“My vehicle was in 2 MAJOR wrecks. One in dec 2021, where I t boned someone who turned in front of me at a green light. $19k in damages. Should have totaled it. But how in the hell the air bags didn’t go off is amazing. Bc my chest hit the steering wheel so hard i had to be treated for it. Not only that but the wreck destroyed my lower vertebrae. After it took the shop 6 months to fix and taken back for problems multiple times. I had it 2 weeks, and the brakes failed. Due to something the shop did I’m sure but I rear ended a lady with no brakes at 45-50 mph. Slamming directly into her even though I saw her. I broke my toe I hit the brakes so hard. But again I hit her directly where all the sensors are and this wreck was worse than the first the vehicle totaled and no air bags went off…. I shattered my left knee from hitting the dash.broke my big toe on my right foot etc. why didn’t my air bags go off…… I bought my vehicle certified pre owned in 2019. There’s no way in either wreck I missed that air bag sensor. I work in the collision industry. I know they have to be hit directly on to make them go off but there is no reason mine shouldn’t have other than a defect. I was alone in the first wreck, the second I was further injured and had to stop treatment I was still undergoing from the wreck before bc of the lapsing. My daughter was also injured in the second wreck. Second wreck happened in July 2022”

“MY HUSBAND AND I WERE INVOLVED IN A 4 CAR PILE UP ON THE 3 LANE HIGHWAY. TRAFFIC ABRUPTLY STOPPED SEVERAL CARS IN FRONT OF US ON THE HIGHWAY AND CARS WERE SWERVING TO MISS THE TRAFFIC STOPPED. THE 2 CARS IN FRONT OF US AND OUR CAR WERE ALL ABLE TO GET STOPPED WITHOUT HITTING EACH OTHER. WE WERE STATIONARY (STOPPED) WHEN WE HEARD SQUEALING TIRES AND WERE REAR ENDED BY A F250 HAULING A TRAILER FULL OF FIREWOOD AT A HIGH RATE OF SPEED. THIS TRUCK THEN PUSHED US FORWARD INTO THE VEHICLE IN FRONT OF US. WHICH THEN PUSHED THAT CAR INTO THE VEHICLE IN FRONT OF IT. SO, OUR VEHICLE WAS HIT FROM BEHIND AND THEN IN THE FRONT. THERE IS SEVERE DAMAGE TO THE VEHICLE, AND IT HAS BEEN DEEMED A TOTAL LOSS BY INSURANCE. MY CONCERN IS THAT NOT ONE AIRBAG DEPLOYED DURING THIS ACCIDENT. IN ADDITION, THE DRIVER'S SIDE SEAT BELT DID NOT LOCK PROPERLY. IT WAS PULLED ALMOST ALL THE WAY OUT AND THEN LOCKED. MY HUSBAND IS NOT A BIG GUY SO IT SHOULD OF LOCKED RATHER QUICKLY AND NOT AT THE END OF THE BELT. AS A RESULT, OF THE MALFUNCTIONING EQUIPMENT MY HUSBAND FLEW FORWARD HITTING HIS FACE AND CHEST ON THE STEERING WHEEL. HE DID SUSTAIN INJURIES TO HIS FACE AND BODY FROM THE ACCIDENT. I WOULD LIKE TO REQUEST THAT THE AIRBAG AND SEAT BELT ISSUED BE LOOKED AT. IT DOES APPEAR THAT SOME OTHER JEEP MODELS OF THE SAME YEAR HAVE SOME RECALLS FOR AIR BAG ISSUES. I HAVE INCLUDED PICTURES OF MY DAMAGED VEHICLE AND THE SEATBELT. *BF 'PARTS OF THIS DOCUMENT HAVE BEEN REDACTED TO PROTECT PERSONALLY IDENTIFIABLE INFORMATION PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).'*JB”

“2017 Jeep Cherokee Trailhawk suffered a Power Transfer Unit failure leading to sudden motive power. Vehicle was pulled over to the side of the road with a narrow shoulder. Exiting the vehicle put driver at risk of being struck by traffic. Driver had to exit the vehicle via the passenger side leading to neck injury. Note Failure of have could have resulted in crash if it had happened earlier. PTU failure confirmed by dealership. Vehicle was under FCA recall O1C NHTSA 25V-011. It should be noted that even though this is a safety issue owners were not told not to drive vehicle, plus the remedy doesn't prevent PTU failure, which can happen without warning in certain conditions.”

“The 4WD warning light illuminated on the dashboard of this 2017 Jeep Cherokee (only 67,000 miles). The vehicle was taken to an authorized Jeep dealership, where a technician officially diagnosed a complete mechanical failure of the Power Transfer Unit (PTU). The estimated cost to replace the failed component is approximately $4,000 out of pocket. This vehicle is experiencing the exact same mechanical PTU failure, dashboard warnings, and safety hazards outlined in the manufacturer's existing widespread PTU recalls and warranty extensions (such as campaigns 25V-011, 20V-343 and the XJ1 warranty extension). However, the manufacturer has excluded this specific VIN from coverage. Stellantis was contacted many times directly regarding this issue and stated they could do nothing to assist because the VIN is not currently tied to an active recall campaign. A sudden internal failure of the PTU presents a severe safety risk, as it can cause an unexpected loss of drive power while the vehicle is in motion or create a rollaway hazard when the vehicle is parked. Because this vehicle has the identical manufacturing defect at low mileage, the manufacturer needs to expand the recall parameters to include excluded VINs facing the exact same safety failure. A deposit of $2,700 has already been paid to order the replacement component, and the vehicle has now been waiting for the necessary part for approximately two months with no resolution.”

It was unknown whether a warning light was illuminated. The driver panicked, depressed the brake pedal and shifted into park(P), but the vehicle failed to stop. The driver engaged the parking brake, and the vehicle came to a stop crossway on two opposite lanes. The vehicle was towed to an independent mechanic, where it was diagnosed with PTU failure related to an extended warranty coverage. The vehicle was later towed to the dealer, where it was diagnosed that the PTU had mechanically failed. However, the contact was informed that the warranty could not be honored because no diagnostic trouble code was retrieved. The vehicle was not repaired. Desiccated Air Bag Inflator Rupture investigation

Critical severity
Evidence: High confidence

Official record covers model years 2017 through 2018.

Symptom
Desiccated Air Bag Inflator Rupture From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L.
Why it matters
The records describe a possible fatal outcome.

What to inspect

  • Ask the inspector to test the named system and record the result.

Ask the inspector to check: Independent pre-purchase inspection.

What may help

Source recordEvidence: High confidence

The records describe a possible fatal outcome.

“Desiccated Air Bag Inflator Rupture From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.”

“Chrysler (FCA US LLC) is recalling certain 2014-2017 Jeep Cherokee vehicles equipped with a two-speed Power Transfer Unit (PTU). Relative movement in the Power Transfer Unit (PTU) between the differential input splines and the transmission output shaft may cause some input spline teeth to wear off, which may eventually cause a loss of engagement between the transmission and the differential inside the PTU. If this occurs, power cannot be transferred between the front wheels and the transmission which results in a loss of drive while the vehicle is in motion and a loss of the Park function while stationary. A loss of drive can cause a vehicle crash without prior warning. A loss of the Park function can cause unintended vehicle rollaway which can increase the risk of a crash or injury.
