONE 4, "YOU'RE HOLDING IT WRONG." I THINK NISSAN SHOULD BUILD THEIR SENSORS FOR THE WAY PEOPLE SOMETIMES SIT IN THEIR SEATS, NOT AS THE WAY THEY WOULD LIKE THEM TO SIT UNDER IDEA CIRCUMSTANCES. THIS IS A GENERAL COMPLAINT ABOUT RECENT-MODEL HEAD RESTS. IT IS NOT LIMITED TO OUR CAR. I JUST WANT TO SAY THAT I THINK SOMEONE AT THE TRANSPORTATION BOARD IS OVERLY ZEALOUS IN THIS. MOST NEW CARS TODAY HAVE THE HEADRESTS VERY FAR FORWARD. IT SEEMS AS IF THE TRANSPORTATION BOARD WANTS THE HEADREST TO TOUCH THE HEAD. I DO NOT LIKE THIS. MY HEAD IS JOSTLED WHEN THE HEADREST TOUCHES MY IT. I THINK EVEN ONE HALF INCH AWAY IS TOO CLOSE, AS THEN IT TOUCHES MY HAIR. THE RESULT OF HAVING IT SO FAR FORWARD IS THAT IT PUSHES MY HEAD FORWARD, TOWARDS MY CHIN, AND MY NECK AND BACK HURTS. AND UNINTENDED CONSEQUENCE, I AM SURE, IS THAT MY WIFE AND I PUSH THE SEATS FORWARD AND THE BACK OF THE SEATS BACKWARDS. THIS HELPS TO KEEP THE HEADREST BACK A LITTLE BIT. BUT, I THEN HAVE TO TILT MY HEAD FORWARD TO KEEP IT VERTICAL, SO IT STILL IS A BIT UNCOMFORTABLE. I THINK YOU DO NOT REALIZE THAT IF YOU MAKE THINGS TOO UNCOMFORTABLE FOR PEOPLE, THEY WILL FIND WAYS AROUND IT AND NEGATE YOUR SAFETY SYSTEM. I JUST WANT YOU TO KNOW THIS. AND, I HOPE YOU WILL CORRECT THE ERROR AND PUSH HEADREST BACK IN THE NEAR FUTURE. *TR

WHENEVER OUTSIDE TEMPERATURES ARE BELOW +/- 38F, THE REGENERATIVE BRAKE SYSTEM BEHAVES VERY AGGRESSIVELY IN THE SENSE THAT THE SLIGHTEST USE OF THE BRAKES RESULTS IN A VERY JERKY BEVAHIOUR OF THE CAR, BRAKING VERY ABRUPTLY AND HARD. USUALLY, A SECOND TAP ON THE BRAKE RESOLVES THE ISSUE TEMPORARILY, BUT THE SYMPTOM COMES BACK SOON AFTER. THIS IS NOT ONLY AN INCONVENIENCE, BUT ALSO POSES POTENTIAL ACCIDENTS IF A CAR BEHIND THE LEAF IS NOT EXPECTING A HARD BRAKING. Nissan North America, Inc. (Nissan) is recalling certain model year 2013-2014 Altima, LEAF, Pathfinder, and Sentra, model year 2013 NV200 (aka Taxi) and Infiniti JX35 and model year 2014 Infiniti Q50 and QX60 vehicles. In the affected vehicles, the occupant classification system (OCS) software may incorrectly classify the passenger seat as empty, when it is occupied by an adult.

Risk: If the OCS does not detect an adult occupant in the passenger seat, the passenger airbag would be deactivated. Failure of the passenger airbag to deploy during a crash (where deployment is warranted) could increase the risk of injury to the passenger.

Remedy: Nissan will notify owners, and dealers will update the OCS software, free of charge. The recall began on April 14, 2014. Owners may contact Nissan at 1-800-647-7261.

Nissan North America, Inc. (Nissan) is recalling certain model year 2016-2017 Nissan Maxima, 2013-2016 Nissan Altima, NV200, LEAF, Sentra, and Pathfinder, 2014-2016 Nissan NV200 Taxi, Infiniti QX60, QX60 Hybrid, and Q50 Hybrid, 2014-2017 Nissan Rogue and Infiniti Q50, 2015-2016 Nissan Murano, Murano Hybrid, and Chevrolet City Express, 2014-2015 Nissan Pathfinder Hybrid, and 2013 Infiniti JX35 vehicles. In these vehicles, the front seat passenger Occupant Classification System (OCS) may incorrectly classify an adult passenger as a child or classify the seat as empty despite it being occupied. As a result, the passenger frontal air bag may be turned off and not deploy in the event of a crash.

Risk: If the passenger frontal air bag does not deploy as intended in the event of a crash, the passenger is at an increased risk of injury.

Remedy: Nissan will notify their owners. Chevrolet City Express owners will be notified by General Motors. Investigations

3 NHTSA investigations on record

Investigations are agency reviews, not findings that a vehicle is defective.

Pedestrian alert sounds

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

Occupant Classification System Failure

In April 2014, Nissan North America launched Safety Recall 14V-138 to address problems with the Occupant Classification System (OCS) algorithm in the following vehicles: model year (MY) 2013-2014 Nissan Altima, Pathfinder, Sentra and LEAF; MY 2013 Infiniti JX35; MY 2014 Infiniti QX60 and Q50; and MY 2013 Nissan NV 200/Taxi vehicles.According to Nissan, the OCS may misclassify adult front passenger occupants and turn off the passenger's frontal air bag for an adult occupant of sufficient weight.In March 2015, the Office of Defects Investigation (ODI) opened this Recall Query after the agency received approximately 124 complaints alleging problems with the OCS after the recall repairs and/or other OCS related repairs were made by Nissan/Infiniti dealers.The majority of the complaints alleged the passenger air bag status light stays on (i.e., indicating passenger air bag is turned off) for adult front passengers of sufficient weight.Many of the complaints stated the dealers have made multiple repairs but the problem still persists.In its June 2015 response to ODI's information request under the Recall Query, Nissan provided approximately 1,121 complaints and field reports on the recalled vehicles.These reports alleged OCS issues similar to the NHTSA reports described above.At this time, NHTSA has received a total of approximately 208 complaints (note: total of 1,271 reports shown in the table above eliminates 58 duplicative reports received by ODI and manufacturer).This Recall Query has been upgraded to an Engineering Analysis (EA15-004) to conduct a comparative assessment and further evaluate the effectiveness of the recall remedy which will include a thorough review of the countermeasures taken by Nissan and its suppliers.The ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers:10583283 10585264 10585349 10585379 10592434 10593399 10593501 10594329 10594614 10594755 10595161 10595645 10595946 10596128 10596459 10596952 10597037 10597452 10597518 10597719 10598537 10598869 10604755 10604834 10605188 10605251 10606565 10606828 10607038 10609649 10610519 10610991 10614467 10614609 10614745 10616905 10617008 10617507 10617903 10618099 10618202 10619059 10619563 10620719 10621840 10622214 10625808 10625981 10625998 10626006 10626023 10626335 10626458 10626732 10628355 10629382 10629460 10629606 10630441 10630502 10630990 10632251 10632588 10632934 10632997 10633714 10633810 10637994 10638591 10639040 10639147 10639324 10639473 10643046 10643843 10644203 10644333 10644341 10644690 10644891 10644985 10648834 10648997 10649414 10649772 10649874 10651360 10651564 10651924 10651982 10652024 10658665 10658844 10659484 10660294 10661195 10661358 10661541 10661795 10661855 10662901 10663033 10663338 10663860 10663978 10668054 10668768 10670538 10672807 10673088 10676471 10678478 10678997 10679512 10680748 10680769 10680975 10681656 10682235 10682902 10682993 10683391 10683689 10689672 10689809 10689981 10691626 10692137 10692558 10692625 10693168 10695312 10700614 10700960 10701008 10701011 10701014 10701018 10701023 10701027 10701031 10701032 10701035 10701045 10701055 10701064 10701074 10701146 10701196 10701228 10701237 10701254 10701301 10701306 10701326 10701367 10701370 10701384 10701415 10701453 10701526 10701558 10701656 10701670 10701946 10702003 10702667 10702798 10703032 10703657 10703766 10703813 10704196 10704988 10705530 10705919 10706621 10712543 10712568 10712575 10713335 10714316 10714330 10714536 10714980 10715132 10715945 10717336 10720918 10720971 10721331 10721746 10723424 10724568 10731167 10731868 10732012 10732065
