“Vehicle was involved in an accident and according to eye witnesses, the vehicle stuttered when pulling out into an intersection. This resulted in a t-bone accident where this vehicle was impacted on the driver's side and the passenger with this vehicle suffered a severe traumatic brain injury. Vehicle was inspected by traffic investigation unit and insurance. Unknown if warning lights or messaging appeared prior to accident.”

“The contact owned a 2022 Honda Accord. The contact stated that while driving 68-70 MPH, the vehicle stopped abruptly, causing another vehicle to crash into the rear passenger's side of the vehicle, and a second vehicle to crash into the rear of the vehicle, causing the trunk to be pushed inside the rear seats of the vehicle. The brake warning light was illuminated. The air bags of the contact's vehicle did not deploy. The air bags of the two other vehicles deployed. The contact stated that the rearview mirror separated and hit her head, causing a concussion. Additionally, the contact sustained whiplash, PSTD, and contusions on the back and on both ankles. The contact received medical assistance. The front passenger seat occupant of the second vehicle that rear-ended the vehicle sustained hip injuries, as the occupant was still recovering from a hip surgery that she had undergone prior to the collision. It was unknown whether the passenger received medical assistance. There was no property damage. A police report was filed. The vehicle was towed to Crash Champions, where it was totaled. The contact hired private investigators who retrieved the EDR data and informed the contact that the emergency braking system had engaged prior to the crash; however, the contact stated that there was no other vehicle or object around the vehicle at the time the emergency braking system had engaged. The vehicle was towed to the dealer, where the contact was informed that special equipment was not available to diagnose the vehicle based on the EDR data. The manufacturer was not notified of the failure. The failure mileage was approximately 37,600.”

“This event occurred after a history of ABS system issues as well as Infotainment system issues since 10k miles. During morning commute at approximately 60 mph, all dash warnings began to chime associated with the Touring features of the sedan. TPMS, Collision Avoidance, Braking System, Lane Departure, etc. AND Electric Power Steering. The steering wheel clicked and immediately became difficult to steer. I found a safe pullover point and managed to return the car safely the short distance home.”

“Inadvertent Automatic Emergency Braking On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles.  The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations.  However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury.  The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources.  In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged.  In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. “Honda (America Honda Motor Co.) is recalling certain 2023 Civic, Acura RDX, Acura Integra, and 2022 Honda Accord vehicles. A ball valve in the vehicle stability assist (VSA) modulator may leak brake fluid, which can result in unintended vehicle movement when the brake hold feature is engaged or an unexpected increase in brake pedal travel. Unintended vehicle movement or an unexpected increase in brake pedal travel can increase the risk of a crash or injury. Dealers will replace the VSA modulator, free of charge. The fuel pump inside the fuel tank may fail. Fuel pump failure can cause an engine stall while driving, increasing the risk of a crash. Dealers will replace the fuel pump module, free of charge. The front passenger seat weight sensor may crack and short circuit, failing to suppress the air bag as intended. An air bag that deploys unintentionally during a crash can increase the risk of injury. Dealers will replace the seat weight sensors, free of charge.
