Due to a faulty Media Oriented Systems Transport (MOST) communication coaxial cable connector, the rearview camera image may not appear on the display. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility." A rearview camera that does not display an image can reduce the driver's rear view, increasing the risk of a crash or injury. Dealers will replace the MOST cable harness and install a straightening cover over the vehicle cable connector, free of charge. Owner notification letters were mailed July 24, 2023. The seat belts retracted as intended during impact, and the lift gate and rear driver's side window were shattered as a result of the crash. In addition, the vehicle occupied one passenger in the front passenger side seat and a second child passenger in the rear row driver's side seat. No warning lights were illuminated. Medical attention was sought for the front passenger for back injuries upon hitting the dashboard during the impact. Medical attention was sought for a strained neck for the driver. The child did not sustain any injuries or seek medical attention. A police report was filed. The local dealer was not contacted. The side, and curtain airbags all deployed simultaneously. These airbags also did not deflate making it very difficult to see and could have been extremely dangerous. There was no crash, no debris, and no external damage to the vehicle. The deployment was sudden, violent, and completely unexpected. The circumstances of this incident match the description of Honda Safety Recall UNW, which identifies a defect in the SRS control unit calibration that can cause airbags to deploy from normal road vibration, potholes, or bumps. My vehicle’s behavior aligns exactly with the recall conditions. I filed a claim with my insurance provider (Allstate), and they issued a denial because the airbag deployment was caused by a manufacturer defect, not a collision or comprehensive event. As a result, I am left with over $10,000 in airbag related repair costs, including replacement of the airbag modules, seat airbags, curtain airbags, SRS sensors, and interior trim damaged by the deployment. I contacted Honda to request reimbursement under the recall, but Honda has refused my claim and pushed me back to my insurance provider. The lack of response from Honda leaves me with significant financial damage caused directly by a known recall defect. I am requesting NHTSA’s assistance in compelling Honda to address this recall related failure, reimburse the repair costs and ensure proper handling of unintended airbag deployments”

The problem was reproduced by a Honda dealer service and while they were checking with the extended warranty to see if the sensor would be paid for, they let me take the van home for regular use. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. “Vehicle: 2019 Honda Odyssey Issue: Unintended airbag deployment consistent with Honda Recall UNW (SRS Control Unit Calibration Defect) Incident Type: Safety defect – airbags deployed without collision Location & Date: Highway 131, outside of Kalamazoo, MI - travel – normal road conditions on July 6, 2026 Insurance Outcome: Denied due to manufacturer defect, will only cover minor wheel damage While driving my 2019 Honda Odyssey on the highway, the vehicle hit a pot hole in the main lane and multiple airbags deployed without any collision, impact, or hazardous event. The side, and curtain airbags all deployed simultaneously. These airbags also did not deflate making it very difficult to see and could have been extremely dangerous. There was no crash, no debris, and no external damage to the vehicle. The deployment was sudden, violent, and completely unexpected. The circumstances of this incident match the description of Honda Safety Recall UNW, which identifies a defect in the SRS control unit calibration that can cause airbags to deploy from normal road vibration, potholes, or bumps. My vehicle’s behavior aligns exactly with the recall conditions. I filed a claim with my insurance provider (Allstate), and they issued a denial because the airbag deployment was caused by a manufacturer defect, not a collision or comprehensive event. As a result, I am left with over $10,000 in airbag related repair costs, including replacement of the airbag modules, seat airbags, curtain airbags, SRS sensors, and interior trim damaged by the deployment. I contacted Honda to request reimbursement under the recall, but Honda has refused my claim and pushed me back to my insurance provider. The lack of response from Honda leaves me with significant financial damage caused directly by a known recall defect. I am requesting NHTSA’s assistance in compelling Honda to address this recall related failure, reimburse the repair costs and ensure proper handling of unintended airbag deployments”

“The sensor on the passenger sliding door failed to work when the engine was on. Once the engine was off, the sliding door would close. The problem was reproduced by a Honda dealer service and while they were checking with the extended warranty to see if the sensor would be paid for, they let me take the van home for regular use. The next day, the door would not close with or without the engine on. We were not home and we could not leave the van in the parking lot or drive home with the door open, with our two young boys in the middle row in their carseats. My husband stood outside with the van for about 15 minutes until the door finally closed and the rear passenger window shattered. The safety of our family was put at risk due to the loose glass shards and due to the fact that the door was no longer functional. The vehicle was inspected by an agent from Louisiana Dealer Services today and they deemed that the broken window and track are due to damage and will not be covered under the extended warranty. There were no warning signs of a broken sensor before, during or after this incident.”

The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. “Inadvertent Deployment of Side Air Bags On October 28, 2025, the Office of Defects Investigations (ODI) opened PE25018 to investigate complaints of inadvertent deployment of the side curtain and/or side thorax air bags. These complaints concern model year (MY) 2018-2022 Honda Odyssey vehicles manufactured by American Honda Motor Co. (Honda). Honda issued recall 26V227 on April 9, 2026, to address concerns of inadvertent side air bag deployment. In the recall filing, the company stated that “[a]s of April 2, 2026, Honda has had 130 warranty claims, 25 reports of an injury, and no reports of death related to this issue from January 24, 2017 – April 2, 2026.” Honda states that the Supplemental Restraint System (SRS) Electronic Control Unit (ECU) contains incorrect deployment parameters for the side thorax and side curtain air bags, which may cause inadvertent deployment when the vehicle encounters strong road impacts, such as driving over potholes, speed bumps, or road debris. Honda stated that the SRS control logic for the second and third rows of seating contained an insufficient deployment threshold margin, allowing signal inputs to be misinterpreted as side impacts and causing inadvertent deployment of the side thorax and side curtain air bags. Recall 26V227 states that dealers will reprogram or replace the SRS ECU with improved air bag deployment parameters. Consumers received notice of this recall in late May 2026. In view of the recall being conducted by Honda, ODI is closing this Preliminary Evaluation (PE). The Agency reserves the right to take additional action if warranted by future circumstances. “Engine failure On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation.  NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008).
