“Inadvertent Automatic Emergency Braking
On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles. The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations. However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury. The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources. In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged. In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. “I was traveling on highway going posted speed of 70mph. There were no cars anywhere close to front of vehicle. Car automatically braked HARD, locking the seatbelt and throwing everything in car forward. It was not a "warning" brake it was a ridiculously HARD brake to attempt to stop car. I am shocked and amazed it did not cause accident or flip. As a result of this sudden hard stop, I have having back and neck pain as well as chest and shoulder pain. There was no reason for this to occur, there was nothing in front of the vehicle at all. There were however cars behind that nearly hit me. I may seek medical if back, neck and shoulder continue to hurt.”

“This is regarding an incident with my 2019 Honda CRV, on 2/12/2025. Conditions were dry, clean and dark with street lighting at approximately 6:15 pm Pacific time. I was driving northward uphill on a paved street and approximately half way up the hill, my car came to an abrupt stop and shut down. My body was jolted forward and I felt an immediate discomfort in my lower back. There was a car behind me with lights on that did not hit me, but had veered left mid-street, possibly to avoid hitting my car, or may have been in the process of making a parking maneuver. I saw nothing in the roadway that would cause my car to stop. There was no occurrence of a warning light or audible notification of a hazard to me. There were two cars stopped at a stop sign ahead at the top of the street, one was in the right turn lane and the one next to it was headed straight south. My car would not proceed. I had to press the start button to restart the car. I drove up to the top of the hill to the limit line of the stop sign. Both of the cars ahead at the top of the hill across the intersection were still stopped, but proceeded once my car came up the hill to the limit line of the stop sign.”

“The contact owns a 2019 Honda CR-V. The contact stated while attempting to come to a stop at a traffic sign, the vehicle did not brake as she depressed the brake pedal. The contact stated that she thought that she rolled into the intersection at approximately 10 MPH and crashed into a vehicle that was crossing the intersection. The contact's front end crashed into the other vehicle's wheel well and the passenger door. The contact's front end was crushed and her air bag was deployed. The contact stated that the police arrived and the Emergency Services unit responded. The contact stated she was treated for lacerations on both arms starting at her wrists and stopping at her elbows and she had bruising on her abdomen. The contact was not transported to the hospital. The contact stated that both vehicles were undriveable and towed away from the accident. The contact stated that prior to the accident she had taken the vehicle to a dealer due to a "brake system problem" message on her instrument cluster. The contact stated that the dealer had examined the vehicle and that when she drove the vehicle off of the lot the "brake system problem" message was no longer showing on the instrument cluster. The vehicle was repaired. The contact stated that the brake warning reoccurred. The contact took the vehicle back to the same dealer and was advised by the dealer that there was no failure of the brake system. The vehicle was not repaired. The contact stated that she had the crash and was not able to return the vehicle to the dealer due to the crash. The manufacturer was not informed of the failure. The failure mileage was approximately 44,000.”

“The contact owns a 2019 Honda CR-V. The contact stated while driving 40 MPH in between two intersections with no nearby vehicles or objects, the vehicle started beeping and the brake warning indicator was flashing on the instrument panel. The vehicle then erroneously suddenly activated the brakes and the vehicle came to an abrupt stop. The contact stated that his wife who was seated in the passenger’s seat sustained bruised from the seat belt to her shoulder. There was no medical attention provided. The vehicle was taken to the local dealer however, the mechanic was unable to duplicate the failure. The contact was also informed that sometimes the metal plates on the road may give a false indicator of a vehicle causing the vehicle to abruptly stop. The vehicle was not diagnosed or repaired. The manufacturer was not notified by the dealer of the failure. The failure mileage was approximately 24,000.”

“I PURCHASED A HONDA CRV FROM HAGERSTOWN HONDA ON 3/16/2019. I LOVE MY CRV. HOWEVER, I'VE BEEN HAVING SERIOUS PROBLEMS WITH MY CAR. THE HONDA COLLISION MITIGATION BRAKING SYSTEM IS CAUSING MY CAR TO JUST STOP WHILE DRIVING, USUALLY GOING 70 MILES AN HOUR OR MORE, DOWN TO ZERO WITHOUT WARNING. THE CMBS THINKS THERE IS SOMETHING I'M GOING TO HIT, HOWEVER THERE ARE NO CARS OR OBJECTS IN FRONT OF ME. THIS HAS HAPPENED TO ME ABOUT 5 TIMES NOW. I HAVE HAD MY CAR BACK TO THE DEALERSHIP TO SEE IF HAGERSTOWN HONDA CAN FIX THE PROBLEM. I HAVE MEET WITH BOB BARNES (SERVICE MANAGER) AND TALKED SEVERAL TIMES ON THE TELEPHONE ABOUT THE ISSUE, THE TECHNICIAN (NICK SHIFLER) READJUSTED THE RADAR, AND A HONDA SENSING INTERVIEW SHEET THAT WAS SENT INTO HONDA USA. IT HAPPENED AGAIN JUST LAST WEEK, WHERE I CAUSED A THREE-CAR ACCIDENT ON EAST BOUND ROUTE 66. I WAS THE ONLY ONE THAT WAS INJURED IN THE ACCIDENT AS FAR AS I KNOW RIGHT NOW, HOWEVER THE OTHER DRIVERS HAD SIGNIFICANT CAR DAMAGE. THIS CAR IS NOT SAFE. SOMEONE IS GOING TO GET KILLED. THE VA STATE TROOPER AT THE SCENE OF THE ACCIDENT TOLD ME NOT TO DRIVE THE CAR BECAUSE IT WAS UNSAFE, AND I AGREE. I CONTACTED BOB BARNES SERVICE MANAGER FROM HAGERSTOWN HONDA AGAIN LAST WEEK, AND HE SAID HE CAN'T HELP ME BECAUSE THEY COULDN'T REPRODUCE THE PROBLEM AND TO CONTACT HONDA USA. MY CAR IS UNSAFE TO DRIVE, BECAUSE YOU NEVER KNOW WHEN THE CMBS SYSTEM IS GOING TO ENGAGE, WITHOUT WARNING. THIS HAS ALL HAPPENED ON HIGHWAY DRIVING.”

“Driving 35 mph in my 2019 CRV EX, and all of a sudden, the brake started braking on its own. There was no warning, and the brake warning light came on several times with the brake braking on and off several times. There were not any objects, cars, or obstructions around. We (friend as passenger) came to a sudden stop when the braking first started up. It felt like major whiplash with jerking like motions. This lasted about 5 seconds and was just shocking. Our chests hurt afterwards, and it felt like the life was taken out of us. I was not using the cruise control/CMBS on at the time, but it was on to use if needed. I called Stirling Honda the next day about the problem and scheduled an appt. to bring the SUV in 6/22/2023. The lady did not seem to be too worried in my opinion. I actually have not been using the cruise control and CMBS at all, and actually have disabled it (turned off). This is a big safety concern, and thank God there was not a car in front of me or in back of me. I cannot imagine being at a higher speed and this happening.”

“On June 17, 2022 I had front end collision. I was stopped at a traffic light and went to move up closer to the car in front of me. When I touched the accelerator my accelerated very quickly in and removed my foot from the pedal. The engine continued to rev even though my foot was on the break. I was not seriously injured. The car that I hit had 2 people in it and they looked ok at the scene. I think they are being evaluated for injuries. I am waiting to take it to Progressive Insurance Network collision center. But they don’t seem to be interested in anything but body work.”

“On October 7, 2025, I was driving to work. There was a steel metal plate of approximately 1 inch height lying flat on the roadway, presumably to cover up a pothole or other utility work. The steel metal plate was near an intersection where I was to be turning left. I was driving relatively slow as it was almost time to make the left turn, about 15-20 mph. When I was almost upon the steel metal plate my car braked to a complete stop because of the Collision Mitigation Braking System. The intense deceleration has had my back in pain all day, and my neck is slowly starting to hurt as well. I feel like I was in a car accident from how quickly my car came to a stop over what appeared to be the steel plate tripping the CMBS. My vehicle is available for inspection upon request. My safety was at risk due to the back and neck pain I am currently experiencing. If a car had been driving behind me, I would have almost certainly been rear ended which could have created further injury. This feature cannot be permanently disabled and needs to be able to be. It's an undue burden on the driver of the car to have to turn it off every time the car starts. I am concerned someone else will drive my car and not know to turn off that feature and they will be injured or in an accident.”

“The contact owned a 2019 Honda CR-V. The contact stated while driving approximately 48-50 MPH, the Collision Mitigation Braking System engaged while there was no other vehicles nearby and the contact sustained whiplash. The vehicle was taken to the dealer, where the contact was informed that the failure could be related to a crack on the passenger's side of the windshield. The vehicle was not repaired and was traded in. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was approximately 45,000.”

“The CMBS suddenly engaged the brakes when there was no other vehicle on the road no less in front of the vehicle. The brakes very forcefully engaged causing the vehicle to slam to a stop. The light has come on previously on occasion with no cause but the brakes have never been engaged till now.”

“Traveling about 30mph on a street, the car activated the automatic breaking for no apparent reason. The screen flashed the orange “BREAK” sign at the same time that the car slammed the brakes on (with no prior warning). There were no cars in front, next to, or around me to the sides. There were no people or animals around and the weather was fully clear. The car behind me honked loudly at the sudden breaking of my vehicle. Everything in my car flew forwards (bags falling off the seat into the foot bed). It was very dangerous and I was lucky to avoid being hit from behind. I believe I injured my back and neck from the short stop as well. And my car just went through a full inspection at Firestone two days ago and everything checked out fine. The breaks activated for absolutely no apparent reason.”

“I was driving my 2019 Honda CRV yesterday and it slammed on the brakes to a complete stop all by itself. It shocked me and jerked my head so badly. If I wasn't wearing my seat belt I would have gone through the windshield. If someone was driving behind me, they would have hit me. Please advise on what I should do. There is not a recall but online I saw there were other occurrences like this. This is the first time this has happened since I bought the car new. I have not taken it to the dealer yet. “Vehicle has had phantom braking and accelerations, occurrences of "Christmas tree dashboard" with all system alerts going off - have been intermittent, far between and unsure how to go about presenting to dealership or auto repair due to this - however THIS OCCURRED TODAY AND CAUSED A COLLISION/ ACCIDENT! My looking into the issue online has shown that this is a problem that many others have also had and that NHTSA has opened an investigation into it with honda! AGAIN - MY VEHICLE IS NOW DAMAGED AND I AM INJURED DUE TO THIS SYSTEM MALFUNCTION/ERROR! Today while in slow moving traffic to turn onto interstate, car started having the phantom breaking and jolting forwards out of my control. All of the system failure alerts started going off (I've been able to clear in the past by turning off and turning on car multiple times in a row and on the 5-7th restart they go away) - while slowing moving forward across railroad tracks - the railroad lights and barriers then also started to decend (unexpectedly, as they had just been down shortly before - less than 2 minutes) - I attempted to accelerate forward to clear my vehicle from the tracks and my vehicle jolted forward - despite my own braking, the vehicle's forward collision mitigation also failed and the brakes did not/would not apply hard enough (or as hard as I was applying) in order to prevent collision with vehicle in front of me. I attempted to veer to the left at the last second to avoid any collision - which limited damage to truck in front of me to very minimal, but my car's front right is moderately damaged. ADDITIONALLY - driver airbag deployed despite speed being under 35mph and collision not at a level that warranted airbag deployment - thus ALSO INJURING ME AND CAUSING ADDITIONAL CAR DAMAGE. NEEDS TO BE ADDRESSED IMMEDIATELY DUE TO PERSON INJURIES AND VEHICLE IMPAIRMENT!”

“On December 30, 2022 I was driving my vehicle between the speed of 40-45 MPH through an intersection on a overcast day mid afternoon around 11:30 AM there were no vehicles in front, behind, or either side of me. I was the only vehicle in the intersection. All of a sudden the car did an automatic hard brake. So hard I was lifted off my seat and my head almost hit the steering wheel. The mid of my back from the force back into to my seat has hurt me for a few days after the incident. Luckily there was no one behind me or I'm sure I would have been rear ended due to the hard sudden stop for no reason at all. A few days later after the holidays on January 4, 2023 I called my local Honda Service Department told them about the incident and asked if they needed to inspect my vehicle or anything they could do to fix the system. I was told there is nothing they can do at this time Honda Corporation as a whole has not done any kind of update to the programing or summited any kind of service requirement to correct. He advised for me to turn the function off every time I turn on my vehicle. At this time Honda or no one else has inspected the vehicle and I have continued to turn the function off so it has not reoccurred at this time and I sure hope to never experience it again. There were no warning lamps or messages the day of the incident. Since I have owed the vehicle on multiple occasions I have gotten a "Brake" message pop up on the dash when nothing is in front of me but never has the vehicle did an auto brake like it did on December 30th.”

“In the past month, or so, my automatic emergency braking system has initiated, for no apparent reason. Last night's incident was the most severe. I was driving about 25-30 miles per hour, accelerating (to get to the speed limit of 35 mph), when the car's automatic braking system slammed on the brakes, causing my car to lunge forward. It was not a slight brake slowdown; it was a strong, immediate, brief stop. I am grateful that nobody was traveling close behind me, otherwise I would have been rear-ended. The very strong motion jerked me so that both of my wrists and my neck are now hurting. I am now very nervous about driving my vehicle; and, especially having passengers with me. I have left a message for the service manager at my Honda dealer (where I purchased my new vehicle in 2019), and am awaiting a return call.”

“When entering an interstate at rush hour, I was the first car in the line of those entering, and there were no cars ahead of me in the remaining length of lane. At the top of the ramp, with nothing in front of me, my car suddenly came to a complete stop with the screen flashing BREAK, telling me the automatic breaking engaged. The force of the break while driving 55 mph caused my body to fly forward (to the extent allowed by the seatbelt) and later a bruise across my chest. Everything loose in my car became a projectile. The car immediately behind me had to swerve to avoid hitting me, avoiding an accident. Problem is not reproducible as the system otherwise "works fine" and the car passes inspections. The car was under a year old at that point, and there were no other issues, warning lights, etc.”

“Rear Differential Seizure
On November 29, 2022, the Office of Defects Investigation (ODI) opened PE 22-011 after reviewing field reports alleging rear differential seizure in 2018-2022MY Honda CR-V and HR-V vehicles equipped with all wheel drive. To date, ODI has not received any consumer complaints. Some of the field reports received alleged the vehicle’s wheels locked up with no warning while the vehicle was in motion. These vehicles were towed to a dealership. Examination of these vehicles showed the driveshaft had broken and the differential unit was completely damaged. The field reports cited a lack of oil due to damaged oil seals as the cause. Honda provided data including consumer complaints, field reports, and warranty claims they received relating to the loss of oil retention and resulting differential damage. Honda’s data cited 1201 incidents with unique vehicle identification numbers (VINs). The rear differential oil seals prevent the oil from leaking out and protect internal components from contaminants. A loss of oil lubrication results in failure of the vehicle differential. Honda stated that the oil seal’s lip contact with the driveshaft may become unstable due to intermittent changes in internal pressure. Their review of field report and warranty data indicated that these failures occurred early in the vehicle’s life thus the failures were covered by the manufacturer’s warranty. Honda stated that in November 2021 they changed the seal specifications and improved the packaging process of the parts for transport. Honda also enacted an enhanced visual inspection process both before and after the oil seal installation on the assembly line. Honda stated that in the event of differential failure, the operator can control the vehicle without stability loss. After the drive shafts break due to differential failure, the operator will hear noises and the vehicle will still function as a 2WD vehicle preventing complete loss of motive power. Based on the following factors, ODI is closing this investigation: · Information regarding field failures does not indicate hazardous outcomes · The failures are occurring early in the life of the vehicle · Repairs are covered under the manufacturer’s warranty · The vehicle can continue functioning as a 2WD without loss of stability control in the event of failure The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the agency reserves the right to take additional action if warranted by new circumstances.”

“Honda (American Honda Motor Co.) is recalling certain 2019 CR-V vehicles. The metal core of the steering wheel may have burrs which can damage the harnesses routed within, potentially disabling the driver's air bag or causing it to deploy without warning.
In the event of a crash, a disabled air bag would increase the risk of injury. An uncommanded deployment of the driver's air bag also increases the risk of injury and a crash.

The brake warning light came on as it was braking.”

The records describe a possible crash, fire, injury, or loss of control.

To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.”
