“The contact owns a 2021 Toyota Rav4. The contact received notification of NHTSA Campaign Number: 23V865000 (Air Bags) however, the part to do the recall repair was unavailable. The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. The contact stated that while driving at approximately 35-40 MPH with traffic ahead slowing down, the vehicle registered the traffic information as an immediate stop ahead and the braking system engaged along with the contact depressing the brake pedal to slow the vehicle down. Upon coming to a stop, a pick-up truck rear-ended the vehicle 3 - 4 times. No air bags deployed. The driver's front seat belt failed to retract during the impact to safely secure the driver. The contact sustained injuries to her knees, shoulders, and arms. The front passenger sustained bruises from the seat belt. The rear passenger sustained head and back injuries. All 3 occupants received medical attention via ambulance to the Emergency Room. A police report was filed. The vehicle was towed due to it being undrivable. The insurance company was currently inspecting the vehicle. The vehicle was not diagnosed or repaired. The dealer was not contacted. The manufacturer was not made aware of the failure. “I was at a complete stop and was rear ended. The person who hit me was going over 45mph, and the impact caused me to hit the car in front of me, which crushed my car at both ends. My (driver) airbag did not deploy, and my head struck the steering wheel. It is available for inspection, and has not been confirmed by a dealer - it is at the insurance company’s tow lot. “My RAV4 PRIME was in stop and go traffic. Brakes did not switch from regen braking to physical brakes. Slammed on the brakes several times, they did not slow the car enough. Eventually the brakes locked up as the RAV4 rear ended the car in front. Pushing that car into the one in front of it, and that one in front of that, and that one in front of that. Causing damage to three other cars. Two other people are claiming medical, the driver went to the ER. Air bags did not deploy. Taking to the repair shop. They were shocked that the air bags didn't go off given the the condition of the frame. Toyota Motor North America (TMNA) did an inspection to read the event data recorder (EDR). Brake were applied at -1.5 seconds before impact. And here is the weird part, saying that the ABS activated then too. There was no notification, and the brakes locked up. Speed went from 46 to 21.1 mph. This occurred at 3,573 miles on only a few month old RAV4. I'm well aware of what ABS feels like when it goes off.”

The van suddenly made an emergency avoidance maneuver to bypass a stationary vehicle completely stopped in the lane. Despite the attempt, the van rear-ended the stopped vehicle. The contact immediately initiated an evasive maneuver to avoid the sudden collision occurring ahead. However, due to proximity and immediate lane obstruction, the contact was unable to avoid an impact and struck the vehicle.
