“While operating my vehicle on a public roadway, the vehicle suffered a sudden, catastrophic loss of transmission hydraulic pressure and vehicle propulsion, accompanied by a dashboard warning light with a blinking "D". The vehicle completely stopped delivering power to the drive wheels while moving with traffic, creating an immediate and severe safety hazard for a rear-end collision on an active road. We pulled into the high school parking lot, stopped the car, then restarted it. It had the error on the dashboard, but we were able to get it safely home, where it was towed to the dealership later. The vehicle was towed to Dick Hannah Honda (the local authorized dealership). The service department scanned the onboard computer (from the parking lot, NO actual physical inspection done) and pulled internal transmission fault codes P0730 (Incorrect Gear Ratio) and P0746 (Pressure Control Solenoid "A" Performance/Stuck Off). Based solely on these electronic fault codes, the dealership stated that the internal mechanics of the ZF 9-speed transmission had completely failed. They stated the transmission is completely unrepairable and quoted a full replacement cost of $11,984.41. Prior to this failure, the vehicle had service documentation and inspection records from authorized Honda franchise technicians, who repeatedly marked the transmission condition and vehicle health check sheets as completely healthy and "GREEN" at every visit for 60,000 miles by American Honda Motor Co. Case Management was contacted (Case #16371194), but they refused any financial goodwill assistance. This sudden operational failure of the ZF 9-speed transmission platform without ANY prior Maintenance Minders given by the computer system. This failure matches the exact software calibration and pressure defects currently cited in ongoing consumer protection litigation involving the 2016-2022 Honda Pilot 9-speed drivetrain.”

“HONDA PILOT 2018 87K MILES DIAGNOSED WITH DTC P0741 TORQUE CONVERTER Vehicle has experienced severe and recurring drivability issues on more than one occasion during normal driving conditions. While attempting to change lanes, the vehicle exhibited delayed acceleration and loss of power. When accelerating from a stop sign, the vehicle bucked and lagged, followed by sudden and unintended forward propulsion. These events occurred without any warning lights or alerts. The unpredictable behavior of the vehicle has resulted in near‑miss accidents and required sudden braking and evasive actions to avoid potential collisions. This erratic behavior occurred without warning, reduced my ability to safely control the vehicle, and created a dangerous situation for my family, surrounding traffic and me. I opened a case with American Honda Motors (#16231734)and they stated that NHTSA is responsible for identifying the VINs that need to be part of the recall and/or Service Bulletin 23-078. I called NHTSA and NHTSA denied such claims.”

The contact was informed that the torque converter needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure, and a case was filed. The contact referenced a TSB; however, the VIN was not associated. The contact was referred to the NHTSA Hotline for assistance. The failure mileage was approximately 87,443.”

“No Restart After Auto Start/Stop Engages On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751.  Based on analysis of data from RQ24013, NHTSA has closed that investigation. The significant number of reports of engine failure in vehicles outside the scope of RQ24013 presents a potential safety risk that warrants further investigation.  ODI is opening this Preliminary Evaluation (PE) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues.  To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.”

“Engine failure On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation.  NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.”

“Honda (American Honda Motor Co.) is recalling certain 2018 Honda Pilot and Ridgeline vehicles and 2018 Acura MDX vehicles. The front driver and passenger powered seats in the Honda vehicles and the driver powered seats in the Acura vehicles may have been assembled with improperly manufactured rivets. In the event of a crash, the rivets can break causing the seats not to be secured to the floor. As such, these vehicles may fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 207, "Seating Systems," and 210, "Seat Belt Assembly Anchorages." In the event of a crash, if the seat does not remain secured to the floor, the seat occupant has an increased risk of injury. Honda will notify owners, and dealers will replace the driver seat frame assembly in Acura MDX vehicles, and replace the driver and front passenger seat slide rail frames in Honda Pilot and Ridgeline vehicles, free of charge.
