“The contact owns a 2022 Honda Odyssey. The contact stated that while their wife was driving approximately 50 -55 MPH, the front passenger side air bag and the curtain air bag, which were expanded from the front passenger seat to the rear passenger seat, suddenly deployed without impact. The sudden deployment caused the driver to immediately stop the vehicle to avoid a further incident. The contact stated their wife had informed them of a pot hole being in the road; however, they could not confirm if the air bags deployed after driving over the pot hole specifically. The driver pulled to the side of the road, and the local police were contacted. A police report was filed. The contact stated that two children had received injuries, one child received a bruise and swelling to the side of their head, and the other child received bruising and scratches to their arms. The contact's wife received bruising and whiplash from the incident. The other two children in the vehicle at the time of the incident received mild whiplash. Medical attention was needed. The vehicle was towed to a body shop, where a mechanic confirmed that no impact had occurred and was unable to diagnose the cause of failure. The manufacturer was not made aware of the failure. The failure mileage was approximately 75,000.”

“While approaching an intersection, a 4 door sedan in front of me rapidly applied their brakes to stop for a yellow light. I did not see they had stopped in time, so I rear ended them. The impact was so severe, the engine from my 2022 Odyssey was pushed into the firewall and most likely is totaled. If the Collision Mitigation Braking System (CMBS) initiated, it did not occur before I slammed on my brakes. The airbags also did not deploy. The car is undriveable. I was taken to the hospital and was found to have a hand injury from most likely my hand hitting the dash. My neck has whiplash and upper back/sternum is sore. I am being referred to an orthopedic surgeon for further hand evaluation. I am confused why the CMBS didn't react quicker and/or more aggressively as well as why didn't the airbags deploy?”

“The right side airbags deployed without any damage or accident occurring to the vehicle. The safety of the driver and backseat passenger was put at risk because the airbags deployed without any warning and completely disabled the vehicle forcing us to the side of the road and trapping the backseat passenger who was in a wheelchair. The vehicle has been assessed by police and reported there was no damage done to the vehicle. The dealership has evaluated and reported there was no reason the airbags should have deployed. An SRS light would come on frequently. Dealership said it was a wiring issue but no permanent fix was ever made or recommended.”

“The contact owns a 2022 Honda Odyssey. The contact stated while driving at 50-65 MPH, the collision mitigation braking system was applied and the vehicle stopped abruptly, causing her children to sustain whiplash. The message "brake" was displayed on the instrument panel. The vehicle was taken to the dealer where it was diagnosed that no issues were found. However, the failure had reoccurred several times while there were no other vehicles or objects around the vehicle. Additionally, the contact stated while her son was attempting to exit the vehicle through the driver's side rear sliding door, he pressed the door lock button; however, the door did not fully open fully and closed abruptly on his left hand. The contact's son was taken to the hospital where it was diagnosed that he sustained severe pain, laceration, and swollenness on his left hand. The dealer was notified of the failure. The manufacturer was notified of the failures and advised the contact that they would arrange for a field technician to inspect the vehicle; however, the vehicle was not inspected. The contact followed up with the manufacturer. A case was opened. The manufacturer advised the contact to acquire a vehicle rental while the inspection was being completed. The vehicle was not diagnosed or repaired. The failure mileage was approximately 950.”

“Passenger side, sliding door. The door closes on its own randomly but becoming more frequent. I have taken the van to the dealer multiple times and they can not figure out what is wrong. There was no warning light or error shown. Today the door shut on my son’s foot as he was getting in the car. The door fully closed and pinned his foot causing him great pain. He could not twist to open the door and free himself. The door did not safety bump open (as it should have if anything was in the way of closing) It was not until I heard his screams of pain that I ran to his side of the car and opened the door, freeing him. I have attached the photo of his foot.”

“I went to open back tailgate with key fob, no sound or anything - tailgate did not open. I tried inside vehicle buttons for back tailgate and would not respond - no sounds or anything. Nothing was obstructing the back gate on the outside or interior of vehicle to prevent it from opening. I found a video of a 2021 Odyssey having water leaks into the car from the sliding door and rusts the power module causing it to short. So, my husband opened the back fuse box and got into the back panel and removed and sure enough there is water damage to the module. It is about $400 to replace and it is only a Honda dealership part. This is a safety issue because there is no way to manually open back tailgate in case of an emergency. If water is slowly leaking and causing corrosion, this needs to be fixed by Honda.”

“I am reporting a recurring and dangerous failure of the infotainment system and electrical control in my vehicle. The following issues occur intermittently but frequently: Safety Camera Failure: The center display screen often goes black, causing the backup camera and side-view cameras to fail completely. This occurs while the vehicle is in reverse, creating a significant blind-spot hazard and risk of collision. I specifically visited the dealership in early June 2025 to inquire about this issue—and I have video evidence to prove it—yet the electronic fault "magically disappeared" (an intermittent issue) when the vehicle was inspected on-site. Despite this, the dealership still charged me a $300 diagnostic fee. Total System Freeze: The GPS navigation, audio system, and all center console controls frequently stop responding or shut down entirely while driving, causing significant driver distraction. Power Management Defect: Most critically, the infotainment system and audio often fail to shut off even after the engine is turned off and the vehicle is locked. This parasitic draw continues until the 12V battery is completely drained, leaving the vehicle stranded and potentially creating an electrical fire hazard. On 03/20/2026, the infotainment system failed to shut down after the engine was turned off, which completely drained the battery. I had to call AAA for roadside assistance to tow/jump-start the vehicle to the dealership. I have a video recording of the system staying powered on while the vehicle was off. Furthermore, I just replaced the 12V battery with a brand new one in February 2026, confirming that this is a parasitic draw caused by a vehicle defect and not a faulty battery. I am unable to upload videos here, but I reserve the right to present them.”

“No Restart After Auto Start/Stop Engages On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. “Inadvertent Deployment of Side Air Bags On October 28, 2025, the Office of Defects Investigations (ODI) opened PE25018 to investigate complaints of inadvertent deployment of the side curtain and/or side thorax air bags. These complaints concern model year (MY) 2018-2022 Honda Odyssey vehicles manufactured by American Honda Motor Co. (Honda). Honda issued recall 26V227 on April 9, 2026, to address concerns of inadvertent side air bag deployment. In the recall filing, the company stated that “[a]s of April 2, 2026, Honda has had 130 warranty claims, 25 reports of an injury, and no reports of death related to this issue from January 24, 2017 – April 2, 2026.” Honda states that the Supplemental Restraint System (SRS) Electronic Control Unit (ECU) contains incorrect deployment parameters for the side thorax and side curtain air bags, which may cause inadvertent deployment when the vehicle encounters strong road impacts, such as driving over potholes, speed bumps, or road debris. Honda stated that the SRS control logic for the second and third rows of seating contained an insufficient deployment threshold margin, allowing signal inputs to be misinterpreted as side impacts and causing inadvertent deployment of the side thorax and side curtain air bags. Recall 26V227 states that dealers will reprogram or replace the SRS ECU with improved air bag deployment parameters. Consumers received notice of this recall in late May 2026. In view of the recall being conducted by Honda, ODI is closing this Preliminary Evaluation (PE). The Agency reserves the right to take additional action if warranted by future circumstances.
