“I was travelling on a two lane surface road with a speed of 45 mph, travelling at or below posted speed and just starting to slow to make a right hand turn when I encountered / wandered over the solid white line into a pothole at the edge of the road where there is no shoulder. Airbags on the passenger side including the sidecurtain and airbag inside the passenger seat deployed instantly. There was no collision and no deployment of the front passenger airbag or any other bags in the vehicle. There was an immediate call from Honda regarding the airbag deployment along with an automated notification to local police. Police in [XXX] responded and made a report and created a case number: #[XXX] I had a passenger in the front seat at the time who received a facial laceration, bruises to the right arm and hip. This is part of a known recall for this model and year of vehicle. Police and local dealership ( Casey Honda of XXX [XXX] ) have both confirmed visually that the airbags have deployed. No warnings, lights preceded the event. I just received the letter in the last few days regarding the recall. Honda has issued a case number for the incident: #[XXX]. I will be following up Honda for an inspection of other accident sensor and systems for safety which can be affected by the deployment. I had taken a hearing test during the year before the accident and have hearing results after the accident. Our resolution for the time was to just push on the door harder and it would continue to slide open. Today, on XXX I was going to put my [XXX] son in his carseat, which is located mid van right side. He snuck infront of me and was standing in the path of the sliding door, it did what it normally does, the same door did not detect his body and began shutting on him. As it was squeezing his shoulders I was pushing back on the sliding door with all my strength, and after a few seconds it opened back up. “I am reporting American Honda’s ongoing failure to safely remedy critical safety defects and recalls on my 2018 Honda Odyssey (purchased new), resulting in severe vehicle degradation and a life-threatening traffic hazard. The vehicle has been to an authorized dealer over 21 times.SAFETY RECALL NEGLIGENCE & POWERTAIN DAMAGE: At 95k miles, the vehicle displayed active check engine codes P0087 (Low Fuel Pressure), P0420, and P0430. The dealer replaced injectors, but severe fuel issues persisted. The dealer utilized stall tactics, claiming they "could not duplicate" ongoing stalling/hesitation symptoms. When the official fuel pump recall was issued, the bulletin mandated priority service for vehicles actively displaying symptoms. My vehicle met these parameters. However, when I attempted to get the recall performed at 111k and 120k miles, the dealer denied service due to part shortages, instructing me to drive the vehicle until it "completely broke down." Because Honda failed to supply recall parts to a symptomatic vehicle, I was forced to drive it a defective car for an additional 46,000 miles. A replacement pump was installed at 166k miles, but the prolonged forced operation directly caused the high pressure pump to fail and raw fuel to melt both catalytic converters. The dealer issued a $6,000 repair estimate for damage engineered entirely by Honda's recall delays.LIFE-THREATENING TCM FAILURE: One day after a related service visit, the Transmission Control Module suffered a catastrophic electrical failure. The engine completely shut off without warning while I was actively facing four lanes of oncoming traffic, creating an immediate, severe collision risk.BROKEN SLIDING DOORS: The power sliding doors failed under 12 months of ownership. “No Restart After Auto Start/Stop Engages
On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. On October 28, 2025, the Office of Defects Investigations (ODI) opened PE25018 to investigate complaints of inadvertent deployment of the side curtain and/or side thorax air bags. These complaints concern model year (MY) 2018-2022 Honda Odyssey vehicles manufactured by American Honda Motor Co. (Honda). Honda issued recall 26V227 on April 9, 2026, to address concerns of inadvertent side air bag deployment. In the recall filing, the company stated that “[a]s of April 2, 2026, Honda has had 130 warranty claims, 25 reports of an injury, and no reports of death related to this issue from January 24, 2017 – April 2, 2026.” Honda states that the Supplemental Restraint System (SRS) Electronic Control Unit (ECU) contains incorrect deployment parameters for the side thorax and side curtain air bags, which may cause inadvertent deployment when the vehicle encounters strong road impacts, such as driving over potholes, speed bumps, or road debris. Honda stated that the SRS control logic for the second and third rows of seating contained an insufficient deployment threshold margin, allowing signal inputs to be misinterpreted as side impacts and causing inadvertent deployment of the side thorax and side curtain air bags. Recall 26V227 states that dealers will reprogram or replace the SRS ECU with improved air bag deployment parameters. Consumers received notice of this recall in late May 2026. In view of the recall being conducted by Honda, ODI is closing this Preliminary Evaluation (PE). The Agency reserves the right to take additional action if warranted by future circumstances. On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation. NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.”
