“On January 28, 2023, my Chevrolet Silverado was involved in a significant multi-impact collision. At the time of the accident, I was a front-seat passenger and my vehicle was fully stopped when it was struck from behind at high speed by a Dodge 2500. The force of the rear impact pushed my truck into the vehicle in front of us, resulting in a secondary front-end collision. This sequence occurred twice. During the collision, the front passenger-side airbag did not deploy, and the front passenger seatbelt failed to lock. As a result, I was thrown violently within the passenger side of the vehicle and struck my head on the dashboard. In contrast, the driver’s seatbelt locked properly, as did the rear seatbelt where my child was seated. Due to the failure of the passenger-side seatbelt to lock, I sustained a severe concussion, a black eye, and other injuries. The vehicle was subsequently inspected by my insurance company. When sheriff’s deputies and state police arrived at the scene, they expressed concern and surprise that the passenger seatbelt did not lock and that no airbags deployed despite the severity of the collision. Shortly after the accident, I contacted GMC regarding these safety concerns. GMC retrieved the vehicle’s Crash Data Recorder (CDR) and conducted an investigation. I was later informed that GMC found no defects or issues with their products. Nearly three years have passed since the accident, and I remain deeply concerned and dissatisfied with GMC’s conclusion. The passenger-side seatbelt did not function as designed, while the driver and rear seatbelts did, and no airbags deployed. I do not believe adequate accountability or explanation was provided regarding the apparent failure of the passenger-side restraint system. Given the force of the impact, I could have been ejected through the windshield.”

“WHILE TRAVELING IN THE LEFT HAND LANE OF A 4 LANE DIVIDED HIGHWAY WE NOTICED THE VEHICLE IN FRONT OF US WAS NOT MOVING. WE WERE GOING APPROXIMATELY 58 MPH. WE COULD NOT MOVE TO THE RIGHT LANE DUE TO VEHICLES ENTERING THAT LANE FROM AN ON RAMP. WE WERE ABOUT 100 YARDS AWAY WHEN WE REALIZED THE VEHICLE IN FRONT OF US WAS NOT MOVING. WE IMMEDIATELY SLAMMED ON THE BRAKES. THE TRUCK DID NOT STOP. WE HAD A VERY SEVERE COLLISION. WE SLAMMED INTO THE REAR OF THE VEHICLE IN FRONT OF US. THAT VEHICLE WENT LEFT INTO THE GUARDRAIL AND WE FINALLY STOPPED ON THE REAR BUMPER OF THE VEHICLE THAT HAD INITIALLY STOPPED WITH HALF OF THEIR VEHICLE STILL IN THE LEFT HAND LANE. TALKING LATER WITH MY SPOUSE WE BOTH REALIZED NEITHER OF US FELT THE ANTI-LOCK BRAKES ENGAGE. THERE WAS NO SOUND THAT IS USUALLY ASSOCIATED WITH BRAKING HARD. I TOOK PICTURES AT THE SCENE. I FOUND IT ODD THAT THERE WERE NO SKID MARKS. IT WAS A CLEAR DAY AND SHORTLY AFTER 11 AM. THE AIRBAGS DEPLOYED. MY HUSBAND REQUIRED TREATMENT AT THE LOCAL ER. THE TRUCK WAS TOTALLED.”

“I had a complete loss of steering and had to have the truck towed to the dealership. This could have caused a very serious accident. There was no previous warning - it just went out. The dealership charged me $2183 to replace the steering gear. According to the literature, the steering function should last to well past 100K miles (my truck had 66,552 miles on it). In addition, there are multiple reports of steering going out online - perhaps a recall is in order. The literature also states that if diagnostic code C1599 then the steering rack is completely fine mechanically but it has simply lost its electrical connection. That is the code that the dealership saw - it sounds like the fix could have been much simpler and cheaper.”

Of the 1,157 reports received, 173 are outside of the scope of recall 25V-274. These engines are installed in Model Years (MY) 2019–2024 Chevrolet Silverado 1500 and GMC Sierra 1500, and 2021-2024MY Chevrolet Suburban and Tahoe, GMC Yukon/Yukon XL, and Cadillac Escalade/Escalade ESV vehicles. The subject engine failures were investigated as part of a Preliminary Evaluation (PE25001).  GM determined that these engine failures were due to multiple supplier manufacturing and quality issues. GM issued recall 25V-274 to address the supplier manufacturing and quality issues produced between March 1, 2021, and May 31, 2024. Based on the issuance of recall 25V-274, NHTSA has closed PE25001. ODI continues to receive a significant number of reports of engine failure in vehicles outside the scope of 25V-274.  This presents a potential safety risk that warrants further investigation.  ODI is opening this Engineering Analysis (EA) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues of vehicles built outside the recall scope. These complainants alleged engine bearing failures resulting in knocking and growling noises from the engine’s bottom end and in some cases more severe consequences such as broken connecting rods leading to engine damage, or engine failure. If the engine fails during vehicle operation, the vehicle will lose propulsion, increasing the risk of a crash. On April 24, 2025, GM issued recall 25V-274 to address the concern with L87 engines produced between March 1, 2021, and May 31, 2024.  GM cited the root cause as a supplier manufacturing and quality issues. GM’s  recall remedy will be either an oil change to an oil with a different viscosity or engine replacement depending on the results of the remedy inspection procedure. To date, ODI has received 1,157 allegations of engine bearing failure. GM submitted 55,464 reports in total, with 29,752 unique VINs. ODI continues to receive allegations of L87 engine failures which fall outside of the scope of recall 25V-274. Based on GM’s recall, ODI will close this PE. NHTSA will continue to investigate complaints of engine failure outside the scope of recall 25V-274 as part ofEngineering Analysis (EA25007). The passenger-side frontal air bag module may have been damaged during assembly, possibly resulting in the air bag not inflating properly in the event of a crash. In the event of a crash, an air bag that does not inflate properly can increase the risk of injury. GM will notify owners, and dealers will replace the passenger side air bag module, free of charge. “General Motors LLC (GM) is recalling certain 2019-2020 Chevrolet Silverado 1500 and GMC Sierra 1500 trucks. The battery positive (B+) cable rings may have been manufactured with excessive glue, potentially causing an intermittent electrical connection between the B+ cable and alternator. An intermittent electrical connection between the B+ battery cable and the alternator could cause the vehicle to stall, increasing the risk of a crash. The intermittent connection could also lead to electrical arcing, which could generate sufficient heat to damage surrounding material, increasing the risk of a fire. GM will notify owners, and dealers will inspect for the presence of glue or other contamination at the B+ cable attachment to the alternator and clean the connection and reattach the B+ cable connection, as necessary. If arcing has damaged the cable, nut, or alternator, the dealer will replace the damaged components. All services will be performed free of charge. GM issued owners an interim notification on February 4, 2020. GM issued second notice and the recall began March 9, 2019. Due to an Electronic Brake Control Module (EBCM) software error, the vehicle's Electronic Stability Control (ESC) and Antilock Brake System (ABS) may become disabled. Furthermore, if this specific error occurs, the vehicle's diagnostics will not illuminate the instrument cluster ESC and ABS malfunction warning lights. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 126, "Electronic Stability Control" and 135, "Light Vehicle Brake Systems." If the driver operates the vehicle unaware that ESC and ABS are not functioning, there is an increased risk of a crash. GM will notify owners, and dealers will reprogram the EBCM, free of charge.
